After putting the pieces of the puzzle together we present the best options for your loved ones needs. We are a hands-on service that assists in finding alternative living situations for those who are unable to continue living at home. Or we can help by finding the appropriate in-home assistance so that your loved one is able to continue to live at home.

For over two decades, I have been honored and proud to serve the seniors of our community. As Publisher of this resource guide, I can assure you that the entire Seniors Blue Book team works tirelessly to provide you with an abundance of resources in the senior realm.
This edition’s Letter From The Publisher is more personal than normal. You see, I lost both of my parents in the past year and a half. It is fair to say I ‘walked the walk and talked the talk’ as my sisters and I navigated the underbelly of caretaking an elderly loved one (times two).
Mom was struggling for nearly a decade and Dad was her caretaker. Dad was a proud Army veteran and a doting husband who didn’t like to ask (or even accept) help as he thought he could fully care for Mom on his own. But, like many caretakers before him, he didn’t truly understand it was to his detriment to try to do it ‘all’. After MANY hospital visits for them both, months of rehabilitation between them, and legal/estate matters, I was reminded
of the inherent value of having access to resources in the palm of my hand. There were setbacks, pitfalls and mistakes made along the way. There were questions beyond questions that came to light as we served as advocates for our folks.
Sadly, a bad fall brought on brain surgery for Dad that eventually incapacitated him. After his passing, Mom quickly followed about 18 months later after succumbing to a surprise diagnosis of Stage IV cancer. Gratefully, they are back together again – as it should be.

An ombudsman advocates for the rights of PACE participants and people who live in assisted living residences and nursing homes.
An ombudsman works with you to make sure you receive the care you want and need. They help residents of long-term care residences and Program for All-Inclusive Care for the Elderly participants maintain their quality of life and receive quality care. If you’re 60 or older, an Area Agency on Aging case manager may be able to provide one-on-one help. Staff will conduct assessments regarding your service needs, help you create a care plan and assist you in getting services in place so you can live safely and independently. Call 303-480-6700.
Do you currently have Medicare, or will you soon be eligible for Medicare? SHIP counselors provide unbiased information, Medicare counseling, information about your benefits and eligibility for cost-saving programs, and enrollment assistance at no cost. Veteran Directed Care is a self-directed program that empowers veterans to live independently with the assistance they need. The Department of Veterans Affairs determines veteran eligibility to participate in the Veteran Directed Care program and provides veterans a budget to self-direct their care and pay trusted caregivers to help them continue living at home. The caregiver support and services program helps individuals in all stages of their caregiving journey access resources and support in their communities. Eligible individuals may qualify for a one-time voucher for in-home respite care to get much-needed breaks from caregiving. Call 303-480-5656.
The Choice Services Program provides up to two hours a week of home care and can offer select types of free transportation assistance to qualified individuals 60 and older living in the Area Agency on Aging’s metro Denver service area. Call 303-480-6700.
The Older Adult Refugees and Friends Program reduces isolation and increases community connections for older adults from refugee and immigrant communities. The program’s services help older adults discover resources, make social connections, meet friends, get navigation support, and learn ways to stay healthy, well and independent. Medicaid members 18 and older living in long-term care settings like nursing homes can receive one-on-one options counseling to learn about returning to communitybased living with Medicaid supports and services to thrive and stay independent. Through options counseling, members may choose to enroll in Medicaid Transition Services, which provides additional resources including life skills training, home-delivered meals and peer mentorship. Trusts keep your wishes private, allow your family to bypass probate, safeguard beneficiaries, and provide detailed instructions on how to handle assets after your passing.
Good trust planning can help with estate taxes, protection for family members with disabilities or other concerns, and provide safeguards if the surviving spouse remarries.
A revocable trust allows the creator to change or revoke the trust and manage trust assets themselves. An irrevocable trust allows you to name a trustee to manage trust assets for you. Someone else controlling your trust assets may be scary, but know they are bound as a fiduciary to act in your best interests
with significant consequences if a trustee mishandles assets.
When considering any type of trust, consult with a knowledgeable attorney to ensure your trust works for you and does exactly what you want and need the trust to do.

A simple accident, like tripping on a rug or slipping on a patch of ice, can change your life.
Every year, more than one in four adults 65 years or older experience a fall contributing to over three million emergency department visits across the country.1 In 2023, Good Samaritan Hospital treated 787 trauma patients due to fall-related injuries, making falls the number one trauma injury seen in Good Samaritan’s Emergency Department. Simply stated, the risk of falling and fall-related problems increases as we age.
Falls can result from a range of personal and environmental factors.
Hazards like dimly lit rooms, steep stairs, and hard-to-reach shelves or storage areas present tripping risks. Additionally, as we age, changes in our vision, hearing, and overall health can raise the risk of falls.
Fall prevention is about identifying and mitigating as many risk factors for falling as possible. Take some time to do an inventory of your home and your physical health to see if there are any risks present. Once you’ve identified potential risk factors, you can take steps to address them.
n Remove home hazards. Remove clutter and items on the floor and

in areas where you walk. Secure electrical cords so they don’t block walkways, and secure or remove any area rugs that bunch or slip.
n Make home modifications. Install grab bars near the toilet and in your shower, install handrails along your stairs, have lightbulbs replaced or install new lights in dark hallways and rooms, and use non-slip mats in your shower or bathtub.
n Stay active. Regular physical activity and exercise keeps your muscles strong, prevents bone loss, and can improve your balance. Walking, water workouts, or Tai Chi are great activities that improve strength, balance, coordination, and flexibility. Be sure to consult your
physician before beginning any new exercise program.
Taking care of your overall health and acknowledging your risk factors for falling is extremely important as we age. If you’re not sure where to start, schedule time with your primary care physician to discuss a plan for you or your loved one to keep you healthy, happy, and safe in your home and community. ~

It is the wish of all who enter the workforce to retire one day.
Most of us do retire but at different ages depending on many factors. Some want to work for as long as possible and in some cases forget to review the benefits only available after the age of 65. It is here where mistakes are made if you do not do any research.
The earliest you can draw Social Security benefits is 62. If you are unable to work and are disabled prior to age 62 the Social Security Administration provides benefits that can support you until retirement. Most people are required to apply for Medicare at 65 or after 24 months
of Social Security disability with the exception of those with other creditable coverage. The reason for this is that the Federal Government and the State Government share the load for healthcare and support for those retiring or disabled. If you do not become aware of these guidelines you could risk being penalized. This is usually a monetary penalty applied to your premiums once you finally get your benefits in place. Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.”

Rehabilitation Hospitals, sometimes referred to as Inpatient Rehabilitation Hospitals, provide intensive rehab to patients who are generally medically stable but need help to recover from injury or other medical conditions. They can be found within a hospital or in a free-standing building. Rehabilitation hospitals were created to provide services less costly on a per diem basis than general hospitals but provide a high level of professional therapies such as physical, occupational and speech therapies. Rehabilitation Hospitals must offer a minimum number of physician visits, increased daily rehab hours (3 hours/day, 5 days/week of PT, OT, and/or Speech), and interdisciplinary services including increased RN oversight and nursing hours per day.

A hospital stay—whether planned or unexpected—often brings the difficult question: What happens next?
For many older adults and their families, the discharge process can feel rushed, confusing, and full of unexplained terms. “Skilled nursing,” “rehab,” “long-term care,” and “transitional care” can all start to sound the same. But each serves a different purpose—and knowing the difference can make a huge impact on recovery and peace of mind.
If you, or your loved one is not quite ready to return home safely, short-term skilled nursing or rehabilitation may be recommended. This doesn’t mean anyone is being “put in a home.” It means a person needs time to regain strength and independence with the support of 24/7 nursing care and therapy. The goal is to help the individual recover enough to return to the community—whether that’s home, assisted living, or another level of support.
On the other hand, long-term care is often appropriate when a person’s needs are ongoing, chronic, or unlikely to improve significantly. This can be a tough pill to swallow for an individual, or families—but it can also bring relief. Good long-term care communities provide safety, structure, medical oversight, and social connection. The best ones strive
to feel as home-like as possible.
Transitional care bridges the gap between hospital and home and can happen in a variety of settings. Think of it as a safety net during recovery. These services can happen in a skilled nursing facility, at home with visiting nurses and therapists, or in a specialized hospital-based unit. Transitional care focuses on reducing avoidable readmissions and helping people meet recovery goals like walking, dressing, or managing medications independently again.
It’s okay to have fears about nursing homes—we’ve all heard stories or carry memories of what they used to be. But the landscape has

COPRA was founded to improve standards in senior placement and referral services. It has worked diligently to promote ethical practices, ensuring that families and seniors receive clear and unbiased guidance when making decisions about senior living options. Over the years, COPRA has:

It is so important to help that individual to have a dietary routine that allows them to stay healthy, active, happy and enjoy their daily experience. Residents with memory care challenges are like intricate puzzles that require observation, trial and error and patience to help figure out. Once you figure them out and gain their trust you can really maximize their enjoyment of food and drink.
It can be an enjoyable experience each day for those residents. Many simple preventable ailments include Urinary Tract Infections (UTI) and Constipation. These simple ailments can cause major confusion, pain, medication challenges, and end up requiring a trip to the hospital and sometimes even worse.
Residential Memory Care Homes can offer a huge advantage:
n the kitchen is adjacent to the dining tables where the residents can smell the food being cooked.
n meals can easily be customized for that individual because the care staff is right there
n food can be more easily managed by the residents that may be prone to choking, staff can remind them to chew and swallow before taking the next bite and ensure there are
small cut-up pieces or add more gravy or sauce to help moisten food or even puree food when called for.
There can also be a caregiver helping the resident to eat or providing a special utensil for easier management of the food, or a special color plate making it easier to see the food due to the resulting contrast that a resident with macular degeneration may need. Providing a plate with separation on the plate so the resident can distinguish the meat from the potatoes from the vegetable on his plate, allowing him to enjoy the taste of that item.
Living often residents are brought to a large dining room with many tables and served a plate of food brought from a distant kitchen. It does not always work very well. Many residents don’t really understand their surroundings, there could be too much stimulation, there might not be any connection to the food. (They miss out on smelling it being cooked or baked for the past hour or two.) They may not even realize it is time for them to eat. The staff does not have enough time to assist those residents that need help. This is not an atmosphere that helps them eat better, often the food gets cold

“Why can’t I do this? I really thought I could take care of my spouse/ parent to the end of their life. I’m not only disappointed in myself; I also feel like a failure. But I’m so exhausted and depressed I can’t hardly get through the day. I have lost hope. I don’t know what to do.”
As a Senior Placement Company, these are the kind of thoughts and feelings we hear from caregivers on a regular basis. Often those who have little to no caregiving experience think caregiving for a loved one declining in mental or physical health shouldn’t be all that hard. Demanding yes, but doable; however, experience with caregiving will reveal otherwise. I’ve had to look a caregiver in their eyes and tell them directly, “You can’t get there from here.” What am I saying? You’ll not make it alone to the end. You’ll burnout before your loved one either passes or moves to a Senior Care Community.
So then we have to talk about guilt and fear. The feelings of failure and the fear of their loved ones future well-being while caregiving becomes overwhelming. I was shocked to learn that statistics tell us that care-giver’s burnout and/ or health decline often supersedes
the death of the one they’re caring for. As hard as it is to face the fact that help is needed; it must be done much sooner rather than later. I’ve NEVER heard a caregiver say, “I should have waited to find help or placement for my loved one.”
Families need to understand that caring for loved ones as they decline will eventually require help from others/ communities in approximately 70% of the population. Reaching out to a Senior Placement Company to find the “right fit” for their loved one sooner rather than later is far better than waiting for an emergency hospitalization to become the precipitating event for much needed care-giving. Until he was 70, Tommie was a cowboy and loved to herd cattle on horseback on his South Dakota Ranch. Tommie suffered a severe heart attack and his dementia quickly progressed.
Tommie’s daughter, Cindy, moved both her parents to Colorado so that she could visit more often. She placed them in a large assisted living facility memory care unit. Tommie’s wife, Dorothy, passed away 7 months after moving to Colorado. Tommie declined quickly in that environment because he wasn’t eating and wandered a lot. Cindy was unable to work because of her daily visits to her father. She made sure that he would eat and was safe. That experience took a toll on Cindy, her husband, and two young children.
They found Jaxpointe at Flower Ct. and moved Tommie two and a half years ago. It is the perfect environment for Tommie to call home for the rest of his life. The backyard walking path enables Tommie to enjoy nature and walk around. After placing Tommie at Jaxpointe, Cindy and her family went on their first family vacation in four years because they were so comforted by the care that he was receiving.
These days, Tommie is doing great. Daily he dons his western shirt, suspenders, jeans and most of the time, his cowboy hat. He will greet you when you come into Jaxpointe, and always tries to join in on conversations. He goes out in the backyard daily to walk or will “try to find his cattle or check on his hay crops”. He is very happy and enjoys every day. There are times that he doesn’t remember family members or their names, but he is thriving in the person that he has become with dementia.
