Food First Approach in Long Term Care Jessica MacDonald NM Season Care Consultant Season Care Dietitian Network What is the Food First Approach? At Seasons Care, we believe in a Food First Approach. Residents who struggle with food intake may not be getting enough nutrients, therefore, nutrition supplements are often used as a means to combat the risk of malnutrition. The Food First philosophy is based on our firm belief that foods are more enjoyable and satisfying than supplementation, and all measures should be taken to adapt the normal diet to fit the Resident’s individual needs, such as providing foods that are high in protein and nutrient dense. The goal is to have Residents consume something before they leave the dining room. What is the Food First Approach? 1. A Resident may require additional nutrition due to a wound, weight loss, malnutrition, poor intake, behaviors, etc. 2. The Registered Dietitian (RD) will provide nutrient dense items using High Energy High Protein (HEHP) Intervention such as High Protein Milk, Super Cereal, High Energy High Protein Pudding, or items and snacks based on the Residents’ preferences. 3. If these are not effective on their own, or if the Resident refuses, the RD will then add in a nutritional supplement to enhance the Food First Approach.

What are the benefits and drawbacks to the Food First Approach? Like all approaches, there will be benefits and drawbacks to everything. Benefits: • Food is one of life’s primal and most innocent delights, both satisfying and eternal • Food uses all 5 senses, supplementation does NOT • It connects with all; everyone has to eat A drawback of this approach would be that food and fluid intake in Long Term Care (LTC) can often be challenging due to common medical conditions, such as dementia, Parkinson’s disease and stroke, that may affect one’s ability or desire to eat. How can we promote the Food First Approach? Promoting the Food First Approach may be difficult in your Home. You need to look at all aspects of your meal and snack service and how everything is presented to your Residents. Everything from optimizing the core menu, individual Resident preferences, snacking between meals, your approach, always having options, and your multidisciplinary approach, are all very important. See below for things that we can do to promote this approach: 2 choice at meal

How can we promote the Food First Approach over the Holidays? Here are just a few examples: HEHP Hot Chocolate with Whipped Cream & Cinnamon To a hot cup of hot chocolate, add in a scoop of your protein powder and top it with whipped cream & cinnamon. HEHP Eggnog Pudding Using a simple instant vanilla pudding recipe, sub eggnog for the regular milk you would use. Incorporate food in activities that your Home hosts over the holidays Examples of this could be Holiday baking and decorating, offering snacks and refreshments at holiday wreath making, stocking stuffing, Christmas tree decorating, holiday dance and carol singing. Jazz up the snack cart for the holidays. If you are using a special cart for snack service, Residents may find it more intriguing and exciting to get a snack. Colour and choices can go a long way in promoting Food First! Food is to be enjoyed by everyone. It is a time for Residents to socialize and share with fellow Residents, staff, and family. If we work together and offer Food First in a more positive way, we can watch our Resident’s flourish!

Making Every Momen Chronicles of a Butter (Part 2) The Butterfly Approach is designed for dementia care settings and focuses on delivering emotion-based, person centered, care that connects people in a dignified, holistic and human way. The Approach is proprietary to Meaningful Care Matters (MCM), an international company supporting culture change within health and care settings. The Approach appealed to the Responsive Group because of its ability to bridge the emotional isolation captured in its 2017 Empathy video Could a greater miracle take place than for us to look through each other's eyes for an instant. Our “traditional nursing stations” were transformed in collaborative space. All leading up to Accreditation Day for each home ~ with successful completion of Meaningful Care Matters’ 100-Point Checklist and of up to six hours of observational audits (using QUIS - Quality of Interactions Schedule - methodology) that score how team members engage with residents and attest to quality of life in the home.

Accreditation becomes the starting point for a new normal of emotionbased, person-centered care where team members bring their full selves to work. PREPARATION 1. Steam broccoli and carrots until just tender; drain well. 2. Saute onion in margarine until tender, add garlic; stir to heat through. 3. Add flour to garlic/onion mixture, then stir with whisk until flour is mixed in. Continue cooking stirring often for about 5 minutes to create a roux. 4. Combine water, milk and base then gradually add to roux, stirring with whisk until incorporated; cook stirring often, until thickened. 5. Add basil, parsley, pollock, crab, mushrooms, carrots and broccoli to sauce. 6. Cook fettuccine as per package instructions. 7. Gently fold pasta into sauce/vegetable mixture; scale into baking pans then sprinkle with parmesan cheese. Bake in oven for 30-40 minutes
