THE CANNON CLUB APPLICATION FOR MEMBERSHIP
This application will not be acted upon unless fully completed, signed, and accompanied by payment of appropriate fees. It is the policy and practice of The Cannon Cub not to discriminate based on race, color, religion, gender, gender identity or expression, sexual orientation national origin, disability, or age.

Application for Membership: I (we) hereby apply for membership to The Cannon Club (hereinafter referred to as the “Club”) and agree to abide by its rules and bylaws as they exist now or are later amended. If approved for membership, I (we) agree to render timely payments of all dues, fees, and other such charges incurred or for which I (we) become obligated. I (we) authorize the disclosure and release of information to The Cannon Club by those persons and entities named herein for the purpose of determining my (our) eligibility for membership.
NOTE: Family Golf Membership includes a spouse and any children living under the household under age 19. Exceptions for this age group if the child is a full-time student pursuing higher education, the age limit would be extended until age 24. Children that age out may transfer to their own membership. The aged-out children are considered a legacy member, as such, they would not be subject to any initiation fee. All other fees and charges for the various categories apply.
initial_____2. Approval for Membership: I (we) understand that membership is contingent upon approval, which shall be at the Club’s sole discretion as exercised by the Membership Committee. I (we) understand that I (we) shall be notified by the Club of the status of the application within two weeks of the date it is received by the Club’s Membership Committee. If approved, I (we) understand that membership is granted for one full year and will be renewed automatically from year to year unless otherwise terminated. It is agreed that if this application is not acted upon favorably, all funds paid to or deposited with the Club, less any charges incurred, shall be refunded immediately.
initial_____3. Payment of Dues: I (we) understand that I (we) must pay the dues consistent with my (our) membership class for a period of twelve months.

Resignation: I (we) understand that resignations must be submitted in writing to membership@thecannongolfclub.com at least sixty (60) days before the end of the period in which the member seeks to resign. Any Cannon Club LLC property such as locker and or wine keys must be returned before your last day of membership. A fee of $25.00 per key will be added to your final statement if the key(s) are not returned. Such resignation shall be effective at the end of that term or at such other date as the resigning Member may designate. I (we) understand that resignations, which occur prior to the end of the required twelve (12) month period, will not nullify the commitment to pay the full dues to the satisfaction of this contract.
If a member wishes to change their membership type from “Golf” to “Social”, notification in writing at least sixty (60) days in advance must be submitted. The change in category will remain in effect for one year or a member can choose to pay the total monthly dues and differences for time missed.
initial_____5. Amounts Owed: I (we) understand that all dues, payments, and other charges incurred, are due and payable upon receipt of the billing statement from the Club. I (we) understand that a Member’s account becomes delinquent and past due on the 15th of the month. A Member who has been notified of delinquency and who fails to pay in full all amounts due to the Club within fifteen (15) days of the Notice of Delinquency may be suspended from membership by the Club at the expiration of the fifteen (15) day period.

Recurring ACH Payment Authorization
You authorize regularly scheduled charges to your checking/savings account. A receipt for each payment will be provided to you and the charge will appear on your bank statement as an “ACH Debit”. I understand that this authorization will remain in effect until I cancel it in writing, and I agree to notify The Cannon Club in writing of any changes in my account information or termination of this authorization at least 15 days prior to the next billing date. If the above noted payment dates fall on a weekend or holiday, I understand that the payments may be executed on the next business day. For ACH debits to my checking/savings account, I understand that because these are electronic transactions, these funds may be withdrawn from my account as soon as the above noted periodic transaction dates. In the case of an ACH Transaction being rejected for Non-Sufficient Funds (NSF) I understand that The Cannon Club may at its discretion attempt to process the charge again within 30 days and agree to an additional $25.00 charge for each attempt returned NSF which will be initiated as a separate transaction from the authorized recurring payment. I acknowledge that the origination of ACH transactions to my account must comply with the provisions of U.S. law. I certify that I am an authorized user of this bank account and will not dispute these scheduled transactions with my bank; so long as the transactions correspond to the terms indicated in this authorization form.
