Survey Resources
Facebook: 1. You can use Facebook to make surveys. -‐go to: http://www.facebook.com/simple.surveys -‐click “get started” Survey Monkey: 2. Create a New Survey Form -‐Click the “Create Survey” button located on the top right of all of the pages in your account to open the create survey page 3. Choose one of three options to create a new survey -‐Create a new survey from scratch -‐copy an existing survey -‐use a survey template 4. Add Questions -‐Click the “Add Question Here” button 5. There are specifics about questions -‐Choose the question type from the drop-‐down menu -‐type the questions into the provided textbox -‐type the answer choices into the provided textbox -‐choose to add any of the additional features -‐click the “Save Changes” button when you are finished 6. You can format your questions with your specific needs -‐Sort/Randomize answer choices -‐add comment field -‐validate text -‐require answer to questions

7. Use the editing options for your questions -‐Move question -‐Copy question -‐Delete question -‐Restore question 8. Add Pages -‐If you have a long survey, you may want to separate it by pages -‐Click the “Add Page Before/After” button to add a new page to your survey 9. Utilize Edit Pages tools -‐Move page -‐Copy page -‐Delete page
10. You can Preview Pages before finishing -‐View Pages -‐Preview your Survey -‐Print your survey design 11. Customize your Design -‐You can customize your design to relate to your organization -‐Edit page and questions numbering -‐add a logo -‐progress bar -‐hide/display survey and page titles -‐edit navigation links -‐required questions 12. Themes -‐Survey Monkey has pre-‐created themes with color schemes that may fit your organization
13. Save your Survey -‐Make sure you hit the “save” button after you edit any of your questions, pages, etc!
14. Send/Publish Survey -‐When you finish creating the survey, Survey Monkey will provide you with a link to your survey that you can e-‐mail or post to your Facebook/Twitter.
