hankfully we are starting to see more smart women with adventurous spirits become the main attraction of some of today’s most popular films and television shows.

One of the characters quite literally at the frontier of modern female heroes was TV’s Dr. Quinn, a physician embarking into the 1870s Wild West and embodied by award-winning actress Jane Seymour. What many people don’t know is the difficult place Seymour was in when she landed the iconic role. Learn more about Seymour’s experience, and get a hint of what she will share at this year’s SmartWomen Luncheon & Expo, in this issue.

You’ve probably heard the phrase, “Work smarter, not harder.” But physicians embracing new and innovative approaches to their procedures would argue, “Work smart, do less.” To the patient, that is. t’s not something people want to talk about in the grocery store,” says Syl Alan Lord, MD, of Metro Surgical, PC, with a laugh. He’s talking about things he deals with every day—conditions and diseases of the colon.

Awareness of minimally-invasive treatments can help patients face colorectal disease

Board certified in general surgery and colon and rectal surgery, Dr. Lord has probably heard it all during his more than two decades as a Savannah physician. He understands it’s not easy to discuss, but with advances in minimally-invasive robotic surgical procedures and colon cancer screening, there’s no reason to suffer in silence. Some of the common conditions that Dr. Lord treats include: • Diverticulitis – A pocket or pouch that develops on the wall of the colon, called diverticula, can become perforated, which can then lead to an infection, causing pain, bloating, nausea, or fever • Hemorrhoids – When venous pressure weakens the veins in the anal and lower rectum areas, they can swell and prolapse (slip forward or down), causing pain and possibly bleeding • Rectal pain or bleeding – This can have a variety of causes, including hemorrhoids or anal fissures, which are small tears in the lining of the anal canal • Abscesses and fistulas – Abscesses are infected gland sites, filled with pus. If an abscess bursts or decompresses itself into the nearest structure, it will cause a fistula, an abnormal connection between the rectum and the skin or another organ • Crohn’s disease and ulcerative colitis – These are types of inflammatory bowel disease, where inflammation of the digestive tract can lead to abdominal pain and other potentially debilitating symptoms

While many cases of the conditions above can be treated with medication and lifestyle changes, the most severe cases must be treated surgically. Dr. Lord treats many of his surgical cases using the da Vinci Robotic Surgical System, an advanced, minimally-invasive approach that gives patients a quicker recovery and less post-operative pain.

Through his years of practice, when Dr. Lord desired a second opinion for his patients, he would send them to Mark Blankenship, MD. Now, the two physicians enjoy working together at Metro Surgical, PC. “Dr. Blankenship has long been considered the most cutting edge and best GI surgeon in the region and we’ve always had a good working relationship,” Lord says. “I was happy to have this chance at joining up with him.”

Both Dr. Lord and Dr. Blankenship encourage patients in Savannah to be up front about any symptoms they have.

“People would rather not talk about their colon,” Lord says. “But I think more people are coming around now that they see the options that are available—not only with colorectal surgery, but with colon cancer screening as well. These advances are saving lives and making a huge difference.”

n the 1970’s, she was a Bond girl. In the 80’s, she was called the Queen of the Miniseries. But in the early ‘90s, she was nearly penniless.

So Jane Seymour called her agent and said, “I need work yesterday. I’ll do anything.”

Jane Seymour An offer came for a movie-of-the-week called Dr. Quinn: Medicine Woman. The TV movie was created as a series pilot, though the studio had little hope for it.

“They said, ‘We know it won’t be a series,’” Seymour recalls. “It’s a woman in the lead, a family drama, a western with horses and children. It won’t work.”

Joyce The Dancer

The character of Dr. Quinn is an intelligent, proper woman who heads out to the Wild West of America.

Years before inhabiting this role, Jane Seymour had already made some adventurous moves herself.

Born Joyce Frankenberg in London, England, her desire as a young girl was to be a ballerina.

“My original intention was to be a classical dancer,” Seymour says. “But I injured myself at 17 and had to segue into acting.”

Her ballet school had also provided drama training, so she felt prepared for the challenge. But with her new direction came the suggestion for a stage name: Jane Seymour.

“We thought it was a good, easy English name. It was only later that we remembered the middle wife of Henry the VIII,” Seymour says. “People thought that they knew who I was because the name sounded familiar, so it worked out well. But I have never shied from publishing my real name too. I’m proud of my heritage.”

Solitaire Draws A Queen

Seymour quickly gained major film and television roles in England, and achieved international fame in 1973 as Solitaire, an exotic Bond girl in the film Live and Let Die. But as exciting as it was to be a part of the iconic film series, Seymour noticed that it made getting serious acting roles more difficult. So she took a chance on America.

“In 1976 I came here, where playing a Bond girl was not a problem,” Seymour says. “I gave myself six weeks to make it in America. And I’m still here.”

Her scene-stealing performances in epic television works brought her both Emmy and Golden Globe awards, and eventually had her dubbed the Queen of the Miniseries. But amidst this artistic success, her marriage was falling apart, and taking her financial future with it. That’s when she took the role in a show that was destined to fail.

however. The show was a surprise hit, one of the last successful Western dramas on television, generating some of the highest ratings of any show for a Saturday night timeslot. Episodes from the six-season series still play in about 100 countries around the world. Along with its talented cast and crew, Seymour believes the show struck a chord because it spoke to today’s issues through a historical lens.

“The show was about the human condition,” Seymour says. “There was not one episode that did not relate to something that was happening right now, even though it was set back in 1870. We did an episode about mercury poisoning, and the day we were filming the L.A. Times had a story about mercury in the water, left over from the Gold Rush.”

As the daughter of a doctor, Seymour insisted on accuracy in the portrayal of a late 1800’s physician, and the show’s creators often consulted the Smithsonian Institute.

“It was not only beautifully written, but also authentic,” Seymour says.

Open Hearts

Seymour continues to play memorable roles on the big and small screen, but acting is only a part of her creative life. She is the author of more than a dozen books, including Remarkable Changes, Making Yourself At Home, and The Road Ahead: Inspiration Stories of Open Hearts and Minds. She is also a painter, whose work has been exhibited in numerous galleries across the country.

Seymour’s art informs her design work, including the Open Hearts jewelry line that she created for Kay Jewelers. The idea behind Open Hearts came from Seymour’s mother, who told her that if we keep our hearts open, love will always find its way in.

Seymour embraces this philosophy not only through her art but also through several philanthropic endeavors. Along with her charitable work with organizations such as the American Red Cross and Childhelp, Seymour has run the Open Hearts Foundation since 2010. Her goal is to empower other non-profit organizations that need help to grow.

“We are like an incubator for these charities,” Seymour says. “We raise money and tell their story, which always pertains to the Open Hearts philosophy of taking a challenge and using it to help others.”

Return To Savannah

Seymour’s first experience of Savannah was actually for the filming of the miniseries East of Eden in the early 1980s, for which she won a Golden Globe.

“Coming from England where antiquities were common it was nice to see how beautifully preserved they were in Savannah,” she recalls.

She hopes that both the challenges and successes within her own story will have a positive impact at this year’s SmartWomen event.

“The most important question to me is how I can share my story in a way that enlightens people,” Seymour says. “I want to empower them with the experience I have had in my life.”

Less is With robotic sentinel lymph node biopsy, gynecological cancers can be staged less invasively

T

hroughout your body there are small round organs, found mostly around the blood vessels, which have that classic thankless job—taking out the trash.

“I sometimes call lymph nodes the garbage disposals of the body,” says gynecologic oncologist Sarah Gill, MD. The lymphatic system carries lymph fluid through the body, collecting viruses, bacteria, and other waste along the way to settle within the lymph nodes so the immune system can properly fight infection and kill abnormal cells. The “trash” is filtered by the lymph nodes, making them an important part of your immune system.

But they serve another purpose for Dr. Gill and other oncologists—lymph nodes can help determine if cancers have spread from their primary tumor site.

Lymph Node Biopsy

Cancer can metastasize through the lymphatic system, which is a common mechanism of spread in most gynecologic cancers. Lymph nodes found close to the primary cancer site are often the earliest sites of this spread.

“When patients with endometrial, early vulvar or early cervical cancers are initially seen, surgical staging is indicated to evaluate whether or not the cancer has spread to other areas of the body,” Dr. Gill says.

In the past, oncologists would perform a complete lymphadenectomy—meaning surgically removing all of the lymph nodes in a specific location in the body—to determine if the patient’s cancer has spread. Surgical staging, and therefore lymph node assessment, is important because it helps the physician determine the best treatment strategy and establish the patient’s prognosis. Unfortunately, a complete lymphadenectomy carries risks and side effects.

“With complete removal of the lymph nodes in the pelvis and groin, there is a high incidence of lymphedema in the legs,” Dr. Gill says. “It causes chronic swelling and can negatively impact a patient’s quality of life.”

Side effects of lymphadenectomy commonly include symptoms of heaviness or tightness in the legs, pain, infections, and decreased range of motion. The most common finding tends to be lymphedema, the swelling of the leg. There are many treatments for the condition, but it does not have a cure.

“Simply put, it feels bad and it looks bad,” Dr. Gill says. “It is bothersome in the very least, and in some cases it is life-changing.”

A New Way

As a way to decrease the risk of these side effects, Dr. Gill and Dr. Wm. Edward Richards, her colleague at St. Joseph's/Candler Gynecologic Oncology & Surgical Specialists, offer a newly established surgical technique utilizing the da Vinci robotic surgical system where only the sentinel node is removed for surgical staging. The sentinel node is the first node that drains a particular organ, and is a reliable indicator of the presence of metastatic cancer.

“Instead of removing many lymph nodes in the pelvis or inguinal area, we inject a dye that allows us to identify the very first lymph node that drains the organ where the cancer originated. We can assess for metastatic disease just as accurately, but we only have to remove that one node,” Dr. Gill explains. “With cervical and endometrial cancer, we use a dye called indocyanine green. It is fluorescent and lights up with the infrared camera of da Vinci robotic surgical system, allowing us to see where that sentinel node is located.”

Different dyes are used for vulvar cancer but the benefit is the same.

“It’s changed our practice tremendously,” Dr. Gill says. “Not only does it significantly reduce the risk of lymphedema, but it also decreases the risk of vascular injuries and damage to nerves.”

Lymphatic System

Smart Treatment In rare cases, the procedure may not clearly identify the sentinel node, and a complete lymphadenectomy may still be needed. Because of the importance of surgical staging, Dr. Gill believes the benefits ultimately outweigh the risks.

“If cancer is found in the lymph nodes, the recommended treatment and prognosis is very different,” she says. “For vulvar cancer, lymph nodes with metastatic cancer could mean the addition of radiation treatment. For cervical cancer, it may change the type of surgery we perform. When a patient is upstaged from a uterine-confined disease to a metastatic cancer, it usually warrants additional therapy with chemotherapy and/or radiation. So you can see that the information gleaned is vital for patients’ wellbeing.”

With the advanced imagery and precision afforded to her from the da Vinci robotic surgical system and the fluorescent dye, Dr. Gill can make those determinations on staging and treatment without increasing the risk of lymphedema from the procedure.

“It’s crucial to assess the nodes, but now we can do it in a much smarter and safer way,” Dr. Gill says. “We can get the same information with less risk to the patient.”

ave you ever taken your own pulse, or had a nurse take your pulse with just the tips of their fingers on your wrist? Your heart rate can be felt through your radial artery, which supplies your hand with blood.

That same artery is now allowing cardiologists to perform a life-changing procedure for patients with narrowed heart arteries.

And there are quite a few of those patients out there—coronary artery disease (CAD) is the most common type of heart disease in the United States. Cholesterol and other deposits create plaque on the walls of arteries. The buildup of plaque in arteries that supply blood to the heart can partially, and sometimes totally, block the blood flow. A common symptom of CAD is angina, or chest pain. It is often an early sign of a heart attack.

Reshaping Blood Vessels To treat blocked arteries, interventional cardiologists like Roy D. Flood, Jr., MD, of SouthCoast Health, perform an angioplasty (“angio-“ means relating to blood vessels, “-plasty” means reshaping or reforming a certain part). In this procedure, Dr. Flood guides a balloon catheter to Roy D. Flood, Jr., MD the narrowed or blocked area and then inflates the balloon, reshaping and expanding the artery walls and improving blood flow. In many cases, a tube called a stent is placed in the re-opened area to ensure that it remains open.

“Patients often feel better quickly, with their energy returning and their symptoms of angina and shortness of breath improving,” Dr. Flood says.

When it was first developed in the late 1970s, angioplasty significantly changed the treatment of clogged arteries, helping many patients avoid the risks and extensive recovery period of open-heart surgery. Now, a new approach to the procedure is benefitting patients even more.

The Radial Artery Access to the heart’s arteries has traditionally begun in the femoral artery, a major blood vessel in a person’s groin area. Though the angioplasty would help the patient’s blockages, they would now possibly face bleeding, swelling, or pain at the puncture site. In fact, 6

patients would have to lie still for hours to prevent bleeding from the femoral artery.

Dr. Flood avoids these complications by accessing the blood vessels through the radial artery in the patient’s wrist.

“When I perform an angioplasty this way, it provides several benefits to my patients,” Dr. Flood says. “They have increased mobility, meaning less down time for the patient, and in many cases there is less bleeding from the site.”

Arterial Plaque

The smaller size of the radial artery makes it easier to stop the bleeding at the puncture site.

And with only the patient’s wrist affected by catheter, patients are up and walking around much sooner.

“Recovery is not only quicker, it’s easier,” Dr. Flood says. “You still have to monitor the insertion site, but the patient is not stuck with immobility.”

Dr. Flood notes that a common anxiety about recovery is being unable to exert your heart enough to walk long distances or climb stairs, but the radial artery approach can at least help with practical issues that may arise.

“We’re more concerned about you bending that leg if you need to take a step up to get in your house,” he says.

Speaking of that return home, Dr. Flood says that patients treated with angioplasty through the radial artery can usually go home on the same day as the procedure.

Not every patient is a candidate for this approach. If someone has calcified arteries, for example, the smaller radial artery could be too difficult to navigate. But Dr. Flood is excited to be able to offer this approach to appropriate patients. During the procedure, patients are sedated but conscious, and the threading of the catheter is painless.

Getting Moving, Staying Healthy “After their procedure is done, patients often tell me it wasn’t as ominous as they thought it would be,” Dr. Flood says. “Especially now with the radial approach, the procedure is less intimidating to the patient.”

Dr. Flood hopes that the improved energy and relief from symptoms that patients will experience after the procedure will help them take better care of themselves and prevent future cardiovascular disease. A patient’s ability to get up and move sooner after a radial artery angioplasty should be the first positive step toward a change in lifestyle.

“In the ideal world, everyone would stop smoking, eat a plant-based diet, take their medications, and manage their diabetes,” Flood says. He adds with a smile, “And I would be out of a job.”

Knowing All

DIMENSIONS One of the largest breast cancer studies in decades is focused on mammography

B

reast cancer awareness is crucial because early detection, especially of aggressive breast cancers, can have such a strong effect on treatment and outcomes for patients. From a clinical standpoint, researchers continue to strive for technological advances in screening and early detection, but what matters most are results. That’s why St. Joseph’s/Candler and the Nancy N. and J.C. Lewis Cancer & Research Pavilion have joined one of the largest studies on breast cancer screening performed in decades.

The Tomosynthesis Mammographic Imaging Screening Trial (TMIST) is the first randomized trial to compare two types of digital mammography for breast cancer screening, 2-D and 3-D. Standard digital mammography (2-D) takes a single picture from two angles of the breast to create a flat image, while 3-D mammography takes multiple low dose images around the breast that are then built into a 3-D image that the radiologist can scroll through, layer by layer.

Savannah women ages 45 to 75 who are planning to get a routine screening mammogram are eligible to participate in TMIST. Participants must receive their mammogram at St. Joseph’s/Candler’s Telfair Pavilion.

“Our participants will be among the 165,000 women across the country helping with this trial,” says Lora Reese, Director of the Telfair Pavilion. “All of us share the same goal—to provide data that will give caregivers and their patients a new understanding of the impact of mammography technology. This study will also help us learn more about the biology of the cancers that both 2-D and 3-D screenings detect, and to determine if the newer technology finds more aggressive tumors associated with a worse outcome sooner when the prognosis is better for the patient.”

their eyes. “Sugar is nothing…what about those of us with a ‘salt tooth?’” We see you, and we just want you to be aware of a few things before you indulge, particularly the risk of high blood pressure. Take our quiz to learn more:

E. All of the above. Cravings can not only be physically based but emotionally as well. And if you are tired, you’re more likely to give in to cravings.

D. All of the above. These foods are part of the Salty Six, a list created by American Heart Association/American Stroke Association to warn consumers about hidden high amounts of sodium. Others on the list are cold cuts and cured meats, soup, and fast food sandwiches. Always compare nutrition labels for the lowest sodium amount per serving.

6. Sea salt is better for the body than table salt because it is less processed.

B. False. By weight, the sodium content of both types is the same: about 40 percent. Because of the larger crystals of sea salt, there may be less sodium by volume, for instance per teaspoon. But there is little nutritional difference—it is more a matter of your preferred taste and texture.

Excellent Organization Skills Contribute To Highest Quality Care From Nurses Roni Kelly, RN 6 North, Candler Hospital

Smart Living: Patients want nurses who have exceptional clinical skills as well as a compassionate bedside manner, but are there additional skills that a nurse should have that perhaps the patient doesn’t see?

Roni Kelly: Yes, great organizational skills are important, too. Our supplies need to be organized in a way that helps us respond to a patient’s needs promptly. This is true not just of the patient’s room but also our work stations and supply carts. Everything needs to be in its right place. And on the flip side of that, anything that does not belong needs to be removed. We always want to be fully focused on patient care and not have distractions, such as clutter, getting in our way.

In fact, St. Joseph’s/Candler recently completed a hospital-wide initiative to remove older equipment and other unnecessary clutter so that we can all experience a more pleasant work environment.

SL: Conventional wisdom says that an uncluttered space at home decreases stress and helps a household operate more smoothly. It sounds like the same is true for a hospital?

RK: Absolutely, and it works for both the patients and the nurses. First of all, it’s safer. De-cluttering decreases the risk of falls. Also, a clean, professional environment makes the patient feel better. And by staying on top of our organizational plan and preventing clutter, we are free to go above and beyond for our patients in other ways. I work three days back-to-back, which allows me to learn my patient’s personal needs on day one. Then on day two and three I know who likes coffee early in the morning, or who needs apple juice or applesauce, for example. So along with clinical care and making the patient feel better, I have time to create a great nurse-patient relationship.
