Desvelando o cuidado humanizado: percepções de enfermeiros em oncologia pediátrica.

Unveiling humanized care: nurses’ perceptions in pediatric oncology.

esde o inicio do século XXI, um forte movimento vinculado à humanização tem permeado discussões na área de saúde. O Ministério da Saúde, em 2000, regulamentou o Programa Nacional de Humanização, que teve como objetivo fundamental o aprimoramento das relações entre profissionais, usuários/profissionais e hospital/comunidade, visando melhorar a qualidade e a eficácia dos serviços prestados pelas instituições e promover uma nova cultura de atendimento.

Humanização é uma expressão de difícil conceituação, tendo em vista seu caráter subjetivo, complexo e multidimensional. A literatura descreve que “humanizar é afirmar o humano na ação e isso significa cuidado porque só o ser humano é capaz de cuidar no sentido integral”.

Desse modo, além da ênfase para a integralidade da atenção, também deve haver um movimento de reflexão crítica dessas relações, considerando a subjetividade dos indivíduos na ocasião do cuidado. Trata-se de um processo que exige ênfase ampla e coletiva, não apenas do ser cuidado,

ince the beginning of the XXI century, a strong movement linked to humanization has permeated discussions in the area of health. In 2000, the Brazilian Ministry of Health regulated the National Humanization Program, which has as its fundamental aim to improve relations between professionals, service users/professionals, and hospital/community, with a view to improving the quality and the efficiency of the services provided by the institutions and to promote a new culture of care.

Humanization is a term which is difficult to conceptualize, given its subjective, complex and multidimensional character. The literature describes that “to humanize is to assert the human in the action and this means care because only the human being is able to care in the holistic sense”.

In this way, in addition to the emphasis on the integrality of the care, there must also be a movement of critical reflection on these relationships, taking into account the subjectivity of the individuals when providing

foco da relação, mas também da preocupação com a estruturação do ambiente e dos profissionais, para que todos usufruam de segurança e conforto. Além disso, envolve ética e estética, o que pressupõe além da técnica, sensibilidade para que os indivíduos formalizem a consciência com o cuidado.

Nesse sentido, a humanização é um processo complexo, que envolve padrões diferenciados na percepção dos profissionais, seus valores e crenças, para haver mudança de comportamento. Nessa relação única e intersubjetiva tem-se um processo singular, não caracterizado por generalizações, já que diferentes profissionais, equipes e instituições terão processos distintos na aquisição de habilidades para uma assistência mais humanizada como visam os programas emergentes.

Esse estudo tem como proposta analisar essa singularidade no processo de humanização, com enfermeiros que trabalham em oncologia pediátrica. Nesse contexto, aliada à estrutura da criança ainda em crescimento e desenvolvimento, tem-se a evolução de uma doença ameaçadora e estigmatizada. Diante disso, a família passa por uma desestruturação, justificada pela mudança na rotina, pelas crenças que cercam a doença e pelo sentimento de indignação ao ver uma criança ser privada dos seus “direitos” em detrimento do sofrimento ocasionado por sucessivas intervenções dentro do hospital, que são grandes geradoras de ansiedade e estresse.

Por isso, a criança com câncer e sua família passam por situações de extrema fragilidade física, psicológica e social. Além do ambiente pouco acolhedor no hospital, os fatores psicológicos que envolvem a criança e a família, como o estresse e a ansiedade durante a internação hospitalar, são identificados como limitantes no cuidado de enfermagem e influenciam a interação do profissional com a família, gerando situações de incompreensão e não cooperação por parte da família nos cuidados.

A enfermagem tem importante papel nesse processo, já que as atitudes de cuidado devem considerar fatores que minimizem essas influências e OM 6

care. This is a process which demands broad and collective emphasis, not only on the one-being-cared-for, the focus of the relationship, but also on the concern with the structuring of the environment and the professionals, such that all may benefit from security and comfort. In addition to this, it involves ethics and esthetics, which presupposes, in addition to technique, sensitivity such that the individuals may formalize consciousness with the care.

In this regard, humanization is a complex process, which involves differentiated standards in the professionals’ perception, values and beliefs, for there to be change in behavior. In this unique and intersubjective relationship, there is a unique process, not characterized by generalizations, as different professionals, teams and institutions have distinct processes in the acquisition of skills for more humanized care, as the emerging programs aim to do.

This study aims to analyze this uniqueness in the process of humanization, with nurses who work in pediatric oncology. In this context, along with the structure of the child who is still growing and developing, there is the progression of a threatening and stigmatizing disease. In the face of this, the family passes through a destructuration, explained by the change in the routine, by the beliefs which surround the disease, and by the feeling of indignation on seeing the child being deprived of her rights as a result of the suffering caused by successive interventions in the hospital, which create anxiety and stress.

Because of this, the child with cancer and her family pass through situations of extreme physical, psychological and social fragility. In addition to the little-welcoming atmosphere in the hospital, the psychological factors which surround the child and the family, such as stress and anxiety during inpatient treatment, are identified as limiting factors in the nursing care, and influence the professional’s interaction with the family, causing situations of incomprehension and non-cooperation on the part of the family in the care.

promovam uma assistência humanizada. Fatores como estabelecer vínculo de confiança e amizade, empatia, fazer com que o paciente sinta-se parte do processo, são elementos que permitem um cuidado além da técnica, considerando a dimensão humana desse processo.

Ao se deparar com esse ambiente, o enfermeiro deve buscar artifícios para cuidar com arte, habilidade empática e muita criatividade. Ademais, o diagnóstico de câncer envolve sentimentos próprios de sofrimento que trazem mudanças profundas na vida das crianças e de suas famílias, por isso, os aspectos socioculturais, emocionais e espirituais devem ser passíveis de cuidados e intervenção, como contextos decisivos na evolução da patologia.

Dessa maneira, é necessário compreender o sentido do cuidado e a subjetividade que envolve esse processo, com atenção especial ao universo da criança, preservando o seu mundo particular e as etapas de desenvolvimento da infância para satisfazer suas necessidades em troca do bem estar, apesar da condição de doença.

Face ao exposto, foram traçados os seguintes objetivos para este estudo: desvelar os elementos do cuidado humanizado prestado à família e à criança com câncer, identificar a percepção dos enfermeiros que trabalham na oncologia pediátrica quanto à humanização da assistência, e verificar em que situações o enfermeiro percebe que a humanização está ancorada ao cuidado prestado.

Método Este estudo trata-se de pesquisa exploratório-descritiva, sustentada pela abordagem qualitativa, que utilizou como referencial teórico os pressupostos da Teoria do Cuidado Humano de Jean Watson.

Em 1979, Watson desenvolveu os carative factors, os quais foram revisados em 1985, sendo introduzido o paradigma de Cuidado Transpessoal, que enfoca o ideal moral, o significado da comunicação e do contato intersubjetivo mediante a co-participação do self como um todo.

Com a evolução da teoria, foi introduzido o clinical caritas processes, que substituíram os carative factors. Esse processo inclui a sacralidade do ser cuidado, a conexão do ser humano para um plano que extrapola o concreto e visual e a proposição do healing para reconstituição do ser como componentes para fundamentar o cuidado. A teoria considera a intersubjetividade enfermeiro-cliente, a partir da ideia de que há sempre influência de um ser sobre outro e está centrada nos conceitos humanísticos do cuidado, olhando para o indivíduo nas dimensões biopsicológica, espiritual e sociocultural.

Os participantes compreenderam nove enfermeiros de um hospital

Nursing has an important role in this process, as the attitudes of care must be considered to be factors which minimize these influences and promote humanized care. Factors such as establishing a link of trust and friendship, empathy, and making the patient feel that she is part of the process, that is, considering this process’s human dimension, are elements which allow a care which goes beyond simple techniques.

In encountering this environment, the nurse must seek techniques so as to care with art, empathetic skill and plenty of creativity. Furthermore, the diagnosis of cancer involves distinct feelings of suffering which bring profound changes in the lives of the children and their families; as a result of this, the socio-cultural, emotional and spiritual aspects must be cared for and subject to interventions, as decisive contexts in the pathology’s progression.

For these reasons, it is necessary to understand the meaning of the care and the subjectivity which surrounds this process, with special attention to the child’s universe, preserving her private world and the stages of childhood development, so as to meet their needs so as to achieve wellbeing, in spite of the condition of having the disease.

In the light of the above, the following objectives were outlined for this study: to unveil the elements of humanized care provided to the family and to the child with cancer, to identify the perception of the nurses who work in pediatric oncology in regard to the humanization of the care, and to ascertain in which situations the nurse perceives that humanization is anchored in the care given.

Method This study is exploratory-descriptive research, supported by the qualitative approach, and used as its theoretical framework the assumptions of Jean Watson’s Theory of Human Caring.

público de oncologia pediátrica da cidade de São Paulo, que atenderam aos seguintes critérios de inclusão: ser enfermeiro e ter vínculo na instituição do estudo. Os sujeitos foram esclarecidos sobre sua participação voluntária, sobre a garantia de sigilo das informações e sobre o uso dos resultados apenas para fins científicos. Esses participantes foram identificados por números, para garantir o anonimato.

Ressalta-se que o projeto recebeu parecer favorável do Comitê de Ética em Pesquisa da Instituição (Parecer n. 534/2006), de acordo com a Resolução n. 196, referente à pesquisa com seres humanos e, posteriormente, os dados foram coletados de novembro de 2008 a fevereiro de 2009, por meio de entrevistas compostas por duas partes, tendo a primeira a finalidade de identificar os sujeitos da pesquisa e a segunda parte foi composta pelas questões semiestruturadas: o que você entende por humanização? Fale-me sobre uma situação em que você acredita ter prestado um cuidado humanizado? O que torna possível o cuidado humanizado as crianças e suas famílias, no contexto da oncologia? As entrevistas foram gravadas em fitas cassete e, para a análise, seu conteúdo foi transcrito na íntegra, sendo utilizada como estratégia a análise de conteúdo, que busca interpretar os dados por meio das etapas: pré-análise e codificação aberta, exploração do material para categorização, tratamento dos dados e interpretação.

Resultados e discussão A descrição dos participantes Participaram do estudo nove enfermeiros, identificados pelos números em sequência: Enf. 1, Enf. 2, Enf. 3, Enf. 4, Enf. 5, Enf. 6, Enf. 7, Enf. 8 e Enf. 9.

Dos participantes, um era do sexo masculino e oito eram do sexo feminino, com idade variando de 24 a 28 anos. Quanto ao tempo de

contact through the co-participation of self as a whole. As the theory evolved, the clinical caritas process was introduced, substituting the carative factors.

This process includes the sacredness of being cared, the connection of human being to a plane which extrapolates the concrete and visual and the proposition of healing for reconstitution of self as components for grounding the care. The theory considers the nurse-client intersubjectivity based on the idea that one being always has an influence on another, and is based on the humanistic concepts of care, viewing the individual in his or her bio-psychological, spiritual and socio-cultural dimensions.

The participants were nine pediatric oncology nurses from a public hospital in the city of São Paulo, who met the following inclusion criteria: to be a nurse, and to be linked to the institution in the study. The subjects were informed as to their voluntary participation, the guarantee of the confidentiality of the information, and on the use of the results only for scientific purposes. These participants were identified by number, so as to ensure anonymity.

It should be noted that the project received a favorable decision from the institution’s Research Ethics Committee (Decision n. 534/2006), in accordance with Resolution n. 196, referent to research involving human beings. The data was later collected, between November 2008 and February 2009, through two-part interviews, the first of which aimed to identify the research subjects, and the second of which was composed of the following semi-structured questions: what do you understand by humanization? Would you tell me about a situation in which you believe you provided humanized care? In the oncology context, what is it that makes humanized care for children and their families possible? The interviews were recorded on cassette tapes and for analysis their content was transcribed in full. Content analysis was used as a strategy, seeking to interpret the data through the following stages: pre-analysis and open coding, exploration of the material for categorization, treatment of the data and interpretation.

formação, a média foi de dois anos e quatro meses. O tempo de atuação na área de oncologia pediátrica variou de dois meses a um ano e seis meses. Dos nove entrevistados, três nunca trabalharam em outra área e os outros seis apresentavam diferentes experiências profissionais prévias como: oncologia com adultos, centro obstétrico, unidade básica de saúde, hematologia e diagnóstico por imagens. Das religiões referidas, quatro enfermeiros se declararam católicos, quatro evangélicos e um espírita.

Os depoimentos Os elementos teóricos deste estudo emergiram das descrições de situações apresentadas pelos enfermeiros e foram organizados a partir dos 10 Clinical Caritas Processes, segundo a teoria de Jean Watson.

Prática do amor, amabilidade e serenidade no contexto de cuidado consciente

Results And Discussion The description of the participants Nine nurses participated in the study, identified by the sequential numbers: N1, N2, N3, N4, N5, N6, N7, N8 and N9.

One of the participants was male, and eight female, with ages varying from 24 to 28 years old. Regarding the time since graduation, the mean was two years four months. The length of service in the area of pediatric oncology varied from two months to one year six months. Of the nine interviewees, three had never worked in another area, and the other six had varying previous professional experience, such as: adult oncology, obstetric center, primary health center, hematology, and screening. Of the religions mentioned, four nurses stated that they were Roman Catholics, four were Evangelicals, and one was a Spiritualist.

The discourses

O cuidado deve ser baseado em um conjunto de sentimentos humanos universais como a bondade, o interesse e a prática do amor por si e pelos outros, sendo assim, o enfermeiro se torna capaz de conectar-se ao outro e com ele. Nesse sentido, emergiram dos discursos aspectos que centralizam esses sentimentos como fundamentais para a prática de um cuidado humanizado na enfermagem: “Sendo humano, tratar o outro com carinho, amor e respeito” e “Tendo que exercer a profissão com amor, principal fruto para a realização profissional.”

Tem que trabalhar com carinho, com amor, que é o principal. Se você não tiver amor, pelo que você faz, pela sua profissão. [...] nunca vai conseguir fazer um trabalho humanizado, vai tornar tudo mecânico. Amor, carinho e dedicação são coisas essenciais para um trabalho humanizado (Enf. 6).

O amor ao outro, à profissão e a si mesmo é um dos elementos que traz a afinidade exigida para a inter-relação do self do enfermeiro e do self do outro (criança, família, comunidade), à plenitude que rege o universo.

The study’s theoretical elements emerged from the descriptions of the situations presented by the nurses, and were organized based on the 10 Clinical Caritas Processes, in line with Jean Watson’s theory.

Practicing loving-kindness and equanimity within a context of caring consciousness The care must be grounded in a set of universal human feelings, such as kindness, interest in and the practice of love for self and others; this being so, the nurse becomes able to connect with others and with herself. In this regard, aspects emerged from the discourses centralizing these feelings as fundamental for the practice of humanized care in nursing: “being human, treating others with tenderness, love and respect” and “having to exercise one’s profession with love, the principal fruit for professional realization.”

You have to work with tenderness, with love, that’s the main thing.

If you don’t have love, for what you do, for your profession, [...] you’ll never manage to provide humanized work, everything will be mechanical.

Love, tenderness and dedication are essential for humanized work (N6).

Love for others, for the profession and for self is one of the elements which brings the affinity required for the inter-relation of the self of the nurse with the self of the other (child, family, community), for the completeness which rules the universe.

Being authentically present and enabling, and sustaining the deep belief system and subjective life world of self and one-being-cared-for

Estar autenticamente presente para possibilitar e manter um sistema profundo de crenças e um mundo subjetivo do self e do ser-para-o-cuidado A manutenção da fé e o respeito à crença do outro são elementos essenciais para conectar-se um ao outro. Mostrar aos pais e às crianças que o enfermeiro estará sempre presente, fazendo o melhor,demonstra a importância de assegurar a presença de um profissional competente e comprometido, junto aos pais e às crianças, em todos os momentos do tratamento. [...] tentar conversar com os pais e falar: olha, está acontecendo isso e a gente está aqui, está tentando. Não desistiremos. Todas as crianças que estão aqui são graves, mas a gente tenta até o final (Enf. 4).

O enfermeiro também para assegurar à família que a assistência prestada é adequada se percebe encorajando a família a ter fé e esperança no tratamento e no profissional. Assim, ajuda no fortalecimento do laço profissional-família e na manutenção da fé e da confiança.

Tentar entender o que está passando na vida dela [família] e conversar, levar para o lado da fé, para quem acredita. [...] falar referente à equipe médica e de enfermagem, que é muito boa, que tem experiência, e tentar dar tranquilidade de que ele vai ser tratado da melhor forma aqui (Enf. 1).

Keeping faith and respect for the beliefs of others is an essential element to connect with one another. Showing the parents and the children that the nurse will always be present, doing her best, shows the importance of ensuring the presence of a competent and dedicated professional alongside the parents and children at all times during the treatment. [...] try to converse with the parents and say: look, this is what’s happening, and we’re here, we’re trying. We shall not give up. All the children here are seriously ill, but we try right to the very end (N4).

So as to assure the family that the care given is adequate, the nurses perceive themselves encouraging the family to have faith and hope in the treatment and in the professionals. Thus, the nurse helps in strengthening the professional-family bond and in maintaining the faith and trust.

Try to understand what is happening in their lives [the family] and converse, bring it toward faith, for those who believe [...] speak about the medical and nursing teams, which are very good, which are experienced, and try to give peace of mind that here he will be treated in the best way (N1).

Cultivating one’s own spiritual practices and transpersonal self, going beyond ego self In this process, Watson emphasizes the need to experience emotions and feelings in caring relationships, and further reveals that for there to be

É essa sensibilidade que faz o contato transcender o mundo físico/material, para integrar também o mundo emocional e subjetivo. O enfermeiro acredita que mostrar sensibilidade aos sentimentos das crianças e familiares, reflete a preocupação do profissional em compartilhar esses sentimentos para ter empatia e integrá-los no cuidado como um processo humano, que requer do profissional a habilidade de escutar.

quando o pai quer conversar, quer falar dos problemas da criança. Mesmo nesse momento você ouve; às vezes até você chora junto (Enf. 6).

Saber escolher o momento certo exige do enfermeiro a sensibilidade para identificar quando um momento não é adequado para a realização de um determinado procedimento, especialmente por se tratar de uma criança. Ao mesmo tempo, percebe a necessidade de impor limites à criança, com uma certa tolerância, para não prejudicar a sua saúde durante o tratamento.

Tem que buscar o melhor momento [...]. Tinha uma criança em fase terminal aqui e a gente ia puncionar o cateter, quando ele disse: ‘ah tia, espera só um pouquinho?’ Então eu falei: tá bom, depois a tia tem que colocar o remédio e puncionar o seu cateter, tá bom? Depois eu esperei uns 30 minutinhos, fiquei lá, conversei, e ele me olhou: ‘agora pode, tia’. Então fui lá, puncionei e ele ficou tranquilinho para tomar a medicação (Enf. 6).

Desenvolver e sustentar uma autêntica relação de cuidado, de ajuda e confiança Para desenvolver a relação de ajuda e confiança, alguns elementos auxiliam para estabelecer uma interação harmoniosa e cuidadosa. A congruência trata-se de uma das características que ajudam a construir um relacionamento verdadeiro, em que o enfermeiro deve agir de maneira honesta e aberta. Você conversa antes, explica o que vai acontecer [...] pode ser que doa ou que não! Nunca mentir! A gente que trabalha com criança percebe isso rápido [...]. Assim a gente consegue fazer com que ele entenda e ajude durante o procedimento. Isso é humanizar, é atenção, conversar, ouvir (Enf. 6).

At the same time, the nurse perceives the necessity to impose limits for the child, with a certain tolerance, so as not to harm her health during the treatment.

You have to wait for the right time [...]. There was a child in the terminal phase here, and we were going to puncture the catheter, when he said: ‘oh nurse, can it wait a little bit?’ So I said: ‘okay, I’ll put the medicine in and puncture the catheter later, okay?’ Then I waited about 30 minutes, stayed there chatting, and he gave me a look: ‘now you can, Nurse’. So I went there, punctured, and he calmed down to take his medication (N6).

Developing and sustaining helping-trusting, authentic caring relationships To develop a helping-trusting relationship, some elements help to establish a caring and harmonious interaction. Consistency is one of the characteristics which help to construct a true relationship, in which the nurse must act honestly and openly. Establishing a relationship with sincerity to have trust is how the professional exercises consistency with the child, going beyond productive work, because he can transcend the rigidity of the function’s expectations.

confiança é a empatia, explicada como a habilidade do enfermeiro para experenciar o universo privativo do outro, e comunicar-lhe algum grau significativo de compreensão. O enfermeiro se percebe olhando para as coisas do ponto de vista da criança e da família, como forma de expressar a empatia, fundamental para o cuidado humanizado.

Eu sempre me coloco no lugar do outro para tudo o que eu faço, não só no trabalho, mas para tudo [...]. A gente tenta entender tudo o que o paciente está passando, para resolver da melhor forma possível (Enf. 7).

A habilidade de reconhecer o sentimento do outro é uma referência comum para a experiência emocional. Ainda assim, ninguém pode passar completamente pela experiência do outro, mas algum grau de dor, raiva, tristeza, culpa e prazer, uma pessoa já pode ter sentido alguma vez, e isso embasa uma referência para desenvolver essa sensibilidade, o respeito e a estima.

Outra característica nessa relação de ajuda e confiança é a aceitação positiva incondicional, que representa aceitar de forma positiva o outro, por meio de pequenos detalhes como o sorriso, a postura do corpo, o toque carinhoso e a capacidade de ouvir. Valorizando a comunicação interpessoal representa o movimento do enfermeiro para estabelecer vínculo com a família, com habilidade comunicacional, e realizar um cuidado humanizado, assegurando as necessidades dos pacientes.

Para o cuidado humanizado, a conversa, a comunicação interpessoal é muito importante. Porque, às vezes, dez, vinte minutos conversando, explicando alguma coisa, consolando, ou mesmo nem falando nada, só ouvindo, já é, para outra pessoa que precisa desabafar, muito, né? [...].

Bastante carinho e paciência para ouvir, eu acho muito importante (Enf. 5).

Esse envolvimento que aproxima o profissional da criança e de sua família, nas situações de câncer, é fruto da construção de uma relação de ajuda e confiança que envolve empatia, congruência e aceitação positiva incondicional e consolida uma referência também nos momentos mais frágeis.

Estando disponível ao lado da família nos momentos difíceis representa o envolvimento com os sentimentos dos familiares e a relação próxima estabelecida pelos enfermeiros, como parte do processo de humanização. [...] tinha morrido a paciente e [...] eu sabia que não ia vê-la ali naquele momento [no velório], ela não ia ver que eu estava lá, mas eu senti a necessidade de dar um conforto para a mãe dela (Enf. 2).

Estar presente e apoiar a expressão de sentimentos positivos e negativos, como uma conexão profunda com o espírito do ser e do ser que cuida do outro A aceitação e a expressão de sentimentos deve estar presente para manter um canal de comunicação entre as pessoas. A não expressão daquilo que se sente leva à inconsistência entre pensamentos e sentimentos, o que pode resultar em ansiedade, estresse, confusão, agressividade e até mesmo medo. Dar apoio e respeitar a criança e a família ao expressarem sentimentos de angústia representa o esforço do enfermeiro para compreendê-los e apoiá-los quando apresentam comportamentos de angústia ou raiva. Valorizar sentimentos e expressões de alegria refere-se à importância do enfermeiro enfatizar sentimentos positivos manifestados pela criança e família.

Tem sempre que entender mesmo quando você tem uma criança que chuta e xinga. Você não sabe o que é [...]. Você vê que a mãe está sofrendo: ‘vocês estão julgando meu filho e não é assim. Ele está doente, a gente não sabe o que fazer com ele’ (Enf. 4).

Os sentimentos justificam pensamentos e comportamentos e, portanto, quem cuida deve estar atento aos sentimentos do outro na manutenção da OM 14

Another essential condition to develop a helping-trusting relationship is empathy, explained as the nurse’s skill in experiencing the other’s private world and communicate to them some degree of comprehension. The nurse notices herself seeing things from the point of view of the child and the family, as a way of expressing empathy, which is fundamental to humanized care.

I always put myself in the position of the other in everything I do, not just at work but for everything [...]. We try to understand everything the patient is going through, to resolve things in the best way possible (N7).

The skill of recognizing the other’s feelings is a common benchmark for the emotional experience. Even so, nobody can go completely through the other’s experience; but people have felt some degree of pain, anger, sadness, guilt and pleasure at some time or other, and this is a base for a reference for developing this skill, respect and esteem.

Another characteristic in this helping-trusting relationship is unconditional positive acceptance, which means accepting the other in a positive way, through small details such as a smile, body posture, caring touch and the ability to listen. Valuing interpersonal communication means the nurse moving to establish a link with the family, with communicational skill, and carrying out humanized care, ensuring the patients’ needs.

For humanized care, the conversation, the interpersonal communication is very important. Because, sometimes, ten, twenty minutes talking together, explaining something, consoling, or even just saying nothing, just listening, for the other person who needs to let it out, that’s a lot, you know? [...]. Enough tenderness and patience to listen, I think that’s very important. (N5).

This involvement, which brings the professional closer to the child and the family, in cancer situations, is the fruit of the construction of a

saúde e na resposta das pessoas à situação de doença, como forma de conexão profunda com o espírito do ser cuidado e do ser que cuida do outro.

Uso criativo do ser, de todas as formas de conhecimento e do ser/fazer, como parte do processo de cuidado para engajar na arte da prática do cuidado e proteção A resolução criativa de problemas deve aliar conhecimento e intuição como parte do processo de cuidar. O método científico para a prática de enfermagem tem igual valor à natureza fundamental da enfermagem (cuidar do outro), que envolve a abordagem humanística. Considerar o universo peculiar da criança para entendê-la, reflete a preocupação de praticar um cuidado voltado para as necessidades da criança, considerando aspectos particulares do desenvolvimento, para melhorar a compreensão e a interação.

Para a criança tudo é novo, tudo é surpresa, mas é só naquele momento também, né? Quando você vai fazer uma punção, é sofrido para ela naquele momento. Depois de cinco minutos ela está pulando! Então, tem que entrar nesse mundo também, que é um mundo que a gente viveu (Enf. 4).

Outro significado identificado nos discursos foi o uso de ferramentas lúdicas para interagir com a criança, e tem semelhança com o que foi colocado anteriormente, já que em ambos notamos a preocupação do profissional em prover uma abordagem diferenciada para a criança ao considerar um cuidado humanizado. Os enfermeiros descrevem estratégias que utilizam para se aproximar do mundo da criança.

Acho que isso é humanizar o cuidado e a gente faz isso com coisas corriqueiras mesmo, deixando para eles darem injeção na gente, fingir que estão puncionando a veia, e tornar isso um pouco mais tranquilo para eles (Enf. 3).

Being available at the family’s side in difficult times means involvement with family members’ feelings and the closer relationship established by the nurses as part of the humanization process. [...] the patient had died and [...] I knew that I wasn’t going to see her there, at that time [at the wake], she wasn’t going to see that I was there, but I felt the need to comfort her mother (N2).

Being present to, and supportive of, the expression of positive and negative feeling The acceptance and expression of feelings must be present to maintain a communication channel between people. Not expressing that which one feels leads to inconsistency between thoughts and feelings, which can result in anxiety, stress, confusion, aggressivity and even fear. Giving support to, and respecting, the child and family represents the nurse’s efforts to understand them and support them when they present behaviors of distress or anger. Valuing feelings and expressions of happiness refers to the importance of the nurse’s emphasizing positive feelings manifested by the child and family.

You must always understand, even when you have a child who kicks and swears. You don’t know what is going on [...]. You see that the mother is suffering: ‘you are judging my son and that’s not how it should be. He is ill, and we don’t know what to do with him’ (N4).

The feelings explain thoughts and behaviors and, therefore, the one who is caring must be attent to the feelings of the other in the maintenance of health and in people’s responses to the situation of the disease, as a way of profound connection with the spirit of the person being cared for and of the being who cares for the other.

Engajar-se numa experiência genuína de ensino-aprendizagem que atenda à unidade do ser na tentativa de significar o estar-junto-com a estrutura referencial do outro O enfermeiro deve se comprometer de maneira genuína em uma experiência prática de ensino-aprendizagem. Oferecer ao paciente informações necessárias para compreender o tratamento, representa o papel do profissional como educador na tentativa de fazer a criança e a família entenderem cada aspecto relacionado ao tratamento, diminuindo a ansiedade e o estresse decorrente das incertezas e dúvidas. Para o enfermeiro, é necessário promover encontros educativos e discussões com a equipe, para realizar discussões sobre o tratamento, e também sobre aspectos relacionados ao processo de humanização entre os membros da equipe.

Tem que existir discussões dentro da instituição de trabalho [...] para todo mundo crescer junto, porque senão é como eu te falei: fica à critério pessoal; cada um faz o que acha melhor, e isso não tem nada de científico (Enf. 3).

Criar um ambiente protetor em todos os níveis, onde se está consciente do todo, da beleza, do conforto, da dignidade e da paz Esse processo está relacionado com o ambiente proporcionado pelo enfermeiro para promover e restaurar a saúde e prevenir a doença. Em quase todos os depoimentos, esses aspectos surgiram como a necessidade de ter uma visão holística da criança e da família. Dessa forma, foi possível identificar a preocupação do profissional em olhar para o paciente como um ser humano completo, mais do que um problema de saúde.

Creatively using self and all ways of knowing as part of the caring processes; engaging in artistry of caring-healing practices The creative resolution of problems must ally knowledge and intuition as part of the caring process. The scientific method for nursing practice has a value equal to the fundamental nature of nursing (caring for the other), which involves the humanistic approach. Considering the child’s specific universe so as to understand her reflects the concern with practising care directed at the child’s needs, taking into account particular aspects of development, so as to improve understanding and interaction.

For the child, everything is new, everything is a surprise, but it’s only in that very moment, you know? When you are going to puncture them, it is suffered in that moment. Five minutes later they’re jumping around! So, you have to enter this world too, which is a world we experienced (N4).

Another meaning identified in the discourses was the use of ludic tools to interact with the child, and there is a similarity with what was described above, as in both we noted the professional’s concern in trying out a different approach for the child, in considering humanized care. The nurses describe strategies which they use to get closer to the child’s world.

I think that this is humanizing the care, and we do this with routine things, letting them give us injections, pretending they’re puncturing the vein, making this a little more tranquil for them (N3).

Engaging in a genuine teaching-learning experience that attends to wholeness and meaning, attempting to stay within the other’s frame of reference The nurse must be genuinely dedicated in a practical experience of teaching-learning. Offering the patient information which is necessary

possível, respeitando tanto a parte psicológica, espiritual, biopsicossocial, completo [...], sempre respeitando o paciente como um todo, e não como uma parte doente (Enf. 9).

Para Watson, essa é a grande diferença entre as perspectivas da ciência tradicional e da ciência humana. A primeira é normativa, reducionista, mecanicista, centrada no método e na doença do corpo físico. A ciência humana não considera o conhecimento como um produto, e sim, como um processo de descoberta, sendo ela transpessoal, metafísica, centrada no fenômeno e a doença vista como resposta às condições humanas pessoais.

Assistir as necessidades humanas conscientemente, com um cuidado humano essencial, o qual potencializa a aliança mente-corpo-espírito Esse processo está baseado em uma hierarquia de cuidados, considerando cada pessoa no contexto global. As necessidades hierárquicas foram compostas da seguinte forma: necessidades de sobrevivência, funcionais, integradoras e de busca de crescimento. Para Watson, o atendimento das necessidades de ordem inferior pode não auxiliar o ser humano em direção à autoatualização.

Tem que explicar o que vai acontecer, o que a criança pode ter, que cai cabelo, tem falta de apetite quando faz o tratamento, e cuidar disso também [...]. A gente explica, entende quando ela não quer comer, incentiva, mas não força, porque se ela não quer, não quer mesmo! (Enf. 8).

NURSING to understand the treatment represents the role of the professional as educator in the attempt to make the child and family understand each aspect related to the treatment, reducing the anxiety and stress resulting from the uncertainties and doubts. For the nurse, it is necessary to promote educational meetings and discussions with the team, to have discussions about the treatment, and also about aspects related to the process of humanization among the team members.

There have to be discussions in the work institution [...] for everybody to grow together, because otherwise it is how I told you: it comes down to personal criteria; each does what she thinks is best, and there’s nothing scientific about that (N3).

Creating healing environment at all levels, whereby wholeness, beauty, comfort, dignity and peace are potentiated This process is related to an environment made available by the nurse to promote and restore health and prevent disease. In nearly all the discourses, these aspects appear as a necessity to have a holistic view of the child and family. In this way, it was possible to identify the professional’s concern in seeing the patient as a complete human being, rather than as a health problem.

Humanization, for me, means treating the patient in the best way possible, respecting equally the psychological, spiritual and bio-psychosocial parts, complete [...], always considering the patient as a whole, and not as a sick part (N9).

For Watson, this is the big difference between the perspectives of traditional science and of human science. The first is normative, reductionist, mechanistic, centered in the method and in the illness of the physical body. Human science does not consider the knowledge as a product but rather as a process of discovery, it being transpersonal, metaphysical, centered on the phenomenon and the illness seen as a response to personal human conditions.

Assisting with basic needs, with an intentional caring consciousness, administering ‘human care essentials’ which potentiate alignment of mind-body-spirit, wholeness in all aspects of care

Tampouco os enfermeiros foram estimulados a falar sobre o assunto, já que não foi explorado esse aspecto dentro dos objetivos do estudo. Porém, o enfermeiro valoriza o vínculo para o reconhecimento do próprio self, da criança e da família, e também como fortalecimento no trabalho profissional.

Nesse processo, tentar conhecer a experiência do outro é fundamental.

Teve aquele apego [...] a gente [equipe e família] é uma segunda família, ao mesmo tempo que ele [criança] foi a óbito, esse sentimento criado foi algo que me conforta e me dá ânimo pra continuar a trabalhar (Enf. 8).

Apesar disso, parece-nos que falta habilidade para ajudar o paciente e a família a explorar os significados que dão à experiência de doença e morte, na dimensão espiritual dentro do contexto da oncologia.

A percepção do enfermeiro sobre o cuidado humanizado traz elementos fundamentais para as relações humanas e estão consistentes com os processos caritas de Jean Watson, bem como com a discussão acerca do cuidado e humanização, que vem crescendo com a Política Nacional de Humanização (Humaniza/SUS), enfocando aspectos como: a individualidade das pessoas, a escuta atenta, a valorização das crenças, da comunicação e a presença genuína. Considerando os relacionamentos humanos como manifestação de subjetividades, esses ingredientes fazem parte da sensibilização dos profissionais e das instituições para enraizar o aspecto existencial e humano nos encontros de saúde. Sendo assim, o foco de um constante trabalho de conscientização das equipes é necessário, incluindo também um cuidado com o profissional, para o atendimento da criança e da família, visto que as rotinas hospitalares são rígidas e ainda se distanciam das necessidades das famílias.

This process is based in a hierarchy of care, considering each person in the global context. The hierarchical needs were composed in the following way: survival needs, functional needs, integrative needs and those of search for growth. For Watson, attending the lower-order needs may not help the human being in regard to self-realization.

You have to explain what’s going to happen, what might happen to the child, that hair falls out, that she’ll lose her appetite when she does the treatment, and care for this too [...]. We explain, we understand when she doesn’t want to eat, we encourage, but we don’t force, because if she doesn’t want to, she really doesn’t want to! (N8).

The pediatric oncology nurses express the need for a complete attendance, going beyond functional or integrative needs - they present holistic thinking for the care of the child and family.

Opening and attending to spiritual/mysterious and existential unknowns of life-death In spite of the spiritual aspect’s importance for caring practice, this element did not emerge spontaneously in the discourses obtained. The nurses were not encouraged to talk about this matter, because it was not explored among the study’s objectives. However, the nurse values the link for the recognition of self, of the child and the family’s self, and also as strengthening in the professional work. In this process, it is fundamental to try to investigate the experience of the other.

There was that attachment [...] we [team and family] are a second family, when he [the child] died, this feeling created was something which comforts me and gives me energy to continue the work (N8).

In spite of this, it seems to us that there is a lack of skill for helping the patient and the family to explore the meanings which they give to the experience of illness and death, in the spiritual dimension within the context of oncology.

The nurse’s perception on humanized care brings fundamental elements for human relationships and is consistent with Jean Watson’s caritas processes, as well as with the discussion on care and humanization, which is growing, with the National Humanization Policy (Humaniza/ Sistema Único de Saúde - SUS), addressing aspects such as: people’s individuality, attentive listening, the valuing of beliefs, of communication and of genuine presence. Considering human relationships as manifestations of subjectivities, these ingredients form part of the sensitization of professionals and of the institutions to entrench the existential and human aspect in the health encounters. This being the case, the focus of a constant work of consciousness-raising of the teams is necessary, for attending the child and family, given that the hospital routines are rigid and remain distant from the families’ needs.

Um referencial teórico como a teoria do cuidado humano de Jean Watson, para a prática do cuidado prestado às crianças e famílias que enfrentam o câncer, é um norteador para a prática do enfermeiro e importante recurso para extrapolar a rigidez das funções que engessam o encontro de cuidado, promovendo, assim, um diálogo mais sensível às necessidades humanas físicas, biológicas, psicológicas, emocionais e espirituais.

Considerações Finais A Teoria de Jean Watson nos leva a pensar na existência do ser, no amor como manifestação de cuidado que transcende o aspecto físico desse processo. Ao utilizarmos os dez elementos que compõem o processo clinical caritas, entendemos que o enfermeiro precisa se arriscar a realizar uma análise, uma transformação em seu próprio íntimo, para que seja possível a promoção do cuidado humanístico.

Esse cuidado envolve o fortalecimento do vínculo entre o profissional, a família e a criança. Alguns sentimentos precisam estar evidentes, como carinho, amor e respeito pelo outro e pela profissão. Da mesma forma, é necessário empenhar-se para estabelecer um relacionamento com empatia e criatividade, encorajar a fé e a esperança no tratamento, agir com sensibilidade e flexibilidade na abordagem com a criança, aceitar a expressão de sentimentos, investir na comunicação e realizar reuniões com a equipe para pensar no cuidado oferecido, a fim de assegurar da melhor forma as necessidades da família. Sendo assim, o enfermeiro exibe um papel fundamental para garantir que o encontro de cuidado envolva consciência e sensibilidade na interação com o outro.

A theoretical framework such as Jean Watson’s Theory of Human Caring, for the practice of the care given to the children and families who face cancer, can guide the nurse’s practice. This is an important resource to go beyond the rigidity of the functions which stifle the care encounter, thus promoting a dialog which is more sensitive to the physical, biological, psychological, emotional and spiritual human needs.

Final Considerations Jean Watson’s theory leads us to think about the existence of self, about the love as a manifestation of care which transcends the physical aspect of this process. In using the ten elements which make up the clinical caritas process, we understand that the nurse needs to risk him- or her-self to undertake an analysis, a transformation within, for the promotion of humanistic care to be possible.

This care involves strengthening the link between the professional, the family and the child. Some feelings need to be evident, such as tenderness, love and respect for the other and for the profession. In the same way, it is necessary to commit oneself to establishing a relationship with empathy and creativity, to encourage faith and hope in the treatment, to act with sensitivity and flexibility in the approach with the child, to accept the expression of feelings, to invest in communication and to hold meetings with the team to think about the care offered, so as to ensure the family’s needs in the best way. This being so, the nurse displays a fundamental role to guarantee that the caring encounter involves consciousness and sensitivity in interactions with the other.

ENFERMAGEM O enfermeiro que vivencia o contexto da oncologia pediátrica conhece elementos humanos essenciais para tornar o cuidado mais próximo do cliente, tal como preconiza a Teoria de Watson, porém pouco se apropria de referenciais para o cuidado. Assim, torna-se mais difícil conciliar estratégias formais e embasadas para ajudar no sofrimento de crianças e famílias. Essa teoria, por se aproximar da percepção de cuidado humanizado trazida pelos enfermeiros, pode ser um caminho para que esses profissionais desenvolvam intervenções no atendimento das necessidades e ajustamentos da família à doença.

A subjetividade das ações de cuidado envolve um olhar para as interações humanas, que estão em constante mudança, exigindo preparo e reflexão crítica dos profissionais que promovem os momentos de cuidado.

Este estudo abre essa possibilidade a partir do conhecimento da percepção e da vivência do enfermeiro no cuidado às crianças e às famílias que vivem a experiência do câncer. Trata-se de um convite à reflexão e uma proposta de mudar o jeito de pensar enfermagem, através da valorização e da percepção do outro e de si mesmo na relação de cuidado.

O décimo processo clinical caritas - A permissão de forças existentes/ fenomenológicas espirituais - nos parece relevante para o contexto da oncologia, no qual os enfermeiros lidam com as questões de vida e morte.

Segundo Watson, se os enfermeiros não forem capazes de olhar para o seu interior e encarar suas próprias questões relacionadas às perdas, não serão capazes de ajudar o outro a descobrir o significado evento difícil que vivenciam. Assim, consideramos esse processo preponderante para o verdadeiro cuidado humanístico, especialmente nos contextos em que precisamos lidar com pessoas no processo de morrer.

“Desvelando o cuidado humanizado: percepções de enfermeiros em oncologia pediátrica” representa o exercício intelectual realizado no percurso deste trabalho, de aprofundar o conhecimento na Teoria do Cuidado Humano de Jean Watson e analisar os depoimentos tendo como base este referencial. Este estudo trouxe-nos um novo conhecimento em relação ao cuidado e que, sem dúvida, trará grandes benefícios aos nossos futuros clientes e famílias, alvo das ações de cuidar.

The nurse who experiences the context of pediatric oncology knows essential human elements for bringing the care closer to the client, as Watson’s Theory calls for, but little appropriates benchmarks for the care. Thus, it becomes more difficult to conciliate formal and grounded strategies to help with the suffering of children and families. This theory, because it comes close to the perception of humanized care brought by the nurses, may be a path for these professionals to develop interventions in attending the families’ needs and their adjustments to the disease.

The subjectivity of the care actions involves focussing on the human interactions, which are in constant change, requiring the professionals who promote the care to have preparation and critical reflection. This study opens this possibility based on the knowledge of the perception and experience of the nurse in the care for the children and the families who live through the experience of cancer. It is an invitation to reflection and a proposal to change the way of thinking about nursing, through the valuing and the perception of self and other in caring relationship.

The tenth clinical caritas process - Opening and attending to spiritual/ mysterious and existential unknowns of life-death - seems relevant to us for the context of oncology, in which the nurses deal with the questions of life and death. According to Watson, if the nurses are not capable of looking within themselves and facing their own questions related to the losses, they will not be capable of helping the other to discover the meaning of the difficult event which they are experiencing. Thus, we consider this process to be overriding for true humanistic care, especially in the contexts in which we need to work with people who are in the process of dying.

“Unveiling humanized care: nurses’ perceptions in pediatric oncology” represents the intellectual exercise undertaken in the course of this work, of deepening the knowledge in Jean Watson’s Theory of Human Caring and analyzing the statements using this framework as a base. This study brought us a new knowledge in relation to care and will, without doubt, bring great benefits to our future clients and families, the target of our caring actions.

Promoção, prevenção e cuidado da hipertensão arterial no Brasil.

Promotion, prevention and arterial hypertension care in Brazil.

elevação da pressão arterial é um sinal de manifestação de doença específica na hipertensão arterial (HAS) secundária, que responde por 5% dos casos. Nos casos de HAS primária ou essencial, é sinal de que um conjunto de fatores compromete o sistema cardiovascular e responde pelos demais 95% dos casos de HAS. Nesses casos, sua manifestação e severidade são influenciadas por fatores como quantidade de sal na dieta, padrão de atividade física, controle do peso corporal, tabagismo e comorbidades, como o diabetes mellitus. Assim, a abordagem dessa condição exige ações de promoção à saúde, prevenção de agravos e cuidado do estado clínico. Diretrizes ordenando essas iniciativas estão bem estabelecidas e divulgadas.

A qualidade do cuidado em HAS tem sido avaliada por meio de diferentes metodologias e indicadores. Estudo recente aponta como indicadores de qualidade na abordagem de HAS a medida da pressão arterial pelo menos uma vez ao ano e o fornecimento de orientação sobre cuidado com o peso corporal, atividade física e dieta pobre em sal.

Para avaliar a atenção dedicada à HAS em pessoas adultas no Brasil consideraram-se as diretrizes recomendadas por política nacional

he increase in blood pressure is a sign of manifestation of a specific disease in secondary hypertension (HTN), corresponding to 5% of cases. In the case of primary or essential HTN, it is a sign that a set of factors is affecting the cardiovascular system, corresponding to the remaining 95% of HTN cases. In these cases, its manifestation and severity are influenced by factors such as the amount of salt in the diet, physical activity pattern, body weight control, smoking and co-morbidities such as diabetes mellitus. Thus, health promotion actions, disease prevention and clinical care are required to approach this condition. The directives that guide these initiatives are well-established and widely disseminated.

Quality of HTN care has been evaluated using different methodologies and indicators. A recent study pointed to the following quality indicators to approach HTN: measuring blood pressure at least once a year and providing guidance on body weight control, physical activity and a low-salt diet.

The directives recommended by the Brazilian national policy and by authors who represent the British Hypertension Society were considered to assess HTN care for adults in Brazil. A previous study found a significant

association between diastolic blood pressure and the variables that represent the care process and concluded that, despite the efforts made to develop methods that assess medical care quality, none of them was fully satisfactory. Care provided by the same doctor throughout time is a variable significantly associated with compensated blood pressure.

Capacity building and sensitization of primary health care professionals increase the adoption of practices such as guidance on risk factors and assessment of target HTN services, variables considered to be health care qualifiers. The present study aimed to estimate the prevalence of promotion, prevention and hypertension care actions for adults in Brazil and to identify its association with decompensated hypertension.

Methods

Estudo anterior encontrou associação significativa entre pressão arterial diastólica e variáveis que representam o processo do cuidado e concluiu que, a despeito dos esforços para desenvolver métodos para avaliar a qualidade do cuidado médico, nenhum foi plenamente satisfatório. O cuidado ao longo do tempo, pelo mesmo médico, é uma variável associada de maneira significativa com a pressão arterial compensada. A capacitação e sensibilização dos profissionais da atenção primária em saúde ampliam a adoção de práticas como orientação sobre fatores de risco e avaliação de órgãos-alvo da HAS, variáveis consideradas como qualificadoras do cuidado. Este estudo teve por objetivo estimar a prevalência de ações de promoção, prevenção e cuidado da hipertensão arterial em adultos no Brasil e identificar sua associação com estado descompensado de hipertensão.

Métodos Realizou-se um inquérito epidemiológico transversal de base populacional, de 2008 a 2009. A população-alvo foi representativa da população brasileira, composta por adultos de 20 a 59 anos, moradores em domicílios particulares na zona urbana de 100 municípios brasileiros.

A amostra foi obtida em processo complexo em múltiplos níveis: a) população em cinco categorias: 1) < 10 mil habitantes; 2) 10 mil a < 20 mil habitantes; 3) 20 mil a < 100 mil habitantes; 4) 100 mil a < 1,1 milhão habitantes; e 5) mais de 1,1 milhão de habitantes; b) setores censitários; e c) domicílios. Para a seleção dos municípios e setores censitários urbanos adotou-se a divisão territorial e a malha setorial utilizada no Censo Demográfico 2000, do Instituto Brasileiro de Geografia e Estatística. O número de setores sorteados em cada município foi proporcional ao número de

A population-based cross-sectional epidemiological study was conducted between 2008 and 2009. The target population was representative of the Brazilian population, comprised of adults aged between 20 and 59 years, living in private homes in the urban area of 100 cities in Brazil.

The sample was obtained in a complex process on multiple levels: a) population in five categories: 1) < 10,000 inhabitants; 2) 10,000 < 20,000 inhabitants; 3) 20,000 < 100,000 inhabitants; 4) 100,000 < 1.1 million inhabitants; and 5) more than 1.1 million inhabitants; b) census tracts; and c) households. The territorial division and network of census tracts used in the 2000 Demographic Census of the Instituto Brasileiro de Geografia e Estatística (IBGE - Brazilian Institute of Geography and Statistics) was adopted to select cities and urban census tracts. The number of sectors selected in each city was proportional to the number of valid sectors and population size.

Individuals who were not legally free or who lived in long-term institutions were excluded from this study.

The parameters for the association tests used a power of 80%, 95% confidence level and ratio of exposure of 1:4; prevalences of outcome from 15% to 60% in the non-exposed group, prevalence ratio of 1.3 and an additional 10% for losses and 15% for confounding factors. Considering the design effect of 2.4, a sample comprised of 10,200 adults was estimated to be sufficient to meet the needs.

The definition of the number of individuals to be located in each urban census tract (standard unit of population group) considered a mean concentration of 1.94 adults aged from 20 to 59 years per urban household. Households were selected in all sectors by systematically ignoring 30 households with a random beginning. All eligible adults were studied in each household.

The inclusion criterion for the category of participant having HTN was to have received a medical diagnosis of this condition, whose predictive positive value is 73.9%, when compared to the diagnosis performed by measuring blood pressure, considered to be the gold standard. The mean to define the outcome was obtained in different moments during the interview using two blood pressure measurements made with a Geratherm Wristwatch Medical AG blood pressure monitor, considering mean blood pressures higher than 140/90 mmHg to be decompensated.

Sex, age, place of residence, economic classification and level of education were used in the analysis of quality of care behavior. The independent variables considered as promotion, prevention and care were as follows: to have consulted a doctor in the previous year, to have received guidance on ideal weight maintenance; to have received guidance on physical activity; and to have had an electrocardiogram (EKG) performed in the previous year. Foram excluídos os indivíduos que no momento da entrevista estivessem privados de liberdade por decisão judicial, ou residindo em instituições de longa permanência.

Os parâmetros para os testes de associação utilizaram um poder de 80%, nível de 95% de confiança e razão de exposição de 1:4; prevalências de desfecho de 15% a 60% no grupo não exposto, razão de prevalência de 1,3 e acrescentando 10% para perdas e 15% para fatores de confusão. Considerando um efeito de delineamento de 2,4, foi estimada como necessária e suficiente uma amostra de 10.200 adultos.

A definição do número de indivíduos a localizar em cada setor censitário urbano (unidade padrão de agregação populacional) considerou uma concentração média de 1,94 adulto de 20 a 59 anos por domicílio urbano. Os domicílios foram selecionados em todos os setores segundo pulo sistemático de 30 domicílios com início aleatório. Em cada domicílio foram estudados todos os adultos elegíveis.

O critério de inclusão na categoria de portador de HAS foi ter recebido um diagnóstico médico dessa condição, cujo valor preditivo positivo é de 73,9%, quando comparado ao diagnóstico realizado mediante aferição da pressão arterial, tomado como padrão-ouro. Por meio de duas medidas de pressão realizadas com aparelho de pressão Geratherm Wristwatch Medical AG, obtidas em momentos diferentes durante a entrevista, foi calculada a média para a definição do desfecho, adotando como estado descompensado da pressão arterial os que apresentassem médias de pressão arterial superiores a 140/90 mmHg. Sexo, idade, região de residência, classificação econômica e escolaridade foram utilizados na análise do comportamento da qualidade do cuidado. As variáveis independentes, consideradas como promoção, prevenção e cuidado, foram: haver consultado no último ano, ter recebido orientações sobre a manutenção do peso ideal, orientação sobre atividade física e realização de eletrocardiograma (ECG) no último ano. Pressão arterial acima de 140/90 mmHg foi considerada estado descompensado da pressão arterial, sendo a variável dependente para a avaliação da qualidade do cuidado O autorrelato de ser hipertenso baseado em diagnóstico médico informado foi validado em estudo recente por autores brasileiros. Self-reporting hypertension based on an informed medical diagnosis was validated by Brazilian authors in a recent study. Likewise, the variables adopted to indicate health promotion, prevention and care were similar to those used by authors in North America in the last decade as well.

In addition to the questionnaire application and blood pressure measurements, weight (Geratherm digital scale) and height (WISO T87 measuring tape) were assessed, thus enabling objective and standardized measurements. All pieces of equipment were of proven quality and their use was authorized by the Agência Nacional de Vigilância Sanitária (Anvisa - National Health Surveillance Agency) and Instituto Nacional de Metrologia, Qualidade e Tecnologia (Inmetro - National Institute of Metrology, Quality and Technology).

Data were recorded electronically in a Personal Digital Assistant (PDA).

At the end of each day of research, PDA data were stored into a portable computer and subsequently sent to the server located at the research headquarters via Internet. This technology also enabled participants’ homers to be located by Global Positioning System (GPS), used as field work quality control. Data were stored and analyzed in Stata 10.0.

Além da aplicação do questionário e das medidas de pressão arterial, foram realizadas medidas de peso (balança digital Geratherm) e estatura (trena Modelo WISO T87), o que possibilitou aferições objetivas e padronizadas. Todos os equipamentos eram de qualidade comprovada e seu uso autorizado pela Agência Nacional de Vigilância Sanitária e Instituto Nacional de Metrologia, Qualidade e Tecnologia.

Os dados foram registrados eletronicamente em computador de mão (Personal Digital Assistant - PDA). Ao final do dia de trabalho os dados do PDA eram armazenados em um computador portátil e transmitidos pela Internet para o servidor na sede da pesquisa. Essa tecnologia também possibilitou a localização do domicílio do entrevistado por sistema de posicionamento global, utilizado como controle de qualidade do trabalho de campo. Os dados recebidos foram armazenados e analisados no Stata 10.0.

A análise bivariada foi realizada pela comparação de proporções, utilizando-se o teste do qui-quadrado com nível de 5% de significância.

Razões de prevalência brutas e ajustadas com intervalos de 95% de confiança foram calculadas por meio de regressão de Poisson, de acordo com modelo de análise hierarquizado (Figura). No nível mais distal foram incluídas as variáveis: sexo, idade, região geográfica, classe econômica e escolaridade. No segundo nível as variáveis: consultar por HAS e receber orientações de promoção à saúde, e no terceiro nível: ter realizado ECG. O estudo foi aprovado pelo Comitê de Ética em Pesquisa da Universidade Federal de Pelotas, de acordo com o protocolo número 152/07, de 23 de novembro de 2007. Todos os entrevistados assinaram termo de consentimento livre e esclarecido.

the chi-square test with a 5% significance level. Crude and adjusted prevalence ratios with 95% confidence intervals were calculated using Poisson regression, according to a hierarchical analysis model (Figure).

The following variables were included at the most distal level: sex, age, geographic region, socioeconomic class and level of education; at the second level: to have consulted a doctor about HTN and to have received guidance on health promotion; and at the third level: to have had an EKG performed.

This study was approved by the Research Ethics Committee of the Universidade Federal de Pelotas, according to protocol number 152/07 of November 23rd, 2007. All participants signed an informed consent form.

Results A total of 12,324 adults were interviewed in the 100 cities of the sample, of which 16.3% (2,004) reported a medical diagnosis of HTN. There were no differences in the distribution of hypertensive individuals by sex among Brazilian regions. The distribution by age showed higher proportions of hypertension in the 50 to 59 year age group, concentrated in the Southeast and Central-West regions, whereas the lower frequency of HTN in this age group was found in the Northeast region (40.7%).

Almost half of the sample of Brazilians with hypertension belonged to socioeconomic class C; a higher proportion of individuals of classes D and E in the North and Northeast regions contrasted with the higher proportion of individuals of classes A and B in the South, Southeast and CentralWest regions. The proportion of individuals with hypertension who had no formal education in the Northeast region was higher than that found in the remaining regions, followed by the North and Central-West regions.

In the latter region, the lowest proportion of adults with hypertension who had not completed one year of school was observed.

Foram entrevistados 12.324 adultos nos 100 municípios amostrados, dos quais 16,3% (2.004) referiram diagnóstico médico de HAS. Não houve diferença na distribuição de hipertensos entre as regiões do Brasil por sexo. A distribuição por idade revelou maiores proporções de hipertensão na faixa etária de 50 a 59 anos, concentrada nas regiões Sudeste e Centro-Oeste, enquanto a menor frequência de HAS nessa faixa etária foi encontrada na região Nordeste (40,7%). Praticamente a metade da amostra de hipertensos brasileiros pertencia à classe socioeconômica C; uma proporção maior de pessoas das classes D e E, nas regiões Norte e Nordeste, contrastou com a proporção maior de pessoas das classes A e B nas regiões Sul, Sudeste e Centro-Oeste. A proporção de pessoas hipertensas com nenhuma escolaridade na região Nordeste foi superior à encontrada em todas as demais regiões, seguida pelas regiões Norte e Centro-Oeste. Nesta última região foi registrada a menor proporção de adultos hipertensos com nenhum ano de escolaridade completado.

Nessa amostra de 1.995 hipertensos, 1.318 (66,1%) consultou por hipertensão no último ano. Desses, 682 (52,4%) realizaram ECG para o monitoramento do coração como órgão-alvo da HAS. A pressão arterial de 1.495 hipertensos (74,6%) dessa amostra foi aferida neste estudo, da qual 634 (42,4%) apresentavam cifras tensionais acima dos limites normais, sendo considerados portadores de um “estado descompensado” da hipertensão.

A proporção de adultos portadores de HAS que consultou no último ano foi significativamente maior (p < 0,000) entre as mulheres (69,0%) do que entre os homens (61,5%). Estratificada por idade, a proporção de adultos que consultou no último ano foi de 59,9% na faixa de idade dos 20 aos 29 anos, de 56,8% dos 30 aos 39 anos, de 64,4% dos 40 aos 49 anos e de 72,1% nas pessoas com 50 anos e mais, diferenças essas significativas (p < 0,000). Esses resultados também mostraram diferenças regionais significativas (p= 0,03), mais frequentes nas regiões Norte (76,3%) e Centro-Oeste (70,5%), enquanto na amostra em geral foi inferior (66,1%). Among this sample comprised of 1,995 individuals with hypertension, 1,318 (66.1%) had consulted a doctor about HTN in the previous year.

Of these, 682 (52.4%) had had an EKG performed to monitor the heart as a target organ of HTN. The blood pressure of 1,495 participants with HTN (74.6%) of this sample was measured in this study, of which 634 (42.4%) had values higher than the normal limits and were thus considered to be decompensated.

The proportion of adults with HTN who had consulted a doctor in the previous year was significantly higher (p < 0.000) among women (69.0%) than men (61.5%). When stratified by age, the proportion of adults who had consulted a doctor in the previous year was 59.9% in the 20 to 29 year age group, 56.8% in the 30 to 39 year age group, 64.4% in the 40 to 49 year age group, and 72.1% in the 50 year and more age group, and these differences were significant (p < 0.000). In addition, the results of this study also showed significant regional differences (p= 0.03), with higher frequencies in the North (76.3%) and Central-West regions (70.5%), whereas the overall sample had a lower value (66.1%).

The stratification of this variable by socioeconomic classes did not show significant differences. When the stratification was performed by level of education in number of years of school completed, those with 12 or more years of school had consulted a doctor with a significantly higher frequency (p = 0.01) (72.0%) than the overall sample (66.0%).

The majority (87.2%) of adults with hypertension monitored their weight.

Almost half (45.4%) received guidance on ideal weight maintenance, of which 77.8% attempted to follow this guidance. One third of them (31.6%) reported having had problems with excessive weight in the previous year; an even higher proportion (38.5%) had a body mass index that indicated obesity.

Guidance on ideal body weight did not show significant difference in prevalence (p = 0.37) between men and women, nor among the different age groups (p = 0.10). However, the distribution of this guidance differed among regions (p = 0.001), with a lower frequency in the Northeast region (37.5%) and a higher frequency in the Central-West region (53.4%). This guidance was significantly (p < 0.000) more frequent in the socioeconomic classes A (66.7%), B (53.6%) and C (43.2%) than classes D (34.0%) and E (37.0%). A significant difference (p < 0.000) was also observed in the stratification by level of education, with lower values among those without

CARDIOLOGIA econômica não mostrou diferenças significativas. Quando a estratificação foi feita por escolaridade em anos completos, aqueles com 12 ou mais anos de escolaridade consultaram significativamente (p = 0,01) mais (72,0%) do que o conjunto da amostra (66,0%).

A maior parte (87,2%) dos adultos hipertensos monitorou seu peso.

Quase a metade (45,4%) recebeu orientação para manter o peso ideal, da qual 77,8% tentou seguir essa orientação. Um terço deles (31,6%) reconheceu ter apresentado problemas por excesso de peso no último ano; proporção ainda maior (38,5%) mostrou índice de massa corporal compatível com obesidade nas mensurações realizadas.

A orientação para manter o peso ideal não mostrou diferença significativa (p = 0,37) nas prevalências encontradas entre homens e mulheres, nem tampouco nas diferentes categorias de idade (p = 0,10). Entretanto, a distribuição dessa orientação diferiu entre as regiões (p = 0,001), sendo menos frequente na região Nordeste (37,5%) e mais frequente na região Centro-Oeste (53,4%). Essa orientação foi significativamente (p < 0,000) mais frequente nas classes econômicas A (66,7%), B (53,6%) e C (43,2%), do que nas classes D (34,0%) e E (37,0%). Diferença significativa (p < 0,000) também foi observada na estratificação por anos de escolaridade; menor entre aqueles sem nenhuma escolaridade (30,2%), crescendo nas categorias de um a quatro anos (42,9%), de cinco a oito anos (45,5%), de nove a onze anos (49,2%) e 12 anos mais (58,6%).

Embora quase a metade dos hipertensos (47,9%) tenha recebido orientação para realizar atividade física no último ano, quase dois terços dos advertidos (63,3%) não mudou seu padrão de atividade física.

Sedentarismo no deslocamento (94,3%) e no lazer (84,5%) foram os comportamentos mais frequentes.

A orientação para realizar atividade física foi menos frequente (p = 0,03) entre os homens (45,3%) quando comparados às mulheres (49,6%).

A prevalência dessa orientação diferiu significativamente (p = 0,005) entre as faixas etárias, de modo crescente: 20-29 anos (35,1%), 30-39 (45,4%), 40-49 (47,8%) e 50-59 anos (50,9%). A diferença dessa orientação nas regiões do Brasil também foi significativa (p = 0,03): menor no Norte (36,5%) e maior na região Centro-Oeste (52,5%). Essa orientação foi mais frequente (p < 0,000) nas classes econômicas A (70,3%) e B (60,5%) do que nas classes C (45,1%), D (35,3%) e E (51,9%). Diferença significante (p < 0,000) foi observada em relação à distribuição por escolaridade. As menores prevalências foram observadas entre aqueles com nenhum ano de escolaridade (32,6%), aumentando nas categorias seguintes: um a quatro anos (42,6%), cinco a oito anos (47,9%), nove a 11 anos (54,4%) e 12 e mais anos (63,1%).

A frequência de realização do ECG não mostrou diferença significativa segundo sexo (p = 0,308; homens 51,8% e mulheres 53,4%). Prevalências significativamente diferentes (p = 0,01) foram observadas entre as categorias de idade: 41,5% naqueles com 20-29 anos, de 46,9% dos 30-39 anos, de 56,2% dos 40-49 anos e de 53,9% de 50 anos e mais.

Esse monitoramento foi significativamente (p = 0,004) mais frequente na região Sudeste (58,5%) em relação à prevalência geral da amostra (52,4%). formal education (30.2%), which increased in the following categories: one to four years of school (42.9%), five to eight years (45.5%), nine to 11 years (49.2%) and 12 years or more (58.6%).

Although almost half of the individuals with hypertension (47.9%) had received guidance on physical activity practice in the previous year, nearly two thirds of them (63.3%) did not change their physical activity pattern. Sedentary habits in the transportation (94.3%) and leisure time domains (84.5%) were the most frequent types of behavior.

Guidance on physical activity practice was less frequent (p = 0.03) among men (45.3%), when compared to women (49.6%). The prevalence of this guidance differed significantly (p = 0.005) among age groups, increasing with age: 20-29 years (35.1%), 30-39 years (45.4%), 40-49 years (47.8%) and 50-59 years (50.9%). The difference in guidance among Brazilian regions was also significant (p = 0.03): lower in the North (36.5%) and higher in the Central-West region (52.5%). Such guidance was more frequent (p < 0.000) in socioeconomic classes A (70.3%) and B (60.5%) than classes C (45.1%), D (35.3%) and E (51.9%). A significant difference (p < 0.000) was observed with regard to the distribution by level of education (32.6%), which increased in the following categories: one to four years of school (42.6%), five to eight years (47.9%), nine to 11 years (54.4%), and 12 years or more (63.1%).

The frequency of EKG did not show significant differences according to sex (p = 0.308; 51.8% in men and 53.4% in women). There were no differences according to socioeconomic class. Discussão Discussion A prevalência de HAS descompensada neste estudo (42,4%) foi menor do que a encontrada em estudo realizado nos EUA (58%). A associação positiva identificada na análise bruta de menores proporções de cifras tensionais descompensadas com as orientações sobre: manutenção do peso ideal, realização de atividade física e a realização do ECG sugere que essas ações possam representar marcadores de cuidado clínico positivo da HAS. Na presente amostra, escolaridade se mostrou mais sensível para revelar diferenças no cuidado de hipertensão do que a classificação econômica, apesar do tecido social heterogêneo em relação à classe econômica e anos de escolaridade. Apenas a metade dos hipertensos realizou ECG no último ano, resultado semelhante a de estudo realizado na Arábia Saudita, com OM 30

The prevalence of decompensated hypertension in this study (42.4%) was lower than that found in a study conducted in the United States (58%).

The positive association between lower proportions of decompensated blood pressure values and ideal weight maintenance, physical activity practice and EKG identified in the crude analysis suggests that these actions may represent positive clinical care markers of HTN. In the present sample, level of education was more sensitive to the differences in HTN care than economic classification, despite the heterogeneous social composition in terms of socioeconomic class and level of education. Only half of the individuals with hypertension had an EKG performed in the previous year, a result similar to that found in a

Apenas metade da amostra recebeu orientações sobre manutenção do peso ideal e realizar atividade física, o que confirma dados de literatura e contribui para a perda de efetividade no manejo dessa situação de saúde.

Frequências relativamente baixas como essas guardam coerência com as prevalências ainda relevantes de obesidade e sedentarismo e, portanto, dignas da preocupação dos agentes públicos de saúde. Dentre o pequeno impacto dessas orientações nas modificações de atitude esperadas em cada uma delas, destaca-se a manutenção do peso ideal, pois ter recebido essa orientação esteve associado positivamente a menor prevalência de cifras tensionais descompensadas.

A inexistência de associação entre consultar com médico no último ano e cifras tensionais descompensadas foi um achado curioso e aparentemente contraditório com evidências publicadas anteriormente. Neste estudo, a variável independente foi “consultar com o mesmo médico” e no presente estudo “consultar com médico”, independentemente de ser o mesmo ou não, o que talvez possa representar diferencial na adesão a medidas recomendadas.

Merece atenção especial o resultado da análise múltipla em que as variáveis independentes que permaneceram associadas ao estado descompensado da pressão arterial foram ser do sexo masculino, ter idade entre 40 e 49 anos e 50 anos ou mais e morar na região Sul do Brasil. Cada variável representa um conceito sintético que, como tal, encerra diversos atributos, tornando inviável, com o delineamento adotado, identificar com precisão fatores passiveis de intervenção que reduzam a prevalência de pressão arterial descompensada.

Finalmente, são pequenas as ações de promoção e prevenção à saúde destinadas a essa população. São necessários novos estudos de abrangência nacional com delineamento apropriado para estudar, nesses componentes conceituais, as possíveis causas determinantes do estado descompensado da pressão arterial, como por exemplo, estudos multicêntricos de corte.

study conducted in Saudi Arabia that included 47% of participants with decompensated HTN and where the EKG was one of the procedures least frequently performed.

Only half of the sample received guidance on ideal weight maintenance and physical activity practice, thus confirming literature data and contributing to the loss of effectiveness of management of this health condition. Relatively low frequencies such as these are in agreement with the still relevant prevalences of obesity and sedentary habits which should consequently be the focus of concern for public health agents.

Ideal weight maintenance stands out as a minor impact that these types of guidance have on the behavioral changes expected from each of them, because to have received such guidance was positively associated with the lower prevalence of decompensated blood pressure.

The lack of association between having consulted a doctor in the previous year and decompensated blood pressure values was a curious and apparently contradictory finding with previously published evidence.

The independent variable in this study was “to have consulted the same doctor” and that of the present study was “to have consulted a doctor”, regardless of being the same one or not, which may represent a difference in adherence to the recommended measures.

Special attention should be given to the result of the multiple analysis in which the independent variables that remained associated with decompensated blood pressure were: being male, being in the 40 to 49 year age group or 50 year and more age group, and living in the South region of Brazil. Each variable represents a synthetic concept that, as such, includes several attributes. Consequently, with the study design adopted, this makes it impossible to accurately identify factors susceptible to an intervention that reduce the prevalence of decompensated blood pressure.

Finally, health prevention and promotion actions aimed at this population are small. New studies with a national scope and an appropriate design such as multicenter cohort studies are required to study the possible determinants that cause decompensated blood pressure in these conceptual components.

O câncer renal Kidney cancers linked relacionado ao arsênico to arsenic in drinking na água potável. water. ingestão elevada de arsênico presente na água potável está associada com um risco aumentado de câncer de ureter e pelve renal, de acordo com um estudo de caso-controle realizado no norte do Chile.

O estudo comparou 122 casos de câncer de rim e 640 de controles baseados na população. Os casos incluíram 76 tipos de câncer de células renais, 24 carcinomas de células transicionais da pelve renal e do ureter (TCC) e 22 outros tipos de câncer nos rins.

Em comparação com os indivíduos cuja média de ingestão de arsênico da água potável era inferior a 400 mg/dia, aqueles cuja média de consumo foi de 400-1.000 mg/dia e maior que 1.000 mg/dia apresentaram um significativo aumento do risco em 5,7 vezes e 11 vezes para o desenvolvimento dos cânceres de pelve renal e ureter, respectivamente, informaram os pesquisadores, liderados por Craig Steinmaus, MD, MPH, online no American Journal of Epidemiology. A ingestão de arsênico elevada não foi associada com um risco aumentado de câncer das células renais.

levated intake of arsenic present in drinking water is associated with an increased risk of renal pelvis and ureter cancers, according to a case-control study conducted in northern Chile.

The study compared 122 kidney cancer cases and 640 populationbased controls. The cases included 76 renal cell cancers, 24 renal pelvis and ureter transitional cell carcinomas (TCC), and 22 other kidney cancers.

Compared with subjects whose average arsenic intake from drinking water was less than 400 μg/day, those whose average intake was 4001,000 μg/day and greater than 1,000 μg/day had a significant 5.7-fold and 11-fold increased risk of renal pelvis and ureter cancers, respectively, researchers led by Craig Steinmaus, MD, MPH, reported online in the American Journal of Epidemiology. Elevated arsenic intake was not associated with an increased risk of renal cell cancer.

Dr. Steinmaus’ group pointed out that northern Chile is one of the best places in the world to investigate the carcinogenicity of arsenic in part because of high exposures to the heavy metal and the availability of good records on past arsenic water concentrations.

They noted that several factors support the biological plausibility of their findings. First, ingested arsenic is a well-established cause of bladder cancer, most of which also is TCC. Second, the kidney is the primary route

Avaliação tomográfica das orelhas contralaterais de pacientes com otite média crônica grave.

O

tite média é uma das doenças mais prevalentes do mundo. Com custos anuais estimados em cerca de US$ 5 bilhões nos EUA, a otite média é a segunda maior causa de consultas ambulatoriais da população americana com até 15 anos de idade. Segundo Sadé et al., a otite média crônica (OMC) afeta de 0,5% a 30% dos indivíduos da comunidade. Estima-se que mais de 20 milhões de pessoas sofram com esta enfermidade em todo o mundo. Harker descreveu uma incidência de colesteatoma de seis por cem mil habitantes/ano em Iowa, nos EUA. No caso da OMC, estatísticas americanas indicam incidência de 18 casos por cem mil habitantes/ano, 4,2 associados à presença de colesteatoma.

A definição de OMC é baseada em características clínicas e patológicas.

A OMC é tradicionalmente caracterizada pela presença de perfurações, colesteatoma, secreção recorrente e perda auditiva. Histopatologicamente, a OMC é definida pela presença de inflamação da orelha média associada a dano tecidual irreversível, independente da presença de perfurações da membrana timpânica.

Tomographic evaluation of the contralateral ear in patients with severe chronic otitis media.

O

titis media is one of the most prevalent diseases in the world. With annual costs estimated at approximately U$ 5 billion in the United States, otitis media is the second leading cause of outpatient visits in the American population younger than 15 years. According to Sadé et al., chronic otitis media (COM) affects 0.5% to 30% of individuals in any community and over 20 million people are estimated to suffer from this condition worldwide. Harker found an incidence of cholesteatomas of 6 per 100,000 inhabitants/year in Iowa, United States. For COM, an overall US statistic reported an incidence of 18 cases per 100,000 inhabitants/ year, 4.2 associated with the presence of cholesteatoma.

The definition of COM is based on clinical and pathological features. COM is traditionally characterized by the presence perforations, cholesteatoma, recurrent ear discharge, and hearing loss. Histopathologically it is defined by the presence of middle ear inflammation, associated with irreversible tissue damage, regardless of the presence of tympanic membrane perforations.

The disease often evolves in a continuum. Abnormalities that at first 35 OM

A doença frequentemente evolui de forma contínua. Anomalias que primeiramente causam sintomas mínimos ou discretos, tais como simples retrações, podem progredir para alterações graves, como bolsas de retração e colesteatomas destrutivos. Este modelo contínuo explica o desenvolvimento progressivo da OMC. Segundo esta teoria, supuração, perfurações e colesteatomas representam os diferentes estágios patológicos de uma mesma doença. A evolução deste contínuo pode ser observada na orelha contralateral. Quando a disfunção tubária deflagra OMC, há uma alta probabilidade de ambas as orelhas serem afetados, ainda que em diferentes níveis de intensidade. Alguns estudos indicam uma tendência de acometimento bilateral nas patologias inflamatórias da orelha média.

Baseadas nessa hipótese, algumas pesquisas começam agora a se concentrar na orelha contralateral dos pacientes com OMC. Costa et al. publicaram no presente periódico a presença de anomalias no exame otoscópico de 75% das orelhas contralaterais de 500 pacientes com OMC; a prevalência de tais achados foi ainda mais elevada no subgrupo de pacientes com colesteatoma (83%). Em estudo histológico, Rosito et al. identificaram anomalias em 91% das orelhas contralaterais de pacientes com OMC. Em estudo funcional desenvolvido pelo mesmo grupo de pesquisadores, avaliação audiométrica foi feita em 463 pacientes com OMC, demonstrando que cerca de 30% tinham algum grau de perda auditiva na orelha contralateral. Em resumo, evidências demonstram que, em pacientes com OMC, a orelha contralateral frequentemente apresenta anomalias clínicas, histopatológicas e auditivas. Contudo, não ha estudos radiológicos sobre as orelhas contralaterais de pacientes com OMC. É importante realizar estudos sobre orelhas contralaterais com avaliação tomográfica.

A tomografia computadorizada (TC) é um excelente método para visualizar tecido ósseo e espaços pneumatizados, sendo, portanto, o melhor método diagnóstico para a avaliação do acometimento de estruturas do osso temporal resultante de inflamação crônica. Há achados bem documentados de OMC em estudos tomográficos. As modernas técnicas de aquisição helicoidal de imagens e cortes de alta resolução permitem a avaliação de estruturas menores com maior precisão.

O entendimento abrangente do papel da orelha contralateral na avaliação de pacientes com OMC é essencial, especialmente quando a otite média é analisada a partir de uma perspectiva contínua. Acreditamos que o estudo da orelha contralateral possa fornecer pistas sobre a fisiopatologia da doença na orelha principal (com doença manifesta), servir de parâmetro de função tubária e contribuir no planejamento terapêutico.

caused mild or minimal symptoms, such as simple retractions, for example, can progress to severe changes, such as retraction pockets and destructive cholesteatomas. This continuum model explains the development of COM in a progressive manner. According to this theory, effusion, perforations, and cholesteatomas represent different pathological stages of the same disease. The evolution of this continuum can be seen in the contralateral ear (CLE). When tubal dysfunction is the trigger of COM, there is a high probability of impairment of both ears, although in different intensity.

Some studies point to a tendency of bilateral involvement in inflammatory pathologies of the middle ear.

Based on this hypothesis, some research has now begun to focus on the contralateral ear of patients affected by COM. Costa et al. published in this journal the presence of abnormalities in the otoscopic examination of 75% of the CLE of 500 patients with COM; the prevalence of such findings was even higher in the subgroup of patients with cholesteatomas (83%). In a histological study, Rosito et al. found abnormalities in 91% of the CLE of patients with COM. In a functional study developed by the same research group, audiometric evaluation was performed in 463 patients with COM and showed that about 30% had some degree of hearing impairment in the CLE. In summary, evidence shows that, in patients with COM, the CLE often presents clinical, histopathological and hearing abnormalities. However, there are no radiological studies of the CLE of patients with COM. It is important conclude contralateral ear studies with a tomographic evaluation.

Computed tomography (CT) is an excellent method to visualize bone and pneumatized spaces. It is therefore the best diagnostic method to evaluate the involvement of temporal bone structures resulting from chronic inflammation.

There are well-documented findings of COM in CT studies. Modern techniques of helical image acquisition and high-resolution cuts allow the evaluation of smaller structures more accurately.

A comprehensive understanding of the role of the CLE in the evaluation of patients with COM is essential, especially when viewing otitis media from a continuum perspective. We believe that the study of the CLE can provide clues to the pathophysiology of the primary ear disease (ear with full-blown disease), serve as a parameter of tubal function, and contribute to the therapeutic planning.

OTORRINOLARINGOLOGIA OTORHINOLARINGOLOGY OM Objective To assess the prevalence of CT abnormalities in the contralateral ear of patients with COM. Method

Objetivo Avaliar a prevalência de anomalias tomográficas da orelha contralateral de pacientes com OMC. Método O presente estudo foi conduzido com pacientes atendidos no Ambulatório de Otite Média Crônica do Hospital de Clínicas de Porto Alegre (HCPA). Os pacientes foram encaminhados para estudo tomográfico com base na avaliação clínica, independente de sua participação no estudo. Apenas os casos mais graves de OMC são encaminhados para TC. Dentre estes estão os pacientes com supuração persistente, os refratários a tratamento medicamentoso, indivíduos com perda auditiva condutiva acima de 30 dB ou associada a perda auditiva neurossensorial e presença de colesteatomas. Assim, a população do estudo foi uma amostra por conveniência de pacientes com OMC, idade mínima de 5 anos, encaminhados para exame por TC, atendidos após 2007, com base nos critérios descritos acima. Incluímos os primeiros 75 pacientes que vieram ao ambulatório com seus resultados de TC. Os critérios de exclusão foram procedimento cirúrgico anterior (exceto tubos de ventilação), malformação congênita conhecida, trauma e outras afecções do osso temporal.

Um neurorradiologista cegado para as informações clínicas e diagnósticos avaliou as tomografias. As imagens foram avaliadas com base em um protocolo com 38 questões em torno das estruturas consideradas importantes para a análise radiológica do osso temporal.

Os exames foram realizados em um tomógrafo helicoidal Philips. Cortes coronais e axiais foram obtidos com espessura de 0,65 mm. Em seguida, com base na apresentação clínica dos pacientes, estabelecemos quais lados representavam as orelhas primária e contralateral. A orelha principal foi definida como aquela que apresentava sintomas mais intensos, maior perda auditiva ou sinais de doença mais avançada ao exame otoscópico.

Os procedimentos e as metas da pesquisa foram explicados aos pacientes, que também assinaram termos de consentimento informado. O presente estudo foi aprovado pelo Conselho de Avaliação Institucional do Hospital de Clínicas de Porto Alegre e recebeu o protocolo número 10-0569.

Patients were referred for CT scans based on clinical judgment alone, independently of participation in this study. Only the most severe cases of COM are referred for CT examination. Among these are patients with persistent effusion, refractory to drug treatment, conductive hearing loss above 30 dB or associated with sensorineural hearing loss, and presence of cholesteatomas. Thus, the study population is a convenience sample of patients with COM, 5 years old or more, who were referred for CT scans, after 2007, based on the criteria described above. We enrolled the first 75 patients who attended the clinic presenting their CT results. Exclusion criteria were previous surgical procedure (except ventilation tubes), known congenital malformations, trauma and other temporal bone pathologies.

A neuroradiologist blinded to clinical information and diagnosis evaluated CT scans. Images were evaluated based on a 38 question protocol focusing on the structures considered to be the most important in the radiological analysis of the temporal bone.

Examinations were performed on a Philips helical CT scanner. Coronal and axial images were obtained with a 0.65 mm slice thickness. Next, based on the patient’s clinical presentation, we established which side represented the primary ear and the contralateral ear. The primary ear was defined as the ear presenting more intense symptoms, greater hearing loss or showing more advanced disease in the otoscopic examination.

Research procedures and goals were explained to all patients, who also signed a written informed consent. This study was approved by the Institutional Review Board of the Hospital de Clínicas de Porto Alegre and registered under the number 10-0569.

Cálculo do tamanho da amostra Não há estudos semelhantes publicados na literatura. De modo a estimar a prevalência de 25% de anomalias, com margem de erro absoluto de 10% e intervalo de confiança de 95%, foi necessária uma amostra de 72 pacientes.

Análise estatística Análises estatísticas foram executadas com o software Statistical Package for Social Science (SPSS) 10.0 for Windows. Apenas descrições de prevalência foram efetuadas. Não houve testes estatísticos.

Discussão Acreditamos que o estudo cuidadoso da orelha contralateral pode melhorar significativamente nossa compreensão da patogenia da OMC. Além disso, a análise meticulosa de ambas as orelhas pode ajudar a estabelecer três aspectos essenciais ao desenvolvimento da OMC: etiologia, estado atual e evolução da doença em termos de velocidade e direção.

Ao analisar e avaliar a orelha contralateral, temos a oportunidade de ver “hoje” o que aconteceu “ontem” com a orelha principal.

O grande número de sinais radiológicos de OMC encontrados nas orelhas contralaterais confirma a importância de se estudar as duas orelhas.

A prevalência de velamento ou espessamento da mucosa da cavidade timpânica foi de aproximadamente 20%. Percentuais semelhantes foram identificados quando avaliamos apenas a região do protímpano. Devemos notar que obliterações nessa área podem ser responsáveis por disfunção tubária, redução da troca gasosa transmucosa e desenvolvimento de otite média. Discussion We believe that a careful study of the contralateral ear can significantly improve our understanding of the pathogenesis of COM. Moreover, a thorough analysis of both ears can help to establish three essential aspects in the development of COM: etiology, current status, and disease evolution, both in terms of speed and direction.

By valuing and assessing the CLE, we have the opportunity to see “now” what happened to the primary ear “yesterday”.

The large number of radiological signs of COM found in the contralateral ears confirms the importance of studying both ears. The prevalence of opacification or mucosal thickening in the tympanic cavity was approximately 20%. Similar percentages were found when we evaluated only the region of the protympanum, and it should be noted that obliterations in this area may be responsible for tubal dysfunction, reduced transmucosal gas exchange, and development of otitis media. Claramente, isso se traduz em pacientes com doença mais grave. Pacientes com colesteatomas, retrações com grande perda auditiva e supuração refratária foram incluídos. É provável que a população de nosso estudo apresente tomograficamente orelhas contralaterais mais acometidas que uma amostra típica de pacientes com perfurações centrais secas. Entretanto, não acreditamos que seria adequado expor pacientes à radiação da TC sem indicação clínica.

Apesar de reconhecermos que os pacientes incluídos no presente estudo talvez possam superestimar a prevalência de anomalias radiológicas, a existência de tais anomalias na orelha contralateral é incontestável e reforça os achados de estudos anteriores. Não obstante, enfatizamos que a validade externa da presente pesquisa é limitada a populações semelhantes, principalmente a pacientes com colesteatoma.

As estruturas da mastoide também exibiram elevada prevalência de anomalias (35%). Isto pode trazer implicações sobre a fisiopatologia da otite média, uma vez que os dados sugerem a ocorrência de distúrbios no desenvolvimento da mastoide, com efeitos consideráveis sobre ambas as orelhas. É importante destacar que, apesar dos achados clínicos e radiológicos associados a doença ativa presente (por exemplo, retrações da membrana timpânica ao exame otoscópico ou espessamento da mucosa da orelha média ao exame por TC), a maioria das alterações descritas na TC são resultado do processo patológico como um todo.

O modelo clássico é aquele da criança que desenvolve OMC com acometimento da ventilação da orelha média e, consequentemente, pneumatização reduzida da mastoide. Mesmo com a ocorrência de resolução terapêutica ou espontânea, a mastoide permanecerá radiologicamente anormal na idade adulta. Erosões ósseas ou distúrbios ossiculares ocorridos no passado poderão ainda ser observados na TC. Este conceito traz duas implicações: apesar de poder denotar maior prevalência de mudanças radiográficas em comparação a pacientes atualmente com doença ativa, o exame também permite a identificação de processos patológicos anteriores que ocorreram na orelha média.

Conclusão Concluímos que a orelha contralateral apresenta alta prevalência de anomalias ao exame por TC em pacientes com OMC grave.

with COM are evaluated, our study population involved patients with chronic otitis media who had an indication to perform CT. Clearly, this translates into patients with more severe disease. Patients with cholesteatomas, retractions with major hearing loss, and refractory effusions were included.

It is likely that our study population presents tomographically more affected CLE than a typical sample of patients with dry central perforations. However, we do not believe it would be appropriate to expose patients to radiation from a CT scan without clinical indication.

Even though we recognize that patients enrolled in this study may overestimate the prevalence of radiological abnormalities, the existence of such abnormalities in the contralateral ear is indisputable and reinforces findings from previous studies. Nevertheless, we emphasize that the external validity of this research is limited to similar populations, mainly, patients with cholesteatomas.

The mastoid structures also showed a high prevalence of abnormalities (35%). This may have some implication in the pathophysiology of otitis media, as the data suggest the occurrence of some disturbance in the development of the mastoid that affects both ears considerably. It is important to highlight that although some clinical and radiological findings are due to active, current disease (for example, tympanic membrane retractions seen at otoscopic examination or mucosal thickening in the middle ear seen on CT), most of the changes described in CT are the result of the entire disease process.

The classical model is that of a child who develops COM, with impairment of middle ear ventilation and, consequently, reduced mastoid pneumatization. Even if a therapeutic or spontaneous resolution occurs, the mastoid will remain radiologically abnormal in adulthood.

Bony erosions or ossicular disorders that took place in the past will still be observed on the CT. This concept has two implications: while there may be an increased prevalence of radiographic changes compared to patients who are currently effectively ill, the exam also allows for the identification of previous pathological processes that occurred in the middle ear.

Conclusion We conclude that the contralateral ear presents a high prevalence of CT abnormalities in patients with severe COM.

possível perceber o aumento na preocupação quanto à saúde de adolescentes por parte dos profissionais da saúde. A atenção à saúde do adolescente vem se tornando uma prioridade em muitos países, inclusive para instituições internacionais de fomento à pesquisa. Percebemos isso pelo aumento nos estudos que incluem este grupo da população. Porém, nos serviços de saúde, o adolescente recebe a atenção ideal? Que tipo de acolhimento ele recebe?

No modelo tradicional de nossa sociedade, o adolescente deixa de ser visto em sua singularidade e especificidades pertencentes a esta fase e passa a ser visto como um sujeito qualquer chegando ao serviço de saúde. Em outras palavras, no modelo atual de atenção, o profissional de saúde estabelece uma relação vertical com o adolescente, impondo-lhe normas de conduta, acreditando que, assim fazendo, proporciona ao usuário qualidade no atendimento. Essa postura coloca o adolescente em uma posição de inferioridade e de passividade, retirando dele não só a liberdade de escolha, como também a responsabilidade por seus atos.

O modelo de atenção integral, conforme claramente é explicitado na Lei Orgânica da Saúde, objetiva ampliar a possibilidade de atuação do profissional, tendo como preocupação não só a singularidade do sujeito, mas também a organização do serviço que lhe é prestado. Neste modelo, a saúde é entendida como um requisito para a cidadania e envolve inter-relações entre o indivíduo, a coletividade e o meio ambiente.

Em se tratando do acesso ao serviço de saúde, é fundamental que o adolescente busque o serviço e compreenda que o profissional da saúde é um mediador importante no cuidado à sua saúde. Acreditamos que a maneira

In many countries, healthcare for adolescents has become a priority, including among international research institutions. This is apparent through the rising numbers of studies addressing this segment of the population.

However, are adolescents receiving ideal care from healthcare services?

What kind of welcome do they find?

In the traditional model of Brazilian society, adolescents cease to be viewed as individuals with specific characteristics pertinent to this phase of life, and are instead seen as ordinary people walking into healthcare facilities. In other words, under the current care model, healthcare practitioners establish vertical relationships with adolescents, imposing standards of conduct on them in the belief that by doing so, they are offering good quality care to these users. This stance places adolescents in a passive position of inferiority, removing not only their freedom of choice, but also responsibility for their own acts The comprehensive healthcare model as clearly set forth in the Brazil’s Basic Health Act strives to extend the possibilities for professional approaches, including concerns not only for the singularity of these young subjects, but also the organization of the services where care is provided. Under this model, health is viewed as a requirement for citizenship, involving relationships between individuals, society and the environment In terms of access to healthcare facilities, it is vital for adolescents to seek out these services, understanding that healthcare practitioners

O atendimento ao adolescente, e não somente a este, pois todo usuário quer discrição sobre seu problema, necessita ser com privacidade e confidencialidade. Na relação com os profissionais de saúde, deve haver a expressão de seu processo de individualização. O sigilo da consulta deve ser assegurado por meio de um debate nos serviços visando a um consenso entre os profissionais.

Requer-se que o profissional de saúde possa lidar com os adolescentes, de maneira participativa e interativa, para que o conhecimento emane com tranquilidade, sem as imposições de uma relação de poder Na prática, percebemos que a procura dos adolescentes pelo serviço de saúde dá-se pelos seguintes motivos: teste de gravidez e consulta pré-natal, busca por anticoncepcionais e preservativos masculinos.

Percebemos que uma situação que frequentemente traz dificuldade para o profissional de saúde é ter que lidar com o início da vida sexual do adolescente. Para tanto, os profissionais precisam demonstrar tranquilidade e segurança ao prestar o atendimento e não recriminá-lo por suas escolhas.

Seguindo a orientação da Organização Pan-Americana de Saúde (OPAS), o Programa de Saúde do Adolescente vem procurando atuar de forma preventiva, incentivando atividades de promoção de saúde. Mesmo assim, percebemos, ainda, uma grande dificuldade da sociedade em debater assuntos ligados à sexualidade, prejudicando a divulgação de informações que poderiam favorecer a adoção de práticas saudáveis de vida.

Embora existam programas de atenção ao adolescente, que já estão sendo implementados há quase três décadas, observam-se mudanças significativas no perfil de morbimortalidade neste grupo populacional, com aumento de problemas que poderiam ser evitados por medidas de promoção de saúde e prevenção de agravos.

Desta forma, pensamos que, de acordo com orientação do Ministério da Saúde, a alternativa viável é modificar a ênfase do serviço dirigido a esta clientela. A consulta nos serviços de saúde deve ter como objetivos, além da prevenção de agravos, o diagnóstico, a monitorização, o tratamento e a reabilitação dos problemas de saúde de forma apoiadora e jamais recriminatória.

are crucial mediators in caring for their health. We believe that the way these adolescents are welcomed at healthcare facilities may attract them, or simply discourage them immediately from their quest for attention.

Healthcare for adolescents - and not only for these youngsters, as all users require their problems to be treated with discretion - must be conducted with privacy and confidentiality. In relationship with healthcare practitioners, their individualization processes must find expression. The confidentiality of consultations must be assured through discussions in these facilities, pursuing consensus among their practitioners.

Healthcare practitioners must be able to deal with adolescents in participative and interactive ways, allowing knowledge to be conveyed tranquilly, without imposing power-based relationships.

In practice, we find that adolescents contact healthcare facilities for the following reasons: pregnancy tests, pre-natal consultations, contraceptives and condoms.

We also note that a situation that is often handled clumsily by healthcare practitioners relates to the start of the sexual life of adolescents. To do so, these practitioners must demonstrate calmness and assurance during while providing care, not blaming them for their choices. According to the guidelines established by the Pan-American Health Organization (PAHO), the Adolescent Healthcare Program has been striving to act in preventive ways, encouraging health promotion activities. Nevertheless, we perceive that society still finds it difficult to discuss topics linked to sexuality, hampering the disclosure of information that could underpin the adoption of healthy life practices.

Although adolescent healthcare programs have been implemented for almost three decades, significant alterations in the morbidity and mortality profiles of this segment of the population are noted, with an increase in problems that could be avoided through steps designed to promote health and prevent disease.

As a result, we believe that, compliant with the guidelines handed down by the Ministry of Health, a feasible alternative would be to modify the focus of the services addressing this clientele. Consultations at healthcare facilities must be objective, in addition to preventing disease, diagnosing, monitoring, treating and rehabilitating health-related problems in a supportive and never recriminatory way.

Entendemos que a acolhida dos adolescentes ao serviço de saúde deve ser cordial, compreensiva, de forma que os mesmos sintam-se valorizados e tenham confiança no profissional que os recebe ali, pois muitos adolescentes enfrentam inúmeros tabus até chegar ao serviço de saúde para requerer alguma ajuda ou esclarecimento sobre sua própria saúde.

Por fim, a primeira possibilidade de efetivação dessas ações de abordagem ao adolescente no serviço de saúde pode estar localizada no momento do acolhimento. O confronto entre as necessidades de saúde trazidas pelos adolescentes e o que o serviço tem a oferecer poderá revelar as mudanças no modelo assistencial. Tais transformações são potenciais construtoras de vínculo, aproximando quem oferece ou presta o serviço de quem o recebe.

Acreditamos que a saúde é um território de práticas em dinâmica construção. O desafio que se coloca a todos trabalhadores é o de repensar valores, questionar a ética, superar as dificuldades, inventar e reinventar maneiras novas e cada vez melhores, mais criativas e capazes de beneficiar a saúde e construir cidadania entre nossos adolescentes.

We believe that the welcome offered by healthcare facilities to adolescents must be friendly and understanding, ensuring that they feel appreciated and trust the practitioners who see them at these locations, as many adolescents must surmount countless taboos until reaching the healthcare facilities in order to request help or information on their own health.

Initially, the first possibility for implementing these actions designed to modify approaches to adolescents at healthcare facilities could focus on the moment of intake. The clash between the health-related needs presented by these adolescents and what these facilities have to offer them could trigger changes in the welfare model. These transformations are potential links for building up a relationship chain between the suppliers of the healthcare services and their recipients.

We believe that healthcare is a field of practices under dynamic construction. The challenge facing all practitioners is to rethink values, question ethics, surmount difficulties, invent ad reinvent new and better approaches that are more creative and better able to enhance the health and construct citizenship among our adolescents.

A amamentação entre filhos de mulheres trabalhadoras.

Breastfeeding among children of women workers.

Organização Mundial da Saúde (OMS), em 2001, definiu que o leite materno deveria ser oferecido de forma exclusiva, para todas as crianças, até o sexto mês de vida. Entretanto, embora tenha ocorrido um importante aumento nos índices do aleitamento materno exclusivo ao quarto mês de vida no Brasil (35,5% em 1999 para 51,2% em 2009), ao sexto mês esse índice não chega a 10%.

O trabalho materno é um dos fatores de risco para interrupção do aleitamento. Estudo de Schwartz et al mostrou que o retorno da mãe ao trabalho foi responsável por 58% do desmame precoce em Michigan e Nebraska (EUA). Resultados semelhantes foram encontrados por Costa et al em estudos conduzidos na cidade de Belém, PA, e por Dearden et al em quatro comunidades da Guatemala.

Dispositivos legais foram criados para minimizar os riscos para a diminuição do aleitamento materno no período de retorno ao trabalho. A Constituição Federal de 5 de outubro de 1988, no seu artigo 7º, parágrafo XVIII, versa: “Licença maternidade, sem prejuízo do emprego e do salário, com duração de 120 dias”. A Consolidação das Leis Trabalhistas (CLT) protege a nutriz, com descansos especiais durante a jornada de trabalho e

n 2001, the World Health Organization (WHO) defined that breast milk should be offered exclusively to all children up to the sixth month of life. However, although an important increase in the indexes of exclusive breastfeeding has occurred up to the fourth month of life in Brazil (from 35.5% in 1999 to 51.2% in 2009), up to the sixth month this index does not reach 10%.

Maternal employment is one of the risk factors for discontinuing breastfeeding. A study conducted by Schwartz et al has shown that the mother’s return to work was responsible for 58% of early weaning in Michigan and Nebraska (USA). Similar results have been found by Costa et al in studies conducted in the city of Belém (Northern Brazil), and by Dearden et al in four communities of Guatemala.

Legal provisions have been created to minimize the risk of reducing breastfeeding in the period of return to work. Article 7, paragraph XVIII of the Brazilian Federal Constitution of October 5, 1988, reads: “Maternity leave, without loss of job and salary, with duration of 120 days”. the workday and description of adequate places for providing care for the infants.

On September 9, 2008, the federal Law nº 11,770 was created.

By granting tax incentives, this law stimulates companies to extend the maternity leave to six months. This law meets the recommendation of the WHO and of the Ministry of Health.

The CLT also determines that companies with more than 30 women workers older than 16 years must offer internal daycare centers or establish agreements with external daycare centers. In addition, by means of collective agreements, unions have claimed the payment of the “daycare center token”, as an alternative to mothers who prefer to use neither the internal daycare centers nor those that established agreements with the company. Nevertheless, many women still have difficulties in breastfeeding their child during the working hours, mainly in large urban centers, where transportation is inefficient and/or the distance between the workplace and the place where the child stays is very large.

Thus, the aim of this study was to analyze employment benefits and factors associated with the maintenance of breastfeeding indexes among working mothers.

Em 9 de setembro de 2008 foi criada a Lei federal nº 11.770, que, mediante concessão de incentivo fiscal, estimula as empresas a ampliarem a licença maternidade para seis meses. Essa lei vem ao encontro da recomendação da OMS e do Ministério da Saúde.

A CLT determina também que empresas com mais de 30 funcionárias com idade superior a 16 anos ofereçam creches internas ou estabeleçam convênios com creches externas. Além disso, por meio de acordos coletivos, os sindicatos reivindicaram o pagamento do “vale creche”, como uma alternativa às mães que preferem não utilizar as creches internas ou as conveniadas com a empresa. Entretanto, muitas mulheres ainda têm dificuldade de amamentar a criança durante o expediente, principalmente em grandes centros urbanos, onde o transporte é ineficiente e/ou é muito grande a distância entre o local de trabalho e o local de permanência da criança.

Desse modo, o objetivo deste estudo foi analisar benefícios trabalhistas e fatores associados à manutenção dos índices de amamentação entre mães trabalhadoras.

Métodos Estudo de desenho retrospectivo de coorte. A coorte original foi delineada para realização num programa preventivo em saúde bucal e durante o dia da campanha de multivacinação (10 de agosto), em Piracicaba, SP, no ano de 2008. Situada no estado de São Paulo, Piracicaba ocupa uma área de 1.377 km2 e, segundo o censo de 2010, apresenta 4.328 habitantes menores de 12 meses, o que representa 1,18% da população total da cidade.

A amostra deste estudo incluiu somente as mães trabalhadoras formais, que possuíam vínculo empregatício e que retornaram ao trabalho antes de a criança completar seis meses de vida. No momento da pesquisa, a criança tinha entre seis e dez meses de idade. Foram excluídas as mães que desempenhavam funções não regulamentadas pela CLT, por não serem contempladas com esses benefícios de incentivo ao aleitamento materno.

Além disso, também foram excluídas as díades que apresentaram algum problema de saúde nos períodos pré e pós-parto imediato, bem como as mães que tiveram filhos gêmeos, prematuros (< 37 semanas) ou com baixo peso ao nascer (< 2.500 g).

This is a retrospective cohort study, the original cohort was outlined to be conducted in an oral health preventive program and during the day of a vaccination campaign (august 10) in the city of piracicaba (southeastern brazil), in the year of 2008, located in the state of são paulo, piracicaba occupies an area of 1,377 km2 and, according to the 2010 census, it has 4,328 inhabitants younger than 12 months, which represents 1.18% of the city’s overall population.

The sample of this study included only formal working mothers, who had formal jobs and who returned to work before the child had reached six months of life. At the moment of the study, the child was aged between six and ten months. Mothers who performed functions not regulated by the CLT were excluded, as they are not entitled to breastfeeding incentive benefits. Besides, the dyads that presented some health problem before and immediately after birth were excluded, as well as mothers who had twins, preterm babies (< 37 weeks) or babies with low birth weight (< 2,500 g).

The sample was constituted by 200 mother-infant dyads, of which 100 (Group A) received, from February to May 2008, guidance and support for the breastfeeding practice within an oral health early prevention program. Data collection in this population was carried out by means of interviews performed during the dyad’s participation in the program.

The other dyads (Group B) did not participate in this program and were approached in the child vaccination campaign, at four vaccination units that had been previously drawn.

Considering that the children are not uniformly distributed across the several vaccination units (clusters), two-stage draw was adopted, with proportional probability to cluster size. In the first stage, the vaccination units were drawn and in the second stage, the children in each unit, in a systematic way. The four drawn units and the number of children whose mothers would be approached per unit were defined considering the total of 40 vaccination units of the previous year and the estimate of vaccinating 24,000 children aged zero to five years. The vaccination expectation at those units was 1,200 children younger than one year.

A amostra foi constituída por 200 díades mãe-lactente, das quais 100 (Grupo A) receberam, de fevereiro a maio de 2008, orientações e apoio para a prática do aleitamento, dentro de um programa de prevenção precoce em saúde bucal. A coleta de dados dessa população foi feita por meio de entrevista conduzida durante a participação da díade no programa. As demais díades (Grupo B) não participaram desse programa e foram abordadas na campanha de vacinação infantil, em quatro postos de vacinação previamente sorteados.

Considerando que as crianças não estão distribuídas uniformemente nas várias unidades de vacinação (conglomerados), adotou-se o sorteio em dois estágios, com probabilidade proporcional ao tamanho dos conglomerados. No primeiro estágio foram sorteadas as unidades de vacinação e, no segundo estágio, as crianças em cada unidade, de forma sistemática.

As quatro unidades sorteadas e o número de crianças cujas mães seriam abordadas por unidade foram definidos considerando o total de 40 unidades de vacinação do ano anterior e a estimativa de vacinar 24.000 crianças de zero a cinco anos. A expectativa de vacinação nesses postos era de 1.200 crianças com idade inferior a um ano.

A amostra proporcionou um poder do teste (1-β) de 0,80 com nível de significância α de 0,05 para um odds ratio de 2,0, probabilidade de resposta de 28% e probabilidade de sucesso de 35%, calculada de acordo com Demidenko.

Utilizou-se um roteiro estruturado de entrevista para obter informações sobre o nível socioeconômico da família, idade dos pais, estado civil, número de filhos, tipo de parto, variáveis relacionadas aos períodos peri e pós-natal, época de retorno da mãe ao trabalho, distância entre o local de trabalho e o local de permanência da criança, cuidador da criança no momento de ausência da mãe e as condições do trabalho materno.

A structured interview script was used to collect information on the family’s socioeconomic level, parents’ age, marital status, number of children, type of delivery, variables related to peri- and postnatal periods, period of mother’s return to work, distance between the workplace and the place where the child stayed, child’s caregiver when the mother was absent and the conditions of the mother’s employment.

Results were evaluated by means of descriptive analysis, of the statistical tests chi-square, Fisher’s exact test and multiple logistic regression analysis through the stepwise forward procedure. Variables with p < 0.25 in the bivariate analysis were tested in the model, and those which adjusted to the model with p < 0.05 remained.

All the analyses were carried out in the statistical program SAS 2008, version 9.2.

The concept of weaning used in the study follows the definition of the WHO, that is, when the offer of breast milk to the child is discontinued.

The study was carried out according to the Ethical Norms and Guidelines of Resolution 196/96 of the National Health Council of the Ministry of Health, and was approved by the Research Ethics Committee of Faculdade de Odontologia de Piracicaba - Unicamp (Protocol nº 073/2004).

Results The majority of the children had families of medium socioeconomic level, and fathers aged up to 30 years. Their mothers were aged up to 28 years, primiparous, lived with a partner, underwent cesarean section, started to breastfeed in less than four hours after birth and stayed with their baby in the room.

Among the mothers who did not participate in incentive groups, 43% weaned their baby before the fourth month. Among those who did not breastfeed during the workday, 34% weaned their baby before the fourth

Foram testadas no modelo as variáveis com p < 0,25 na análise bivariada, permanecendo as que se ajustaram ao modelo com p < 0,05. Todas as análises foram realizadas no programa estatístico SAS 2008, versão 9.2.

O conceito de desmame utilizado no estudo segue a definição da OMS, ou seja, quando ocorre a interrupção da oferta de leite materno à criança.

O estudo foi realizado de acordo com as Normas e Diretrizes Éticas da Resolução 196/96 do Conselho Nacional de Saúde do Ministério da Saúde e aprovado pelo Comitê de Ética em Pesquisa da Faculdade de Odontologia de Piracicaba - Unicamp (Nº de protocolo: 073/2004).

Resultados A maioria das crianças apresentava família de nível socioeconômico médio, pais com até 30 anos de idade e suas mães tinham até 28 anos, primíparas, contavam com a presença do companheiro, passaram por parto tipo cesárea, tinham iniciado a amamentação em menos de quatro horas após o parto e permaneceram com seu filho em alojamento conjunto.

Entre as mães que não participavam de grupos de incentivo, 43% desmamaram antes do quarto mês. Entre as que não amamentavam durante a jornada de trabalho, 34% desmamaram antes do quarto mês. Dentre as mães que voltaram a trabalhar depois do quinto mês após o parto, 82,9% desmamaram após o quarto mês.

A análise de regressão logística múltipla mostrou que as chances de parar a amamentação antes do quarto mês foi maior para as mães não participantes do programa de incentivo (OR = 3,04 [IC95% 1,35;6,85]) e que não tinham intervalo de 30 minutos durante a jornada de trabalho (OR = 4,10 [IC95% 1,81;9,26]); e para as crianças que utilizavam chupeta (OR = 2,68 [IC95% 1,23;5,83]) ou mamadeira (OR = 14,47 [IC95% 1,85;113,24]).

antes do quarto mês de vida, tais como o fato de a mãe não conseguir amamentar a criança durante a jornada de trabalho, não poder usufruir do intervalo de 30 minutos por turno trabalhado e o uso de chupeta e mamadeira. Hight-Laukaran et al mostraram que o tempo médio de aleitamento materno é maior entre mulheres que permanecem com seus filhos no local de trabalho. Além disso, Oliveira & Silva ressaltam que os empregadores que respeitam as leis trabalhistas e facilitam a amamentação no local de trabalho contam com funcionárias mais produtivas e com menores índices de falta ao trabalho.

A época de retorno ao trabalho mais frequente foi entre o terceiro e quarto mês após o parto. Esse resultado já era esperado, tendo em vista que a legislação trabalhista vigente no momento da coleta do dado dava o direito a quatro meses de licença maternidade remunerada às trabalhadoras formais. Houve tendência em manter o aleitamento por mais de quatro meses entre as mães que voltaram a trabalhar depois do quinto mês após o parto. Esses dados indicam que a lei de ampliação da licença maternidade de quatro para seis meses poderá contribuir para o aumento do período de aleitamento.

A presença de posto de coleta de leite materno na empresa não mostrou relação com a manutenção do aleitamento. Estudo realizado em empresas na cidade de São Paulo, SP, sugere que a instalação de postos de coleta de leite humano no local de trabalho facilita a manutenção do aleitamento materno quando o número de funcionários do sexo feminino não viabiliza a instalação de uma creche. No entanto, para que esse benefício atue de modo a promover a manutenção do aleitamento, a mãe precisa receber informação e treinamento para ordenhar, armazenar e oferecer seu leite adequadamente.

A presença de creche no local de trabalho ou o acesso à creche externa não mostrou relação com a interrupção precoce do aleitamento. Rea et al relatam que não há diferença significativa entre o tempo médio de aleitamento materno ao comparar mulheres cujos filhos permanecem em creche OM 52

Some variables showed a significant relationship to weaning before the fourth month of life, such as the mother not being able to breastfeed the baby during the workday, not having the 30-minute break per shift and the use of pacifier and bottle. Hight-Laukaran et al have shown that the average time of breastfeeding is higher among women who stay with their babies in the workplace. Furthermore, Oliveira & Silva highlight that employers who respect the labor laws and facilitate breastfeeding at the workplace have more productive women workers with lower indexes of absences.

The most frequent period of return to work was between the third and fourth month after birth. This result was already expected, in view of the fact that the labor law that was in force at the moment of data collection entitled formal women workers to four months of paid maternity leave.

There was a tendency of maintaining breastfeeding for more than four months among mothers who returned to work after the fifth month after birth. These data indicate that the law that extends the maternity leave from four to six months may contribute to increase the breastfeeding period.

The presence of a breast milk collection station in the company was not related to the maintenance of breastfeeding. A study conducted in companies in the city of São Paulo (Southeastern Brazil) suggests that the installation of breast milk collection stations at the workplace facilitates the maintenance of breastfeeding when the number of female employees does not enable the installation of a daycare center. However, so that this benefit promotes the maintenance of breastfeeding, the mother needs to receive information and training to pump, store and offer her milk adequately.

Presence of a daycare center in the workplace and access to an external daycare center were not related to early interruption of breastfeeding.

Rea et al report that there is no significant difference in the average time of breastfeeding when women whose children stay at a daycare center outside the workplace are compared to women who do not have this

fora do local de trabalho e mulheres que não dispõem desse benefício. Isso pode ser devido ao fato de que as empresas geralmente não fornecem transporte para o deslocamento das funcionárias até o local de permanência da criança. Assim, a dificuldade de acesso a creches localizadas fora do local de trabalho da mãe pode resultar em redução do tempo destinado ao aleitamento, constituindo-se em um fator de risco ao aleitamento.

Estudo com trabalhadoras da cidade de Campinas, SP, que utilizaram o benefício de creche no local de trabalho mostrou índice de aleitamento materno exclusivo de 66% ao quinto mês de vida da criança. Além disso, foi possível observar que o desmame iniciou por volta do quinto mês de vida por orientação da creche, visando uma melhor adaptação da criança às rotinas do estabelecimento.

O cumprimento das leis de proteção à amamentação parece ser fundamental em uma sociedade como a brasileira, em que quase 25% da população economicamente ativa é constituída por mulheres com filhos menores de seis meses de idade. Segundo Hight-Laukaran et al, 92% das mulheres que não recebem o beneficio de creche no local de trabalho introduzem substitutos para o leite materno.

O Estado de Santa Catarina obteve um índice próximo a 85% no cumprimento dessas leis, por meio de algumas estratégias. Entre elas, a exigência do registro de creches próprias e conveniadas na Delegacia Regional do Trabalho, a obrigatoriedade da empresa em proporcionar às mulheres o conhecimento de seus direitos e o modo de utilizá-los, a modificação do tipo de inspeção e controle das creches e empresas e a divulgação dos direitos da mãe trabalhadora pelos meios de comunicação.

benefit. This may be due to the fact that companies generally do not provide the employees with transportation to the place where the child stays. Thus, difficult access to daycare centers located outside the mother’s workplace may result in a reduction in the time destined to breastfeeding, constituting a risk factor to breastfeeding.

A study with women workers in the city of Campinas (Southeastern Brazil) who used the benefit of having a daycare center at the workplace showed an index of exclusive breastfeeding of 66% in the child’s fifth month of life. Moreover, it was possible to observe that weaning began around the fifth month of life following the guidance provided by the daycare center, aiming at a better adaptation of the child to the institution’s routine.

The enforcement of laws that protect breastfeeding seems to be fundamental in a society like the Brazilian one, in which almost 25% of the economically active population is constituted by women with children younger than six months of age. According to Hight-Laukaran et al, 92% of the women who do not receive the benefit of having a daycare center at the workplace introduce substitutes for breast milk.

The State of Santa Catarina obtained an index close to 85% in the enforcement of these laws by means of some strategies. Among them, the requirement of the registration of company-owned and external daycare centers at the Regional Labor Inspection Office, the company’s obligatoriness to explain to women their rights and the way to use them, the modification in the type of inspection and control of daycare centers and companies, and the dissemination of working mothers’ rights by the media.

Segundo Roig et al, o uso de mamadeiras e chupetas e a falta de informações estão relacionados com a duração do aleitamento materno.

Os dados do presente estudo mostram a importância de oferecer informações sobre os direitos garantidos por lei que facilitam a manutenção do aleitamento às mães trabalhadoras. Munida desse conhecimento, a mulher-mãe-nutriz pode lutar pelos seus direitos, fiscalizando e exigindo o cumprimento dos benefícios trabalhistas pelas empresas. Porém, há que se oferecer, também, apoio e informação sobre o manejo da lactação, aconselhando a mãe a, por exemplo, evitar a introdução de bicos artificiais e ordenhar e armazenar seu leite para que seja oferecido à criança nos momentos de sua ausência.

Essas iniciativas, em conjunto com a ampliação do tempo de licença maternidade, poderão ter um importante papel na manutenção da prática do aleitamento materno.

These results corroborate the study by Osis et al, who observed that women breastfeed for a longer period of time when they receive some kind of support, either from the family or professional support. According to Roig et al, the use of bottles and pacifiers and the lack of information are related to the duration of breastfeeding.

The data of the present study show the importance of offering information to working mothers on the rights that are guaranteed by law and that facilitate the maintenance of breastfeeding. Having this knowledge, the woman-mother-feeder can fight for her rights, inspecting and demanding the enforcement of the employment benefits by the companies. However, it is necessary to offer, also, support and information on the handling of lactation, counseling the mother to, for example, avoid the introduction of artificial teats and to pump and store her milk so that it is offered to the child when she is absent. These initiatives, together with the extension of the length of the maternity leave, may play an important role in the maintenance of the breastfeeding practice.
