Easy Baked Salmon Ingredients: 1 pound salmon filet 1 cup cherry tomatoes (cut in half) 5 green onion (sliced) ¼ cup chopped fresh parsley ¼ tsp. salt ¼ tsp. pepper Directions: 1 Spray shallow baking dish with nonstick cooking spray. Place salmon filet in dish, skin side down. Cover with tomatoes, onions and parsley. Sprinkle with salt and pepper.

2 Bake in the oven at 425 degrees for 15 minutes or until flaky.

A Trusted Guide for Your Medical Journey

I

f you were traveling to a faraway foreign land, to a place you’ve never been where you don’t speak the language and don’t know a soul, chances are that you’d insist on having an experienced guide to help make sure your journey was a success.

You’d want someone who’s connected to all the right people, knows all the shortcuts to take, all the places worth visiting and which ones to avoid. That would make all the difference.

That’s very much what it’s like for most people facing joint replacement or spine-related surgery for the first time. They’re looking for a guide – or in this case, a nurse navigator – to steer them with confidence through every step of their medical journey.

A nurse navigator plays many roles for the benefit of patients.

This includes acting as an educator, advocate, liaison, coach, coordinator, guide and problem solver. At AdventHealth Shawnee Mission, the navigator’s role is to help you understand your care from the moment of diagnosis through treatment and recovery. Of course, for each patient, the path of treatment and recovery is unique.

“When I speak to patients, I really want to know the history of their back pain and what they have done in the past, their current symptoms and how this pain affects their ability to feel whole,” said Heather Roy, spine nurse navigator, AdventHealth Shawnee Mission. “I urge patients to call or email me anytime with questions or concerns, allowing an easy point of contact.”

“I first connect with patients after they have been scheduled for surgery. It can be a scary time as they face the unknown. Because of this, AdventHealth provides free, pre-surgery education classes to all patients undergoing spinal surgery as well as hip and knee replacement,” said Amber Keller, orthopedic nurse navigator, AdventHealth Shawnee Mission.

Whether it’s a joint replacement or spine procedure, it’s only natural for patients to have a lot of questions to answer and fears to address. Navigators strive to build relationships with each patient and caregiver by providing continual support and resources to meet the evolving needs of patients along their medical journey.

“For me, the most rewarding part of the process is when you see or hear the “light bulb” go off and know that the patient truly understands their diagnosis, care plan and why we do the things that we do. When you are able to communicate in a way they understand, the relationship is strengthened and their spine health journey flows more smoothly,” said Roy. “Throughout each individual medical journey, we collaborate and network with multiple departments such as Emergency Care, Physical Therapy, Primary Care Providers, Pain Management, specialty offices and the Wellness Department. We also participate in multidisciplinary team rounds when patients are hospitalized,” said Keller.

“Because I maintain communication with the patients’ care team throughout their

journey, if any concerns do arise, I inform their providers via a phone call or message. This enables providers to know what education has been provided to the patient and allows for providers to adjust the patient visit appropriately,” said Roy. Whether a patient’s surgery is an outpatient procedure or requires a hospital stay, the nurse navigator helps patients experience the whole-person approach to care they deserve – from pre-surgery classes to post-operation rehab and therapy, helping them heal in body, mind and spirit.

“When you are able to develop a relationship with patients,

providing informational and emotional support comes naturally.

The continual communication I have with patients helps me to understand their fears, challenges and also inform them of next steps early in the process,” said Roy. Each and every day, our nurse navigators are providing critical support and services to orthopedic and spine patients – all aimed at making their journey as smooth and successful as possible.

“From our very first phone conversation prior to surgery to reviewing education material before their transition home, a personal connection truly makes all the difference,” said Keller.

Walk S your way to a quicker recovery

o much important planning and preparation happens before surgery – taking care of your health, learning as much as you can about the procedure and getting to know the people who will be taking care of you. After all, planning ahead can help ensure you have a successful procedure.

But many times, there isn’t nearly as much thinking in advance about the best way to recover after surgery. The goal is for you to get healthy, comfortable and discharged as soon as possible.

Of course, healing after surgery always takes time. But how much time should it take?

That often depends on how quickly you can regain your mobility.

For example, early walking after surgery is one of the most crucial things you can do to prevent problems. It improves blood flow, speeds wound healing and promotes the flow of oxygen throughout your body. It also strengthens your muscle tone.

“Early mobility essentially means getting a patient moving as soon as possible after a surgical intervention. A common example would be getting a patient up, out of bed and even walking in the afternoon of a morning hip

Early mobility can also refer to range of motion or movement of the area that was operated on.

With a knee surgery, for example, getting the knee flexing and extending and getting the muscles around the joint contracting and being active as soon as possible can reduce the negative effects of bed rest and time off the feet.

Just as important, the dangers of avoiding early mobility are quite serious. The more time that passes before getting a body part moving after surgery, the harder it is going to be to gain that mobility and strength back. Scar tissue develops quickly, and joints stiffen up.

“It does not take long for deconditioning, muscle atrophy, bone density reduction, balance deficits or the risk of pneumonia to arise when someone spends time being immobile. This is why it is so important to get moving quickly and remain as active as possible given any restrictions one may have as a result of the surgical intervention,” said Fix. Of course, some patients may resist early mobility, especially if they think they aren’t able to do anything for a while after surgery.

bodies aren’t ready to handle the movement, or that moving may be too painful.

“Making sure the patient knows the benefit of getting moving – or the risks of not moving – is usually enough to encourage them to try,” said Fix. “When we can tap into what truly motivates the patient and help them see the light at the end of the tunnel, we’re able to help them break through any resistance.”

Measuring progress Depending on the surgical procedure, there are many ways to determine how well a patient is recovering – gains in range of motion or mobility, gains in strength, the ability to do more repetitions or more resistance on an exercise, etc. Progress is also measured in the clinic using questionnaires that track scores based on patient responses.

“The most important way we track patients’ progress is by improvements in their ability to get back to the things that they identified as their goals. How well are they able to participate in their daily functions and life activities they need and want to be doing?

That’s the progress that really matters,” said Fix.

What are some of the benefits and risks of joint replacement?

The decision to get a joint replacement (arthroplasty) usually occurs after the failure of conservative care such as therapy, medication, weight loss (if needed), activity modification, etc. The goal is to eliminate pain and allow a return to normal, painless activity.

As with any surgery, there are risks which include infection, damage to nerves or vessels, blood clot, dislocation (hip arthroplasty) and stiffness. The procedure also requires effort on the patient’s part to participate in a rehabilitation program to optimize the outcome.

What has changed about today’s joint replacements?

Both the design of the implants and the materials used to manufacture the implants continues 10 | AdventHealth | SPRING 2020

to improve, which translates to greater longevity and less or later need for revision. Additionally, less invasive surgical technique and better perioperative management allows faster mobility and shorter hospital stays.

Joint replacement is even being done on an outpatient basis.

Earlier, more accelerated, rehabilitation gives patients a faster return to normal activity, such as work, exercise, etc.

Are joint replacement patients getting younger?

Not only are joint replacements being done in younger patients, but they are also being done in older patients. As each generation becomes more active, and as humans live longer, their activity and longevity cause joints to wear out and increases the demand for intervention.

Speaking of longevity, how long do the joint replacements tend to last?

In 2019, The Lancet (a weekly peer-reviewed medical journal) reported almost 60 percent of hip arthroplasties still functioning well at 25 years. In 2017, they reported 4 percent failure of hip and knee arthroplasties after 10 years, increasing to 15 percent for hips and 10 percent for knees after 20 years.

FREEDOM FROM SMOKING Free yourself to live your life as a non-user of cigarettes, chewing tobacco or vaping.

Bi-weekly meetings led by an ALA Facilitator, are available to help you make the lifestyle changes to be a successful quitter. Sessions include practical skill building and coping strategies once you have quit the habit to help you stay on the road to a healthier lifestyle! Are you a parent of a vape user? These meetings will provide information about vaping, the effects of it, what to look for, and how to keep an open dialogue with your child. Each class includes group discussions and interaction.

Every pound you lose takes four pounds of pressure off your knees.

Talk to your doctor before getting started.

Try to stretch at least three times a week but start with a light warmup to loosen up the joints, ligaments and tendons.

Keep Moving

Low-Impact Activities

The more you move, the less stiffness you’ll have.

Whether you’re reading, working or watching TV, take breaks from your desk or your chair and get active.

Know Your Limits

Stronger abs and back muscles help your balance. That means you’re less likely to fall or get injured.

If you hurt for more than 48 hours, you may have overdone it. Don’t push so hard next time.

Dairy products are the best sources of calcium, but other options are green, leafy vegetables like broccoli and kale.

Stand and sit up straight to protect joints from your neck down to your knees. Walking and swimming can also help.

Ice is a natural pain reliever. If you have a sore joint, apply a cold pack or ice wrapped in a towel.

The best choices are activities that don't pound your joints, like walking, bicycling, swimming and strength training.

Eat More Fish Salmon and mackerel are good sources of omega-3 fatty acids, they also lower inflammation, which causes joint pain.

Qualify Supplements If you want to try supplements, ask your doctor what’s safe and what might affect your medicines or health conditions.
