HEALTH AT HOME - NONMEDICAL, SKILLED HOME HEALTH, AND HOSPICE CARE

This section offers three different comparison grids; the first features Non-Medical and Personal Assistance at home, the second contains Skilled Home Healthcare, and the third grid showcases Hospice Services for end of life care. First familiarize yourself with the different services that are offered by each agency. You will find definitions about each service at the beginning of this section. Many options exist; we hope this will help you find a provider to meet your specific needs.

Utah’s population will soon exceed 4 million people. People who are 65 and older make up about 15% of the total population, and in the near future, are expected to represent one in every five residents. This demographic trend impacts our households, culture, economy, environment, and increasing needs of older adults.

Utah benefits from the wisdom and contributions of our older adults, and we recognize that the next generation will be more diverse, live longer, and desire to make meaningful contributions to our great state. If we don’t have a meaningful plan for an aging population, we expect to see an increase in the struggles for meaningful employment, affordable housing, food and financial security, and access to quality health care. These trends are not new, but without deliberate planning, the challenges will accelerate and overshadow the opportunities that should be available to everyone. The purpose of the strategic plan for intelligent aging is to make sure Utahns are engaged and valued across the spectrum of aging into older adulthood.

S.B. 104 Master Plan For Aging was signed into law March 15, 2023, and directs the Utah Commission on Aging to periodically publish a plan that will guide the aging experience over the future decades and make sure all Utahns are able to navigate the challenges and opportunities of older adulthood.

This is a call to action for state and local governments, private and public entities, and individuals to engage innovatively, proactively, and collaboratively to create a Utah where people of all ages can thrive and age with the grace and dignity they deserve. This plan helps lead the way.

AFFORDABLE ADULT DAY CARE

Neighborhood House provides quality, affordable day care and support services to children and aging or disabled adults in the Salt Lake Valley.

COMMUNITY

Open Monday -Friday | 7:30am-5:30pm

Our program caters to adults 18+ who have disabilities and need support during the day. Disabilities may include cognitive impairment, dementia, developmental disability, TBI, and/or other chronic conditions. Neighborhood House is committed to principles of inclusion, independence, and valued membership in community. Our intergenerational programming supports that commitment through regular events that creates a connection between adults and youth and normalize aging and disabilities within the community around them.

AFFORDABLE CARE

Payments on a sliding fee scale and door-to-door transportation within bus boundaries ensure convenience and affordability for all.

HOME AWAY FROM HOME

The adult program focuses on meeting each person‘s needs, including social, emotional, physical, and cognitive aspects. Certified staff lead recreational therapy activities, assist clients in selecting their preferred activities and engagement levels throughout the day, and provide language translation services or any other necessary assistance as needed. Connection with community is provided through regular outings, contracted performers or teachers, and engagement with volunteers or student interns. The weekly program offers a variety of activities including yoga, horticultural therapy, arts & crafts, dance, music, visits to the sensory room, and intergenerational activities. Person-centered plans are developed upon enrollment by a recreational therapist who also regularly reviews progress alongside program staff.

For caregivers needing respite, enrollment of a family member in day care is a “win-win”. Having a loved one at Neighborhood House Adult Day Center is an opportunity for enhanced quality of life of both client and caregiver.

A Hub for Utah’s Seniors: Area Agencies on Aging

Area Agencies on Aging (AAAs) are local, nonprofit organizations that serve as a crucial resource for older adults, caregivers, and their families. Established under the federal Older Americans Act, their mission is to help seniors stay independent and healthy in their own homes and communities for as long as possible. They are a one-stop shop for information and assistance on a variety of aging-related issues.

SERVICES THEY OFFER

While services can vary by location, AAAs typically provide a broad range of support programs. One of the most well-known services is nutrition support, which includes home-delivered meals like Meals on Wheels and congregate meal sites where seniors can eat together and socialize. They also offer caregiver support, providing respite care, training, and resources to family members who are caring for a loved one. Other vital services include care management, where a professional helps develop a care plan, and legal assistance and benefits counseling to help navigate complex programs like Medicare, Medicaid, and other public benefits. AAAs also provide transportation assistance and act as advocates for seniors at the local level.

Be Your Own Brain Health Advocate

Nearly 119 years ago, Alzheimer’s disease was first diagnosed by Dr. Alois Alzheimer. Today, as 38,300 Utahns among more than 7 million Americans struggle with the disease, not one person has survived the diagnosis.

However, as research into Alzheimer’s proceeds at a record pace, scientists have recently developed a number of lifestyle and medical approaches that can potentially reduce the risk or, hopefully, prevent the onset of Alzheimer’s.

“The bad news is that age is the number one risk factor for developing Alzheimer’s,” said Stacie Kulp, executive director of the Alzheimer’s Association of Utah. “The good news is researchers believe about 45% of all cases can be prevented or delayed.”

Following are results of research studies, presented at the Alzheimer’s Association International Conference, that provide proactive steps people can take to preserve their own cognitive health.

Hearing aids slow cognitive decline

Researchers estimate 65% of adults over age 60 have hearing loss. This study, looking at a subgroup of older adults with hearing loss who were at higher risk for cognitive decline (about ¼ of the total study population), showed those at highest risk for cognitive decline who utilized hearing aids and hearing counseling for three years cut their cognitive decline in half.

Manage your blood pressure

People treated by FDA-approved medications to a top (systolic) blood pressure reading of 120 instead of 140 were 19% less likely to develop mild cognitive impairment, according to a Wake Forest Baptist Medical Center study. Those people also showed fewer signs of damage on brain scans, and there was a possible trend toward fewer cases of dementia.

Gut health & constipation

16% of the world’s population struggles with constipation – more among older adults due to fiber-deficient diets, lack of exercise and use of certain medications. Researchers found that bowel movement frequency of less than every 3 days was associated with 73% higher odds of subjective cognitive decline and other long-term health issues.

Get vaccinated

Getting an annual flu vaccination was associated with a 40% decrease in the risk of developing Alzheimer’s disease over the next four years, according to researchers from University of Texas’ McGovern Medical School who found that even a single flu vaccination could reduce the risk of Alzheimer’s 17%.

Cognitive benefits of volunteering

A University of Colorado study of an adult population aged 70 to 79 showed that volunteering was related to 16% lower odds of developing dementia (24% lower among women).

Wildfire smoke

It’s worse than you thought. More than 42,000 wildfires in the U.S. in 2024 burned 9 million acres of land. A 10-year study has shown that smoke from wildfires is sig-nificantly more hazardous for our cognitive health than other forms of fine particulate matter air pollution.

Researchers analyzed health records from more than 1.2 million Southern California residents 60 or older, none of whom had been diagnosed with dementia at the beginning of the study. They found an increased risk of dementia due to wildfire smoke exposure, even with less exposure than to other air pollution sources. Wildfire smoke may be more hazardous because it is produced at higher temperatures, contains a greater concentration of toxic chemicals and, on average, the particulate matter is smaller than that from other sources.

Bacon and sausages

All things in moderation. People who eat about two servings a week of processed red meat have a 14% higher risk of dementia than those who eat less than about three servings a month, according to Harvard researchers. Examples of processed red meat include bacon, hot dogs and sausage. A serving equals about 2 slices of bacon or one hot dog.

Get vaccinated (part 2)

Getting a vaccination against pneumonia between the ages of 65 and 75 reduced Alzheimer’s risk up to 40%

according to a Duke University study.

Diabetes/weight loss drugs may protect the brain. Liraglutide, a glucagon-like peptide drug, appears to slow cognitive decline and protect the brain from shrinkage, according to Phase 2b clinical trial results. Researchers at London’s Imperial College studied liraglutide, a GLP-1 agonist similar to Ozempic and Zepbound, which have been shown to help with diabetes and weight loss and reduce heart disease risk.

o People in the study who received liraglutide had slower decline in cognitive function after one year compared to those who received the placebo. The drug appeared to reduce shrinking nearly 50% in the parts of the brain that control memory, learning, language and decision-making.

“Ultra-processed” food might hurt our brains.

Researchers studied over 10,000 people over eight years and found that high consumption (more than 20% of daily caloric intake) of ultra-processed foods led to a 28% faster decline in cognitive function.

o Ultra-processed foods include sodas, breakfast cereals, white bread, potato chips and frozen foods, such as lasagna, pizza, ice cream and fries.

ELDER ABUSE PREVENTION

Safeguarding Our Vulnerable Adults

Utah Adult Protective Services (APS) is a vital social service program dedicated to protecting vulnerable adults from abuse, neglect, and exploitation. Their mission is to ensure that all adults—particularly those who are at risk due to age or disability—can live with dignity, respect, and safety. APS professionals work tirelessly to investigate allegations of mistreatment and connect individuals with the support they need to remain safe in their homes and communities.

A “vulnerable adult” in Utah is defined as someone age 65 or older, or a person between 18 and 64 with a physical or mental impairment that substantially affects their ability to protect themselves. This includes individuals who cannot provide for their basic needs, manage their resources, or comprehend the consequences of remaining in a harmful situation.

Utah APS investigates four primary types of maltreatment:

• Abuse: This includes physical, emotional, or sexual harm. Signs may include unexplained bruises, fearfulness, or social isolation.

• Neglect: When a caregiver fails to provide the necessary food, clothing, shelter, or medical care a vulnerable adult depends on.

• Self-Neglect: When an individual, due to a mental or physical impairment, fails to provide for their own essential needs, threatening their health and well-being.

• Exploitation: The illegal or improper use of a vulnerable adult’s funds, property, or assets for someone else’s benefit.

The services provided by Utah APS are short-term and focused on intervention and protection. Once an investigation is complete, APS works to connect the vulnerable adult with resources that can address their needs, such as home health services, meal delivery, or legal assistance. It’s important to note that APS cannot force a person to accept services unless they are found to be a danger to themselves or others.

Anyone who has reason to believe a vulnerable adult is being abused, neglected, or exploited is legally required to report it. If there is an emergency and the person is in immediate danger, call 911.

For all other reports, contact Utah APS. You can make a confidential report by calling their hotline at 1-800-371-7897 (Monday-Friday, 8 a.m. - 5 p.m.) or by submitting a report online 24/7 on the Utah Division of Aging and Adult Services website. Your report can be anonymous, and it is the first step toward ensuring a vulnerable adult receives the help they deserve.

Falls are a leading cause of injury for older adults, but they are not an inevitable part of aging. Taking simple, proactive steps can dramatically reduce your risk, helping you maintain your independence and confidence. By focusing on three key areas—your home, your health, and your community—you can build a safer, more active lifestyle.

Your home environment is the first place to start. Start by walking through each room with a critical eye. In the bathroom, a high-risk area, install grab bars near the toilet and in the shower or tub. Always use non-slip mats on wet surfaces. In hallways and on staircases, ensure there is bright lighting and sturdy handrails on both sides. On all floors, remove throw rugs and secure any loose carpeting or electrical cords that could be a tripping hazard. Simple changes like these can make a huge difference.

Beyond your home, your physical health plays a crucial role. A fear of falling can sometimes cause people to become less active, which ironically weakens muscles and increases fall risk. Combat this by engaging in

regular physical activity that improves strength and balance. Activities like Tai Chi, yoga, or even a simple heelto-toe walk can build stability. It’s also vital to talk with your doctor or pharmacist about your medications. Many prescriptions, even over-thecounter drugs, can cause dizziness or drowsiness, so a regular review is an important preventative measure. Lastly, get your vision and hearing checked annually, as changes in either can affect your balance and awareness.

For those in Utah, there are excellent local resources to help. The Utah Department of Health and Human Services offers programs like Stepping On, a workshop that teaches fall prevention techniques, and Enhance TFitness, an exercise class focused on strength and balance. Many of these programs are low-cost or free and are a great way to learn from experts and connect with others. By making a few small adjustments and staying engaged in your health and community, you can significantly reduce your risk of falling and continue to enjoy life to the fullest.

Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.”

Medicare Part A Hospital Insurance

Generally no cost (If you paid Medicare taxes while working)

$1,676 deductible (per benefit period)

It is possible to pay the $1676 deductible and other daily charges for multiple stays & extended care.

Plan covers: Inpatient hospital care, home health care services, hospice, inpatient skilled nursing & more.

Providers are paid by Medicare assignment.

No coverage outside USA.

No out-of-pocket maximum.

Many people add a Medigap plan, Part D or an Advantage Plan (Part C) to fill the “gaps” in the coverage.

Medicare Part B Medical Insurance

Most people pay monthly $185 (Premium could be higher based on income)

$257 annual deductible. (this can increase each year)

80/20 after deductible.

Plan covers: health care providers, ambulance, emergency care, outpatient surgical, wellness & other Medicare approved benefits

Utah Commission on Aging and the Master Plan for Aging

The Utah Commission on Aging (UCOA) was established to prepare the state’s residents, businesses, and policymakers for the challenges and opportunities of an aging population.

With its creation by statute in 2005, the UCOA fulfills a crucial role by educating stakeholders about the wide-ranging impact of aging on society. The commission’s activities and initiatives are aimed at enhancing public awareness and developing strategies to ensure that aging is met with adequate support and resourc es for both older adults and their caregivers.

One of the UCOA’s key initiatives is sponsoring and managing UtahAging.org, which serves as the official statewide virtual resource center for older adults. This platform provides a wealth of information and resources designed to help Utahns navigate the aging process by offering access to best practices in public policy, education, and research. Through collaboration with expert stakeholders, the UCOA fosters a community-driven approach to aging that promotes resource-sharing, awareness-building, and innovative solutions to challenges related to growing older.

The Utah Master Plan for Aging (MPA)

Utah’s Master Plan for Aging (MPA) is a forward-thinking framework that addresses the aging process across the adult lifespan, with a special focus on the later stages of life. This roadmap provides guidance to state and local governments, businesses, agencies, and the general public on how to prepare for and adapt to an aging population in the face of shifting demographic trends.

The primary objective of the MPA is to inform policies, programs, and funding decisions that empower Utah residents to live fulfilling and meaningful lives as they age. The MPA applies innovative and evidence-based solutions to meet both individual and collective needs. The MPA will measure its impact through a set of key indicators that evaluate social outcomes over the short and long term. An annual report on these indicators is published at the end of each fiscal year starting June 30,

2025, ensuring transparency and allowing stakeholders to track progress. The Utah MPA was developed with guidance from the Center for Health Care Strategies (CHCS) and is supported by state leadership, data-driven analysis, diverse stakeholders, and measurable outcomes. The selected social impact priorities were identified through collaborative input and the latest research. By building on existing successes and addressing gaps in the current system, the MPA will minimize redundancies, improve efficiency, and transform how aging is experienced in Utah.

Goals and Vision for the Future

The Utah MPA outlines a future where every individual in the state can enjoy a life that is meaningful and fulfilling, regardless of age. The plan envisions a society where all Utahns can:

1. Live with dignity, respect, and a sense of purpose.

2. Reside in age-friendly, healthy environments.

3. Have access to suitable, affordable, and adaptable housing.

4. Enjoy financial security and freedom from economic hardship.

5. Pursue employment based on merit without facing ageism.

6. Access personalized, high-quality healthcare.

7. Benefit from a supportive, caring community network.

8. Experience social connections free from isolation.

9. Live independently with autonomy and self-determination.

10. Make decisions with confidence and functional literacy.

11. Be protected from exploitation and abuse in all its forms.

12. Safely use technology to enhance their lives.

Social Impact End Goals

The Utah MPA establishes six core social impact end goals to guide policy and action for future decades. Each of these goals is accompanied by specific outcomes, strategies, and key performance indicators to ensure measurable progress. These goals will shape the state’s approach to aging and serve as a dynamic, evolving plan that will be adjusted and refined annually.

1. Age-Friendly Ecosystems: Creating environments that support successful aging by promoting age-friendly principles in communities, healthcare systems, workplaces, and public health initiatives.

2. Live Well Everyday: Focusing on

sustaining health and wellness throughout the aging process and ensuring that Utah adults have access to resources and services that support their physical, mental, and social well-being.

3. Affordable Aging: Ensuring financial sufficiency so Utahns can age with dignity. The MPA emphasizes the importance of working, saving, managing, and protecting assets in order to sustain economic viability throughout life. It also addresses ageism and the risk of financial exploitation that all adults encounter.

4. Plan for Life: Empowering Utahns to live self-determined lives with dignity and autonomy by fostering personal empowerment, helping older adults maintain their independence and stay engaged with their communities.

5. Networks for Care: Enhancing support systems for both caregivers and care recipients by providing access to education, resources, and support services.

6. Embrace Technology: Increasing affordable access to technology continues to play an important role in society, the MPA seeks to ensure that older adults are empowered to use technology safely and effectively. By promoting digital literacy and the safe use of devices, Utahns can make informed decisions that positively impact the way they age and the choices they make.

In conclusion, the Utah Commission on Aging and the Master Plan for Aging represent a comprehensive effort to address the challenges and opportunities posed by an aging population. Through collaborative partnerships, data-driven strategies, and a commitment to equity and innovation, Utah is preparing for a future where every resident can age with dignity, purpose, and support.

Are You In Need of Care?

Are you 65 and older? Did you serve active duty during a period of war? Are you a spouse or surviving spouse of a war time Veteran?

VA Aid and Attendance is a pension for Veterans who are aging and served during a period of war. This pension is also available for their spouses and surviving spouses. If you live at home with family assisting you, have home care coming in, live in Independent Living or Assisted Living you may be eligible for the Aid and Attendance pension. Aid and Attendance is directly deposited into your bank account to help with your care expenses.

AREAS OF CARE:

Independent Living is considered Custodial Care. Custodial Care includes meals, light housekeeping, transportation and 24/7 staff on duty.

Assisted Living is a higher level of care based on medical needs. Activities of daily living include bathing assistance, medication management, ambulatory assistance, dressing, incontinence etc.

Home Care covers two areas. You can hire a Home Care Agency which is private pay at an hourly rate to come to your home to assist you with non-medical needs. The second area involves a family member providing care.

If we can provide documentation to the VA showing that you need care or custodial care, you may be eligible for Aid and Attendance. You will need to disclose the following:

n Military documents showing dates of service

n Marriage certificate for widows only

n Death certificate for widows only

n Social Security benefit letter and proof of any other income

n Assets – current bank statement, CD’s, Stock/Bonds, Mutual Funds, IRA’s/401K’s

n Long Term Care policies

n The VA has a 3 year look back period into any asset transfers How long will it take to get approved? 4-6 months

If I divorced the Veteran, am I still eligible? No, unless you remarry another active-duty Veteran for one year.

Is my home considered an asset? No, unless you sell the home.

If I am on VA disability, am I able to receive Aid and Attendance? It depends on how much disability you receive.

24 HOUR ON CALL - Refers to someone being available 24 hours a day to speak with you.

ALZHEIMER’S/DEMENTIA CARE –

Non-skilled care provided for clients with memory issues includes but not limited to establishing a routine, reminders for activities for daily living, rerouting if necessary and assistance with sleep issues or ‘sun downing’. By using creativity and caregiving skills, you can adapt routines and activities as needs change.

BONDED - Being bonded means that a bonding company has secured money that is available to the consumer in the event they file a claim against the company.

HOMEMAKER/COMPANION –

Non-medical assistance with meal preparation, laundry, light housekeeping, errands, activities, outings, companion-ship and other household duties. These agencies are licensed by the state.

HOURLY MINIMUM – This refers to the minimum number of hours that an agency provides service.

INSURED - This refers to what happens if someone gets hurt on the job. If the company isn’t insured, then the

claim gets filed against the homeowner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance.

LICENSURE – A home care agency, whether medical or non-medical, shall be issued a license consistent with the type and extent of services provided.

CLASS A – A home care agency that provides any skilled healthcare service. Agencies with a Class A license may also provide personal care services.

CLASS B – A home care agency that provides only personal care services. An agency with a Class B license shall not provide any skilled healthcare service.

MED-ALERT SYSTEMS – Personal medical alert products activated by pushing a button.

STATE REGISTRATION – The State of Utah does not license homecare placement agencies. Placement agencies are registered with the state. Registration ensures that placement agencies are adhering to certain minimum standards such as background checks for caregivers and insurance requirements for agencies and caregivers. ~

Aging With A WISE Purpose

Utah is known for its youthful population, but it also has one of the fastest-growing senior populations in the country.

To address this demographic shift, Governor Spencer Cox has launched the Aging With a WISE Purpose initiative, a comprehensive effort to prepare the state for a future where more Utahns are living longer. WISE is an acronym that represents four key pillars of aging well: Wealth, Independence, Security, and Engagement.

The WISE initiative aims to create a more elderly-friendly Utah by focusing on these four areas:

• Wealth: This pillar addresses the financial challenges faced by older adults, including the rising cost of living, scams, and the need for sound financial planning. The goal is to ensure seniors have the financial stability to live comfortably and securely.

• Independence: This focuses on helping older Utahns remain in their homes and communities for as long as possible. This includes access to affordable and accessible housing, reliable transportation, and in-home support services.

• Security: This pillar is about protecting vulnerable adults from abuse, neglect, and exploitation. It also includes ensuring access to quality healthcare, fall prevention programs, and mental health

The WISE initiative is not just about state-run programs. It’s a call to action for the entire community

resources to promote physical and emotional security.

• Engagement: This pillar emphasizes the importance of social connections, purpose, and community involvement. It aims to combat social isolation and loneliness by supporting senior centers and other community programs that offer opportunities for lifelong learning, volunteering, and social activities.

As part of the initiative, the Utah Department of Health and Human Services has been hosting listening sessions across the state. These sessions are designed to gather direct feedback from older adults and their caregivers about their concerns and needs. The information collected from these sessions will be used to develop a 10-year statewide plan on aging.

The WISE initiative is not just about state-run programs. It’s a call to action for the entire community –from policymakers and businesses to local non-profits and families – to work together to create a society where older adults are valued and have the resources to thrive. 77% of adults, aged 50-plus, want to stay in their own homes as they age. Aging in place promotes life satisfaction, a positive quality of life, and self esteem.

My Adult Children Just Don’t Understand

If you are a senior with adult children, talking to them about your aging can be a sticky subject. Why is it so hard? You raised them, taught them and loved them so why is talking about your needs so difficult?

One third of adult children said they find it difficult to talk with their parents about their aging needs because of the parent-child relationship. When they were young, their parents put their own needs aside to meet the needs of their children. Often it isn’t until adulthood that the children even realize that parents have needs. Parents’ needs are often self-managed, or at least not often discussed. When parents eventually require assistance, children are often not accustomed to getting involved.

Adult children who feel stuck in the parent-child role are less likely to begin conversations with their parents regarding age-related issues. Here are some examples of topics adult children are reluctant to bring up:

• 33% are not comfortable asking parents if they need help around the house

• 30% don’t know how to talk about giving up driving

• 42% don’t know how to talk about leaving their home

• Few are comfortable talking about cognitive changes

• Rarely are children comfortable

asking about personal care issues like incontinence, bathing or hygiene.

It isn’t that adult children don’t want to talk with their parents about age-related issues, they often just don’t know how to bring it up. Because of the parent-child role, children often expect their parent to initiate conversations. Children may fear they will be treated like children. Seniors often fear their children don’t care or they are just too busy.

As difficult as it is, the conversation needs to take place. As parents, it will go better if you initiate the conversation. Here are some suggestions of how to have an effective conversation:

• Invite them to attend a meeting by saying “I want to talk with you about my aging”

• If possible, have the meeting at your home, on your turf

• Give an honest update on your situation. Tell them what you do and do not need help with

• Tell them your wishes and needs, but keep the conversation light

The most important thing to remember is that seniors and adult children should consider each other as adults. Set aside the parent-child role and talk to them as you would another adult whom you love and trust. You may be surprised at the response.

What’s the Difference Between Home Health and Private Duty Care?

We are often asked, “What’s the difference between Home Health and Private Duty care?” The biggest difference is the payor source. Home health is usually funded by health insurance, Medicare, or Medicaid.

Private Duty care is typically paid for by the individual or family. To qualify for home health your loved one must have a “qualifying event” and a physician’s order to begin care. Home Health is usually not a long-term solution, and the goal is to improve a patient’s function in the short term.

Alternatively, Private Duty care can be temporary or long term. Private duty care is flexible and can be tailored to meet your loved one’s and family’s needs. Private duty is flexible and can be anywhere from every day for 24 hours or a few times a month for an hour or two. This does not require a physician’s order.

The primary goal of Private Duty is to ensure individuals can safely stay in their home. Private Duty can be a great resource to the family. This care can allow the family caregivers to take a break. This is a great time to have Private Duty caregivers come and assist the client.

Here are some Advantages of Private Care Duty:

• Fills in gaps after Home Health care has ended and family cannot be there.

• Allows the family caregivers to leave their loved one at home confidently in the care of another (for vacations, appointments, social gatherings, work.)

• Private Duty Home Care is able to take long term care insurance for client reimbursement.

• Reduced hospital readmissions.

• Licensed and Insured.

• Constant training of caregivers.

• Nursing Services provided by some agencies.

Concerned about the risks of rehospitalization for a senior loved one? Understanding the causes and incorporating the correct support can help break the cycle of repeated hospital visits and improve the quality of life for seniors.

Rehospitalization, the need for seniors to return to the hospital shortly after discharge, is a growing concern in healthcare. It can lead to increased medical complications, stress, and higher healthcare costs. For seniors, frequent hospital readmissions can significantly diminish their overall health and well-being.

Why Are Seniors at Risk?

Several factors commonly contribute to high rehospitalization rates among seniors. Age-related changes in health make it more difficult for seniors to recover fully after a hospital stay. These changes include weakened immune systems and decreased mobility. Additionally, many seniors manage multiple chronic conditions, which can complicate recovery and increase the likelihood of additional health setbacks.

Home Health Care Definitions

24 HOUR CARE – Refers to “round the clock treatment” in a home environment. The care may be shared by multiple caregivers or by a single caregiver.

24 HOUR ON CALL – Refers to someone being available 24 hours a day to speak with you.

CARDIAC CARE – Refers to services including patient assessment, specific diet information, and management of the disease process, medication management and education. Cardiac care is a program designed to help a person with heart disease return to health. It may consist of exercise, modifying risk factors to prevent further heart damage, or psychological rehabilitation.

DIABETIC CARE – Refers to services including patient assessment specific diet information, management of the disease process, medication management and education on all the above. Services are provided by Registered Nurse under the supervision of a M.D. Some agencies may have certified diabetic nurse instructors or cardiac care specialty RN’s in their employ.

ENTERAL FEEDING – Enteral feeding (tube feeding), is a way to deliver nutrients through a tube if you cannot take food or drink through your mouth.

HOSPICE – Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. The

goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment.

HOUR MINIMUM – The least number of hours a Non-Medical Home Care Agency will be available for hire. Medicare certified Home Health Agencies are not on a specified time frame.

INTRAVENOUS THERAPY (IV) –

Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance.

MED-ALERT – Company offers a pendent or home alert system that notifies staff that user is in distress and needs immediate assistance.

NON-MEDICAL HOME CARE –

Assistance with Activities of Daily Living (ADL’s), homemaking, companionship and other nonmedical in home care. These services are available for hire after, or in conjunction with Medicare certified services. Private payment, Medicaid and Veterans Benefits are a few of the ways personal care can be paid for.

PSYCHIATRIC CARE – Refers to the agencies’ ability to offer services related to psychiatric support with the prevention, diagnosis, and treatment of mental

illness. Services may include counseling or administering of medications.

SOCIAL WORKER – A clinical social worker whose purpose in health care is to enhance, promote, maintain, and restore the best possible social abilities of the patient or family. Services provided may be preventive, developmental, or remedial in nature.

THERAPY SERVICES

(ST, PT, OT, RT) –

Speech Therapy – Refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech, language, cognitive skills or swallowing.

Physical Therapy – Refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs and enhance basic mobility skills.

Occupational Therapy – Refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Patients may have mental,

physical, developmental, or emotional disabilities. Therapists also help them to develop, recover, or maintain daily living and work skills. Occupational therapists help clients not only to improve their basic motor functions and reasoning abilities, but also to compensate for permanent loss of function.

Respiratory Therapy – Refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the therapeutic use of medical gases and their equipment, environmental control systems, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise, respiratory rehabilitation, assistance with cardiopulmonary resuscitation, and maintenance of natural, artificial, and mechanical airways.

TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allowing air to get to the lungs, or remove secretions. ~

A Brighter Future for Brain Health

The Alzheimer’s Association U.S. Study to Protect Brain Health Through Lifestyle Intervention to Reduce Risk (U.S. POINTER) is a groundbreaking two-year clinical trial that has demonstrated how lifestyle changes can significantly improve cognitive function in older adults at risk for cognitive decline. This study is the first of its kind to be conducted in a large, diverse group of Americans, and its positive results offer a clear roadmap for promoting brain health.

The Four Pillars of the U.S. POINTER Study

The U.S. POINTER study tested two different interventions—a highly structured program and a self-guided program—that focused on four key areas of lifestyle modification. Both approaches improved cognition, but the structured program, which provided more support and accountability, showed greater gains. The four

pillars of the intervention are:

1. Physical Activity: Participants engaged in a combination of aerobic, resistance, and stretching exercises. This emphasizes the importance of a well-rounded fitness routine for brain health.

2. Nutrition: The study promoted adherence to the MIND diet, which is a hybrid of the Mediterranean and DASH diets. This diet focuses on foods known to support brain health, such as green leafy vegetables, nuts, berries, and whole grains, while limiting red meat and sweets.

3. Cognitive Challenge and Social Engagement: The intervention included activities designed to stimulate the brain, such as cognitive training through programs like BrainHQ, as well as encouraging social engagement to combat loneliness and isolation.

4. Heart Health Monitoring: Participants regularly reviewed their health metrics, such as blood pressure and cholester ol, with a clinician and set goals to improve their cardiovascular health.

How to Implement the Findings

The findings from the U.S. POINTER study can be implemented on both an individual and a community level. For individuals, the key takeaway is that it’s never too late to adopt a healthier lifestyle. You can start today by incorporating these pillars into your daily routine:

• Move more, sit less. Find an exercise you enjoy and make it a regular part of your week.

• Add color to your plate. Focus on a healthy diet rich in fruits, vegetables, and whole foods.

• Keep your mind active and stay connected. Learn a new skill, join a club, or volunteer to keep your brain challenged and engaged with your community.

• Know your numbers. Regularly

monitor your blood pressure, cholesterol, and blood sugar, and work with your doctor to keep them in a healthy range.

For communities, the study highlights the value of creating accessible and supportive programs that combine these lifestyle interventions. Community centers, healthcare providers, and local organizations can collaborate to offer classes, support groups, and resources that help people adopt these brain-healthy behaviors.

The Alzheimer’s Association is now working to bring these interventions to communities across the U.S., making it easier for everyone to protect their cognitive function and age well.

Hospice Care Definitions

HOSPICE – Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment.

Services Provided:

N-NURSE – Assesses patient’s physical needs, develops and coordinates the plan of care, ensures symptom control and management, provides patient/family teaching as needed

SW-SOCIAL WORKER – Assesses patient and family emotional and social needs, develops a plan of care, provides counseling, referrals and support to meet identified needs

CHAPLAIN – Assesses patient and family spiritual needs, develops a plan of care, provides counseling, assists with memorial preparations.

HOME HEALTH AIDE – Provides direct personal care to patient, emotional support to patient and family, reports identified needs to RN.

VOLUNTEER – Provides respite and needed non-medical services for patients and their families, offers support at time of death and during bereavement.

Levels of Care:

RO-ROUTINE CARE – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management.

Care is provided at the individual’s private residence, a retirement community, an assisted living, or a skilled nursing facility.

RE-RESPITE CARE – Provided when the family needs time to rest or go out of town for a short time. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is responsible for financial arrangements with the Longterm care facility.

GIP – General In-Patient Care –Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period.

CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs. It also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be. The criteria for continuous care may include: Severe pain or symptoms which are

out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope. Continuous care is typically reserved for end of life or other crises that requires a constant presence by a nurse or care team.

24/7 ADMISSIONS – If a patient qualifies for hospice care, they should be admitted as early as the family requests. Many agencies admit patients to hospice, 24/7, evenings weekends, and holidays.

AROMATHERAPY – Uses plant materials and aromatic plant oils, including essential oils, and other aromatic compounds for the purpose of altering one’s mood, cognitive, psychological or physical well-being. This is not reimbursed by Medicare.

GRIEF SUPPORT GROUPS – Grief is the normal and universal reaction humans have to the loss of those they love. When your loved one passes, you may go through a period of bereavement and experience grief. Many provide Grief Groups that meet and address issues of grief in a supportive group setting.

INDIGENT PATIENTS – Does the hospice accept non funded or indigent patients? This is usually on a case by case basis and at the determination of the hospice company. Many have a non-profit

organization that can help support the care of non-covered patients.

IN-PATIENT

CARE – These hospices offer full hospice services in their on-campus unit or building. The patient moves into the inpatient care facility to receive their hospice services rather than in the patients’ home setting.

LICENSED MASSAGE THERAPY

Used as a method for relaxation to alleviate stress, anxiety and pain with a light hands-on approach, as well as providing an alternative to those who can’t take medication or prefer not to, massage therapy has become essential in hospice care. This is not reimbursed by Medicare.

MUSIC

THERAPY – Music Therapy is an established healthcare profession that uses music to address physical, emotional, cognitive, and social needs of individuals”. A music therapist creates goals and objectives for their patients and uses music activities and music experiences (listening, composition, improvisation, or playing/singing a familiar song) in order to achieve those goals. This is not reimbursed by Medicare.

PALLIATIVE CARE – Specialized medical care for people with serious illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment.

PCP-OVERSIGHT – Can the hospice patient retain their own primary care physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care. Many times the hospice medical director will work in conjunction with the patient’s own

physician. The patient, family and hospice team can determine which physicians will oversee the care and certify the hospice eligibility criteria.

PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare.

VA CONTRACT – If the patient is not a Medicare Recipient and is covered by the VA, the VA will make payments directly to their contracted agencies. If a patient is a Veteran, but is a Medicare Recipient, they may choose any Medicare approved hospice agency for their care. ~

Senior Housing Definitions

AGE RESTRICTIONS

– Some communities have a restriction on the youngest age they are allowed to accept. These restrictions vary by community.

ALZHEIMER’S / DEMENTIA

ASSISTED

LIVING COMMUNITY

Licensed assisted living facility or skilled nursing facility that specialize in delivering care to victims of Alzheimer’s and dementia. Communities are typically bedroom-style rental units with common areas for dining and activities. Many are locked for residents’ safety. Specifically trained staff usually serves the special needs of this population. Amenities vary for each community.

ASSISTED LIVING COMMUNITY

– Typically apartment-style with kitchenettes and private baths. Some offer companion accommodations. Personal assistance with Activities of Daily Living (ADLs) bathing, grooming, dressing, toileting, eating and ambulation. Rates vary from an all-inclusive to base rates with levels of care. Amenities vary for each community.

TYPE 1 CARE

– Residents may receive significant assistance with 2 ADL’s and minimal assistance on more than 2 ADL’s daily. The resident must also be able to evacuate a community under their own power and have stable health. Residents receive 24/7 monitoring

and may have an independent home health agency help with services.

TYPE

2 CARE – Residents receive full assistance with ADL’s and are able to permit aging in place. Residents may require the assist of one person for transfers and evacuations. Residents receive 24/7 monitoring and may have an independent home health agency help with services.

DAY

CARE – Any provider can elect to provide day/night services on a short or long term basis. Rates typically include a place to rest, one or two meals and snacks and activities for the day. Licensed communities are prohibited by law to promote these services without a specific license.

DEPARTMENT OF HOUSING & URBAN DEVELOPMENT (HUD)

– helps apartment owners offer reduced rents to low-income tenants.

HOUSING CHOICE VOUCHER PROGRAM –

(Sec 8) Find your own place and use the voucher to pay for all or part of the rent. Participating households are required to pay the greater of 10% of their gross annual income or 30% of their adjusted income towards rent. To qualify, a household’s annual income must not exceed the applicable income limit for the

area as adjusted by family size. (Sec 236) Property owners in this program make mortgage payments based on a 1% mortgage interest rate. HUD then provides a subsidy to their lender to cover the difference between 1% and the market interest rate on the property’s loan. Participating households are required to pay rent equal to the greater of 30% of their adjusted income (not to exceed the market rent) or the basic rent amount set by HUD for that particular property. Any amount paid by the household that is more than basic rent is considered excess rent, which the owner pays back to HUD in repayment of the subsidy.

INDEPENDENT HOUSING

Apartments and communities for adults 55+ with little or no senior support services (e.g. no meals, etc...)

INTERMEDIATE CARE – 24

hour

inpatient care residents who need licensed nursing supervision and supportive care, but do not require continuous nursing care. Residents may receive full assistance with ADL’s and transfers. Residents receive meds from a nurse following a doctor’s orders and may receive outpatient rehab services. Facilities may use a nurse as a their consultant and the residents receive periodic assessments from a licensed practitioner.

NEW CHOICE WAIVER - MEDICAID

WAIVER – An assisted living waiver may be used to transition individuals out of nursing facilities or to provide persons at risk of institutionalization with options for remaining in their communities. Waivers are designed to provide an alternative to more costly nursing home care.

PRICE DEPENDENT ON LEVELS OF CARE

Flexible levels of service geared to accommodate different needs over time. Rates may vary depending on the changing levels of care while one maintains the same physical residence.

PUBLIC HOUSING (PH)

– Affordable apartments for low-income families, the elderly and persons with disabilities.

RESIDENTIAL HOME

– Assisted living homes are private residential homes that typically have a small number of residents who live together and receive care from live-in caretakers. Adult family homes offer room and board for seniors who want a more private, home-like community. Assistance with activities of daily living such as bathing and dressing are provided. Amenities and nursing services vary greatly between homes. Ask about their services when you tour. They are also referred to as personal board and care homes. Most accept private pay; Medicaid in some instances.

RESPITE CARE / SHORT-TERM STAY

– Otherwise known as a short-term stay. Any provider of IL, AL, ALZ or SNF care can elect to provide fully furnished accommodations to a person for a short period of time. Rates usually quoted on a daily basis and are typically inclusive of all services and amenities.

RETIREMENT COMMUNITY –

Housing designed specifically for mature adults. Typically Studio, 1 BR or 2 BR rental apartments with full kitchens, and private baths are available. Services include meal plans, transportation, housekeeping, laundry and maintenance. Amenities are usually abundant; formal libraries, computer stations, swimming pools, gardens, and other social gathering spaces. Rates are monthly and typically based upon size/ location of apartment.

SECURED UNIT

– A secured unit dedicated to Alzheimer’s/Dementia residents to prevent wandering outside the community without supervision.

SKILLED CARE

– 24 hour licensed nursing services, eight hours of which are RN covered. Patient may receive rehab services either inpatient or outpatient and may be fully dependent for ADL’s.

Patients receive medications by an RN according to a licensed physicians orders.

SKILLED NURSING FACILITY –

Facilities provide 24/7 care for residents and offer full assistance with ADL’s. Daily rates typically include a furnished bedroom and a private or shared bath, meals, activities, transportation, phone service, cable television and all personal/nursing care. Payor sources usually accepted are Medicare, Medicaid, VA, some insurance plans and private sources.

TRACH CARE

– Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allow air to get to the lungs, or remove secretions.

VENTILATOR - Moves breatheable air into and out of the lungs, to provide the mechanism of breathing for a patient who is physically unable to breathe, or is breathing insufficiently.

WANDERGUARD

- Offered at some facilities. A resident with memory loss or confusion wears a small discrete pendant. Each external door in the care home is fitted with a radio detector which detects when a pendant moves near an exit. The WanderGuard will sound an audible alarm, alerting the staff of a patient wandering. ~
