

Accessing HealthRelated Social Needs
(HRSN) Benefits to Help Your Family Thrive
We know a healthy family life includes much more than doctor visits and medicine. Where you live, what you eat, and how you manage everyday challenges all play a big role in your well-being. That’s why some Oregon Health Plan (OHP) members may now qualify for Health-Related Social Needs (HRSN) services — extra support for families navigating tough times.
What Are HRSN Services?
Health-Related Social Needs are services that help meet basic needs that affect your health. These include:
Climate Supports
Helps you stay safe and comfortable during extreme weather. You may qualify for items like:
• Air conditioners (for heat waves)
• Air filters (for wildfire smoke)
• Portable heaters (for cold weather)
• Mini-fridges (for medications)
• Portable power sources (to keep medical devices running)
Housing Supports
Designed to help you stay housed, including:
• Assistance with rent or utility costs
• Support to avoid losing your home
• Help working with landlords
• Medically necessary health and safety changes to your home
Nutrition Supports
Parent Newsletter Month 2025

Makes it easier to eat healthy, especially with nutrition-related health conditions:
• Help to buy food, including fruits & vegetables
• Medically-tailored meals
• Education on eating well on a budget
Who Can Get These Services?
You may be eligible if you’re an OHP member going through certain life changes, including:
• Involvement with Oregon Child Welfare (past or present)
• Leaving residential mental health care (within the last year)
• Recently becoming eligible for Medicare/ Medicaid
• Released from detention or incarceration (within the last year)
• At risk of losing your housing
Other qualifying health conditions include:
• Pregnancy
• Complex physical, mental, or behavioral health needs
• Developmental disabilities
• Children under 6 years old
Each benefit has its own specific eligibility rules, so it’s worth checking to see if you qualify.
How Can You Find Out if You’re Eligible?
There are multiple ways to see if you are eligible.

OHP members who are pregnant or parenting young children (0-5 years old) can contact Pollywog to complete an eligibility screening and be referred for HRSN services. Contact us by phone/text 541-917-4884 or email pollywog@linnbenton.edu. ¡Hablamos español!
