EDITORIAL EDITOR’S LETTER OM análise das cefaléias realizada pelo Dr. Pietro Peres joga luzes sobre a neurofisiopatologia da enxaqueca. O autor relaciona essa patologia com estados fisiológicos da pessoa, como ciclos menstruais, apneia, depressão e outros, destacando-se a perfomance do sistema mesolímbico e do hipotálamo nesse contexto. Conclui que a melatonina e os aminoácidos excercem influências positivas na comunicação neural e, por consequência, melhoras no estado enxaquecoso.

Destaca, ainda, a importância de um estudo mais minudente da temática, porque se sabe pouco a esse respeito.

Os biofármacos, - medicamentos biológicos ou imunobiológicos -, estão ganhando espaço no contexto da saúde. Diferentemente dos medicamentos de origem tradicional – química -, os biofármacos são derivados de material vivo, seja planta, animais ou micro-organismos. Por isso, eles detêm grande peso molecular e alta complexidade estrutural, além de serem testados cinco vezes ou mais que os medicamentos tradicionais. Esses remédios biológicos são desenvolvidos para atingir moléculas específicas do sistema imunológico do paciente, alcançando-se rápida recuperação.

O pós doutor Antonio Carlos Lopes destaca a importância da medicina na qualidade de vida e bem

estar humano. Ilustra essa realidade se referindo ao cuidado que o médico deve ter com o paciente, a existência de hospitais devidamente equipados.

Defendeu, ainda, a necessária qualificação do ensino médico ministrado no Brasil. Enfim, para ele é preciso humanizar a medicina.

Testes utilizando o sangue da mãe, por intermédio do sequenciamento dos gens, são capazes de detectar, com precisão, a síndrome de Down no feto. Aliás, com a análise do genoma dos futuros pais é possível detectar muitas doenças que poderão surgir no descendente e, assim, preveni-las. Outro aspecto importante é educar a população e os profissionais de saúde acerca dessa realidade para que os avanços científicos possam produzir efeitos.

A esclerose múltipla, entidade nosológica que compromete o sistema nervoso central, terá cura, em breve, segundo o Dr. Gianvito Martino. O procedimento consiste em injetar células neurais estaminais no sistema nervoso central. Em seguida, elas alcançarão as áreas lesadas, fazendo com que essas áreas produzam a mielina, restaurando-se a comunicação entre as células nervosas, melhorando o funcionamento cerebral.

Segundo o Ministério da Saúde brasileiro, a educação universitária púbica recebeu recursos no valor de 117 milhões de reais. Esse valor foi

repassado aos 34 hospitais universitários que fazem parte do Programa Nacional de Reestruturação dessas entidades.

Outras abordagens estão veiculadas nesta edição, a exemplo de dermatologia, educação física, hematologia, nutrição, oftalmologia, oncologia e psicologia.

Also highlights the importance of further study of that important issue, because little is known about it.

The biopharmaceuticals - biologics or immunebiological - are gaining ground in the health context.

Unlike the traditional medicines of origin - chemistry - the biopharmaceuticals are derived from living matter, whether plant, animal or micro-organisms.

Therefore, they hold large molecular weight and high structural complexity as well as being tested five times or more than the traditional medicines.

These biological drugs are developed to meet specific molecules from the patient’s immune system, achieving rapid recovery.

well being. Illustrates this reality referring to the medical care they should provide to patient, the existence of well-equipped hospitals. Defended also the necessary qualification of medical education taught in Brazil. Finally, for him it is necessary to humanize medicine.

Com a descrição do professor Aristides Leão, um brasileiro de destaque na área da fisiopatologia das cefaleias, que relatou a depressão alastrante cortical, ou depressão alastrante de Leão, pôde-se relacionar o fenômeno clínico da aura da enxaqueca com tal mecanismo, que, mais recentemente, pôde ser observado em estudos de neuroimagem funcional no ser humano.

Por intermédio das ferramentas de neurofisiologia, sedimentou-se a importância da hiperexcitabilidade cortical, que explica a fotofobia, fonofobia, osmofobia presentes na enxaqueca. Dando um passo adiante, a teoria de que a irritabilidade do humor tão presente nos períodos críticos e intercríticos da enxaqueca seria também resultado de uma hiperexcitabilidade cortical mesolímbica. Com a avalanche de descobertas de mutações e polimorfismos relacionados à enxaqueca, o componente genético contribuiu para o entendimento da hiperexcitabilidade cortical e da aura, abrindo-se a perspectiva de ser a enxaqueca uma doença de canais iônicos, assim chamada canalopatia.

No entanto, outros sistemas também são sugeridos como importantes na fisiopatologia da enxaqueca por meio dos estudos genéticos. Stephen Peroutka levantou o importante papel da dopamina na enxaqueca, estrada que vem sendo explorada cada vez mais em estudos na área das cefaleias. O sistema adrenérgico também vem sendo estudado por alguns grupos de pesquisa.

Nos últimos anos, o processo de sensitização central que foi introduzido na área das cefaleias por Rami Burstein vem sendo considerado de extrema relevância, elucidando o fenômeno de alodínia, presente na crise de enxaqueca. É tido como fator importante de falha na resposta terapêutica aos

With the description of Mr. Aristides Leão, a leading Brazilian in the area of the pathophysiology of headache, reported that cortical spreading depression, or spreading depression of Leão, one could relate the clinical phenomenon of migraine aura with such a mechanism, which, more recently, was observed in functional neuroimaging studies in humans.

Through the tools of neurophysiology, cemented the importance of cortical hyperexcitability, which explains photophobia, phonophobia, osmofobia present in migraine. Avec l’avalanche de découvertes de mutations et de polymorphismes liés à la migraine, la composante génétique a contribué à la compréhension de l’hyperexcitabilité corticale et de l’aura, ouvrant la perspective d’être la migraine une maladie des canaux ioniques, dite canalopathie.

Cependant, d’autres systèmes sont également suggérés comme importants dans la physiopathologie de la migraine par moyen des études génétiques.

Stephen Peroutka a mis en évidence l’important rôle de la dopamine dans la migraine, une route qui est explorée de plus en plus en des études dans le domaine des céphalées. Le système adrénergique est aussi étudié par certains groupes de recherche.

Dans les dernières années, le processus de sensitivité centrale qui a été introduit dans le domaine des chéphalées par Rami Burstein est considéré d’extreme rélévance, élucidant le phénomène de l’allodynie, présent dans la crise de migraine. Já no campo teórico, a formulação idealizada por William Young, concebendo o conceito dos módulos em cefaleias, foi de grande avanço para o seu entendimento. Publicado em 2001, a teoria modular das cefaleias materializa aquela ideia que já habitava os conceitos de muitos que trabalham com dor de cabeça, explicando com muito mais sentido as transições e atipias que ocorrem nas cefaleias primárias, como, por exemplo, as síndromes clustertic e cphtic, ou a cefaleia em salvas com aura, e a enxaqueca com fenômenos autonômicos.

Outros pontos que não podem ser esquecidos são o possível papel da substância periaquedutal cinzenta e do óxido nítrico nos mecanismos causais das cefaleias. Nosso grupo de pesquisa vem explorando alguns aspectos da fisiopatologia da enxaqueca. Em 2001 levantamos o possível envolvimento do hipotálamo na enxaqueca crônica, o que despertou secundariamente interesse pelo fascinante papel da melatonina como mecanismo fisiopatológico nas cefaleias. A importância dos aminoácidos excitatórios e inibitórios, em especial o glutamato, é outro desmembramento da linha de pesquisa em fisiopatologia. Detalharemos estes mecanismos a seguir.

MELATONIN AND HEADACHES The interaction between melatonin and headache is broad and complex. Some headaches have clear circadian rhythmicity as Hypnic headache and cluster headache, with other circannual variation in cluster headache and migraine cyclical, and, finally, the

D’autres points qui ne peuvent pas être oubliés sont le rôle possible de la substance périaqueducale grise et de l’oxyde nitrique dans les mécanismes causaux des céphalées. Notre groupe de recherche explore certains aspects de la physiopathologie de la migraine. En 2001, nous avons mis en évidence la possible implication de l’hypothalamus dans la migraine chronique, ce qui a suscité secondairement l’intérêt pour le rôle fascinant de la mélatonine comme un mécanisme physiopathologique dans les céphalées. L’importance des aminoacides excitateurs et inhibiteurs, en particulier le glutamate, est un autre démembrement de la ligne de recherche en physiopathologie. Nous allons détailler ces mécanismes ci-dessous.

LA MÉLATONINE ET LES CÉPHALÉES L’interaction entre la mélatonine et les céphalées est large et complexe. Certains céphalées présentent une claire rythmicité circadienne comme la céphalée Hypnic et la céphalée en salves, d’autres avec une variation circannuel, la céphalée en salves et la migraine cyclique et, finalement, la migraine menstruel avec une rythmicité mensuelle.

MELATONINA E AS CEFALEIAS A interação entre melatonina e cefaleias é ampla e complexa. Algumas cefaleias apresentam nítida ritmicidade circadiana como a cefaleia hípnica e a cefaleia em salvas, outras com variação circanual, a cefaleia em salvas e a enxaqueca cíclica, e, por último, a enxaqueca menstrual com ritmicidade mensal.

Muitos efeitos biológicos da melatonina a caracterizam como uma potencial candidata a fisiopatologia e tratamento da enxaqueca. Seus efeitos são de potencializar o GABA, inibir o glutamato, varrer óxido nítrico, modular ação da serotonina, dopamina e analgesia opioide, agir como anti-inflamatório, além de ter estrutura molecular semelhante a indometacina, molécula de muito interesse na área das cefaléias. Recentemente, mostramos que a melatonina 3 mg foi eficaz na prevenção da enxaqueca. Na enxaqueca, níveis diminuídos de melatonina e alteração na sua curva de secreção foram detectados.

Clinicamente, crises podem ocorrer à noite, mudanças de ritmo de sono desencadeiam crises de enxaqueca. Pacientes com enxaqueca dormem menos, têm latência de sono maior e mais despertares noturnos. Estudo em duzentos pacientes com enxaqueca episódica e crônica revelou que pacientes que mudaram seu horário de sono, ou relataram viagens cruzando fusos horários, relataram piora das crises.

Foi relatada em pacientes com enxaqueca crônica alteração dos níveis de melatonina com avanço do seu pico, níveis menores em insônia, apontando para uma disfunção cronobiológica.

Na cefaléia em salvas, há importância da melatonina e ritmicidade bem estabelecida. Um estudo duplocego controlado placebo mostra que a melatonina é superior ao placebo em cefaleia em salvas episódica e crônica. Os níveis de melatonina encontram-se diminuídos em pacientes com cefaleia em salvas. A relação entre cefaleia em salvas e aumento de temperatura é provavelmente mediada pela alteração da secreção de melatonina.

GLUTAMATO, GABA, COMORBIDADES, NEUROMODULADORES O sistema nervoso central sofre grande influência de aminoácidos. Os aminoácidos são divididos em inibitórios e excitatórios em relação à sua função cerebral. O maior neurotransmissor excitatório no cérebro é o glutamato e o de maior importância inibitória é o ácido gama-aminobutírico (GABA). Ambos ativam receptores ionotrópicos (GABA-A, ligantes a canais iônicos) e metabotrópicos (GABA-B, G protein-coupled) e são geralmente associados a comunicação neural.

Many biological effects of melatonin as a potential candidate characterize the pathophysiology and treatment of migraine. Study in two hundred patients with episodic migraine and chronic revealed that patients, who switched their sleep schedule or reported traveling across time zones, reported worsening crises.

It was reported in patients with chronic migraine changing levels of melatonin with advancing its peak, lower levels in insomnia, pointing to a chronobiological dysfunction.

In cluster headache, there is the importance of melatonin rhythmicity and well established. A dudplocego placebo controlled study shows that melatonin is superior to placebo in episodic cluster and chronic headache. Melatonin levels are decreased in patients with cluster headache. The relationship between cluster headache and temperature increase is probably mediated by changes in the secretion of melatonin.

GLUTAMATE, GABA, COMORBIDITIES, NEUROMODULATORS The central nervous system is greatly influenced by amino acid. Amino acids are divided into excitatory and inhibitory regarding their brain function. The major excitatory neurotransmitter in the brain’s glutamate and of utmost importance is the inhibitory gammaaminobutyric acid (GABA). Both activate ionotropic

Nous avons récemment montré que la mélatonine 3 mg a été efficace dans la prévention de la migraine. Dans la migraine, des niveaux diminués de mélatonine et d’altération dans sa courbe de sécrétion ont été détectés.

Sur le plan clinique, des crises peuvent survenir pendant la nuit, des changements du rythme de sommeil déclenchent des crises migraineux. Des patients migraineux dorment moins, ont une latence de sommeil plus grande et plus des réveils nocturnes.

Une étude faite chez deux cents patients souffrant de migraine épisodique et chronique a révélé que des patients qui ont changé son horaire de sommeil, ou qui ont rapporté des voyages en croisant des fuseaux horaires, ont rapporté une aggravation des crises.

Il a été rapporté chez les patients souffrant de migraine chronique une altération des niveaux de mélatonine avec un avancement de son apogée, des niveaux inférieurs en insomnie, indiquant une dysfonction chronobiologique.

Dans les céphalées en salves, il y a l’importance de la mélatonine et une rythmicité bien établie. Une étude double-aveugle contrôlée placebo montre que la mélatonine est supérieure au placebo dans la céphalée en salves épisodique et chronique. Les niveaux de mélatonine se trouvent diminués chez les patients atteints de céphalées en salves. La relation entre la céphalée en salves et l’augmentation de la température est probablement médiée par l’altération de la sécrétion de mélatonine.

LE GLUTAMATE, LE GABA, LES COMORBIDITÉS, LES NEUROMODULATEURS Le système nerveux central est fortement influencé par les aminoacides. Les aminoacides sont divisés en excitateurs et inhibiteurs par rapport à leur fonction cérébrale. Le plus grand neurotransmetteur excitateur dans le cerveau est le glutamate et ce de plus grande importance d’inhibition est l’acid gamma-aminobutyrique (GABA). Tous les deux activent des récepteurs ionotropiques (GABA-A, de liaison aux canaux ioniques) et métabotropiques (GABA-B, G protein-coupled) et sont généralement associés à la communication neurale .

O glutamato vem sendo responsabilizado e tido como crucial no desenvolvimento da sensitização central na fisiopatologia da cefaleia. Mostramos em estudo recente que os níveis de glutamato no líquido cefalorraquidiano de pacientes com enxaqueca crônica estava aumentado comparado a controles e ainda maiores naqueles com comorbidade com fibromialgia. A fibromialgia consiste em um processo de sensitização central caracterizada pela alodínia de pressão presente nos pontos dolorosos característicos da síndrome.

Na comorbidade psiquiátrica levanta-se o papel do GABA em condições como depressão e ansiedade (especialmente GABA B) e também em insônia, situações frequentemente associadas à enxaqueca. Podem ser o glutamato e o GABA alvos terapêuticos a serem atingidos na descoberta de novas moléculas, assim como tratamentos que possam interferir na transmissão glutamatérgica possam ser candidatos à profilaxia da enxaqueca e fibromialgia, como os neuromoduladores.

DERRETENDO O ICEBERG

É fundamental que este conceito chegue aos nossos pacientes, aos sofredores de dor de cabeça, pois pode ser uma das chaves para o processo de enfrentamento da dor pelo doente. Sabemos que eles, quando estão numa crise, se encontram frequentemente em estado de circadiana como a cefaleia hípnica e a cefaleia em salvas, outras com variação circanual, a cefaleia em salvas e a enxaqueca cíclica e, por último, a enxaqueca menstrual com ritmicidade mensal. It is important to understand the pain system as a defense mechanism of the body, essential for their survival. Living beings have not evolved over the millennia have developed a system analgesia / pain that did not have a definite biological importance. It is paradoxical, because the pain is there to protect the individual, but ends up causing you grief.

Medicamentos biológicos: inovação e complexidade como opção de tratamento para muitas doenças.

uitas pessoas podem ter utilizado algum tipo de medicação imunobiológica, mas quando perguntadas se poderiam definir ou citar alguma das opções, a maioria não souberam responder. Medicamento biológico é derivado de material vivo (plantas, animais ou microorganismos), geralmente, baseado em proteínas ou ácidos nucleicos, a exemplo de insulina, vacinas e dos derivados de sangue. São produtos desenvolvidos a partir da biologia molecular e, geralmente, usados no tratamento de doenças crônicas, como hepatites A e B, câncer, artrite e psoríase, dentre outras. Os biológicos possuem grande peso molecular e alta complexidade estrutural, quando comparados com substâncias ativas biologicamente produzidas por síntese química, como a maioria dos medicamentos tradicionais.

“Os medicamentos biológicos para artrite reumatoide, lúpus e psoríase, trouxeram uma evolução no tratamento destas doenças. Isto porque os biológicos substituem as altas doses de corticoides presentes em alguns medicamentos”, afirma Morton Scheinberg, coordenador de pesquisas clínicas e diretor científico do Hospital Abreu Sodré-AACD (especializado em sistema músculo esquelético) e clínico reumatologista e imunologista do Hospital Israelita Albert Einstein.

“The biologic drugs for rheumatoid arthritis, lupus and psoriasis, brought an evolution in the treatment of these diseases. This is because biological options replace the high doses of steroids present in some medicines”, says Morton Scheinberg, coordinator of clinical research and scientific director of Hospital Abreu Sodré-AACD (specializing in musculoskeletal system) and clinical immunologist and rheumatologist at the Albert Einstein Hospital.

According to the expert, treatment of rheumatoid arthritis, for instance, is predominantly in outpatient with the use of anti-inflammatory and aims at restoring the quality of life and prevent joint destruction. “However, in approximately two thirds of cases, patients do not respond to treatment with

Os medicamentos biológicos, também denominados biofármacos, atuam de forma mais seletiva e são elaborados utilizando matéria-prima de origens: • biotecnológica: proteínas obtidas a partir de células modificadas geneticamente, e • biológica: extraído de microorganismos, órgãos e tecidos de origem vegetal ou animal, células ou fluídos de origem humana ou animal.

A principal diferença entre os medicamentos biológicos (moléculas grandes, semelhantes a proteínas produzidas pelo próprio organismo) e os tratamentos tradicionais (moléculas pequenas com ação em múltiplos locais) são os alvos atingidos. Os medicamentos biológicos são desenvolvidos para atingir as moléculas específicas do sistema imunológico, responsáveis pelo surgimento da doença.

A complexidade molecular consiste em outro diferencial entre os biológicos e os medicamentos convencionais. Enquanto as moléculas dos medicamentos tradicionais são pequenas e simples, as de um biofármaco possuem uma estrutura espacial mais complexa e com peso mais elevado. Como são produzidos por organismos vivos, os biológicos são sensíveis a qualquer mudança no processo de manufatura, alterando os parâmetros de fabricação, equipamentos e ingredientes. Por estes motivos, produzir uma cópia de um biológico não segue os mesmos procedimentos de produção e controle de qualidade utilizados na fabricação e comercialização dos genéricos. Além disso, essa denominação não se aplica às cópias de biofármacos, sendo estes chamados, dependendo do país, de biossimilares, similares bioterapêuticos (OMS) ou “follow-on biologics” (FDA).

The initial step in the development of a biological product is to genetically modify a cell or microorganism, in order to produce the protein chosen. The cell or organism is maintained, replicates, and grown over manufacturing process.

It gets, then, to the stage of manufacture in which the medicament is formulated and stabilized by clinical trials for the therapeutic treatment and is finally produced. Importantly, any change in the process should be subjected to further assessment by the authorities.

Currently, there are immune-biological drugs approved by the National Agency for Sanitary Surveillance (ANVISA) to combat diseases such as rheumatoid arthritis, juvenile idiopathic arthritis, ankylosing spondylitis, psoriatic arthritis and psoriasis. These diseases have very effective treatments available, similar to etanercept, which acts by blocking the tumor necrosis factor - TNF (a kind of protein that stimulates inflammation), produced in excess, and thus interrupt the inflammatory cascade that is associated with these disorders.

A produção de medicamentos biológicos exige constante avaliação durante o processo de manufatura. Os biofármacos são cerca de cinco vezes mais testados do que os medicamentos convencionais: enquanto são realizados de 40 a 50 testes críticos de um produto químico, um biológico chega a ser testado 250 vezes.

O passo inicial para o desenvolvimento de um medicamento biológico é modificar geneticamente uma célula ou microorganismo, com intuito de produzir a proteína escolhida. Esta célula ou microorganismo fica conservada e réplicas são cultivadas ao longo do processo de fabricação. A partir de determinadas condições de crescimento, dá-se início à colheita da proteína, juntamente, com outros resíduos. Posteriormente, esse material passa por um processo de purificação, que contempla vários passos até atingir o componente ativo desejado.

Chega-se, então, à fase de fabricação, na qual o medicamento estabilizado e formulado passa por ensaios clínicos para o tratamento terapêutico e finalmente é produzido. É importante ressaltar que qualquer modificação no processo deve ser submetida à nova avaliação pelas autoridades.

Atualmente, existem medicamentos imunobiológicos aprovados pela Agência Nacional de Vigilância Sanitária (Anvisa) para o combate de doenças como artrite reumatoide, artrite idiopática juvenil, espondilite anquilosante, artrite psoriásica e psoríase. Essas doenças contam com tratamentos disponíveis muito efetivos, a exemplo do etanercepte, que atua bloqueando o Fator de Necrose Tumoral – TNF (espécie de proteína que estimula a inflamação), produzido em excesso, e interrompem assim a cascata inflamatória que está associada a essas patologias.

A humanização da Medicina ara exercer a Medicina é preciso ter foco no indivíduo, em sua qualidade de vida e bem estar. Nesse caso, falar em humanização pode parecer redundante. Em certas situações, fica inclusive a impressão de tratar-se apenas de recurso retórico.

O fato, no entanto, é que vivemos no Brasil, país até bem posicionado na geografia econômica mundial, mas absolutamente miserável em políticas sociais. Dia a dia, vemos na imprensa hospitais sucateados, pacientes jogados em corredores à espera de internação, mal tratados e desrespeitados. Faltam recursos à assistência adequada e, pior do que isso, não há vontade política nem postura cidadã de boa parte dos gestores, tanto nos sistemas público quanto no privado.

Não bastasse a carência de recursos e a incompetência administrativa, há outras agravantes que comprometem a qualidade do atendimento, tornando nossa medicina e a saúde perigosamente desumanizadas. Começamos pela formação médica, cada vez mais frágil e inconsistente, atingindo, inclusive, escolas médicas tidas como modelo. Surgem novas faculdades todos os dias numa roleta russa que visa somente à quantidade. O resultado é um mercado inflado anualmente por profissionais com capacitação insuficiente.

Com mão de obra excedente, o Estado e empresários da saúde seguem a cartilha da mercantilização. Praticam honorários vis, obrigando médicos a acumular vários trabalhos para compor uma renda minimamente digna. Assim, boa parte se submete a plantões de 24 horas, seguidos por jornadas de 12

The fact, however, is that we live in Brazil, a country well-positioned in the economic geographic world, but absolutely miserable in social policies. Day by day, we see in the press scrapped hospitals, patients played in corridors waiting for admission, mistreated and disrespected. Insufficient resources to care and, worse than that, there is no political will or good citizen attitude of managers in both the public and the private systems.

Not only is the lack of resources and administrative incompetence, there are other aggravating factors that compromise the quality of care in Brazil, making our medicine and health dangerously dehumanizing. We start doing wrong in the medical training, which is increasingly becoming fragile and inconsistent, reaching even medical schools taken as model schools. New faculties arise every day as “Russian roulette” (recipe for the disaster) that aims only to quantity. The result is an inflated market annually by professionals with insufficient training.

With surplus labor, business and the state of health following the playbook of commoditization. Practicing unfair fees, forcing doctors to accumulate multiple jobs to compose a minimally decent income. Where many of them put themselves under 24-hour shifts, followed by journeys of 12 hours the next day,

horas no dia seguinte, só para citar um exemplo. Enfim, colocam em risco a própria integridade, além de também por em risco os pacientes. Todos esses problemas somados à incompetência administrativa, transformam nossa medicina em caso de polícia.

Diante de tal quadro, humanizar a medicina não é chavão nem exercício de retórica. É uma necessidade imperiosa, que passa pela mudança de mentalidade de todos os agentes do sistema.

Vivemos tempos de grande avanço econômico e tecnológico, mas nada substitui o tratamento humanizado, nada é mais importante do que a medicina à beira do leito. Como sempre digo, o doente deve morrer de mãos dadas com o seu médico. Não podemos aceitar a norma do duplo anonimato – doente não tem nome e nem médico.

Humanizar a medicina é mais simples que parece e a Medicina deve ser aprendida ao lado de quem sabe, pelo testemunho da presença daquele que ensina que deve ter formação para tal.

Na futura EPM é preciso valorizar a graduação, a relação médico-paciente, o humanismo e ensinar o aluno a pensar, aprender a construir seu conhecimento através do exemplo e assistência do preceptor.

Médico que não gosta de gente não pode exercer a Medicina e nem ensiná-la. Para isso, torna-se fundamental na EPM a valorização do docente da área profissional, contemplando seu currículo não com publicações que não levam a nada, mas com pontuação acadêmica para suas atividades de ensino, assistência e extensão. Todos devem ter oportunidade e merecem estímulo. Por isso, EPM para todos!

As I always say, the patient should die holding hands with the doctor. We can not accept the double standard of anonymity where patient has no name and no doctor.

Humanize medicine is simpler than it seems and medicine should be learned next to people who know it perhaps the testimony of good professionals teaches what must be trained to do so.

In the future of EPM we need to enhance graduation, the doctor-patient relationship, humanism and teach students to think, learn to build their knowledge by example and assistance of their preceptors.

Doctor who does not like people can not practice medicine, nor teach it. For this, it is essential in the evaluation of the EPM area teaching as professional and not contemplating just the resume with publications that lead nowhere, but with academic scores as for teaching, service and outreach. Everyone should have the opportunity and deserve the encouragement. Therefore, EPM is for everyone!

Vitiligo’s Cure may come from the Brazilian cerrado

Cura do vitiligo pode vir do cerrado mbora não exista nenhum método de prevenção para o vitiligo, uma planta do cerrado brasileiro, Brosium Gaudichaudii Trécul, conhecida popularmente como “mama cadela”, está sendo utilizada no tratamento da doença. A substância atua no processo de repigmentação das áreas afetadas pela doença. Atualmente, no mercado farmacêutico, existem métodos que envolvem aplicações de cremes, loções e fototerapia (exposição ao sol com uso de substâncias fotossensibilizantes).

Os resultados do tratamento à base da planta são surpreendentes tanto em adultos como em crianças: em 80% dos casos ocorreu total eficácia, ou seja, os pacientes voltaram a ter a pigmentação natural da pele; enquanto no restante foram apresentadas melhorias no quadro, com o estacionamento da doença.

O sucesso do tratamento depende de alguns fatores tais como: estado psicológico, estágio da patologia e disciplina do paciente. Isto é, obedecer as prescrições médicas, não abandonar o tratamento em nenhum estágio, mesmo que as manchas já tenham desaparecido, além de não ingerir bebidas alcoólicas. Do contrário, pode haver um retorno das manchas.

A “mama cadela” é conhecida pela população do Centro Oeste no tratamento de diversas enfermidades. Por esse motivo, foi praticamente por acaso que a sua ação foi descoberta em 1968. Na época, o Dr. Anuar Auad, dermatologista, percebeu que a pigmentação de um paciente que tratava de pênfigo, também portador de vitiligo, desapareceu depois de banhos com a planta.

m estudo britânico constatou que a prática regular de atividade física ajuda a proteger o coração, ainda que iniciada tardiamente, após os 40 ou 50 anos.

O trabalho, publicado na revista científica Circulation, constatou que pessoas que faziam as duas horas e meia de exercícios recomendadas apresentavam índices menores de marcadores inflamatórios em seu sangue.

Os marcadores inflamatórios são importantes porque, segundo os especialistas, sua presença em grandes quantidades foi associada a um aumento nos riscos de problemas cardiológicos.

A pesquisa contou com a participação de mais de 4 mil pessoas e foi conduzida por cientistas da University College London, em Londres.

A descoberta não é inédita, uma vez que outros estudos já comprovaram os imensos benefícios para a saúde dos exercícios físicos, porém pesquisadores puderam verificar a redução dos problemas cardíacos, mesmo para aquelas pessoas que começam a praticá-los na meia-idade.

A boa notícia é de que não é preciso fazer exercícios pesados na academia.

Caminhadas vigorosas e até jardinagem já contam para preencher a cota de duas horas e meia de atividade moderada por semana, acrescentaram os especialistas.

British study found that regular physical activity helps protect the heart, though started late, after 40 or 50 years old.

The work, published in the scientific journal Circulation, found that people who had two and a half hours of exercise as recommended had lower rates of inflammatory markers in their blood.

Inflammatory markers are important because, according to experts, their presence in large quantities was associated with an increased risk of cardiac problems.

The research counted the participation of more than 4000 people and was conducted by scientists at the University College London, in London.

The discovery is not new, since other studies have shown the innumerous health benefits from physical exercise, but researchers could verify the reduction of heart problems, even for those people who started practicing them in middle age.

The good news is that you do not need to do heavy exercises in the gym.

Brisk walking and even gardening has come to fill the quota of two and a half hours of moderate activity per week, the experts added.

Exame de sangue da mãe detecta síndrome de Down. Novo teste sequencia genes do DNA fetal encontrado no plasma materno.

Técnica, ainda em fase de pesquisa, evita procedimentos invasivos usados hoje para detectar a doença. estando apenas o sangue da mãe, uma equipe de pesquisadores conseguiu detectar com precisão a síndrome de Down no feto, por meio do sequenciamento dos genes.

Isso evita procedimentos invasivos de diagnóstico pré-natal, como a punção de líquido amniótico, que podem causar risco à gravidez.

A equipe de 24 pesquisadores, liderada por Dennis Lo Yuk Ming, da Universidade Chinesa de Hong Kong, demonstrou que a análise do material genético pode eliminar a necessidade desses procedimentos na gravidez de alto risco.

This avoids invasive procedures for prenatal diagnosis, such as puncture of the amniotic fluid, which can cause risk to pregnancy.

The team of 24 researchers, led by Dennis Lo Yuk Ming, from the Chinese University of Hong Kong, showed that the analysis of genetic material can eliminate the need for these procedures in high-risk pregnancies.

“Most pregnancies were studied in the first quarter, suggesting that it is possible to implement the same test in early pregnancy,” said Lo and colleagues about the genetic sequencing.

The test looks for fragments of fetal DNA in the mother’s blood and determines if there is triple chromosome 21, which causes Down syndrome.

A equipe de Stephen F. Kingsmore, do Centro Nacional para Recursos do Genoma, EUA, optou por vasculhar doenças com origem hereditária, comprovada e baseada em genes ditos recessivos. A pesquisa foi feita com mais de cem amostras de DNA, não com pacientes.

Nesses casos de doenças causadas por genes recessivos, o problema é provocado por apenas um gene.

Para ser afetada, a criança precisa herdar a forma recessiva, não dominante, do pai e da mãe. Se pai e mãe têm ambos uma mutação no mesmo gene recessivo, então qualquer feto estará com 25% de chance de ter a doença, como é o caso da fibrose cística (que afeta o pulmão) ou a doença de Tay-Sachs.

Kingsmore e colegas lembram que já foram identificadas 1.139 dessas doenças com origem em genes recessivos. Doenças hereditárias provocam 20% das mortes de crianças no mundo.

Enquanto a pesquisa de Lo sobre síndrome de Down envolve fetos (é “prénatal”), o objetivo do segundo estudo é testar se os pais são portadores dos genes que provocam a doença, um estudo “pré-concepção”.

Graças a checagens pré-concepção e ao aconselhamento genético dos pais, foi possível, por exemplo, reduzir em 90% os casos da doença de Tay-Sachs, problema neurodegenerativo e incurável, entre os judeus asquenazes (da Europa central).

APLICAÇÃO De acordo com Kinsgmore, a principal questão associada a esse tipo de teste, para que ele seja disseminado, é a educação dos médicos e do público sobre as informações genéticas: como interpretá-las e como aplicá-las. “Isso vai requerer o treinamento de muitos conselheiros genéticos”, disse o pesquisador à Folha.

Ele afirma que pretende começar a oferecer o teste nos EUA em breve.

“Vai levar vários anos para informar e educar os médicos sobre o teste. Nós achamos que o ritmo da adoção vai se relacionar com o custo, que deve ser de cerca de US$ 500 (R$ 837)”, afirma Kingsmore. A próxima estapa da pesquisa vai envolver pacientes voluntários.A ideia é escanear até 580 doenças simultaneamente.

Comentando o futuro dos testes genéticos, também na “Science Translational Medicine”, o pesquisador Laird Jackson, do Hospital da Criança de Filadélfia, EUA, disse que esses estudos jogam luzes sobre velhas preocupações éticas, legais e sociais associadas com a tecnologia genética.

“A tecnologia está aqui, continuando a se desenvolver, e vai se tornar mais barata, mais rápida e, portanto, mais disponível.”

A questão agora é educar consumidores e profissionais sobre o poder dos testes.

“Deve haver uma discussão aberta sobre suas qualidades e a respeito de sua implementação”, declarou Jackson à Folha.

Other published research demonstrates the potential of genomic sequencing to discover hereditary diseases.

The article is in the latest issue of “Science Translational” shows a test that analyzes the genome of prospective parents to detect 448 diseases.

The team of Stephen F. Kingsmore from the National Center for Genome Resources, USA, opted to scour with hereditary diseases, based on proven and recessive genes said. The survey was conducted with over one hundred DNA samples, not patients.

In such cases of diseases caused by recessive genes, the problem is caused by only one gene.

To be affected, the child must inherit the recessive form, not dominant, from father and mother. If both father and mother there is a mutation of the same recessive gene, so will any fetus with a 25% chance of having the disease, such as cystic fibrosis (affecting the lungs) or Tay-Sachs desease.

Kingsmore and colleagues point out that it was identified 1139 diseases originating from recessive genes. Inherited diseases cause 20% of all child deaths worldwide.

While the research from Lo on Down syndrome fetuses involves (is “prenatal”), the purpose of the second study is to test if parents are carriers of the genes that cause the disease, a study “pre-conception”.

Thanks to preconception check-ups and genetic counseling of parents, it was possible, for example, to reduce by 90% the cases of Tay-Sachs, and incurable neurodegenerative problem among Ashkenazi Jews (of central Europe).

ENFORCEMENT According to Kinsgmore, the main issue associated with this type of test, for it to be disseminated, is the education of physicians and public on genetic information: how to interpret them and how to apply them. “This will require the training of many genetic counselors,” said researcher to Folha.

He says he plans to start offering the test in the U.S. soon.

“It will take several years to inform and educate physicians about testing. We think that the pace of adoption will relate to the cost, which should be about $ 500 (R$ 837,00),” says Kingsmore. The next stage of the research will involve volunteer patients. The idea is to simultaneously scan up to 580 diseases.

Commenting on the future of genetic testing, also in “Science Translational Medicine”, the researcher Laird Jackson, from the Children’s Hospital of Philadelphia, USA, said that these studies throw light on old ethical, legal and social issues associated with genetic engineering. “The technology is here, continuing to unfold, and will become cheaper, faster, and therefore more available.”

The question now is to educate consumers and professionals about the power of the tests.

“There should be an open discussion about their qualities and their implementation,” said Jackson to Folha.

crescimento no número de casos de câncer, em todo o mundo, desde os tipos mais comuns até outros de menor incidência, como o Linfoma Não Hodgkin, tem aumentado a procura por tratamentos da doença. Especificamente sobre esse tipo da doença, a possibilidade de cura pode estar no próprio corpo do paciente – o auto transplante de medula óssea.

O procedimento é realizado para promover o restabelecimento fisiológico do indivíduo após intensas doses de quimioterapia.

Segundo o Dr. Celso Massumoto, diretor da Clínica Oncocenter, de São Paulo, a indicação desse tratamento é analisada de acordo com o estágio de desenvolvimento do Linfoma Não Hodgkin, bem como a idade e as condições do paciente. O hematologista explica que nos casos em que o tratamento é indicado, o paciente se submete a altas doses de quimioterapia – até 30 vezes maior que as aplicações usuais. Por consequência, o procedimento destroi células cancerígenas, além doutras responsáveis pelo bom funcionamento do organismo, como as imunológicas e as do sangue.

O autotransplante de medula óssea consiste na coleta de células progenitoras hematopoéticas (TCPH). Essas células são responsáveis pela produção de glóbulos vermelhos, brancos e plaquetas. As células progenitoras estão localizadas no interior da medula óssea, contida no centro dos ossos, e também na corrente sanguínea. “As células empregadas no transplante devem ser geneticamente compatíveis com o paciente. Por isso, são retiradas de seu próprio corpo”. Após o procedimento quimioterápico, essas células são reinjetadas no paciente. Ao fazer o autotransplante, o corpo é estimulado a produzir novas células sanguíneas, responsáveis por levar oxigênio e nutrientes para todo o organismo.

Como é feito o autotransplante? Num processo semelhante à hemodiálise, o paciente tem o sangue separado por um equipamento denominado máquina de leucaférese. Cerca de 300 ml sangue é obtida pelo aparelho, contendo as células progenitoras coletadas e, em seguida, é criopreservada. Ao final da quimioterapia, são devolvidas as células progenitoras previamente retiradas e descongeladas. “De acordo com o caso, as chances de curas são de 70% em quadros de Linfoma Não Hodgkin”.

Massumoto aponta que após o procedimento, o paciente necessita de cuidados especiais, visto que estará extremamente deficiente de células imunológicas.

Assim, não raramente, é indicada sua permanência em ambientes dotados de filtros nas entradas de ar condicionado. O contato com outras pessoas e com a alimentação também requer maior atenção. A retomada das atividades cotidianas acontece gradualmente, de acordo com as avaliações médicas.

om base em uma consulta pública lançada pelo Ministério da Saúde em 24 de setembro, o Sistema Único de Saúde (SUS) passará a adotar novas regras para a realização da cirurgia bariátrica. Uma delas é a diminuição da idade mínima para se submeter ao procedimento, que passa de 18 para 16 anos. Outra é a mudança da técnica cirúrgica a ser aplicada no atendimento público. Além disso, novos exames passarão a ser obrigatórios.

As alterações farão parte de uma nova portaria, a ser anunciada nos próximos dias e com previsão de entrar em vigor em 2013. Especialistas afirmam que as iniciativas são positivas, mas não garantem a resolução de um dos maiores problemas: a espera por uma cirurgia do SUS pode levar até dez anos. Para o cirurgião Ricardo Cohen, presidente da Sociedade Brasileira de Cirurgia Bariátrica e Metabólica (SBCBM), outras mudanças também deveriam ser consideradas. ‘É importante incluir os adolescentes, porém já existe um gargalo grave para a cirurgia em adultos. O problema é como o SUS vai resolver isso’, diz.

A endocrinologista Rosana Radominski, presidente da Associação Brasileira para o Estudo da Obesidade (Abeso) e parceira do Instituto Movere, que trata da obesidade infanto-juvenil, afirma que a inclusão dos jovens de 16 a 18 anos trará impacto positivo. ‘O adolescente com 16 anos já amadureceu e dois anos, nessa fase, é um tempo muito grande. Eles vão ganhar muito com a possibilidade de fazer a cirurgia mais cedo.’

Para o ministro da Saúde, Alexandre Padilha, a decisão de reduzir a idade está de acordo com recomendações internacionais e atende a uma reivindicação do Conselho Federal de Medicina. ‘Estudos mostram que o tratamento precoce da obesidade grave pode evitar a ocorrência de uma série de problemas correlatos, como problemas cardiovasculares.’

De acordo com a portaria, cinco novos exames passam a ser obrigatórios antes da operação, entre eles a ultrassonografia de abdome total, a ecocardiografia transtorácica e a prova de função pulmonar completa com broncodilatador.

Segundo o cirurgião Irineu Rasera, titular da SBCBM, a decisão de incluí-los é acertada, desde que a aplicação desses exames não seja obrigatória. ‘Muitos pacientes precisam desses exames e não os encontram na rede pública. Se a decisão for que os exames estarão disponíveis, vai ser positivo. Mas tornar obrigatória a realização de todos esses exames pode engessar o sistema.’

Com a nova portaria, a técnica cirúrgica adotada pelo SUS passará a ser a gastroplastia vertical em manga, no lugar da ultrapassada gastrectomia vertical em banda. Para Cohen, trata-se de uma adequação da portaria ao que já é praticado nos centros cirúrgicos. ‘Essa cirurgia já não é feita desde o começo dos anos 1990, pois está associada a reincidência de ganho de peso.’

PLÁSTICA O pacote traz a oferta de mais uma cirurgia plástica indicada para quem perdeu peso. Hoje, é possível fazer a correção do abdome. A técnica que o SUS passará a oferecer inclui reparação na área dorsal.

O Ministério deve começar a pagar uma equipe multidisciplinar, encarregada do tratamento. ‘Muitos serviços já ofertam esse tipo de assistência. Mas eles não recebiam pelo atendimento, algo que agora vai mudar’, disse Padilha.

A nova política muda os critérios para credenciamento de hospitais. Além de infraestrutura mínima, a instituição terá de formar uma rede com um centro de atenção básica e outro de média complexidade.

As mudanças incluem reajuste de 20% do valor pago pelas cirurgias.

Outras reivindicações da SBCBM que ainda não foram atendidas incluem a existência de um cadastro unificado de candidatos ao procedimento, a adoção da laparoscopia e a existência de uma política que priorize os pacientes diabéticos (mais informações nesta pág.). Padilha reitera que a consulta pública está aberta. ‘Outras mudanças poderão ser incluídas. Vamos avaliar as contribuições da sociedade.’

A cura da esclerose múltipla

ientistas italianos dizem que podem estar se aproximando da descoberta da cura para a esclerose múltipla, doença que atinge o sistema nervoso central.

A terapia, desenvolvida por cientistas do Instituto San Raffaele, de Milão, se baseia na tentativa de recompor a mielina, substância que falta às vítimas da doença.

Em condições ideais, a mielina reveste as fibras nervosas do cérebro, mas nos pacientes com esclerose múltipla - englobada no grupo das doenças desmielinizantes - a lesão na camada de mielina provoca distúrbios na transmissão dos impulsos nervosos.

O tratamento vem sendo desenvolvido há três anos pela equipe do instituto, chefiada pelos cientistas Gianvito Martino e Angelo Vescovi. Ele consiste em injetar células nervosas, denominadas neurais estaminais, diretamente na veia ou no cérebro. “As células neurais estaminais são levadas para todo o sistema nervoso, e uma vez que chegam às zonas lesadas começam a se transformar em células que produzem a mielina. Somente as zonas inflamadas, ou onde existe algum dano, captam essas células reparadoras”, explica Martino.

Os médicos italianos deverão começar a fazer os testes da nova terapia em macacos em alguns meses. Se os experimentos derem certo, em cinco anos, a técnica poderá ser testada em seres humanos.

The cure for Multiple Sclerosis

talian scientists say they may be approaching the discovery of a cure for multiple sclerosis, a disease that affects the central nervous system.

The therapy, developed by scientists at the Institute San Raffaele, Milan, is based on the attempt to restore the myelin, a missing substance in the victims of this disease.

Ideally, the myelin covers the nerve fibers in the brain, but in patients with multiple sclerosis – putted together in the group of demyelinating diseases - a lesion in the myelin sheath that causes disturbances in the transmission of nerve impulses.

The treatment has been developed three years ago by the institute’s staff, led by scientists Gianvito Martino and Angelo Vescovi. It consists in injecting nerve cells called neural stem directly into the vein in the brain. “The neural stem cells are taken to the entire nervous system, and once they get to the damaged areas they begin to turn into cells that produce myelin. Only inflamed areas, or areas where there are some damage, capture these reparative cells”, explains Martino.

The Italian doctors should start doing the tests of the new therapy in monkeys in a few months. If the trials go well, in five years, the technique could be tested in humans.

A esclerose múltipla atinge principalmente pessoas entre 20 e 30 anos e é resultado da destruição desse revestimento das células nervosas, o que impede a comunicação entre essas células e, com isso, o funcionamento normal do cérebro.

As terapias usadas até hoje são baseadas principalmente no uso de medicamentos que apresentam uma eficácia limitada e efeitos colaterais.

“Até agora, as terapias para esse tipo de doença se concentravam em destruir as células, por assim dizer, enlouquecidas do sistema imunológico.

Mas esse novo estudo tem uma abordagem diversa e se baseia na reconstrução da mielina”, afirma Martino.

O cientista diz que ainda não foi registrado nenhum efeito colateral, mas ressalta que isso não significa que a nova terapia terá sucesso garantido.

“Quando esses experimentos começarem a ser feitos no homem nós usaremos, a princípio, células nervosas de fetos, já que o transplante ainda não é possível e poderia haver rejeição pelo organismo.”

Apesar disso, os dois médicos dizem que as perspectivas até agora são boas. Segundo eles, a introdução dessas células no organismo não seria um processo tão complexo, já que não será necessário manipular as celulas neurais depois do implante.

“Nós implantamos essas células exatamente como elas são e não precisamos nem instruí-las nem implantá-las no lugar da lesão, deixando que elas livremente procurem a parte inflamada ou lesada. Essas celulas têm por natureza como principal função a de reparar danos”, explica o professor Angelo Vescovi.

Segundo os pesquisadores, até hoje era impensável o uso de células neurais como forma de cura para uma doença tão complexa como a esclerose múltipla.

Embora a causa da doença ainda seja desconhecida, sabe-se que os sintomas podem ser aliviados com medicamentos, desde que realizado um diagnóstico precoce.

“We implemented these cells exactly as they are and had no need to instruct them or to deploy them in the place of the injury, letting them freely look for the inflamed or injured part. Such cells are by nature as the main function of repairing damage”, explains Professor Angelo Vescovi.

According to researchers, until now it was unthinkable using neural cells as a way to cure a disease as complex as multiple sclerosis.

Although the cause of the disease remains unknown, it is known that the symptoms can be relieved with medication, if performed early diagnosis.

m cientista americano diz ter encontrado uma maneira de proteger a capacidade física de comunicação do ser humano com base em um estudo que conduziu sobre os padrões cerebrais do renomado físico britânico Stephen Hawking.

Philip Low espera que, com o tempo, Hawking possa “escrever” as palavras com o seu cérebro, substituindo, assim, o atual sistema de reconhecimento de voz do físico britânico, que interpreta os movimentos musculares de seu rosto.

O cientista defende que a inovação poderia evitar o risco da síndrome do “encarceramento”, quando os movimentos do corpo são paralisados (com exceção dos olhos), mas as faculdades mentais se mantêm ativas.

Paralelamente à descoberta, a empresa de tecnologia Intel também está trabalhando em uma alternativa semelhante.

Low diz que criou um software que pode analisar os dados e detectar sinais de alta frequência que se acreditava terem se perdido por causa do crânio.

“Uma boa analogia para entender o processo é quando você se afasta de uma sala de concerto onde há música tocada por uma série de instrumentos”, disse o pesquisador à BBC. “À medida em que vai mais longe, você deixa de ouvir elementos de alta frequência, como o violino e viola, mas continuar a ouvir o trombone e violoncelo.

Bem, quanto mais longe você estiver do cérebro, mais se perdem os padrões de alta frequência”, acrescenta.

ESFORÇOS DA INTEL Em janeiro passado, a Intel anunciou que também havia começado a trabalhar na criação de um novo sistema de comunicação para Hawking, depois de um pedido do físico ao cofundador da empresa Gordon Moore.

A fabricante de chips tenta, agora, desenvolver um novo software de reconhecimento facial 3D para acelerar a velocidade com que Hawking pode escrever.

consumo diário de chocolate escuro pode ajudar a reduzir levemente a pressão sanguínea, segundo indica uma análise de 20 estudos. A pesquisa foi feita pelo grupo Cochrane – colaboração internacional de milhares de especialistas que revisam estudos já realizados.

A causa seria o cacau, principal ingrediente do chocolate, que relaxa os vasos sanguíneos. A teoria é que o cacau contém o flavonoide, que faz o corpo humano produzir a substância química chamada óxido nítrico, que “relaxa” os vasos, facilitando a passagem do sangue e, por consequência, diminuindo a pressão sanguínea.

Os estudos anteriores, combinados pela análise Cochrane, haviam apresentando resultados variados. Retina artificial pode ajudar alguns cegos a enxergar

or duas décadas, Eric Selby não enxergava e dependia de um cão guia para se locomover. Mas depois de receber o implante de uma retina artificial em seu olho direito, ele pode detectar coisas normais, como o meio-fio e a calçada enquanto caminha pela rua.

“Basicamente, são flashes de luz que você tem que traduzir em seu cérebro, mas é incrível que eu possa ver alguma coisa”, disse Selby, um engenheiro aposentado de Coventry, na região central da Inglaterra.

Há mais de um ano, o homem de 68 anos recebeu um implante artificial chamado de Argus 2, feito pela empresa norte-americana Second Sight, inserido cirurgicamente em seu olho direito. Reguladores holandeses são esperados nas próximas semanas para decidir sobre o pedido da empresa para comercializar o aparelho na União Europeia. Artificial retina may help some blind people to see

The computer processes scenes captured by the camera and converts them into visual information in the form of an electronic signal that is sent to the implant. The device stimulates the good cells remaining in the retina, causing them to relay the data to the optic nerve.

The implant is intended only for people with a specific type of retina’s problem, hereditary, when people still have some functional cells. About one in 3,000 people are blind due to this group of hereditary diseases, called retinitis pigmentosa, and may become potential recipients by artificial retina.

O acompanhamento de um médico pode diminuir o sofrimento causado por esse problema frequente. essoas que desenvolvem câncer geralmente costumam sofrer também com a dor. A situação torna-se mais crítica em fases avançadas da doença, já que a dor acomete de 70% a 90% desses pacientes.

“A ideia de que sentir dor faz parte da rotina é equivocada”, alerta Mário Luiz Giublin, anestesiologista responsável pela Clínica de Dor do Hospital de Clínicas da Universidade Federal do Paraná. “Pelo contrário: as pessoas devem ser tratadas para sentir o mínimo de dor possível”, complementa.

Geralmente, ela é causada pela pressão ou invasão que o tumor exerce em alguns órgãos, nervos ou mesmo ossos. Quando alguns vasos sanguíneos são vítimas dessa compressão, a dor também costuma surgir. “Há ainda as dores com componentes neuropáticos, que atingem de 30% a 40% dos pacientes oncológicos”, enumera Giublin. Elas são caracterizadas pela disfunção no processamento da dor, ou seja, quando há problemas no caminho nervoso percorrido pelos sinais dolorosos. “Nesses casos, a abordagem é um pouco mais complexa”, diferencia o especialista, e o uso de analgésicos comuns não é indicado.

A melhor opção, como sempre, é procurar um especialista em dor para receber o diagnóstico e a terapia corretos. “É preciso difundir informações, pois a dor oncológica costuma ser subtratada – apesar de haver tratamentos para ela”, explica. No Brasil, desde 2001 existe o Paraná Sem Dor, programa que é fruto de uma parceria entre Secretaria Estadual de Saúde, Clínica de Dor e Serviço de Oncologia do Hospital de Clínicas da Universidade Federal do Paraná. O objetivo é tratar a dor fornecendo medicamentos e capacitando médicos do SUS para lidar com o problema. Segundo Giublin, que implementou o programa, inicialmente o projeto era voltado apenas para pacientes com câncer. “Ao longo dos anos, o Paraná sem Dor de Câncer passou a se chamar Paraná sem Dor e ampliou sua atuação e assistência”, conclui o médico.

É primitiva porque quase sempre os atos que nos parecem adaptativos por suas consequências, em verdade, não são os que poderiam produzir os melhores resultados, ainda que nem sempre o saibamos. E por não saber, vamos tendo um tipo de compreensão da realidade e forma de resolver os problemas que se nos apresentam diariamente de modo pessoal, como se fosse o melhor.
