Dear Jordan Family and Friends, In one way or another we all have a testimony about benefitting from the quality care that the Jordan Health Center has brought to so many of us throughout the Rochester area. I am fortunate enough to share my own personal story.

All that separated me and my family from quality health care services was a “shortcut.” This shortcut between Atkinson Street and Adams Street was the route I took to see how many people waited in line on the porch of the home of our neighborhood physician, Dr. Anthony L. Jordan. I would hurry back home through the same shortcut to tell my mother the number of others waiting in line to see him that day. You see, my family lived on Atkinson Street in the Third Ward during the 1960s through the 1970s. As the eldest of my three other sisters— “Sarah Jones’ daughters”—as we were known back then, there was nothing we didn’t share simultaneously. From the measles, mumps, and chicken pox and whatever else was going around our neighborhood, made us regular patients of Dr. Jordan’s. So overtime, this shortcut was our worn path to healthcare. Dr. Jordan was the first doctor that we knew and who treated us like family. He not only looked like us in our community as an African American male, he was a true role model who personified kindness, respect, and he taught us a lesson or two in professionalism and overall health and wellness. Dr. Jordan provided quality, affordable healthcare for any ailment we had, regardless of our ability to pay. And for his dedicated leadership and sacrifices and for so many other reasons he will forever be our hometown hero.

Tonight’s tribute and honor should be both heartfelt and hopeful because we can all acknowledge that Dr. Jordan’s reach extended well beyond the length of his stethoscope. This is truly a heartfelt celebration as a community of neighbors, medical personnel, nurses and other former employees and of course, our incredible patients of Jordan Health Center. Thanks to all of you! We remain hopeful because of your continued support for the mission of the Jordan Health Center which is to “provide community-based comprehensive healthcare of the highest quality with dignity and respect to all regardless of ability to pay.” Thanks to each of you!

Tonight, I am honored to celebrate the Jordan Health Center’s major milestone – 50 years of supporting the needs of thousands of people in our community through hard work, dedication and a passion for helping others. Here’s to fifty more years!

As one of the first five Federally-Qualified Health Centers in the Nation, Jordan holds a special place in American history, and Jordan’s place in Rochester history cannot be overstated. The individuals who founded Jordan made a commitment to this community that has stood the test of time.

As you probably know, I worked at Jordan during the early years as the Community Coordinator of the People's Health Council. My mother, the late Lena Gantt, was one of the founding Board members. It is gratifying to see that the sacrifices that she, Dr. Anthony Jordan, Father Laurence Tracy, Mildred Johnson, Dr. Kenneth Woodward, William Hall, Jorge Colon, Edna Craven and others made were not in vain.

What began as the Baden Street Dispensary over 100 years ago became the Anthony Jordan Health Center. What began as individual neighborhood health centers that were affordable and accessible to all has blossomed into the Anthony Jordan Health Corporation, a system of comprehensive health centers, primary care offices and an Urgent Care Center serving over 30,000 patients in Rochester and Canandaigua.

While Jordan has faced many challenges over the years, you have always been able to overcome them, and Jordan has never wavered from the original commitment that was made to this community 50 years ago. It is for this reason that Jordan continues to thrive today.

I reminisce about the last 50 years with great pride, and I am sure there will be another celebration 50 years from now. Congratulations on reaching this milestone. The best is yet to come.

Jordan Health From the President and CEO September 15, 2018 Dear Friends: It has been your support of neighborhood-based health care that has sustained us over these many years. As community, you came together as organizations, grassroots leaders and elected officials to say that the City of Rochester needed a medical and social safety net and fought to make that vision a reality. You never turned your backs on the most vulnerable residents of this community and you never gave up on the center that would later become the Anthony L. Jordan Health Corporation (Jordan Health). Thank you for your amazing dedication and generous spirit.

Our hearts and strength flow from the vision and values of Dr. Anthony Jordan and Dr. Kenneth Woodward. A vision that has endured and grown to service the health needs of the Greater Rochester community without regard to a person's financial or social status. Although our birthplace was humble, from our beginnings over a 100 years ago in a northeast settlement house, the "yellow submarine", born in the basement of the Baden Street Settlement's Hanover Houses, became one of the first of five Federally Qualified Health Centers (FQHCs) grantees in the country in 1968. Our extended family of care providers, support staff and collaborating partners, has served thousands of active patients, accounting for hundreds of thousands health care visits over the years. In this, our 50th year, we are proud to stand with 1200 FQHCs nationally, serving 27 million patients a year.

Jordan Health's essence has always been "Centered Around Community". Our healthcare champions, Ors. Jordan and Woodward believed in a community where access to high-quality health care would be a right for every resident, that, healthcare should be centered around community. That was true yesterday, is true today and will be true tomorrow. Their legacy will always drive our mission: "To provide community-based comprehensive healthcare of the highest quality to all regardless of ability to pay".

On our golden anniversary, we want to take the opportunity to say thank you. Thank you to our patients, employees, clinicians, former and present Board of Directors, community-based organizations and community partners for fifty years of support and encouragement. Thank you, Baden Street Settlement and Action for Better Community and CON EA for your early advocacy and commitment. Thank you to the people, too many to mention, who were the boots on the ground and helped pave our way, such as the late Mrs. Lena Gantt, the late Great Mrs. Stubbs (Ellen), Timothy Downs, Delaine Cook-Green, Dr. Walter Cooper, Dr. Evan Charney and Dr. Bridgette Wiefling. AWARDS ELLEN VOLPE JORDAN HEALTH CHAMPION AWARD THE HONORABLE DAVID F. GANTT The Honorable David F. Gantt, the representative of the 137th Assembly District in the New York State Legislature, has spent his life fighting against the inequalities that exist in the city of his youth and in the State of New York. His sense of community was instilled in him by the extraordinary example set by his mother, the late Lena Gantt, as a community advocate and an active member of the Anthony L. Jordan Health Board of Directors. He was also influenced by Minister Franklin E. Florence, Sr. and Raymond Scott of F.I.G.H.T., and the late James McCuller of Action for a Better Community. Their efforts gave him the unique understanding of why it has been so important to sustain this neighborhood safety net over these last fifty years. Mr. Gantt spent the early part of his career working as a youth counselor for the City of Rochester, then as an administrator for the Jordan Center. He was then elected to the Monroe County Legislature in 1973 before being elected to the New York State Assembly in 1982. Today, fifty years later, he continues to fight for the viability and growth of Jordan Health.

We are honored to salute Mr. David F. Gantt for his unwavering commitment to the health of our community, with the 2018 Ellen Volpe Jordan Health Champion Award â&#x20AC;&#x153;For 50 years, Jordan Health has provided quality medical care for the people of Rochester, primarily for the less fortunate in our community. Jordan Health Champion Award was established for 2011. This unique award honored the community service by an individual promoting and advocating for quality neighborhood accessible health care. In 2017, it was renamed in honor of the late Ellen Volpe, Nurse Practitioner, to highlight current and former Jordan Health employees who have made an extraordinary impact in the lives of our patients and community

AWARDS COMMUNITY PARTNER AWARD UNIVERSITY OF ROCHESTER MEDICAL CENTER, STRONG MEMORIAL HOSPITAL In 1968, in the basement of Baden Street Settlements’ Hanover Houses, The University of Rochester School Of Medicine became one of the first medical schools in the nation to support a community health center. The Rochester Neighborhood Health Center referred to as the “Yellow Submarine” and later renamed the Anthony L. Jordan Health Center was the beginning of a fifty year community partnership between Jordan Health and the University of Rochester Medical Center (URMC), Strong Memorial Hospital. Over the years our partnership has expanded to include the Rochester Integrated Health Network, formed together with Rochester Regional Health and Regional Primary Care Network, to promote the integration and coordination of accessible and affordable medical and behavioral health care services. Jordan has been able to enhance health care to our patients with Strong Labs, Highland Pharmacy and Highland Breast Imaging, which in 2013 brought mammography into our neighborhood. Many of our cardiology and endocrinology patients are able to receive their specialty care within our center. We are honored to salute URMC, Strong Memorial Hospital for their fifty years of commitment and dedication to the mission of Jordan Health, with our 2018 Community Partner Award.

Dr. Buddhi Shrestha has been an advocate and leader for the underserved for over fifty years. As a dental resident at the University of Rochester’s Eastman Institute of Oral Health, he provided for the dental needs of children at the Rochester Neighborhood Health Center, later renamed the Anthony L. Jordan Center. In the early 1970’s, Dr. Shrestha began working as a pediatric dentist for the Jordan Health center, making the conscious decision to keep his talents in his adopted city of Rochester. He continued his work in statewide efforts of the New York State Oral Health Technical Assistance Center at the Regional Primary Care Network. In the early 2000’s, as a member of the Board of Directors, he worked with then medical director Dr. Bridgette Wiefling and was instrumental in preventing the closure of the Anthony L. Jordan Health Center. He remains committed to the mission of Jordan Health, now serving in his eighteenth year as a member of the Corporate Board of Directors, as Past Board Chair and currently as the Chair of the Health Services and Quality Improvement Committee. Dr. Shrestha has been recognized for his efforts in making dental health services available to needy populations through numerous awards, including the Kenneth Woodward Award (1999); Herman Biggs Memorial Award (2002); Elmer Green Award (2010); Anthony L. Jordan Health Center’s Community Champion Award (2011); Myron Allukian National Lifetime Achievement Award (2016) and East Institute for Oral Health Director’s Award (2017). He continues to be committed to oral health and improving access to dental care for children particularly in Rochester’s underserved populations. We are honored to salute Dr. Buddhi Shrestha with our 2018 President’s Award.

YESTER DAY The rich legacy of Jordan Health was born out of a demand from the Rochester community to provide quality neighborhood-based healthcare that was affordable and accessible to all.

The organization is named in honor of Anthony L. Jordan, M.D. A primary care physician who served patients out of his Adam Street home, Dr. Jordan accepted barter for his services from patients who insisted on payment, and made house calls to provide the vital healthcare services he felt each person deserved.

Kenneth Woodward, M.D., also a staunch advocate for healthcare accessibility, helped secure federally subsidized status for a comprehensive health center, originally named the Rochester Neighborhood Health Center. He played an integral role in building the Anthony L. Jordan Health Center and affiliated facilities.

Overall, it was the shared actions of dedicated citizens, in partnership with vital community-based organizations, that led to the centerâ&#x20AC;&#x2122;s establishment and ongoing growth.

TO DAY Jordan Health continues to serve generations unbounded by race, ethnicity or geography. Our dedicated healthcare and human services professionals collaborate in a network of 10 primary care offices and health centers located across Greater Rochester and in Canandaigua. With approximately 30,000 patients, we are uniquely positioned to help a population challenged by limited resources and complex health conditions often associated with being economically disadvantaged and medically underserved.

Our patient-centered approach incorporates teams of clinicians, clinical support staff, behavioral health staff, care managers, clinical pharmacists, medical records managers and financial counsellors to provide an elevated level of well-rounded quality care.
