Forging ahead... Here are some significant events from last year:

T

hirty-five years later the truth of this statement is even more evident. Perhaps the only differences are the speed at which change occurs and the complexity of these changes. To survive in this fast-paced, fluid environment we must learn new ways of thinking and acting. We need to become adept at strategic thinking. We need to build in opportunities for reflection, allowing us to adapt to better meet the demands of the future. At times it can feel overwhelming but with the incredible commitment and support from our staff, volunteers (including the Board of Directors), members and clients, we can not only survive – we can forge ahead to a better future.

In June 2012 we discontinued providing x-ray services. A combination of the steady drop in the number of people using the service, the high cost of replacing the aging equipment and the retirement of our technician made this seem like the right time to request a reallocation of the funding to more indemand programs. As a result we are expanding our chiropody program. In February 2013 the Board hosted a strategic planning session with a panel of experts from Sunnybrook Health Sciences, Toronto CCAC, TC LHIN and Thorncliffe Neighbourhood Office. It was an excellent start to our planning process which will also include a session to engage our members at the AGM. February 2013 also saw the opening of the Oriole Food Space. Based on a vision to “build a healthy community, together, through food”, Flemingdon Health Centre, North York Harvest Food Bank, Working Women Community Centre and the City of Toronto worked together for almost 4 years to make this space a reality. Its programs include a food bank, community kitchens, gardening, healthy eating workshops and settlement services. In March 2013 we completed an extensive evaluation of our South Asian Diabetes Prevention Program

(SADPP). This innovative project, developed by FHC in 2009, provides diabetes screening and education using culturally-relevant and language-specific resources and activities to South Asians who are 4-6 times more likely to develop diabetes than the general population. We anticipate that the results of the evaluation will prove just how successful the project has been. As a further indication of its value, TC LHIN funding for the project will move from project to ongoing base funding in 2013/14. Looking forward: FHC is scheduled to migrate to a fully Electronic Medical Record (EMR) system in September 2013. This is part of a transition plan for all Community Health Centres in Ontario (more than 70) and will be one of the largest deployments of a common EMR solution in the whole of Canada. It will ensure our alignment with the provincial direction for electronic medical records and facilitate higher quality care through improved access to a comprehensive client record by health care providers. To be successful this transition will require a substantial amount of additional time and effort on the part of our staff and in the short-term will reduce the number of clients we can see. We believe the long-term benefits of an EMR will be worth any initial disruption to our service delivery.

Peer Health Ambassadors With the culmination (March 31, 2013) of our Trillium Peer Health Ambassadors Project, almost 1,400 community members have participated in peer-led programs. Peer-led programs were facilitated in the following languages: Mandarin, Urdu, English, Spanish, Bengali, Cantonese, Armenian and Farsi.

Residents of 12 Thorncliffe Park Drive attended a 6 week Chronic Disease Self-Management program.

Peer Ambassadors conducted a Spanish Community Kitchen program.

“Joining Our Threads” Art Valley Park Exhibit

A new expressive arts program encompassing visual art and storytelling was held over 6 weeks with arts organization Diasporic Genius for 12 participants.

An exhibition of textile art created by youth, women and families from our community expressing their unique identities through fabric collage.

Together we developed our creative imagination as we shared stories that brought us closer together.

Physical & Recreational

WALKING GROUP SUMMER 2012 Certified Nordic Pole instructor helped prevent osteoporosis, reduced impact on hip and knee joints, and increased our walking group’s respiratory capacity by 25%.

YOGA We launched a new fitness program in the winter, involving both a registered and drop-in groups. Seven community members attended the 10-week registered program, which consisted of different health workshops each week, followed by a fitness class.

Volunteering

Flemingdon Health Centre endeavours to provide meaningful volunteer opportunities for everyone. Adults, youth, and seniors are all welcome and encouraged to be involved in all opportunities at both of our locations: Flemingdon and Fairview sites. In 2012-13, over 350 youth & adult volunteers contributed approximately 3000 volunteer hours at FHC.

My City is a newcomer youth initiative that aims to decrease isolation for young people who are new to Canada. The program teaches youth to use public transit and visit different parts of the city. They also have an opportunity to be involved in community work. This year, 50 youth were involved and each contributed 12 volunteer hours.

Education & Teaching We ran 5 computer classes per week in our Resource Access Centre, including a beginners Type and Talk, Intermediate Excel, and a Women’s class. That’s an average of 16 students learning each day!

Prenatal program – new collaboration with BBNow, Adventure Place

Flemingdon Health Centre has a long standing tradition as a teaching centre beginning in 1972.

Women’s Computer classes, Microsoft Office training courses

We are affiliated with the University of Toronto through the Department of Family and Community Medicine. We also receive students from the University of Western Ontario, McMaster University, Ryerson University, The Michener Institute and a variety of community colleges. We receive students from Medicine, Social Work, Health Promotion, Nursing, Chiropody, Nutrition, and Health Administration studies.

Support

Breastfeeding Support Group: New pilot partnership with Toronto Public Health and Thorncliffe Neighbourhood Office to provide breastfeeding support to new moms and families. Flemingdon Health Centre also hosts a Breastfeeding Clinic at our Fairview site. This clinic is in collaboration with Toronto Public Health to provide breastfeeding education. Both programs are drop-in and open to all pregnant and breastfeeding families. Parenting & Play

Women Supporting Women attracted an average of 23 women per week for each 10 week cycle. Mindfulness Based Stress Reduction: A six week group program teaching a variety of meditation practices to help manage stress and effectively deal with chronic pain, depression or anxiety.

AUGUST 2014

Advocacy The Oriole Foodspace opened in February in partnership with Flemingdon Health Centre, North York Harvest Food Bank, Working Women Community Centre, and the City of Toronto. We have worked for four years to build this space through advocacy and partnership building. The Trillium Foundation has provided the start up funds. Current programs include: Food Bank, Community Kitchens, Youth Community Kitchens, Kids’ Community Kitchens and Settlement Support. Flemingdon Health Centre joined other health care providers and community groups to rally against the changes to Interim Federal Health Insurance. These changes have reduced which health services are available to refugees and refugee claimants.

Youth The Step Up March Break program, geared to youth 13 to 16 years old was held for 24 youth for the 6th year. SERVE Canada and Flemingdon Health Centre partnered to deliver this program. Through teamwork, mental health and diversity education, community projects, and staff mentoring, the youth participants gained valuable experience and greater awareness, practised their interpersonal and cooperation skills, increased their connection to the local community/city and kept engaged during their March Break.

DIABETES PREVENTION Estimates of diabetes prevalence in Ontario show that 14% of the South Asian population living in Ontario are living with Type 2 Diabetes, compared to 5.2% of the general population. (Lipscombe & Hux, 2007). In response to this, Flemingdon Health Centre has been offering the South Asian Diabetes Prevention Program since 2009, addressing healthy eating, physical activity and stress as it is affected by resettlement to Canada. The two-part Program includes:
