Aging gracefully is a goal that many of my patients want to achieve. In a sense, the cosmetic aspect of our profession is like being a clock-winder, winding back the clock of aging, thereby giving my patients a rejuvenated natural look. As such, my colleagues and I are always exploring different combinations to get the most rejuvenated look for you. In some cases you may need laser resurfacing, and in others Botox® Cosmetic and fillers will do the trick. Most recently we did a patient with one of my new go-to combination treatments, Ultherapy® and Voluma XC™. Our ideal patient for this particular combination is a person who is starting to experience volume loss in the cheeks and is starting to see sagging skin due to a lifetime of sun exposure and gravity. As you can see from the before picture below, this patient, who is in her early 70s, is experiencing both these symptoms.

First of all, I want to thank you for the wonderful reception that we’ve received from new and established patients at our new Katy/Cinco Ranch clinic. We are delighted to be part of this growing community and look forward to engaging more with our new hometown in 2014. Our upcoming events are on the back. Our winter Skinny shares our insights on some of the most exciting and effective cosmetic dermatology treatments and combination therapies we offer. Our first article is by Dr. Jennifer Peterson, who joined us in Katy in January. Dr. Peterson is a fellowship trained laser expert who last year was voted one of our area’s Top Doctors by Houstonia magazine. She shares with you the benefits of combining Photofacial and fractionated laser resurfacing to address facial photoaging. Next, Dr. Syed Ali discusses the transformative effects of combining our most effective skin tightening procedure, Ultherapy®, and our newest filler, Voluma XC™. She also completed a prestigious fellowship in Cosmetic Laser Dermatology in La Jolla, California, and has participated as an investigator in numerous cosmetic dermatology and surgery studies. When we look at our skin in the mirror, we can see there is not just a single change to our skin surface that makes it appear older. Instead, there are a multitude of changes which occur as part of the aging process and sun exposure. These changes hallmark what dermatologists call “photoaging”. Brown spots, sallowness, rough texture, increased redness, broken blood vessels, enlarged pores, fine lines and wrinkles all comprise photoaging. Unfortunately, there is not a single laser that can effectively treat all the signs of photoaging. To overcome this challenge, I use a combination of lasers, Lumenis’ M22™ intense pulsed light (IPL) and new ResurFX™, together on the same day to effectively improve multiple signs of photoaging. The M22 IPL, known as our Photofacial procedure, works by delivering pulses of light to the skin surface to target melanin found in increased amounts within brown spots and hemoglobin, a molecule contained in red blood cells, to target broken blood vessels and redness. Using this technology, the M22 IPL lessens the appearance of broken blood vessels, redness, brown spots and large pores. The ResurFX is a fractionated laser device which creates fine columns of heat in the top layers of the skin which stimulates increased collagen production and ultimately improves the appearance of fine lines, large pores, rough skin texture and sallowness. These fine, microscopic columns also create tiny channels, which allow the skin to more effectively remove excess brown pigment from beneath the skin surface and thus further improve the appearance of brown spots and irregular skin tone. This combined treatment can achieve better, quicker results by addressing the multiple causes of photoaging in the same visit. For a typical facial treatment, we perform the M22 IPL Photofacial first followed by the ResurFX laser resurfacing. The whole treatment typically takes less than 30 minutes. Prior and during the procedures we take steps to keep you comfortable. You can expect a few days of mild to moderate redness and swelling following your laser therapy. Makeup can be used the following day so downtime is usually minimal. I recommend a series of 3-6 treatments spaced 4-6 weeks apart depending upon the severity of my patient’s sun damage. A Case Study

We started this patient’s treatment with an Ultherapy session. Ultherapy is the only FDA-approved device for lifting and tightening skin. With Ultherapy, we can tighten and lift using safe ultrasound waves in a single comfortable outpatient session. The best part is that there is minimal downtime. It is the perfect treatment for lifting the brows and cheeks (1), eliminating jowls, re-defining the jawline, tightening redundant neck tissue (2) and smoothing wrinkles. Then 6 months later we followed up with Voluma XC to restore some of her lost volume. Voluma XC is the newest of the hyaluronic acid (HA) fillers released by Allergan®, the makers of Botox Cosmetic and Juvéderm®. HA is a natural substance found in your skin and is used to refill lost volume due to the sun, shifting fat pads and aging. It is particularly great for restoring the fullness of the cheeks (3). Voluma XC is an exciting product; not only does it yield amazing 1 and immediate results but it also can last up to a year longer than other HA fillers. It is the first and only filler FDA-approved for deep injection in the cheek area to correct agerelated volume loss. The result of these two together can be quite dramatic. Laser-Fast Hair Removal: Vectus™ by Maria Antesoda

Maria Antesoda, RN Joined SBA in 2000 Maria has worked in skin care since 1996 and been a practicing nurse in cosmetic dermatology since 2000. She received her Associates Degree in Nursing from Houston Community College.

With nearly 13 years of experience performing laser hair removal at our practice and 16 years overall, I can honestly say that the Vectus™ is the most exciting new machine for this treatment out in the marketplace. The Vectus provides faster and more comfortable treatments with its unique larger, flatter cooled applicator. Like other machines in the same class, the Vectus laser targets the pigment in the hair follicle and destroys it without harming the surrounding skin. However, unlike its predecessors and current competitors, this laser’s comfort level is so high that topical anesthetic is rarely necessary. Treatment times have also decreased significantly; it’s nearly over before you even realize it’s begun, and there’s no downtime. We are able to treat most skin types, including very light to moderately dark skin, most hair colors and almost all areas of the body. Permanent laser hair reduction is achieved on average in 6 to 8 treatments spaced anywhere between 4 to 6 or 6 to 8 weeks apart. If you are considering permanent hair reduction, winter time is the perfect time to start so that you are ready to go before beach season begins! It’s also the perfect time to treat because your skin is paler now and the laser can deliver more of its energy to the melanin in the hair follicle.
