Dear Readers, I know I am not the only one excited to say farewell to 2020. It has been a year like no other. Although we are not quite “back to normal,” there is undoubtedly more hope in the air these days. During times of great strife, it is so important to try and stay positive. After all, with hard times comes growth and change. We all know the pandemic has wreaked havoc on the economy and has caused illness for so many.

Now more than ever, we must create some positivity in our lives. Maybe it’s practicing a random act of kindness for a friend, or supporting a small business.

I know I am very much looking forward to what the new year has to bring. The month of February is American Heart Month. Let’s try and raise some awareness about the importance of heart health and urge our friends and loved ones to prevent heart disease. Did you know that 47% of all Americans have at least 1 in 3 key risk factors for heart disease? These days, when it comes to factors we can control, we must do just that! Live a healthy lifestyle and keep your blood pressure and cholesterol levels normal.

Total Hip Arthroplasty Anterior Approach vs. Posterior Approach David Walinger, PTA

Total hip arthroplasty (THA) is used worldwide as one of the most effective procedures for relieving pain, restoring function, and improving quality of life in patients with painful or deformed hip joints. Improvements in hip arthroplasty procedures have led to faster recovery, shorter hospitalization, and higher patient satisfaction.

In the United States, more than 300,000 patients between 50-80 years of age undergo total hip replacement. The most common cause for requiring a THA is due to osteoarthritis or injury. Symptoms of osteoarthritis include pain, joint stiffness, and decreased range of motion. THA has the ability to relieve pain and improve quality of life. Among all the joints in the human body, the hip joint has a faster, more durable and successful recovery following replacement. The hip joint is a ball-and-socket joint.

The prosthetic (artificial) portion of the hip joint consists of two parts, a cup-like component that is attached to the acetabulum (portion of the pelvic bone), and a shaft with a round top that attaches to the femur (thigh bone of the leg). The damaged parts of the hip bone are removed, contoured and replaced with the prosthetic. These material combinations can include metal-on-polyethylene ,metal-on-metal, and ceramic-on-ceramic.

Total hip replacements can be performed with two techniques which include an anterior approach or posterior approach. The anterior approach is considered less invasive in comparison to the posterior approach. During a posterior approach, while lying on your side, your orthopedic surgeon will create a 4-7 inch incision behind the hip along the outer buttock

area. The gluteus maximus muscle is split but will not require repair. Two hip external rotator muscles are detached and then reattached to bone and will heal within 4-6 weeks. The anterior approach is usually performed creating an incision along the front upper thigh separating but not detaching muscle to avoid nerve damage. Physical Therapy

Early postoperative therapeutic exercises are necessary for preventing blood clots and increasing blood circulation to your lower extremities. Early exercises are also important to strengthen muscles and maintain joint integrity. Following surgery, you will meet your supervising physical therapist in the recovery room to begin exercises implemented in your personal treatment plan. It may feel uncomfortable at first, but these exercises will speed your recovery and reduce your postoperative pain.

Outpatient physical therapy sessions will help restore range of motion and increase your proximal hip stability. Weight-bearing activities will help restore strength to optimal levels as well. This level of physical therapy focuses on activity-specific rehabilitation for your personal goals, such as a return to work or other activity. Your physical therapist will incorporate exercises that simulate those activities. These may include lifting techniques, pushing, pulling carts, climbing ladders, agility exercises, or light jogging depending on your recovery and activity level.

Today, 80% of all hip replacements last for up to 20 years. Some are not always successful, leaving various factors which increase the chances of failure including an individual’s weight, compliance of hip precautions as well as level of activity and overall health status. Performing strengthening exercises is essential for stabilizing the hip during recovery.

Excessively high impact exercises can cause too much force and can increase the likelihood of complications.

Endometrial Receptivity Array (ERA) Endometrial Biopsy Since the birth of the first in vitro fertilization (IVF) baby 40 years ago, the field of reproductive endocrinology has undergone major advances in medications, retrieval procedures, and laboratory techniques.

Despite these innovations, the cumulative live pregnancy rate per egg retrieval is 35-48% for women 35 to 37 years old and 13-18% for women over the age of 40.

Even though we are able to obtain eggs and sperm to create an embryo, the live birth rate is still less than 50 % nationally. The area that is still an enigma is the “window of implantation.”

This denotes the appropriate time to transfer an embryo to maximize the implantation potential of an embryo. Over the years we have found that a specific sequence of events must occur for an embryo to implant and grow. Currently, it is preferable to transfer a blastocyst embryo, which occurs 5 days after fertilization. The blastocyst is transferred into the uterine cavity and the embryo hatches out and makes direct contact with the uterine lining. When it does, both the endometrium and the blastocyst adhere to each other. The embryo begins to undergo a process of invasion to establish a blood supply with the mother. For this sequence of events to occur, the lining of the uterus must have a specific component; this is called receptivity.

In the past, we would assess uterine receptivity indirectly using an ultrasound. The lining had

to be a specific thickness and pattern. In recent years, studies have shown this is not an accurate assessment. As a result, we may be transferring embryos at the inappropriate time. A test has been developed that directly evaluates the uterine lining for receptivity: endometrial receptivity analysis (ERA).

This is a genetic test which uses a small sample of a woman’s endometrial tissue to evaluate whether or not the uterine lining is prepared and is able to accept an implanting embryo. This test analyzes the endometrial lining at the molecular level, evaluating for 248 genes. It detects a specific genetic signature which identifies the lining as being receptive for an embryo. If it is not ready (pre-receptive) or over-mature (postreceptive), the test will denote changes required to make the lining receptive.

In the era of personalized medicine, a “one size fits all” policy is no longer acceptable.

With IVF, we individualize ovarian stimulation protocols based on a patient’s/couple’s medical situation. We now recognize the importance of individualized uterine preparation protocols to facilitate an appropriate uterine environment to optimize treatment outcome. COVID-19 and the Digestive Tract

Although widely known to affect the respiratory system, in many cases gastrointestinal symptoms may play a role or be part of the presentation. It is important for the general population to recognize these symptoms in order to act upon them with appropriate testing and quarantine.

What gastrointestinal symptoms are seen with COVID-19 infection?

Although fever, loss of taste, cough, fatigue and shortness of breath are common presentations of COVID-19 infection, gastrointestinal symptoms are frequently noted. Complaints of loss of appetite, nausea and diarrhea are often seen in infected patients. These digestive symptoms may be noted in combination with typical respiratory symptoms, but some patients will experience the intestinal symptoms first or as their only manifestation of the illness.

What’s With The CBD Craze?

The Endocannabinoid System (ECS) In order to understand what cannabidiol (CBD) is, we first need to learn more about the endocannabinoid system or ECS. This system was discovered in the early ’90s and has been researched ever since. The ECS is a network of receptors (CB1 and CB2) that are present throughout our body. They reside in places like the heart, lungs, brain, gut, skin, etc. Our bodies produce molecules (endocannabinoids) such as anandamide (AEA) and 2-arachidonoylglycerol (2-AG), which bind to these receptors, allowing our internal functions to run smoothly. This is known as homeostasis.

CB1 receptors are mainly found in the central nervous system, and CB2 receptors are found in the peripheral nervous system, especially immune cells. When our bodies don’t produce enough molecules to bind to these receptors, we go toward a state of imbalance. This can lead to inflammation, free radical production, immunodeficiency, etc.

CBD is a naturally occurring component derived from the hemp plant. People often confuse the cannabis plant with the hemp plant.

The cannabis plant contains tetrahydrocannabinol (THC) while the hemp plant contains little to no THC. You will not get the “psychoactive” effect or “high” with hemp-derived CBD. CBD’s molecular structure is similar to that of our own endocannabinoids so it has the ability to bind to the CB1 and CB2 receptors. When CBD binds to these receptors, it allows for balance or homeostasis. Our bodies are bombarded daily with free radical production as a result of stress, poor diet, obesity, inflammation, smoking, etc. By offering our ECS high-quality CBD products, it gives our bodies a fighting chance to combat any potential imbalances we may have.

What to look for in CBD

It is important to know that since the Farm Act of 2018 was passed, it took the hemp plant off the controlled substances list. How Fast Action Helped 87-Year-Old Freehold Woman Survive a Major Stroke Eileen Kelleher, 87, of Freehold, New Jersey, has been a caretaker her entire life. It was only fitting that she receive outstanding care when she needed it the most.

“My mom’s stroke literally happened right before my eyes,” says Eileen’s daughter, Margaret Kelleher. “We were at home, having a conversation when she left the room for a moment. She walked back over to my desk, opened her mouth to say something and nothing came out. She was staring straight ahead and started to slump over. I told her she was having a stroke and guided her gently to the floor as I called 911.”

Margaret requested her mother be taken to Jersey Shore University Medical Center, where Eileen had been treated for a transient ischemic attack (TIA) – also called a mini stroke – six years ago and for a dangerous spike in her blood pressure just months earlier.

Before Margaret arrived at the hospital, she received a call from neurosurgeon Maureen Darwal, DO, who had already evaluated Eileen in the emergency room. Eileen was alert but unable to speak or move the right side of her body. She explained Eileen needed emergency surgery to remove a blood clot in her brain, which was causing her stroke.

“Everyone was working so fast,” Margaret recalls. “My mom was in surgery within 90 minutes of the onset of her symptoms at home.”

She credits the speed of care – in addition to the skill of all of the medical personnel and the advancements in medical technology – with enabling her mother to pull through a significant stroke with no permanent damage.

“Time is always critical when it comes to stroke care,” explains Dr. Darwal, a member of the neurosurgical team at Altair Health. “In Eileen’s case, our surgical approach also played a big factor in her health.”

Dr. Darwal determined Eileen was a great candidate for a thrombectomy – a minimally invasive procedure whereby a small catheter is inserted into the femoral artery near the groin and navigated to the blood clot in the brain. The clot is then removed by a retrievable stent.

“Our advanced imaging revealed there was a blockage in a large vessel in Eileen’s brain, but the area of her brain in danger of permanent damage was still receiving some blood flow,” Dr. Darwal continues. “By performing a thrombectomy as quickly as we did, we successfully removed the blockage and saved Eileen from having any lasting damage from her stroke.”

Following the procedure, Eileen stayed in the ICU for 48 hours.

Eileen was discharged just three days after surgery. With no evidence of permanent damage in follow-up CT scans, she didn’t require any rehab or even in-home physical therapy. However, she was required to wear a heart monitor to help determine the root cause of her stroke.

Within the first few hours of wearing the monitor, the doctors diagnosed Eileen with atrial fibrillation. This explained the clot that traveled up to Eileen’s brain and began blocking normal blood flow.

Eileen has since had a permanent heart monitor implanted. She is committed to doing everything she can to manage her AFib and avoid another stroke, demonstrating the same grit she has shown in the face of many challenges throughout her life.

Legal Professionals When Abuse is Profuse, Always Refuse to Lose!

Surviving and Thriving – Domestic Violence

New Jersey has one of our nation’s strongest set of laws in place to protect victims of domestic violence. Domestic violence appears in many forms, including cyber abuse, emotional / mental abuse, financial abuse, physical abuse, sexual abuse and a whole host of other combinations.

The Prevention of Domestic Violence Act of 1991 is a statute found within New Jersey’s Code of Criminal Justice, specifically at N.J.S.A. 2C:25-17 through 2C:25-35. The law is found within the criminal laws; the court process is considered civil in nature and final restraining order (FRO) hearings / trials are held in the New Jersey Superior Court, Chancery Division, Family Part. Given the state of COVID-19 and the attendant stressors accompanying it, our practice has seen a tremendous spike in domestic abuse and violence cases, giving us all a great cause for concern.

This article aims to give an extremely basic and brief overview to a complex area of the law in New Jersey. Victims of domestic abuse and violence can use these set of laws through a skilled attorney to navigate through a difficult period of their life and through a legal process that seems intimidating. First, who exactly qualifies as a victim? Our state law defines a victim of domestic violence as any person who is eighteen (18) years of age or older or who is an emancipated minor and who has been subjected to domestic violence by a 1) spouse, 2) former spouse, 3) present household member, 4) past household member, 5) any person, regardless of age, with whom the victim has a child in common, 6) anticipates having a child in common (pregnancy), or 7) has or has had a dating relationship with. See N.J.S.A. 2C:25-19d.

When victims of domestic abuse and violence muster the courage, strength and temerity to finally say enough to their abusers, the process usually begins with a 911 call if there is an active situation or when the victim can safely escape the situation and goes to either the municipal police department or Superior Court. The process can seem overwhelming but either a police officer or court staff will take the initial information and connect the victim before a municipal court judge, a domestic violence hearing officer or a Superior Court judge to obtain a temporary restraining order (TRO). This is known as an ex parte proceeding, meaning that the abuser is not present nor an attorney from their side. An interpreter can be made available and accommodations for any disability if needed. By law, the victim is put under oath and a copy of the testimony is supposed to be kept, either through stenographic medium, tape or longhand notes (if he or she is not physically present). See Rule 5:7A(d). The victim is asked information regarding the predicate (underlying) act or set of acts causing them to seek out a TRO. There is a box to be checked off on the first page of the TRO and the law provides nineteen (19) categories that can fit, including false imprisonment, sexual assault, harassment and stalking. See N.J.S.A. 2C:25-19a(1)-(19). Additionally, they are asked for information about past history regarding domestic violence and abuse. If during the shock or pressure of this harrowing ordeal a victim forgets information, he or she can either themselves, or better yet, through an attorney, get an amended TRO that includes past acts of violence not initially included. See State of New Jersey Domestic Violence Procedures Manual § 4.5.7. For most victims at the onset of seeking a TRO, an attorney is not involved; however, it is imperative that the first call they make after getting safe from a current act(s) of domestic abuse and violence is to one. At that point, the judge either grants or denies the TRO.

If the TRO is granted, you will be provided a copy of the TRO, which indicates certain things the abuser is prohibited from doing, including, but not limited to, contacting you by any means nor through third parties, to have the abuser vacate the household, prevent them from showing up to your work and other protective measures. Law enforcement are directed to serve a copy of this TRO to put the person on notice that a judge has granted this protection. Then by court rule, this event must be heard within ten (10) days to determine whether an FRO is to be granted. See Rule 5:7A(e). Usually, and especially during COVID-19, a reasonable postponement to get an attorney, from either the victim or the abuser, is honored. It should be noted that unlike the criminal justice system, where an attorney will be appointed for you if you cannot afford one, this is inapplicable

in the domestic violence arena in New Jersey. It is common, however, for the abuser to be charged criminally as well in either municipal court or Superior Court, depending on the severity of the underlying domestic violence act.

Once the day arrives for the FRO hearing / trial, you will need all your witnesses present along with any evidence that supports your case. This holds true even during the pandemic wherein FRO hearings / trials are being held through virtual court, usually on a Zoom or Microsoft Teams platform. In our state court system, as the victim / plaintiff, you bear the burden of proof. Fortunately, for domestic violence victims it is the lowest burden of proof, a preponderance of the evidence which essentially means that the evidence is more likely true than not true (essentially slightly over a fifty [50] percent standard).

See N.J.S.A. 2C:25-29a. In order for the victim of domestic violence / plaintiff to win, he or she must prove, by a preponderance of the evidence, that 1) the court has jurisdiction to hear this case or, in other words, whether they are a victim of domestic violence as defined above; 2) that they have proven by preponderance of the evidence that at least one predicate act occurred and 3) that they have proven by a preponderance of the evidence that issuance of an FRO is necessary to prevent further abuse or future harm. See generally N.J.S.A. 2C:25-29a(1)-(6); Silver v. Silver, 387 N.J. Super. 112 (App. Div. 2006).

If the victim is successful in obtaining an FRO, it is a major victory for him or her and a major impediment for the defendant / abuser. A judge has the power under this comprehensive act to order a host of things, including, but not limited to, exiting the residence, revocation of a person’s firearms identification purchaser card, a monetary penalty, subject the person to drug testing, fingerprinting, a financial penalty, payments of restitution, payments for rent or mortgage, custody of a pet, parenting time, mandated professional domestic violence counseling. An FRO can have immigration consequences for a non-citizen, can affect a background check, military or security clearance, and if violated, will subject the violator to prosecution in the Criminal Part of the Superior Court for an indictable crime of contempt. An FRO in New Jersey, unlike other jurisdictions, is permanent – it does not need to be renewed and generally it is not removed absent an exception.

Nasal Congestion And The Dangers Of Over-The-Counter Nasal Sprays Nasal congestion is a common problem.

Almost everyone has had a stuffy nose at some point in their lives. Causes can vary, including allergies, sinusitis, viral infection, and anatomical abnormalities. Most people self-treat before seeking medical advice. Over-the-counter intranasal decongestants, drugs such as phenylephrine, can bring near instant relief. These medications are meant for short-term use only, three days or fewer.

If used for more than three days, overuse of intranasal decongestants can lead to worsening nasal obstruction and dependence on the medication. The name of this condition is rhinitis medicamentosa.

Emotional Investing “Only when you combine sound intellect with emotional discipline do you get rational behavior.” - Warren Buffet We like to think that since the advent of modern portfolio management practices, investing in stocks and bonds has become a cerebral, analytical process with no room for emotion. The truth is that most investors, even institutional investors, are buffeted (no Pun intended) by emotional turbulence from time to time, and that truth is reflected in the volatility of the financial markets.

But if a little emotionalism when it comes to investments is unavoidable, too much emotion can be hazardous to your wealth. Here are four symptoms of problem emotions, financial behavior that is inconsistent with sound investment practice.

• Fear of loss. Investors are generally motivated by fear or by greed. Behavioral scientists have learned that, for many people, the pain of loss is larger than the sense of satisfaction from a gain of the same size. Similarly, some investors will accept larger risks in order to avoid a loss than they will in seeking a gain. Taken to an extreme, fear of loss leads to investment paralysis. An excessively risk-averse investor may park funds in ultra-safe, low-yielding bank deposits or short-term Treasury securities until a decision is made, accepting long periods of low returns. Or winning investments may be sold off too quickly in an attempt to lock in gains, while losing investments manage to stay in the portfolio indefinitely.

• Following the herd. It’s difficult to be a contrarian, to find value that everyone else has overlooked. Many people find it easier to go with the crowd, to own the current hot stock or hot mutual fund. At least that way, if the investment does poorly, one has plenty of fellow sufferers with whom to commiserate. But when “crowd” is defined as one’s family and friends, the crowd’s investment goals may be very different from one’s own.

• Hair-trigger reflexes. Markets move on news. In many cases, the first market response is an overreaction, either to the up side or to the down. Sometimes “news” is only new to the general public, and it’s already been reflected in the share price through trading

by those with greater knowledge. The true importance of any news event can only be discerned over the longer-term. Generally, it’s better to watch the market react to news than to be a part of the reaction. Remember that market dips may present the best buying opportunities but they’re also the toughest times, emotionally, for making a commitment to an investment.

• Betting only on winners. Some 85% of the new money going into domestic equity mutual funds goes to funds with Morningstar ratings of four or five stars, according to one estimate. This may be one reason that the government requires this disclosure for investment products: Past performance is no guarantee of future results. The disclosure is required because it is true. High returns are usually accompanied by high risks; ultimately, those risks may undermine performance.

Abnormal returns, whether they are high or low, tend to return to the average in the long run. Investing on the basis of the very highest recent returns runs a significant risk of getting in at the top of the price cycle, with a strong chance for disappointment.

To avoid impulsive decisions that may be tainted with emotion, one needs an investment plan. The best way to moderate the impact of stock and bond volatility in difficult markets is to own some of each. Assets do not move up down in lockstep. When stocks rise, bonds may fall. Or at other times, bonds also may rise when stocks do. The movements of each asset class can be mathematically correlated to the movements of the other classes. Portfolio optimization involves the application of these relationships to the investor’s holdings.

Expected returns need to be linked to the investor’s time horizon. Longer time horizons give the investor more time to recover from bad years, more chances to be in the market for good years. We like to say it is time not timing that is most important when developing a portfolio for a client.

Toms River X-Ray, CT, And MRI Center: The Last Of The Family Run Radiology Practices As a Radiologist, Dr. Cyrus Khorrami is a medical doctor who specializes in diagnosing and ultimately directing the treatment of disease and injury by using medical imaging techniques such as X-rays, CT, MRI, mammography, and ultrasound. Born into a family of physicians, he has been surrounded by medicine his entire life. “Growing up, I would hear fantastic stories my parents told about how they were able to change people’s lives. I knew this was what I wanted.” He adds, “Radiology answers the question of why someone is sick or why they are in pain. We work closely with all types of medical professionals to diagnose and treat patients. Every case is unique, which makes this field challenging. Reading hundreds of studies a week, I potentially diagnose and help hundreds of patients each week, which makes my job extremely rewarding.”

After graduating from George Washington University, he moved to Philadelphia and conducted research at the University of Pennsylvania in creating a melanoma vaccine.

He stayed in Philadelphia and attended medical school at Temple School of Medicine.

After obtaining his medical degree, Cyrus spent the next four years at Cooper University Hospital for his radiology residency. After becoming a Board Certified Radiologist, he went on to Johns Hopkins Medical Center in Baltimore for a Cross Sectional Body Imaging Fellowship. Although his work has taken him all over the tri-state area, he moved back to Ocean County and enjoys living close to his family.

Both of Dr. Khorrami’s parents are physicians. His father, Parviz Khorrami, is also a Radiologist and founded Toms River X-Ray, CT, and MRI Center. The practice began in 1981 when his father decided to leave the hospital system and open his own practice. He opened a small office on Route 37 with a single X-ray machine. Dr. Parviz Khorrami has slowly built his practice up over the last 30 years to now having over 40 employees and offering all imaging modalities. “We are the oldest and the largest privately owned imaging center in the area,” he says.

Patients will immediately feel a different level of comfort when treated at Toms River X-Ray, CT, and MRI Center. “Large corporations or hospitals run most radiology practices,” he explains. “Ours is a family-run practice, which is very unusual in radiology.

There used to be lots of privately owned radiology centers. Over the years, it became

harder and harder to stay independent. Those other family run, privately-owned centers either closed down or were absorbed by larger radiology groups. It is a story we have all seen a million times. The family owned stores cannot compete with the corporate chains.

“My father founded our practice. My mother has been the office administrator for many years. I serve as the medical director. As with any family-run business, we take pride in what we do and work hard to provide the very best service possible. In my experience, larger corporations have less flexibility and less a person touch. We try to be as helpful and understanding as we can. We care deeply for our patients and strive to treat each and every one like they are a part of our family. I believe that is the reason we are able to prosper.”

Dr. Khorrami enjoys the field of radiology because there are constantly new technologic breakthroughs and advances in imaging happening. He adds, “It is extremely important to have the most state-of-the-art methodologies and to hire and retain the best staff.” He makes it a priority to keep in close contact with the referring physicians he works with. “All doctors should work together as a team to provide the very best and most complete care for our patients.” Part of his responsibility involves working closely with other referring physicians to help guide diagnosis and treatment for his patients.

Once he is able to help figure out the cause of a patient’s symptoms, he can quickly direct appropriate treatment. This can potentially result in catching life-threatening illnesses as quickly as possible.

There are many cough and cold medications on the market that can help with your cold or cough symptoms. Decongestants in a pill form or a nasal spray can help with a stuffy nose. Antihistamines can help with itchy, watery eyes. Cough suppressants help control a nagging cough.

Cough drops or throat lozenges help with a sore throat. Tylenol or Motrin can help any body aches or fever. Before taking any medications, please consult with your pharmacist or doctor to be sure the medication you are buying doesnâ&#x20AC;&#x2122;t aggravate any existing medical conditions or interact with any current medication you are taking. The Live-In Caregiver Deborah Walsh,

Most of us understand the benefits of having a live-in caregiver and remaining in the comfort of one’s home. Consider for a moment the perspective of the caregiver who must move out of their own home, leave family behind, and move into a client’s home. We certainly understand the point of view of the client but want to take a moment to view it from the standpoint of the caregiver and to highlight the work that they do.

Both the caregiver and client come from different backgrounds and personal histories. A caregiver must attend to a client’s most personal needs. A client must allow the caregiver to be privy to her personal life. Both are difficult. Yet, each needs the other and they compromise to make the situation mutually beneficial.

Cultural differences, food choices, routines and more, come into play. A caregiver must navigate the client’s family relationships and discover how to fit into the family dynamic. She often advocates and speaks to family with and for the client as she gains more knowledge of the client’s needs and routines.

We take for granted our dining routines; eating and mealtime is significant for all of us. In the caregiving situation, food is a key part of the equation for both the caregiver and the client. Often the caregiver is from a different culture. Buying familiar food and

meal preparation is something that takes considerable work so that both are content with the living situation. This process, the give and take, is significant in building the relationship.

Companionship is another foundational element to building the relationship.

Our caregivers spend time playing board games, cards, puzzles and more to interact with their clients. Going to dinner or lunch together or to church services and the beauty salon are also often on the day’s agenda. This special time together fosters the bond.

The nurse who oversees the situation is always in touch and is essential in supporting this relationship. He or she will guide the caregiver and the client in getting past small hurdles and issues. We are often pleased to see the interesting combinations of caregiver and client who come together with HomeWorks. It is not unusual to have a caregiver from Africa preparing an Italian dish for a client or a client from the Mid-West enjoying a Jamaican dish. It can be an enriching life experience.

We work hard to make sure personalities are compatible and the relationship can develop in a positive way. The tenderness, patience, and love expressed by our live-in caregivers are very special and often mimic a familial bond.

If either you or your future spouse has or may inherit substantial assets, or if either of you has children from previous marriages, you may want to consider a prenuptial agreement. Discuss your financial history

Marriage is the union of two separate individuals ... and their finances. While talking about money can be a stressful topic for many couples, you’ll want to sit down and discuss your financial history and your future spouse’s financial history before you merge your money.

Start out by taking stock of each of your respective financial situations. You should each make a list of your individual assets (e.g., investments, real estate) and any liabilities (e.g., student loans, credit card debt) you may have. This is also the time to address items such as how much each of you earns and if either of you has additional sources of income (e.g., interest, dividends).

Agree on a system for budgeting/maintaining bank accounts

Right now, you are probably accustomed to managing your finances in a way that is comfortable for you and you alone. Once you are married, you and your spouse will have to agree on a system for budgeting your money and paying your bills together as a couple.

Either of you can agree to be in charge of managing the budget, or you can take turns keeping records and paying the bills. If both of you are going to be involved in the budgeting process, make sure that you develop a record-keeping system that both of you understand and agree upon. In addition, you’ll want to keep your records in a joint filing system so that both of you can easily locate important documents.

Once you agree on a budgeting system, you’ll be able to establish a budget. Begin by listing all of your income and expenses over a certain time period (for example, monthly). Sources of income can include things such as salaries and wages, interest, and dividends. Expenses can be divided into two categories: fixed (e.g., housing, utilities, food) and discretionary (e.g., entertainment, vacations). Be sure to include occasional expenses (e.g., car maintenance) as well. To help you and your future spouse stay on track with your budget: • Try to make budgeting part of your daily routine • Build occasional rewards into your budget (e.g., going to the movies) • Examine your budget regularly and adjust/make changes as needed This might also be a good time to decide whether you and your future spouse will combine your bank accounts or keep them separate. While maintaining a joint account does have its advantages (e.g., easier recordkeeping and lower maintenance fees), it is sometimes more difficult to keep track of the flow of money when two individuals have access to a single account.

If you do decide to combine your accounts, each spouse should be responsible for updating the checkbook ledger when he/she writes a check or withdraws funds. If you decide to keep separate accounts, consider opening a joint checking account to pay for household expenses.

Map out your financial future together An important part of financial planning as a couple is to map out your financial future together. Where do you see yourself next year? What about five years from now? Do you want to buy a home together? If you decide to start a family, would one of you stay at home while the other focuses more on his or her career?

Together you should make a list of short-term financial goals (e.g., paying off wedding debt, saving for graduate school) and long-term financial goals (e.g., retirement). Once you have decided on your financial goals, you can prioritize them by determining which ones are most important to each of you. After you’ve identified which goals are a priority, you can set your sights on working to achieve them together.

Resolve any outstanding credit/debt issues Since having good credit is an important part of any sound financial plan, you’ll want to identify any potential credit/debt problems either you or your future spouse may have and try to resolve them now rather than later. You should each order copies of your credit reports and review them together. You are entitled to a free copy of your credit report from each of the three major credit reporting agencies once every 12 months (go to annualcreditreport.com for more information).

For the most part, you are not responsible for your future spouse’s past credit problems, but they can prevent you from getting credit together as a couple after you are married. Even if you’ve always had spotless credit, you may be turned down for credit cards or loans that you apply for together if your future spouse has a bad track record with creditors.

As a result, if you find that either one of you does have credit issues, you might want to consider keeping your credit separate until your or your future spouse’s credit record improves.

Consider integrating employee and retirement benefits

If you and your future spouse have separate health insurance coverage, you’ll want to do a cost/benefit analysis of each plan to see if you should continue to keep your health coverage separate. If your future spouse’s health plan has a higher deductible and/or co-payment or fewer benefits than those offered by your plan, he or she may want to join your health plan instead. You’ll also want to compare the premium for one family plan against the cost of two single plans.

In addition, if both you and your future spouse participate in an employer-sponsored retirement plan, you should be aware of each plan’s characteristics. Plans may differ as to matching contributions, investment options, and loan provisions. Review each plan together carefully and determine which plan provides the better benefits. If you can afford to, you should each participate to the maximum in your own plan.

Assess your insurance coverage needs

While you might not have felt the need for life and disability insurance when you were single, once you are married you may find that you and your future spouse are financially dependent on each other. If you don’t have life or disability insurance, you will want to have policies in place in order to make sure that your future spouse’s financial needs will be taken care of if you should die prematurely or become disabled. If you already have life and disability insurance, you should reevaluate the adequacy of your existing coverage and be sure to update any beneficiary designations as well.

You should also take a look at your auto insurance coverage. Check your policy limits and consider pooling your auto insurance policies with one company (your insurance company may give you a discount if you insure more than one car with them). As for renters/homeowners insurance, you’ll want to make sure your personal property and possessions are adequately covered.

NOTE: If you and your future spouse obtain joint credit, both of you will become responsible for 100% of the debt.

dvanced Memory Research Institute of New Jersey (AMRI) was founded in 2013 by Dr. Sanjiv Sharma. Dr. Sharma is a board-certified geriatrician who specialized in memory loss. Got An Itch That Can’t Be Scratched?

Nina Paonessa, DO, FACOS As we enter the New Year, a common ailment affects many individuals. This is the time of year when people make New Year’s resolutions to eat “healthy” and diet in order to lose weight for spring break or in preparation for the summer. This is also the time of year for football season and tailgating parties and all of the foods that are associated with it. Because of these unique situations, many people suffer from aggravating symptoms of perianal itching, burning, and discomfort as well as a “wet” or moist feeling in the perianal area.

What is pruritus ani?

Pruritus ani is a Latin term literally meaning “itchy anus.” It is an uncomfortable sensation around the anus which produces the desire to scratch. Scratching causes irritation of the perianal skin, which in turn causes the desire to scratch more. The skin becomes more irritated and damaged, which causes the desire to scratch even more. This becomes a vicious cycle.

Pruritus ani affects 1% to 5% of the population. Although it affects both men and women, it is four times as common in men. This condition is most often encountered in patients in their 40s to 60s. Pruritus ani is treated medically. An accurate diagnosis is mandatory for proper care.

What causes pruritus ani?

The symptom of pruritus or itching is common to many anorectal conditions.

Disorders such as hemorrhoids, skin tags, fecal soilage and/or incontinence, anal fistulae, anal fissures and anal warts all may be associated with itching. This is termed secondary pruritus ani. However, pruritus ani may be independent of these conditions. This is called primary pruritus ani. The cause of the syndrome in these cases is usually from topical agents applied to the perianal skin and/or the pH of the stool. The pH of the stool is determined by foods which we eat.

In addition to inadequate hygiene, overzealous or aggressive hygiene using irritating soaps, fragrances and lotions may cause pruritus ani. Overzealous cleaning and the use of topical steroids can destroy natural skin barriers and cause trauma to the anal skin, making the problem worse. As such, attempting to keep the perianal skin too clean, is harmful and may worsen the problem. The use of baby/toilet wipes is a common culprit for many patients suffering from pruritus.

It is very important to note that there are many medical conditions that may cause perianal itching and are treated differently from basic, primary pruritus ani. There are infectious conditions such as bacterial skin infections, fungal infections, parasitic infections (pinworms or scabies) and viral infections (anal warts). Skin conditions such as psoriasis, seborrheic dermatitis, contact dermatitis, and lichen sclerosus may cause secondary perianal itching. Local cancers such as Bowen’s disease or Paget’s disease are also potential causes of pruritus ani.

How is pruritus ani diagnosed?

In order to accurately diagnose pruritus ani, your physician will obtain a complete history and physical examination. The timing and duration of the symptoms are important factors. Toileting rituals/behaviors, stool frequency and consistency, and hygiene/cleansing routines are noted in order to help make a diagnosis. Travel history, medical history and medications taken are also important. It is crucial to include any supplements taken, topical ointments/creams used and whether or not baby/toilet wipes are being used. A careful diet history is also taken in order to identify any possible contributing agents.

Your doctor will perform a physical examination. This will include a careful inspection of the perianal skin and a digital rectal examination. Common findings are irritated, swollen, reddened skin that may have linear ulcerations/cracks. Sometimes the perianal skin may be thickened. An examination of the anal canal via an anoscope will also be performed in order to ensure there is not any intra-anal pathology causing

the symptoms such as hemorrhoids, warts, a polyp or tumor. Considering the serious diseases that may cause the symptoms of pruritus ani, it is very important not to selfdiagnose, but rather to seek consultation with a physician. In certain situations, a small skin biopsy may need to be performed.

How is pruritus ani treated?

The first goal of treatment of pruritus ani is to make a correct diagnosis. That means determining if the condition is primary pruritus ani or secondary pruritus ani caused by a medical condition/disease. Once this has been accomplished and it is confirmed that the patient has primary pruritus ani, the goal of therapy is to restore clean, dry and intact perianal skin. This will help to alleviate the symptom of itching. The first step is to alter the consistency of the stool so as to avoid seepage of stool from the anus. This may be accomplished with diet modification and adding a fiber supplement.

The second step in treatment is to discontinue any offending agents such as cortisone ointments, hemorrhoid creams, lotions, perfumes, soaps and baby/toilet wipes.

Topical agents containing alcohol are very irritating to the perianal skin. Likewise, any topical agent that causes the perianal skin to remain moist should be avoided.

This includes Vaseline, witch hazel, A & D ointment, antibacterial ointments, etc.

Additionally, it is important to remember not to over-clean the perianal area. This causes further skin breakdown and irritation. Believe it or not, plain water is the best way to keep the perianal skin clean and healthy.

Finally, the last step in treatment is to modify the diet to avoid a change in pH. Foods related to pruritus ani include caffeine, colas, citrus, dairy, tomatoes, nuts and beer. As mentioned earlier, these foods are very common during this time of year while watching football or trying to lose weight.

Where can you find help?

Dr. Paonessa is very familiar with diagnosing and treating pruritus ani as well as all the other diseases/conditions that may be mistaken for pruritus ani. Since there are many serious diseases that may be mistaken for primary pruritus ani, it is very important to see a physician trained in treating this condition. It is incredible how a fairly simple ailment such as pruritus ani can alter a patient’s daily life and affect his/her quality of life. Don’t hesitate to pick up the phone and call for a consultation. Stop suffering and begin to live life again.

Christine Genovese, LPC Genovese Counseling Services, LLC Christine Genovese is a Licensed Professional Counselor with offices in Tuckerton and Somers Point. Raised in Absecon, she has combined her love of education and healing and started her private practice in 2018. After a successful career in education, Christine retired as a teacher and school counselor from the Galloway Township school system. During that time, Christine earned a Master’s Degree in Education from Marygrove University and a Master’s Degree in Counseling Psychology from Georgian Court University. Anna Brocco, PharmD, MBA Director of Operations and Clinical Research Advanced Memory Research Institute Anna Brocco grew up in Toms River, New Jersey and currently lives in Forked River with her husband and toddler son. She is a graduate of Rutgers, The State University of New Jersey, Ernest Mario School of Pharmacy with her doctorate in pharmacy and later, a master’s degree in business. Throughout pharmacy school and after graduation, Anna worked at Community Medical Center in the pharmacy department where she specialized in IV medication preparation.

Anna joined Advanced Memory Research Institute of NJ in 2014 as a clinical research pharmacist and then in 2015, she became the operations manager and then the director of operations and clinical research. Anna uses her clinical trial and business knowledge to bring more research opportunities to the center so that all those who wish to participate in research can have the opportunity.

Anna is also an adjunct clinical professor of pharmacy practice at Fairleigh Dickinson University, School of Pharmacy and Health Sciences. In this role, she educates future pharmacists about clinical research and about Alzheimer’s disease and memory loss.

Doctor Teresa Menadier recently joined Gastroenterologists of Ocean County after completing her Gastroenterology fellowship at the University of Virginia. She is a graduate of Tufts University. She obtained her medical degree from Rutgers-New Jersey Medical School in 2012 where she was elected President of the NJMS chapter of the Alpha Omega Alpha Honors Society.

Nina Braunwald: A Main Artery In The Surgical World Happy New Year! We all have been waiting for this moment with great anticipation. A chance for restored faith, renewed hope and new beginnings. Although many of you may not realize it and may even deny it, COVID-19 has made us quite resilient and resourceful.

As we begin the new year and the winter months ahead, it is important to remain motivated Facebook.com. and energized. February is National Heart Month and what better time to honor Dr. Nina Starr Braunwald, the first female to be certified by the American Board of Thoracic Surgery in 1963.

Nina Starr was born on March 2, 1928 to Morris and Mary Starr in Brooklyn, New York.

Nina attended public schools and had an early interest in working with her hands. Nina had an interest in art and medicine and began her post high school education at the Washington Square College of Arts and Sciences at New York University. Nina’s father was an influence in her life, sparking her interest in the medical field as he was a physician. Nina graduated with a Bachelor of Arts degree in 1949 and immediately transitioned into pursuing a medical degree from New York University’s College of Medicine. In 1952 Nina began an internship and residency at Bellevue Hospital in New York and would continue her work there as the first female surgeon on staff.

Nina Starr married Eugene Braunwald, who was also her classmate in college and in medical school.

Eugene specialized in the field of cardiology. Nina left her residency and completed a postdoctoral fellowship with a renowned cardiac surgeon from Georgetown University. Nina earned a Master’s degree in Surgery and decided to transfer to Georgetown to complete her residency requirements. During her residency, Nina took a great interest in the field of cardiac surgery. Nina embraced the challenges that came with the training and competition in the field of cardiac surgery. Nina is known to have received training from the chief of the Clinic of Surgery at the National Institutes of Health in Bethesda, Maryland (https://www.ncbi.nlm. nih.gov/pmc/articles/ PMC5408644). Nina Braunwald was promoted to Deputy Chief of the Clinic. In 1968 Nina and Eugene Braunwald relocated to San Diego, California, after Eugene received a job offer. Nina obtained a position as an Associate Professor of Surgery. Nina would later go on to establish the cardiac surgical program at the University of San Diego, California.

Dr. Nina Braunwald was not finished adding to her lists of “firsts.” After relocation back to the East Coast, Dr. Nina Braunwald became the first woman to join the surgical faculty at Harvard University in the late 1960s.

At the time Dr. Nina was entering into the medical field, let alone as a cardiac surgeon, women were not afforded the same level of professionalism as men. However, Dr. Nina Braunwald kept the focus of her research on the patient, not the politics. Although modest about her career, through extensive research as a cardiac surgeon and with her mentors, Dr. Nina Braunwald developed the “bioprosthesis.” This was a prosthetic valve used to replace the mitral valve in the heart. Dr. Nina Braunwald not only designed the bioprosthesis, she fabricated it too. The first surgery was performed on a 44-year-old female. The significance of the mitral valve is that it regulates the blood flow from your lungs into your left heart ventricle, which is the main pumping chamber of your heart. Dr. Nina Braunwald continued to advance the prosthetic from a cloth-covered mechanical valve known as the “Braunwald-Cutter valve” to the stented aortic homograft, which utilized human tissue to treat chronic thromboembolic disease.

Dr. Nina Braunwald was truly a female pioneer in the field of cardiac medicine. She was an incredibly talented cardiac thoracic surgeon who was also a researcher in a field that was dominated by men. Dr. Nina Braunwald had the ability to keep pace with “the boys” and continued to work through all of her pregnancies. It has been reported that she even returned to work just 10 days after delivering her children.

Nina and Eugene Braunwald had three daughters. Nina worked hard to maintain a work/life balance. It is said that she would be a part of the morning routines with her daughters and be home for dinner in the evenings and bedtime. Remember in the 1960s there was no internet or the luxury of working from home, so any additional work that needed to be done after hours, Nina would return to the office after her children were in bed. Nina was known to always put family first. Sadly, Dr. Nina Braunwald departed this life at the young age of 64 on August 5, 1992. Eugene Braunwald established a foundation and scholarships to support women in the field of cardiac surgery in his wife’s beautiful memory.

Dr. Nina Braunwald has shown us that women can have an incredibly successful career and a family too! She never gave up and continued to challenge herself in the best interest of others and in medicine. In the pursuit of your new year’s goals and in the best interest of your “heart” (emotionally and physically), I encourage everyone to laugh a little, to dance a lot and don’t forget to breathe!

Rita King, LCSW is a mental health clinician providing psychotherapy services to adults and children. Rita has worked with individuals with mental illness in the community mental health setting, criminal justice system and private practice setting. Rita is a “De-escalation for Individuals with Special Needs” training facilitator and has trained approximately 1,200 law enforcement professionals to date. Rita is also a CIT trained mental health professional. Rita has a strong passion for women’s history and in her free time she continues to raise awareness of the importance of women’s history through lectures and community speaking engagements.

We are grateful for the journey you have taken with SCAN ever since we transitioned our programming to an online platform, and we appreciate that you warmly welcomed SCAN into your home. SCAN Welcomes Reverend Ronald L. Sparks, the Pastor of Bethel AME Church in Freehold to the Board of Directors as Vice President.

Reverend Sparks will be joining Tracey Wolfman, President; Siobhan Connelly, Secretary; and Dana Montanelli, CPA, Treasurer. In addition to pastoring, Reverend Sparks is a member of the Freehold Clergy Association, works with Casa Freehold, an organization supporting immigration issues, and is a member of the NJ Black MultiFaith Alliance. He, along with other faith and community leaders was instrumental in reestablishing the Freehold chapter of the NAACP. Reverend Sparks believes it is important for all to understand our history. “Because if we understand our history, we will realize that we are not standing alone, but rather, we are standing on the shoulders of those who stood before us,” he said. We are very excited for Reverend Sparks to take on a bigger role at SCAN.
