Springing Power of Attorney With the theme of this edition being ‘Spring into Happiness,’ it seemed only fitting to talk about Springing Powers of Attorney.

A Power of Attorney, also known as a POA, is a document that allows you to appoint an agent to transact financial, legal, or business transactions on your behalf. In the POA document you are known as the principal, and the person you appoint to act on your behalf is known as your agent or attorney in fact. Keep in mind your agent or attorney in fact does not have to be an attorney; it can be your spouse or some other relative, a friend, or an associate.

For Powers of Attorney known as Durable Powers of Attorney, the authority granted to your agent takes effect immediately.

Many people mistakenly think a POA does not take effect until they become incapacitated. There is another type of Power of Attorney, known as a Springing Power of Attorney, that does not become effective unless or until you are incapacitated. This kind of POA ‘springs’ into life when it is needed. The key to a Springing Power of Attorney is the event that triggers the POA’s effectiveness: the principal’s inability to handle his or her own affairs due to his or her incapacity.

On its face this may sound like an easy precaution to take, but in reality it can be problematic. A Springing Power of Attorney requires a doctor’s examination and report. Oftentimes, two doctors’ certifications are required. It can take weeks to get the Certified Elder Law Attorney, Annemarie Schreiber answered questions and offered informaton on the Caregivers First TV show, hosted by Lynette Whiteman, Executive Director of Caregiver Volunteers Of Central Jersey. It’s hard to believe that it was just over one year ago that the world as we know it changed forever. Although the physical (and mental) effects of Covid-19 are daunting, we can still find the silver lining. One thing the virus has shown us is that we were all guilty of taking the “little” things for granted. Whether that means the ability to visit grandparents, or sending your kids to school, or attending a concert, we will never look at those things the same way again. After the long months of isolation and quarantining, spring 2021 is a welcome distraction.

Between the longer days and mild weather, it’s hard to feel anything but hopeful. In fact, studies show that taking your workout outside can improve your mental and physical well being. Whether you indulge in a post dinner walk or a recreation adult sports league, the warmer temperatures will allow you to enjoy the time outside. Spring is the ideal time of year to enjoy simple pleasures.

The flowers are blooming, the sun is shining, and we can enjoy the benefit of extra daylight. “Springing forward” has benefits for all.

There is much to look forward too on the horizon. Besides the promise of time spent outdoors, St. Patrick’s Day is just around the corner on March 17th and Easter follows in April. Chronic Ankle Instability Rebecca Kashdan, DPT

With every step you take, your feet are the only connection between your body and the ground. Your feet support you and transmit the force from each step through your ankles, through your lower leg, and to the rest of your body. Many people have, or will have, a sprained ankle at some point throughout their lifetime. An ankle sprain can impact the way you move, whether you are a competitive athlete or simply walking around the grocery store, and can lead to a chronic condition called ankle instability.

What is a sprained ankle and why is chronic ankle instability a big deal?

Your ankle is made up of several bones that not only allow you to point and flex your ankle, but also allow you to tilt your foot inward (inversion) and outward (eversion) so that you can adjust to uneven surfaces such as cobblestones, a sandy beach, or divots in your backyard. The bones in your ankle are connected by several ligaments, a type of connective tissue, on both the inside and outside to increase the stability of the joint.

When you experience a sprained ankle, usually the ankle rolls inward, resulting in overstretching the ligaments on the outside of the ankle. Occasionally the ankle is forced outward, resulting in overstretching of the ligaments on the inside of the ankle; however, that is much less common. Ankle sprains are often graded based on the severity of overstretching with grade I meaning the ligaments are overstretched but not torn, grade II meaning there is a mild to moderate tear, and grade III meaning there is a significant tear in the ligaments. Symptoms may also include swelling, pain, bruising, and difficulty putting weight on the affected

foot. If you have experienced a sprained ankle with any of these symptoms, you should see your doctor.

Chronic ankle instability is the result of several factors causing a person to sprain their ankle repeatedly. The first factor is overstretched or torn ligaments, resulting in decreased ankle stability. The second factor is muscle imbalance. When the ligaments are stretched or torn, the muscles and tendons surrounding the ankle may also be stretched and irritated, resulting in muscle weakness. The third factor is impaired proprioception. Proprioceptors are tiny nerve receptors located in soft tissue throughout the body that sense your position in space (these receptors allow you to touch your pointer finger to your nose with your eyes closed). The proprioceptors in the ankle are extremely important when it comes to balance and the ability to adjust your ankle on uneven surfaces to prevent falling. When the tissues surrounding the ankle are disrupted, the proprioceptors do not collect the information that they need to help you adjust. All of these factors combined make up an ugly recipe for repeated ankle sprains as well as increasing the risk for falling. An unstable ankle can change the way the forces are transmitted from the foot through the rest of your body, causing problems beyond your foot and ankle.

Physical therapy can help!

Physical therapy can help to break the cycle of chronic ankle instability by preventing future ankle sprains. We use exercises that focus on normalizing muscle imbalances, restoring normal motion, and improving ankle proprioception. Your physical therapist will evaluate your imbalances in order to create a program specifically designed for you. For instance, if you feel fine while standing on flat, stable surfaces but you have difficulty on a plush carpet or an uneven sidewalk, your therapist may have you practice standing and walking on unstable surfaces to functionally strengthen your ankle-stabilizing muscles.

Physical therapy will not repair a torn ligament; however, the therapists at All-Care are trained to develop a program that will promote healing as well as maximize the stability in your ankle by strengthening the surrounding musculature.

Is barefoot exercise good for me?

While proper footwear can provide excellent support for a person with chronic ankle instability, exercising barefoot in a controlled environment helps build natural stability. Often patients with ankle instability perform physical therapy exercises without shoes to increase reliance on the natural stabilizing muscles instead of relying on support from your shoes. You may notice that if you stand on one foot with a supportive sneaker and compare it to standing on one foot without shoes, you tend to “wobble” more without the shoes. That is exactly why we focus on barefoot stability. While barefoot exercise in a controlled environment can be beneficial, high-impact barefoot training such as running or jumping without proper conditioning can lead to many other injuries.

She continued her education at Rutgers University and received her Doctoral degree in Physical Therapy in 2015. Rebecca has completed clinical rotations at Duke Regional Hospital in North Carolina as well as Applewood Estates Rehab in New Jersey with a focus on post-op orthopedic, cardiovascular, bariatric and neurological conditions.

Rebecca has a particular interest in manual therapy, athletic injuries, balance dysfunction and pathologies of the foot and ankle. She has also worked with several pre-professional dancers from The Rock School for Dance Education as well as the University of the Arts in Philadelphia and is currently a member of the International Association for Dance Medicine & Science. Rebecca firmly believes that understanding the needs of her patients is the key to successful recovery.

Helping Small Miracles Happen

Recurrent Pregnancy Loss Pregnancy loss is a devastating yet common event that occurs in about 15-25% of pregnancies. Most of these losses take place before the end of the first trimester.

However, fewer than 5% of all women will experience two consecutive losses. About 1% of women may be afflicted by three or more losses. Recurrent pregnancy loss (RPL) is defined as having more than two pregnancies identified by ultrasound which did not result in a live birth. The most common cause of these losses, or as more commonly called, “miscarriages,” is an embryonic chromosomal abnormality such as Down syndrome. Other conditions that may contribute to RPL include the antiphospholipid syndrome and anatomic factors. Less likely causes include hormonal or metabolic factors, infections, or clotting disorders. Although the overall prognosis for patients afflicted by RPL is quite good, this diagnosis is emotionally difficult to deal with for the patient. The immense sense of hopelessness that patients may experience can be devastating. For this reason, it is important to understand the patient’s feelings and concerns in order to provide adequate care.

As a woman gets older, her chance of miscarriage increases. This happens mainly because age affects the machinery responsible for genetic recombination within her eggs. In women under 35, the risk of sporadic miscarriages is at most 12%. For women over 40, this risk is close to 50%. In a small proportion of cases, the cause for pregnancy loss is due to abnormality. Preimplantation genetic screening can be done to reduce the chance of a chromosomally abnormal pregnancy.

Another entity responsible for RPL is called antiphospholipid syndrome. In this case, the mother carries antibodies which can affect the ability of the

embryo to adequately grow and be nourished within the uterus. Blood tests on the mother can be done to identify these antibodies, and a blood thinner may be indicated to improve on the chance of a live birth.

Uterine abnormalities are anatomic factors which are responsible for up to 37% of all RPL. The most common uterine abnormality is a uterine septum. Other conditions, such as doubling of the uterus or defects in the shape of the uterus, are less common. Nonetheless, a sonohysterogram or a hysterosalpingogram can be done to evaluate the shape of the uterus. These abnormalities are more commonly associated with second trimester losses.

Surgical interventions can lead to very successful pregnancy outcomes.

Hormonal problems such as thyroid, prolactin, and poorly controlled diabetes can account for a small proportion of RPL. A blood test can identify these conditions. Depending on the results, medications can be used to optimize these problems.

Other inherited clotting disorders are less likely to contribute to pregnancy losses. A thorough history from the patient can help identify if there is a need for additional blood tests to identify a propensity to blood clotting.

In summary, it is imperative to provide a diligent investigation of RPL as well as give emotional support in the form of psychological counseling. These patients deserve a caring provider who can listen and help reduce anxiety by clearly communicating and answering all questions and concerns in a timely fashion.

Spring Cleaning

“Have nothing in your home that you do not know to be useful or believe to be beautiful.” — William Morris The winter is finally over and with spring comes new fragrances and pollens outside. To fully embrace the new season, some decide to clean their homes by moving everything and doing a full clean and assessment of what should be kept or thrown out.

We collect treasures over our lifetimes that represent memories and letting go of those treasures can be hard.

An article from the Journal of Gerontology: Series B, titled “The Material Convoy after 50” discusses how people divest themselves of possessions as they age. David J. Ekerdt and Lindsey A. Baker through analysis of data from a national panel conclude that “After age 50, people are progressively less likely to divest themselves of belongings. After age 70, about 30% of persons say that they have done nothing in the past year to clean out, give away, or donate things, and over 80% have sold nothing.”

Although the authors suggest that with a larger set of possessions, “their continued keeping can be a predicament for oneself and others,” they explore many reasons why less is divested as we age. The reasons they bring up that we find compelling are: 1. The longer we live, the more possessions we accumulate that signify our identity and “The ongoing collection of belongings can secure continuity of the self in the face of aging and vulnerability.”

2. As we age, we feel less up to the task of assessing value and divesting where there isn’t any: “a major reason for less possession management is the rising

risk of poor health at older ages that can limit the capacity to carry out the cognitive, physical, social, and emotional tasks of divestment.”

3. We’ve divested already: “Most obviously, people may have already divested (perhaps by having moved) and so are content with their collection of belongings.”

This is the time for switching over from winter to spring, embracing hopefulness and creativity, and enjoying sunshine and outdoor activities. But the COVID-19 pandemic has shifted all of that. This spring will be much different from what we were expecting.

Fear is in the air. We are worried for ourselves and others. We are all dealing with altered daily rhythms and an unknown future. We don’t know how long this pandemic will last. How long we will be cut off from our loved ones. How long until life feels “normal” again.

Eckhart Tolle writes, “If uncertainty is unacceptable to you, it turns into fear. If it is perfectly acceptable, it turns into increased aliveness, alertness and creativity.”

Uncertainty is not going away anytime soon, so we are forced to go through a process of acceptance. This acceptance can foster that kind of “increased aliveness, alertness and creativity” that we all long to feel in springtime.

Care for your mind Our minds are trained to quickly go into “fix-it” mode. The coronavirus pandemic is beyond our control and there is no easy fix. Meditation, writing down your thoughts, reading, and focusing on something else can help. Don’t let yourself get caught up in pandemic panic; care for your mind so it doesn’t fixate on fear.

Care for your spirit To care for your spirit, do something that fills you up. Go for a walk, paint, hug your pet, listen to your favorite music. Pick something that you know is restorative for you.

Every challenge in life can be an opportunity for growth, learning, and change. We can be inspired this season by the beauty of stillness, home, family, and nature. We can all practice gratitude and love each other into blooming, even from a distance.

March Is Colon Cancer Awareness Month Written by: Jai Mirchandani, MD Gastroenterologists of Ocean County

What is a Cancer Screening?

A screening test is a recommended medical test to examine for precancerous lesions prior to the development of cancer.

Examples of those kinds of tests include: Mammography to detect breast cancer; Pap Smears to detect cervical cancer and a Prostate Exam, which examines for early prostate cancer. Colonoscopy is a screening test to detect and prevent colorectal cancer.

What is the colon and rectum?

The colon is a 6 feet long tube, which absorbs excess water and salt into the blood, and converts waster into stool. The rectum is the end of the colon, where waste/stool os stored until it leaves the body during a bowel movement.

What is a polyp and why are they important?

A polyp is a growth out of the wall of the colon. Polyps are either benign (noncancerous), or pre-malignant (pre-cancerous). Polyps are important because certain polyps can grow large over time into colon cancer. Usually patients have NO SYMPTOMS, if they have a polyp. Polyps are caused by genetic and hereditary factors, as well as dietary and environmental factors.

What is a colonoscopy?

A colonoscopy is a test where a flexible tube is inserted into the rectum. Through this tube, a gastroenterologist can visualize the inside of a patient’s colon. Using a colonoscope the gastroenterologist can identify small polyps, and remove them internally. The procedure takes approximately 30 minutes. Patients are given sedation to make them comfortable throughout the entire procedure. The day before the procedure patients must drink a solution to clean their intestines prior to the procedure. If a patient’s colonoscopy is normal, they most likely will not need another procedure for 10 years.

What happens to patients who develop Colorectal Cancer?

Patients who develop colorectal cancer must undergo further treatments, including possible surgery, chemotherapy and radiation. Ultimately they have a shorter life expectancy.

What is the most important thing to know about Colorectal Cancer?

The most important thing to know about colorectal cancer is that it is a common form of cancer, and all patients, including men and women are susceptible to developing colon cancer. Most types of colorectal cancer are preventable however, with appropriate screening.

Colonoscopy is covered as a screening tool by Medicare and most insurances. Make sure to talk to your doctor to see if you should be screened for colon cancer.

How common is Colorectal Cancer, and do I have to worry about it?

Colorectal cancer is the 3rd most common cause of Cancer in both men and women. It is estimated by the American Cancer Society that colorectal cancer will be the second most common cause of cancer death in 2016. The lifetime probability of developing colorectal cancer is approximately 1 in 20 for both men and women, making it very common.

What are some potential symptoms of Colorectal Cancer?

Symptoms of Colorectal cancer include blood in the stool, anemia (low blood counts), changes in bowel habits, abdominal pain and weight loss. Often, however, patients have no symptoms at all.

What are risk factors for developing Colorectal Cancer?

Family history and genetic factors are one of the biggest predisposing factors to the development of colorectal cancer. If you have a close family member who had either colorectal cancer or colon polyps, you are at an increased risk for developing colorectal cancer. Environmental or dietary factors, such as diets high in red meats, alcohol or tobacco use can predispose patients to colorectal cancer. Also patients who are overweight or have advanced diabetes are also at an increased risk for developing colorectal cancer.

What are protective factors for Colorectal Cancer?

Diets high in fruits, vegetables and fiber are protective for colorectal cancer. Also, a diet high in calcium has been seen to be a protective for colon cancer as well. Exercise has been seen to be a protective factor as well to prevent colon cancer.

Who should be screened for Colorectal Cancer?

Any man or woman over the age of 50 years old should be screened for colorectal cancer. Patients with family members with a history of colorectal cancer, or colon polyps, as well as certain symptoms may need to be screened at an earlier age.

How do patients get screened?

A patient can get screened by stool tests to check for blood, X-ray or CT scan tests, as well as certain blood tests that can assess if patients are at risk for developing colorectal cancer. The best test for screening for colorectal cancer is a colonoscopy, as this test can visualize pre-cancerous polyps, as well as remove these polyps before they have a chance to grow and become colorectal cancer.

Nasal Congestion And The Dangers Of Over-The-Counter Nasal Sprays Nasal congestion is a common problem.

Almost everyone has had a stuffy nose at some point in their lives. Causes can vary, including allergies, sinusitis, viral infection, and anatomical abnormalities. Most people self-treat before seeking medical advice. Over-the-counter intranasal decongestants, drugs such as phenylephrine, can bring near instant relief. These medications are meant for short-term use only, three days or fewer.

If used for more than three days, overuse of intranasal decongestants can lead to worsening nasal obstruction and dependence on the medication. The name of this condition is rhinitis medicamentosa.

Rhinitis medicamentosa is drug-induced swelling of the nasal passages due to a rebound phenomenon. The more you use the medicine, the more stuffy your nose becomes. People find themselves trapped in a vicious cycle.

Here are some clues that you have rhinitis medicamentosa:

1. Your main and only symptom is nasal congestion.

2. You cannot identify triggers typically seen with allergies.

4. You experience withdrawal symptoms when you try to quit the spray, such as headaches, sleep trouble, restlessness, and anxiety.

Recipe Chicken In Puff Pastry Compliments of Chef Steph

Serves 4

1 sheet puff pastry, thawed and rolled out into a 14in square.

4 boneless chicken breasts, cooked 1/3 cup herbed soft cheese 1/4 cup fresh parsley , chopped 1 large egg Optional ingredients-chopped spinach, diced roasted red peppers, additional cheese, etc.

Preheat the oven to 400 degress.

Roll out puff pastry, cut into four equal sized pieces.

Place 1 1/2 tablesoons of herbed cheese in the center of each piece.

Top herbed cheese with a chicken breast and then chopped parsley Next, fold the corners of the pastry over the chicken breast.

Place chicken pastry seam side down on a baking sheet covered in parchement.

Lightly brush the top with a whisked egg.

Bake for 20-25minutes or until crust is golden brown.

The Benefits of Tax-Advantaged Savings Vehicles Taxes can take a big bite out of your total investment returns, so it’s helpful to look for tax-advantaged strategies when building a portfolio.

But keep in mind that investment decisions shouldn’t be driven solely by tax considerations; other factors to consider include the potential risk, the expected rate of return, and the quality of the investment.

Tax-deferred and tax-free investments Tax deferral is the process of delaying (but not necessarily eliminating) until a future year the payment of income taxes on income you earn in the current year. For example, the money you put into your traditional 401(k) retirement account isn’t taxed until you withdraw it, which might be 30 or 40 years down the road!

Tax deferral can be beneficial because:

• The money you would have spent on taxes remains invested • You may be in a lower tax bracket when you make withdrawals from your accounts (for example, when you’re retired) • You can accumulate more dollars in your accounts due to compounding Compounding means that your earnings become part of your underlying investment, and they in turn earn interest. In the early years of an investment, the benefit of compounding may not be that significant. But as the years go by, the long-term boost to your total return can be dramatic.

Taxes make a big difference

Let’s assume two people have $5,000 to invest every year for a period of 30 years. One person invests in a tax-free account like a Roth 401(k) that earns 6% per year, and the other person invests in a taxable account that also earns 6% each year. Assuming a tax rate of 24%, in 30 years the tax-free account will be worth $395,291, while the taxable account will be worth $308,155. That’s a difference of $87,136.

This hypothetical example is for illustrative purposes only, and its results are not representative of any specific investment or mix of investments.

Actual results will vary.

The taxable account balance assumes that earnings are taxed as ordinary income and does not reflect possible lower maximum tax rates on capital gains and dividends, as well as the tax treatment of investment losses, which would make the taxable investment return more favorable, thereby reducing the difference in performance between the accounts shown.

Investment fees and expenses have not been deducted. If they had been, the results would have been lower. You should consider your personal investment horizon and income tax brackets, both current and anticipated, when making an investment decision as these may further impact the results of the comparison. This illustration assumes a fixed annual rate of return; the rate of return on your actual investment portfolio will be different, and will vary over time, according to actual market performance. Tax-advantaged savings vehicles for retirement One of the best ways to accumulate funds for retirement or any other investment objective is to use tax-advantaged (i.e., tax-deferred or tax-free) savings vehicles when appropriate.

Traditional IRAs — Anyone who earns income or is married to someone with earned income can contribute to an IRA. Depending upon your income and whether you’re covered by an employer-sponsored retirement plan, you may or may not be able to deduct your contributions to a traditional IRA, but your contributions always grow tax deferred. However, you’ll owe income taxes when you make a withdrawal.* You can contribute up to $6,000 (for 2020 and 2021) to an IRA, and individuals age 50 and older can contribute an additional $1,000 (for 2020 and 2021).

Roth IRAs — Roth IRAs are open only to individuals with incomes below certain limits. Your contributions are made with after-tax dollars but will grow tax deferred, and qualified distributions will be tax free when you withdraw them. The amount you can contribute is the same as for traditional IRAs. Total combined contributions to Roth and traditional IRAs can’t exceed $6,000 (for 2020 and 2021) for individuals under age 50.

SIMPLE IRAs and SIMPLE 401(k)s — These plans are generally associated with small businesses. As with traditional IRAs, your contributions grow tax deferred, but you’ll owe income taxes when you make a withdrawal.* You can contribute up to $13,500 (for 2020 and

2021) to one of these plans; individuals age 50 and older can contribute an additional $3,000 (for 2020 and 2021). (SIMPLE 401(k) plans can also allow Roth contributions.)

Employer-sponsored plans (401(k)s, 403(b)s, 457 plans) — Contributions to these types of plans grow tax deferred, but you’ll owe income taxes when you make a withdrawal.* You can contribute up to $19,500 (for 2020 and 2021) to one of these plans; individuals age 50 and older can contribute an additional $6,500 (for 2020 and 2021). Employers can generally allow employees to make after-tax Roth contributions, in which case qualifying distributions will be tax free.

Annuities — You pay money to an annuity issuer (an insurance company), and the issuer promises to pay principal and earnings back to you or your named beneficiary in the future (you’ll be subject to fees and expenses that you’ll need to understand and consider). Most annuities have surrender charges that are assessed if the contract owner surrenders the annuity.

Annuities generally allow you to elect to receive an income stream for life (subject to the financial strength and claims-paying ability of the issuer).

There’s no limit to how much you can invest, and your contributions grow tax deferred. However, you’ll owe income taxes on the earnings when you start receiving distributions.*

Tax-advantaged savings vehicles for college

For college, tax-advantaged savings vehicles include: 529 plans — College savings plans and prepaid tuition plans let you set aside money for college that will grow tax deferred and be tax free at withdrawal at the federal level if the funds are used for qualified education expenses. These plans are open to anyone regardless of income level.

Contribution limits are high — typically over $300,000 — but vary by plan.

Coverdell education savings accounts — Coverdell accounts are open only to individuals with incomes below certain limits, but if you qualify, you can contribute up to $2,000 per year, per beneficiary. Your contributions will grow tax deferred and be tax free at withdrawal at the federal level if the funds are used for qualified education expenses.

Series EE bonds — The interest earned on Series EE savings bonds grows tax deferred. But if you meet income limits (and a few other requirements) at the time you redeem the bonds for college, the interest will be free from federal income tax too (it’s always exempt from state tax).

NOTE: Investors should consider the investment objectives, risks, charges, and expenses associated with 529 plans. More information about specific 529 plans is available in each issuer’s official statement, which should be read carefully before investing. Also, before investing consider whether your state offers a 529 plan that provides residents with favorable state tax benefits. The availability of tax and other benefits** may be conditioned on meeting certain requirements. There is also the risk that the investments may lose money or not perform well enough to cover college costs as anticipated.

For withdrawals not used for qualified higher-education expenses, earnings may be subject to taxation as ordinary income and possibly a 10% federal income tax penalty.

Tax deferred is not the same as tax free.

“Tax deferred” means that the payment of taxes is delayed, while “tax free” means that no income taxes are due at all. For example, with a Roth IRA, after-tax dollars are contributed, but qualified distributions (those satisfying a five-year holding period and made after age 59½ or after becoming disabled) are free from federal income tax.

Though tax considerations shouldn’t be your only investing concern, by putting your money in tax-advantaged savings vehicles and investments when appropriate, you’ll keep more money in your own pocket and put less in Uncle Sam’s. *Withdrawals prior to age 59½ may be subject to a 10% federal income tax penalty unless an exception applies. **Other state benefits may include financial aid, scholarship funds, and protection from creditors.

The Keys to Beating Back Pain in Your 80s “I just can’t believe how great I feel,” says Jaqueline Camuso, 82. You would never know she had been grappling with intense back pain on and off for nearly 12 months.

After seeing three physicians, undergoing multiple treatments for spinal stenosis – a narrowing of the spaces between vertebrae that compresses nerves in the back – and finally getting relief, she’s now sharing her experience to educate others about key lessons for beating back pain in your 80s, or at any age.

“After injuring my back, my primary care doctor told me I needed to see a pain management doctor. That physician gave me three epidurals, one each month, but I didn’t feel any pain relief until the third injection,” explains Jaqueline. “I was finally relieved, but seven months later, the pain returned. I got another epidural, but the pain persisted. That’s when they told me it was time to start exploring surgery.”

When teeth are missing, the sinus that lies above the top jaw will expand downward into the ridge and cause bone loss in the gum ridge. This is a correctable problem as well, according to Dr. Banks. “A Sub-Antral Sinus Augmentation – commonly called a ‘Sinus Lift Procedure’ – can be performed, sometimes even at the time of implant placement. This procedure increases the amount of bone in the area where we would like to place a dental implant. Again, a bone replacement product or even the patient’s own bone can be used.”

A common misconception is that the bone is placed into the sinus. How common is colorectal cancer and who is at risk?

Colorectal cancer is the second most common cancer in the United States among both men and women. It affects 140,000 people annually and causes 50,000 deaths. The average person has a one in 20 chance of developing colorectal cancer in his/her lifetime. This risk is increased if there is a family history of colorectal polyps or cancer, and is even higher still in those patients with a history of breast, uterine or ovarian cancer, and patients with a history of extensive inflammatory bowel disease such as ulcerative colitis or Crohn’s colitis. Additionally, a person’s risk for developing colorectal cancer increases significantly with age.

If the cancer is diagnosed and treated in the earliest stages, approximately 80-90% of patients are restored to their normal health. The cure rate decreases to 50% or less when the cancer is diagnosed in the later/advanced stages. Less than 5% of all colorectal cancer patients require a colostomy or “bag,” thanks to modern technology. Additionally, modern technology has made possible minimally invasive colorectal surgery using laparoscopic or robotic surgery techniques.

Both laparoscopic and robotic colorectal surgery are types of minimally invasive surgery in which small incisions are made to perform major surgery. This allows for shorter hospital stays, decreased incisional pain and earlier return to work and other activities.

Can colorectal cancer be prevented?

Colorectal cancer is one of the few cancers that is preventable. Obtaining a screening colonoscopy at regular intervals is the single best way to prevent colorectal cancer. Your colorectal surgeon or specialist is able to review the guidelines for screening colonoscopy based upon your age, medical history and family history. Although not definitively proven, studies published in the literature mention additional factors that may lower your risk of developing colorectal cancer and include eating a high-fiber, low-fat diet, maintaining a healthy weight, and engaging in regular, vigorous exercise.

It is generally agreed that nearly all colon and rectal cancer begins in benign polyps, called adenomas. These pre-malignant polyps, or growths, occur in the wall of the colon and/or rectum and may eventually increase in size and become cancer, or medically speaking, carcinoma. This is known as the “adenomacarcinoma sequence.” Several factors have been mentioned in the literature that may increase a person’s risk for developing colorectal cancer, including age, a diet high in fat and cholesterol, an inactive/sedentary lifestyle, obesity, smoking and environmental exposures.

What are the symptoms of colorectal cancer?

Unfortunately, polyps and early cancers do not produce any physical signs or symptoms. Therefore, symptoms usually indicate late or advanced disease. The most common symptoms of colorectal cancer are rectal bleeding and changes in bowel habits such as constipation or diarrhea. These symptoms are common in many other disorders of the gastrointestinal tract, so it is very important to have a thorough examination should you experience them. NOT ALL RECTAL BLEEDING IS DUE TO HEMORRHOIDS. It is very important for patients to remember this and seek evaluation if they are experiencing rectal bleeding.

Other symptoms such as abdominal pain and weight loss are usually late symptoms and quite possibly indicate advanced/extensive disease.

How is colorectal cancer diagnosed?

Evaluation of colorectal cancer is first begun with a complete history and physical exam, including a digital rectal exam and possibly testing the stool for blood. A colonoscopy is the most definitive test used to diagnose colorectal cancer. A colonoscopy is performed by a colorectal surgeon or other specialist in order to get a direct visualization of the inside of the colon and rectum. A colonoscopy is a safe, painless, outpatient procedure performed under a light sedation. Based upon the findings on colonoscopy, your colorectal surgeon or other specialist may recommend other testing such as blood work, a CT scan of the abdomen and pelvis, pelvic MRI and/or ultrasound.

Anna Brocco grew up in Toms River, New Jersey and currently lives in Forked River with her husband and toddler son. The Magnificent “Wordologist” Amanda Gorman

As I sit down to write the next feature, I am reflective over the past year, January 6, 2021 and most recently the inauguration of our nation’s President.

As I logged into my computer at work to watch the inauguration, I was just in time to view the 22-year-old Harvard graduate’s presentation of hope and peace to the world. The ability to put into words the current climate of the world with such elegance and power leaves a person, well, speechless.

I was so enamored by Amanda Gorman I had to learn more about her. I watched her speech about 10 times and then as many YouTube interviews I could find. Amanda has a twin sister and another sibling. Amanda was raised by a single mother in California. Amanda was interested in writing at a young age due to her limitations with her speech. Amanda’s writing became her voice. Amanda began performing her written work in the second grade. Amanda’s interest in literature continued to grow, and it has been reported that a speech by Nobel Prize laureate Malala Yousafzai inspired Amanda to become a youth delegate for the United Nations in 2013.

By 2014, Amanda received the award of Youth Poet Laureate of Los Angeles. Amanda used her words to advocate for social change and the underserved. In the same year, Amanda “founded” “One Pen One Page,” which is known to be a non-profit organization that “encourages youth advocacy, leadership development and poetry workshops.”

All of this greatness before college! By 2015 she was published. At the young age of 19, Amanda was awarded the United States first female African American National Youth Poet Laureate award. Amanda credits former First Lady Michelle Obama for her work in the national award.

First Lady Jill Biden was moved by Amanda’s writings after observing Amanda recite her famous poem “In this Place” at the Library of Congress in Washington, DC. Amanda has contagious energy and is incredibly positive.

I particularly enjoyed her presentation of a poem that was created from her experience during an audition for The Lion King on Broadway. Amanda Gorman described real-life interactions with her mother and sister that are Poets.org. hilarious, horrifying and a true portrayal of family life.

Amanda reflected feeling “broken” at not getting her big break for a part in The Lion King and yet she ends up being selected to write the inaugural poem with few restrictions. Amanda very openly shares that she grew up with a speech impediment and, even when she was a finalist for the inauguration presentation, had her doubts of being selected to write and deliver “The Hill We Climb.” A poem that was started conservatively over the course of a few weeks and then, after the event of January 6, 2021, Amanda seized the opportunity to be the voice of peace.

speech impediment, poverty, single-parent households, and the pressures of an Ivy League education. In an interview with James Corden (January 21, 2021), Amanda shared the events that happen after an inauguration.

As any young adult would do after getting a good grade, scoring the winning goal or making their bed, Amanda’s reaction and mindset was “maybe she could get a new iPhone.” A young individual so talented, it is quite humbling to see that yes, she is real!

Amanda is very self-aware as she described herself as “a skinny black girl, descendant from slaves and raised by a single black mother can dream of becoming a president.”

On inauguration day, Amanda Gorman stood before the nation poised, strong, articulate, and delivered a message that will continue to resonate with us for decades. Amanda Gorman has made it known to America that Instagram.com she has a dream to be on the ballot in the 2036 Presidential election. Amanda is not stopping at the inauguration. Amanda recently debuted a children’s book entitled Change Sings. The book is about how a young girl has the power to change through music. Amanda shared in an interview with James Corden that she wanted to write a book in which young girls are activists and not observers in the story. Additionally, it has been reported that Amanda was offered a modeling contract with IMG Models, and by the time of this publication she will have performed before Super Bowl 2021 to honor three pandemic heroes. There is no doubt that Amanda will continue to shine bright through her powerful prose.

The historic events of the world over the last year have dimmed the lights for many. I appreciated Amanda’s description of how we have “braved the belly of the beast.” A message quite fitting on many levels in each of our lives. In “The Hill We Climb,” Amanda brings back to focus that we are amazingly resilient wherein she reminds us that “even as we hurt we hope” and we will “rebuild, reconcile and recover.” Amanda gave us more than a simple poem on inauguration day; she gave us inspiration to live each day to its fullest.
