• Margarita Persico Overweight children and teens are caught in a “tsunami of terrible things,” like cardiovascular complications, type 2 diabetes, and kidney failure, said Dr. Abhinash Srivatsa, a pediatric endocrinologist at Children’s Hospital Boston.

Such illnesses were once common only to adults, added Srivatsa, who also teaches at Harvard Medical School.

Type 2 diabetes — a condition in which the body either does not produce enough of the hormone insulin, needed to convert sugar into energy, or cannot properly use the insulin it does produce — is linked to obesity, physical inactivity and genetics in certain groups, like African Americans, Asian Americans, Hispanic Americans, American Indians and Alaskan Natives, according to data from the Centers for Disease Control and Prevention.

Childhood obesity has become one of the top 10 health concerns in the United States, according to the National Health and Nutrition Examination Surveys (NHANES). The survey shows a sharp increase in the rate of overweight children and adolescents over the past three decades.

In the first survey, conducted from 1976 to 1980, the rates ranged from 5 percent to 6.6 percent; in the last survey, stretching from 2003 to 2006, they reached from 17 percent to 17.6 percent.

Obesity also causes other complications, such as increased cholesterol, slow or non-healing wounds, liver inflammation, blindness, high blood pressure, heart attacks and strokes, said Srivatsa, who frequently sees patients with high blood pressure at Children’s Hospital Boston’s Optimal Weight for Life (OWL) program, a clinic for obese, overweight children and/or with type 2 diabetes.

At the OWL, almost one in four children has liver inflammation, also known as nonalcoholic steatohepatitis (NASH) and “fatty liver,” said Dr. David Ludwig, a Harvard pediatrician who directs the OWL program.

branch in Chelsea, recommends parents to take their children out every day to exercise and plan weekend activities, since the body uses insulin better by regular exercising.

“We recommend less than two hours a day of TV. We recommend getting the kids outside… for an hour every day,” emphasizes Nelson.

“If it’s cold out, you get your hat, you get your gloves, you get your jacket, but you get outside.”

But many doctors believe disease management and prevention are not enough to turn things around.

Srivatsa believes policies need to be put in place by the government to control what he calls “aggressive marketing.” He knows of several physicians, who are working as intermediaries with the legislature and other decision makers to change the system.

One of them is Ludwig, who Time Magazine called the “obesity warrior.”

“That’s really where the trend can be reversed — public health policies and educating children about healthy habits, giving them more time in school for physical activity because there’s so much pressure on performance and exams,” Srivatsa says.

He says physical activities are now restricted in school and replaced with academic material.

But the good news is type 2 diabetes can be prevented or delayed, according to the Centers for Disease Control. Pediatricians and health educators in the U.S. are trying to identify the problem’s root to catch the illness before it’s too late to reverse it.

Nelson and Srivatsa emphasize that adopting healthy habits can prevent complications that lead to diabetes. Among them are drinking more water instead of juices, sodas or sports drinks; avoiding fast food; and trading television for more outdoor activities.

“We focus on telling them to eat more natural foods and less man-made foods,” emphasizes Srivatsa.

“Anything that comes in a packet, like chips or processed or refined [foods]... [are] usually likely to be unhealthy. And [drinking] more water [helps] because … beverages with sugars still are one of the biggest culprits in this epidemic.”

Physicians and health care providers recommend keeping these kinds of foods out of the home. Ludwig uses the term “safe home environment,” which means people should not have unsafe foods in the house, where kids can consume them.

“Food also is a poison in some ways for kids who are already suffering from liver disease and who are on the borderline of getting diabetic,” says Srivatsa. “Too [many] calories [are] a poison.”

Want to Reduce Your Risk of Dying Prematurely?

Smoking and hypertension cause most premature deaths. Find out about the other factors you need to keep an eye on • PLoS Medicine Hundreds of thousands of deaths every year in the US could be prevented by tackling just a few risk factors, according to a study published earlier in the year in the journal PLoS Medicine.

Dr. Majid Ezzati of Harvard University and colleagues estimated the toll of poor diet, high blood pressure, cigarette smoking, sedentary lifestyle and other factors by determining how 2005 mortality data would change if each were eliminated.

They found, if no one in the US smoked, there would have been 467,000 fewer deaths from smoking-related causes, and if everyone had their blood pressure controlled at optimal levels, 395,000 fewer people would have died.

“Targeting a handful of risk factors has a large potential to reduce mortality in the US, substantially more than the currently estimated 18,000 deaths averted annually by providing universal health insurance,” Ezzati and his team say. However, they add, even though there are proven ways to help people quit smoking and reduce their blood pressure, “blood pressure and tobacco smoking declines in the US have stagnated or even reversed.”

Ezzati and his research colleagues identified a few dietary, metabolic and lifestyle factors also accounted for a substantial number of deaths in the US:

El tabaco y la hipertensiĂłn son las principales causas de mortalidad prematura en EE.UU. “Las patologías coronarias fueron consecuencia, en primer lugar, de la hipertensión (responsable del 45% de las muertes cardiovasculares), el sobrepeso-obesidad, el sedentarismo, el colesterol alto, el tabaco, el exceso de sal, ácidos grasos y el bajo consumo de omega-3”.

Por su parte, el tabaco produjo el 33% de los fallecimientos por cáncer durante el año 2005, aunque también influyó en las patologías respiratorias y la diabetes, en las que también tuvieron un papel importante otros factores, como el alcohol, el azúcar en sangre, el sedentarismo, el sobrepeso y el bajo consumo de frutas y verduras.

Entre los factores relacionados con la dieta, la alta ingesta de sal tuvo un gran efecto, ya que ocasionó el 4% de las muertes. Respecto al

consumo de alcohol en exceso, éste fue la causa de 90,000 muertes, producto de enfermedades cardio-vasculares, accidentes de tráfico y violencia.

Además, este factor se presentó de forma diferente en mujeres y en hombres. “El 70% de los fallecimientos atribuibles al alcohol ocurrieron en hombres”.

Otra de las notables diferencias entre sexos consiste en que la hipertensión fue la mayor causa de mortalidad prematura en ellas (19%), mientras que fumar se posicionó como el primer factor de riesgo de muerte entre ellos, según los resultados de la investigación.

A diferencia de estudios previos, en los que se confirmaba que factores de riesgo modificables son responsables de un gran número de muertes, Ezzati incluye nuevos hábitos relacionados con la dieta y factores metabólicos (colesterol, tensión arterial, obesidad...) y realiza comparaciones.

With Help, You Can Quit Smoking Massachusetts’ model tobacco cessation benefit spurs astounding drop in smoking rates A “model” tobacco cessation benefit offered to Massachusetts’ Medicaid participants has produced an unprecedented 26% drop in smoking rates in only two and a half years, and has already been linked to decreases in heart attacks, hospitalizations for asthma and COPD, and a significant decrease in birth complications.

Researchers from the Massachusetts Tobacco Cessation and Prevention Program (MTCP) found that up to 38% fewer MassHealth cessation benefit users were hospitalized for heart attacks in the first year after using the benefit, and that 18% less benefit users visited the emergency room for asthma symptoms in the first year after using the benefit. Researchers also found that there were 12% fewer claims for adverse maternal birth complications since the benefit was implemented.

The Massachusetts Executive Office of Health and Human Services said more than 75,000 people — a full 40% of MassHealth members who smoke — have used the benefit to try to quit smoking. Cost savings are being studied, and all indications suggest they will be significant.

and a national coalition of business, labor, health care professionals and nonprofits have endorsed a nation wide call for action (www.ACTTION toquit.org) to increase access to tobacco cessation benefits for smokers who want to quit. The head of Partnership for Prevention, the disease prevention advocacy organization in Washington, D.C., that organized the national effort, said the Massachusetts experience makes a strong case for expanding cessation benefits.

MTCP and MassHealth worked together to design a model benefit that includes all FDA-approved medications to quit smoking, behavioral counseling, and features very low co-pays to reduce barriers to access. Beginning in July 2006, MassHealth began providing coverage of smoking cessation as part of the state’s healthcare reform initia-

tive. MTCP promoted the new benefit through radio and transit ads and extensive community outreach. The benefit was introduced into an environment that encourages quitting smoking: Massachusetts has smoke-free workplaces, high cigarette taxes, and a non-smoking social norm, all of which contribute to smokers wanting to quit.

Smoking remains the number one preventable cause of illness and death in the Commonwealth and in the United States. More than 8,000 Massachusetts residents die annually from the effects of smoking, and tobacco use is associated with $4.3 billion in excess health care costs in Massachusetts each year.

In Massachusetts, 77% of adult cigarette smokers want to quit, 60% of smokers have tried to quit within the past year, and 44% report that they plan to quit in the next 30 days.

Con Ayuda se Puede Dejar de Fumar Programa para dejar de fumar de MassHealth ocasiona un drámatico descenso en ataques cardíacos La administración del gobernador Deval Patrick anunció recientemente que el nuevo beneficio de MassHealth para ayudar a fumadores a dejar el cigarrillo está dando resultado. En tan sólo un año, los usuarios del beneficio para dejar de fumar han experimentado un importante descenso en el número de hospitalizaciones debido a problemas de corazón, un declive en las visitas a salas de urgencias y clínicas ambulatorias por motivos de asma.

En los primeros dos años y medio de la existencia del beneficio, más de 75,000 miembros de MassHealth han intentado dejar de fumar. Este número representa un 40% de los miembros de MassHealth que son fumadores, un nivel sin precedentes en el país.

“Estos hallazgos son de importancia nacional, especialmente en vista de las decisiones que se tomarán durante la reforma del sistema de salud”, dijo el

New Mammogram Advice Raises Questions, Concerns For many women, getting a mammogram is already one of life’s more stressful experiences.

Now, women in their 40s have the added anxiety of trying to figure out if they should even be getting one at all.

A government task force said in midNovember that most women don’t need mammograms in their 40s and should get one every two years starting at 50 — a stunning reversal and a break with the American Cancer Society’s long-standing position. What’s more, the panel said breast self-exams do no good, and women shouldn’t be taught to do them.

The news seemed destined to leave many deeply confused about whose advice to follow.

“I’ve never had a scare, but isn’t it better to be safe than sorry?” asked Beth Rosenthal, 41. “I’ve heard of a lot

of women in their 40s, and even 30s, who’ve gotten breast cancer. It just doesn’t seem right to wait until 50.”

For most of the past two decades, the American Cancer Society has been recommending annual mammograms beginning at 40, and it reiterated that position. “This is one screening test I recommend unequivocally, and would recommend to any woman 40 and over,” said the society’s chief medical officer, Dr. Otis Brawley.

But the U.S. Preventive Services Task Force, a government panel of doctors and scientists, concluded that such early and frequent screenings often lead to false alarms and unneeded biopsies, without substantially improving women’s odds of survival.

Breast cancer survivors who were diagnosed at a young age were among the more vocal critics of the new guidelines.

“This sure seems like a big step backwards to me,” said Debbie Hayes, who was diagnosed with Stage 2 breast cancer at age 33 after finding a lump during a self-exam. “People are being diagnosed even in their early 20s,” said Hayes.

Most women in their 40s interviewed for this article said they planned to stick with the old mammography

recommendations, at least for now.

“I have two young children,” said Amber Smart, a 47-year-old mother in Agoura Hills, Calif. “There’s a lot of years left that they need me.”

But there were those who saw the new guidelines as potentially a relief, a development that could save women from endless stress, false positives and perhaps needless procedures. “I can’t tell you how many friends I have who’ve gone through severe worries from false scares,” said Maren Waxenberg, a Manhattan mother.

The new guidelines are for the general population, not those at high risk of breast cancer because of family history or gene mutations.

Y las cuarentonas tienen ahora razones adicionales para sentirse nerviosas ante las contradicciones surgidas entre los médicos en torno a la conveniencia de someterse a esos exámenes antes de los 50 años.

Un grupo de expertos contratados por el gobierno recomendó a mediados de noviembre que las mujeres comiencen a hacerse mamografías a los 50 años, no a los 40, como era la norma, y que se los hagan cada dos años, no anualmente.

Esto contradice la que ha sido la postura tradicional de American Cancer Society.

En otra conclusión sorprendente, los expertos dijeron que los exámenes que se hacen las mismas mujeres no son confiables y que no se las debería alentar a que continúen con esa práctica.

El informe generó confusión. “¿No es mejor prevenir que curar?”,

preguntó Beth Rosenthal, de 41 años.
