YEAR IN REVIEW

2024 in 5 Words

After another incredible year at Share Ourselves, I find myself reflecting on the moments that defined 2024. While so much transpired, there are five words that stood out— words that capture our journey, our impact, and the heart of this community.

GROWTH: In 2024, we expanded our reach in powerful ways. At our Mission Viejo clinic, we doubled our footprint , allowing us to serve more medical patients and we introduced dental services at this location. We also launched a school-based health center at the BESST Center in Costa Mesa, bringing critical medical and behavioral health care directly to students and their families across the district.

HOPE: Every day, families walk through our doors carrying uncertainty—but they leave with something else: hope. From a mother receiving groceries to keep her children fed, to a patient accessing life-changing medication, hope is what fuels our mission.

COMMUNITY: Change happens when people come together, and this year, our community showed up in incredible ways. From the volunteers who gave their time, to the donors who made our programs and services possible, to the partners who helped expand our impact, every act of kindness made a difference.

HEALING: At Share Ourselves, healing is more than medicine—it’s compassion, dignity, and care for the whole person. This year, our clinics, Food Pantry, and community programs ensured that thousands of people received the help they needed to heal, inside and out.

FUTURE: As we look ahead to 2025, we see a future filled with possibility. A future where more families have access to care, more neighbors receive support, and more lives are positively impacted.

But there’s one more word that truly defines this year: You.

Whole-Person Care

Whole-person care is a patient-centered approach that recognizes that health is influenced by more than just medical conditions —it’s shaped by social, behavioral, and environmental factors.

As a Federally Qualified Health Center (FQHC), Share Ourselves is committed to providing comprehensive, quality health and social services to underserved individuals and families, regardless of their income or insurance status. This means addressing not only immediate medical concerns but also the broader social determinants of health, such as food insecurity, unstable housing, and lack of transportation.

For many in our community, fragmented healthcare systems create barriers that lead to delayed treatment, worsening conditions, and unnecessary emergency room visits. As an FQHC, Share Ourselves plays a critical role in bridging these gaps by offering integrated, coordinated services tailored to each individual’s needs.

A patient managing diabetes, for example, may require more than medication—they might need access to nutritious food, support in navigating insurance, or behavioral health assistance to identify and work through barriers to successfully managing their blood sugar levels. By focusing on the whole person, we empower our patients to take control of their health and work toward long-term stability.

Whole-person care is especially crucial in Orange County, where economic disparities and rising costs of living make it difficult for many families to prioritize their health. Through our commitment to whole-person care, we aim to break the cycle of poverty and poor health, ensuring that everyone has the opportunity to live a healthier, more stable life.

What is a Federally Qualified Health Center?

A Federally Qualified Health Center (FQHC) is a community-based healthcare provider that receives funding from the federal government to offer comprehensive primary care services to underserved populations. As an FQHC, Share Ourselves must meet certain requirements and provide care regardless of a patient’s ability to pay:

• Serve Medically Underserved Areas: Be located in communities with limited access to healthcare.

• Offer Comprehensive Services: Medical, dental, behavioral health, pharmacy, and more.

• Offer a Sliding Fee Scale: Patients are charged based on their income and ability to pay.

• Be Governed by a Community Board: At least 51% of board members must be patients.

FQHCs play a critical role in reducing health disparities by providing accessible and affordable healthcare to low-income, uninsured, and underinsured individuals.

Share Ourselves strengthens the healthcare system by providing preventive care, chronic disease management, and affordable services, which helps reduce emergency room visits, lower hospitalization rates, and improve overall healthcare efficiency. As part of a nationwide network of FQHCs, our whole-person care approach contributes to the $24 billion in annual savings* nationwide by ensuring individuals receive timely, comprehensive care in a costeffective manner.

We Strive for Excellence in all we do, especially in expanding access to quality healthcare. In 2024, we doubled our capacity for medical services at our Mission Viejo clinic and introduced dental care for the first time. We also opened a new school-based health center at the BESST Center in Costa Mesa, bringing essential healthcare to students, families, and the surrounding community.

Lead with Dignity

For nearly a decade, Paige has relied on Share Ourselves for support through some of life’s most difficult moments. Struggling with addiction and job loss, Paige remained determined to rebuild her life, earning her bachelor’s degree at 54. Yet financial instability and a lack of affordable housing forced her into homelessness. After experiencing homelessness and immense personal hardship, she found more than just assistance—she found a community that treated her with dignity and compassion. In the face of these overwhelming challenges, Share Ourselves became her lifeline, providing essential resources, care, and a sense of belonging.

Champion Justice

Now, housed and thriving, Paige is a testament to the power of perseverance and community support. Her journey wasn’t just about securing housing—it was about reclaiming her independence and sense of purpose. With a hopeful outlook, she is eager to give back, particularly by working with animals, a lifelong passion she now has the opportunity to pursue. Paige’s story reflects Share Ourselves’ deep commitment to Lead with Dignity, fostering long-term stability and ensuring that no one is defined by their past struggles.

Championing Justice means ensuring that everyone, regardless of their circumstances, has equal access to health and social services. Through Care Management, we help individuals navigate complex systems, removing barriers to essential resources and empowering them to achieve better health and stability.

When Maria’s husband suffered a severe brain injury, she and their five children faced overwhelming hardship. Without his income, they lost their home and were forced to live in their van, struggling to meet basic needs. Navigating healthcare and social services alone felt impossible. That’s when Share Ourselves stepped in. A dedicated care manager connected them to medical care, food assistance, and housing resources, helping Maria apply for financial aid and advocating for the family’s long-term stability. Maria’s experiences demonstrate our value: Champion Justice—ensuring that no one has to face hardship alone and that every family has a fair chance at a healthier, more secure future.

We go beyond traditional healthcare by offering social services designed to support overall well-being. These services—including care management, food distribution, housing navigation resources, financial assistance, and mail services—are designed to help our patients receive the support they need in all aspects of life.

Through programs like Adopt A Family, Back to School, Family Service Night and Thanksgiving Food Distribution, volunteers uplift neighbors facing economic hardships. “Our OC community clinics provide care to about 300,000 of our CalOptima Health members. CalOptima insures about 950,000 people in Orange County. That’s a third of the county. That means a third of the county lives below the federal poverty level. We cannot do this work without the support of our partners and that includes clinics like Share Ourselves.”

Collaboration is at the heart of Share Ourselves’ mission. With over a third of Orange County residents living at or below the federal poverty level, partnerships with other healthcare providers, social service agencies, community organizations, and local businesses are critical in executing our mission.

By working together with partners such as CalOptima Health, Hoag Hospital, and fellow nonprofits, Share Ourselves strengthens the safety net that so many individuals and families rely on. These partnerships enable us to expand access to care, coordinate services, and provide comprehensive support that goes beyond medical treatment.

Whether it’s connecting patients to housing resources, offering nutritional assistance through our Food Pantry, or ensuring they have access to affordable medications, our ability to meet these needs is amplified through strong, strategic collaborations.

Beyond healthcare, our partnerships also extend to schools, faith-based organizations, and corporations that share our vision of a healthier community.

Programs and services like our Back to School initiative, Thanksgiving food drives, Adopt A Family, and First In Family scholarship program demonstrate how collective action can make a meaningful difference in people’s lives. By uniting our efforts, we can provide not only essential healthcare services but also the social and economic support needed for long-term well-being.

How does Share Ourselves use revenue to further its mission?

While patient revenue directly supports the care provided, additional funding sources play a crucial role in ensuring we can provide whole-person care. These funds help us expand services, provide uncompensated care for those unable to pay, and support essential resources like our Food Pantry, social services, and community programs. We also reinvest in our operations to ensure long-term sustainability, allowing us to continue serving those who rely on us most.

Share Ourselves is proud that an average of 80 cents of every dollar we spend is invested in delivering care and comfort to those in need, with total administrative and fundraising expenses at 20%.
