The HSE Management System is foundational to the safe, reliable operations of our facilities.”

Our HSEMS systematically identifies, assesses and manages risks to safeguard employees, contractors and the environment. It ensures ongoing risk identification, prioritization and control while promoting continuous improvement in implementing HSE policies, meeting leadership expectations and upholding core values.

The HSEMS comprises four distinct phases with 15 interrelated elements:

Identify hazards, risks and regulatory requirements then establish strategic plans and objectives accordingly.

Properly implementing each element is essential for HSEMS effectiveness. Each phase builds on the previous one to ensure comprehensive risk management.

Implement tools for managing identified risks and requirements. Ensure effective risk management through detailed monitoring and auditing.

Review the HSEMS to identify strengths, gaps and opportunities for improvement.

POLICY AND LEADERSHIP

Phillips 66 leaders play a crucial role in implementing and driving HSE policy adherence. They ensure policies are clearly communicated to all stakeholders and promote a dynamic safety culture aimed at preventing incidents. By actively engaging the workforce on HSE issues, leaders foster an environment where safety is prioritized, and employees are empowered to identify and address unsafe practices and conditions.

RISK ASSESSMENT

Our risk assessment practices encompass comprehensive hazard identification, risk evaluation and analysis, and risk mitigation strategies. We thoroughly evaluate tasks, projects and products using scalable methods tailored to the severity of the risk.

We update risk assessments using a variety of resources. In addition to regulatory requirements, these may include incident investigations, audits, HSE metrics analyses, and stakeholder feedback.

Phillips 66 uses Process Hazard Analysis (PHA) to systematically analyze the design of each process to look for possible causes of process safety events. For events with the most severe consequences, we go even further and use Levels of Protection Analysis (LOPA). Safeguards identified in LOPA are called Independent Protection Layers (IPLs).

We use the Hierarchy of Controls to identify and mitigate hazardous scenarios effectively.

Phillips 66 business units monitor and uphold legal requirements and operational standards with unwavering commitment. Responsibilities are clearly assigned to ensure seamless identification, communication and continuous monitoring of all regulatory obligations, including laws, permits and commitments across our operational regions.

4 STRATEGIC PLANNING, GOALS AND OBJECTIVES

Each business unit maintains an HSE strategic planning process, documenting annual and five-year goals and objectives. These goals drive continuous improvement in environmental stewardship, occupational health and safety, process safety and sustainable development. Progress is reviewed annually by leadership with employee and stakeholder input, ensuring alignment with our commitment to a strong HSE culture and performance target achievements.

STRUCTURE AND RESPONSIBILITY

The organizational structure of HSE roles, responsibilities and accountabilities are documented and communicated, including site-level employee participation. Leadership ensures highly qualified subject-matter experts and resources are available for sustainable and successful HSEMS management and execution. Changes in personnel and organizational structure are actively managed, ensuring continuity and effectiveness.

6 PROGRAMS AND PROCEDURES

We establish and maintain comprehensive programs and procedures to manage significant risks and ensure legal requirements and Standards of Operation compliance. Training, auditing, reviews and revisions are regularly scheduled and executed to ensure ongoing effectiveness and relevance. These resources are accessible to employees, contractors and government agencies as needed.

ASSET AND OPERATING INTEGRITY

Business units implement comprehensive programs to minimize HSE risks related to operations and equipment failure. These programs ensure assets are designed, fabricated, installed, operated and maintained properly. Key components include quality assurance/quality control, defined inspection and maintenance intervals for critical equipment, safe operating procedures, risk assessment and management for deviations, and the use of Management of Change procedures and Pre-Startup Safety Reviews.

EMERGENCY PREPAREDNESS AND RESPONSE

Each facility has a written Emergency Response Plan outlining how potential emergency scenarios are identified and mitigated, including employee training and drills. The plan is thoroughly reviewed annually. At the corporate level, the company maintains a Crisis Management Plan and deploys Regional Response teams to support major emergencies, ensuring prompt and effective responses.

AWARENESS, TRAINING AND COMPETENCY

Each business unit ensures all employees have the knowledge and competencies to perform their jobs safely and responsibly. Training covers HSE policy, compliance, technical skills, emergency preparedness and mandated certifications.

Contractors receive orientation and HSE training, with participation verified. Business units adapt to changing requirements through a training needs analysis and a training matrix, specifying required competencies. We diligently monitor and report training compliance, fostering a culture of excellence and safety.

NONCONFORMANCE, INVESTIGATION AND CORRECTIVE ACTIONS

Business units adhere to structured processes for reporting, investigating and resolving nonconformances, near misses, and incidents. Root causes and management system deficiencies are promptly identified, with corrective actions addressing immediate and systemic issues. Insights and lessons learned are actively shared across the organization, promoting safety and continuous improvement.

11 COMMUNICATION

Business units facilitate effective internal HSE communication, emphasizing education and employee engagement. They foster open dialogue between management and staff, communicate critical HSE information to external stakeholders, and document each step for transparency and accountability.

DOCUMENT CONTROL AND RECORDS

Our robust document control processes ensure accessibility and effective life-cycle management of HSE documents, with regular reviews and revisions to maintain accuracy.

MEASURING AND MONITORING

Business units actively measure and monitor operational activities to ensure HSEMS goals and objectives are met. This proactive process evaluates the effectiveness of operational safeguards and controls while tracking health, occupational safety, process safety and environmental performance. Performance measures are shared regularly with leadership to analyze trends, assess impact and identify improvement opportunities.

AUDITS

At Phillips 66, each business unit audits the effectiveness of HSE controls and ensures compliance with legal and operational standards. Our auditing program promotes transparency and includes joint ventures, partnerships and contractors, fostering a culture of accountability and excellence throughout all operations. We prioritize continuous improvement through periodic reviews and prompt corrective actions to enhance our safety and environmental stewardship.

REVIEW

Periodically, business units conduct a comprehensive assessment of the HSEMS by analyzing key metrics, procedures, programs, activities, audit findings and incident statistics. This process systematically evaluates system strengths and identifies and addresses gaps and continuous improvement opportunities to advance HSE objectives.
