Congratulations to 2 UEC Transformation Programme
• The Frailty Team on the launch of the Frailty Assessment Unit, where in the first 6 weeks of opening 183 patients were seen with an average of 70% of patients going back home on a Pathway 0
• Stroke specialist nurse Anupama Suresh on receiving positive feedback regarding communication skills with family and use of the stroke pathway
• General Medicine Hot Clinics have been introduced in Medicine
• Standardised Board and Ward Rounds are being embedded across Medicine
