Welcome!

It is a pleasure having you with us tonight to honor two very impressive health care leaders and celebrate the Partners in Care Foundation.

Since 1997 Partners has been driving the new shape of care with a goal of improving the lives of people with complex social/health needs – people with disabilities, the chronically ill, the disadvantaged, and the elderly. During these years, we have served almost two million people needing social care services –be it for care coordination, care transition services, evidencebased health self-management programs, or other resources and information.

The agency was founded with one big idea in mind –that people should be able to safely enjoy living in the community and their home for as long as they can. That medical concerns alone should not drive people to medical care facilities too late – be it ER use, an extended hospital stay or a nursing home. Our Visiting Nurse roots reminded us that there were fixable things in people’s environments that made a dramatic difference in whether or not they were frequent visitors to the emergency room, challenging to discharge or ended up moving to a care facility.

We saw how prescription medications interactions could make people dizzy and fall. We saw a lack of basic transportation preventing people from getting to medical appointments. We saw how fragmentation of families and social conditions – not to mention a complex health system –meant that many older folks ended up confused about how to navigate the system to access the services for which they were eligible. And we knew with the application of simple and inexpensive interventions, many of these symptoms could be mitigated, allowing individuals to safely stay in their home, usually both healthier and happier.

Little did we know 25 years ago we were observing social circumstances that in the past five years have now become universally known as the social determinants of health.

Partners is a change agent, not only addressing the needs of those here in our own backyard, but statewide by consulting on efforts

that improve people’s lives through CalAIM. We are actively engaged in conversations on the use of resources to best achieve state and local goals. This includes formulating and presenting solutions to health plans, state agencies, and funders. In this role, we are representing the views and needs of community-based organizations as eligibility, covered services, reimbursement rules, and regulations are reviewed and revised.

Nationally, we co-lead a major initiative, the Partnership to Align Social Care - partnering with major health plans, federal agencies, medical providers, and community-based organizations –to co-design a new and better coordinated social and health service delivery eco-system. We believe this effort will strongly align the roles of all stakeholders around the needs of people with disabilities, the chronically ill, the disadvantaged, and the elderly.

Ensuring that service structures are in place and supported by policy and regulation is crucial, as is training the community health workers and other social care staff necessary to meet the needs of a growing aging population, which we are now doing through our Workforce Development Center.

As we reflect on 25 years of service and leadership, we see the change that is both possible and happening as providers, health plans, and community-based organizations work together. Even more encouraging is the recognition of our role in guiding this change.

2023 Award for Vision & Excellence in Healthcare Leadership

Johnese Spisso assumed the position of President of UCLA Health, CEO of UCLA Hospital System and Associate Vice Chancellor of UCLA Health Sciences in 2016. She is a nationally recognized academic healthcare leader with more than 40 years of experience. As CEO, she leads UCLA’s hospital system consisting of the UCLA Ronald Reagan Medical Center, UCLA Mattel Children’s Hospital, Resnick Neuropsychiatric Hospital at UCLA, UCLA Santa Monica Medical Center, the expansive network of over 200 UCLA Health primary and specialty care clinics throughout Southern California, the UCLA Health Homeless Health care program and the Faculty Practice Group. She is responsible for all aspects of the health system and the regional outreach strategy in the local, state, national and international programs.

Spisso is active in community leadership and has served as the Los Angeles Community Chair for the Leukemia and Lymphoma Society Light the Night Walk as well as the Los Angeles Community Chair for the American Heart Association’s Go Red for Women Luncheon. She has received numerous awards and recognition throughout her career, recently being named to Modern Healthcare’s Top 25 Most Influential Women Leaders in 2019, LA Business Journal’s 500 Most Influential Leaders in Los Angeles in 2020, Modern Healthcare’s Top 50 Clinical Leaders of 2020 in the U.S., and the Los Angeles Business Journal Women of Influence Award for Health Care in 2021 and 2022.

2023 Lifetime Achievement Award

Robert K. Ross, MD, is President and Chief Executive Officer for The California Endowment, a private, statewide health foundation established in 1996 to address the health needs of Californians. Prior to his appointment in September 2000, Dr. Ross has previous experience as Director of Health and Human Services Agency in San Diego County and as Commissioner of Public Health in the City of Philadelphia. During his tenure as President & CEO at The California Endowment, Dr. Ross has provided leadership in supporting the vision of underserved communities and grassroots leaders for a healthier California and a healthier America.

Dr. Ross has been actively involved in community and professional activities at both the local and national level. He served as a member of the President’s Advisory Commission on Educational Excellence for African Americans; Co-Chaired the national Diversity in Philanthropy Coalition; and has served as a member of the California Health Benefit Exchange Board, the Rockefeller Philanthropy Advisors Board, National Vaccine Advisory Committee, and on the boards of Grantmakers in Health, the San Diego United Way, and the Jackie Robinson YMCA. He is a Diplomate of the American Academy of Pediatrics, served on the President’s Summit for America’s Future and as chairman of the national Boost for Kids Initiative. He was honored by the Council on Foundations as the Distinguished Grantmaker of the Year for 2008 and received the American Public Health Association’s highest award in 2020. Most recently he has been invited to serve as Stanford University’s Distinguished Visiting Professor for the 2023 academic calendar.

2022 Arthur M. Southam, MD

Arthur M. Southam, MD was honored as Executive Vice President Health Plan Operations and Chief Growth Officer of Kaiser Permanente. Dr. Southam is dedicated to delivering high-quality, affordable health care and coverage to more Americans. Dr. Southam joined Kaiser Permanente in 2001 and had national responsibility for Health Plan marketing, sales, service, and administrative activities that support growth and care.

2021 Paul S. Viviano

Paul S. Viviano was honored as President and CEO of Children’s Hospital Los Angeles. Mr. Viviano is recognized as both a major health care leader and prominent advocate of child health. For over three decades, he has directed academic, nonprofit, and for-profit health care organizations delivering excellence in clinical care. Mr. Viviano joined Children’s Hospital Los Angeles as President and CEO in 2015 and serves as a member of its Board of Directors and Board of Trustees.

Paul Torrens, MD, was honored for over 40 years at the Fielding School, patiently guiding, mentoring and advising generations of students, helping shape many distinguished careers in public health. Equally important has been his long career in both healthcare management and policy development. As a healthcare executive, Dr. Torrens held numerous executive positions in hospitals and health care organizations and served on the governing boards of Blue Shield of California and PacifiCare Behavioral Health of California.

In the health policy arena, he served in a wide variety of advisory and consulting capacities to governmental and nongovernmental organizations both in the United States and additionally in 18 foreign countries. Thanks to his vision and leadership, the Paul Torrens Health Forum is a premier source of discovery and conversation about the prevailing public health issues of the day.

2019 Elaine Batchlor, MD, MPH

Elaine Batchlor, MD, MPH, was honored as the CEO of Martin Luther King, Jr. Community Hospital, a state-of-the-art, community-oriented safety net hospital that provides compassionate, quality care to the South Los Angeles Community. Throughout her career, Dr. Batchlor’s number one priority has been to improve access and quality of care for underserved communities utilizing innovative and collaborative approaches. Her work to increase access for underserved populations has been recognized as an example of leading best practices and adopted throughout California.

2018 Diana S. Dooley

Diana S. Dooley was honored as the Secretary of the California Health and Human Services Agency by Governor Jerry Brown at the beginning of his term in 2011. She used federal law, together with state legislative and administrative actions, to make improvements in the quality of healthcare delivery, increase transparency and consumer engagement, and promote the integration and coordination of medical, behavioral, and social services in the furtherance of whole-person care.

Sachin H. Jain, MD was honored as President and CEO of SCAN Group and Health Plan, where he is charged with leading the organization’s growth, diversification, and emerging efforts to reduce healthcare disparities. Under Dr. Jain’s leadership, SCAN’s revenues topped $3.4B and the organization now serves 220,000 patients.

James R. West was honored as the President and Chief Executive Officer of PIH Health, an integrated delivery system comprised of two hospitals, 22 medical offices, home health services, a medical foundation, and an independent physician association.

Molina Family & Molina Family Healthcare, Inc.

Molina Healthcare began in 1980 as an small medical clinic in Long Beach, California. As an emergency room physician, C. David Molina, MD saw the need for an alternative to going to the ER.

2010 Patrick Soon-Shiong, MD

Dr. Patrick Soon-Shiong was honored as a physician, surgeon, and scientist. He pioneered treatments for both diabetes and cancer, publishing over 100 scientific papers, and holds 50 US patents on groundbreaking advancements across a myriad of fields.

2006 Kathleen Brown

Kathleen Brown was honored as the former Treasurer of the State of California and Board member of the Los Angeles Unified School District.

Innovating Effective Social Care Solutions

For the past 25 years, Partners in Care Foundation has been innovating effective solutions addressing problems associated with social care. Today, our talented team leads the way in designing effective strategies to serve the most vulnerable populations among us as they navigate the complexities of today’s health care systems.

We know that once people leave a medical facility they are often on their own. An example of this is 23-year-old Steve. Recently discharged from a nursing facility following a stroke, he is now unable to perform the basic activities of daily living. Partners ’ Social Care Transitions Programs addresses non-medical threats that hinder successful transitions to home and ensures that recovering individuals like Steve have access to appropriate social care they need at home.

Just identifying who needs help isn’t enough. Those most in need of social care are often also the hardest to reach. They need to be found, enrolled, and fully served.

Our Engagement Center exceeds industry outreach success standards. This talented team achieves exceptional success in contacting and enrolling participants into a variety of social care services supporting their medical care and ongoing health.

Our team and technology successfully close the gaps that threaten the alignment of social and health care. Their skill has made them a sought-after supplier for health plans and providers throughout the United States.

Alignment Leads to Healthier Lives

Our programs align social care and health care, allowing people to lead happier and healthier lives.

A good example of this is Danielle. She takes seventeen different medications for health conditions related to congestive heart failure. She reported shortness of breath, extreme weakness in her legs, and had trouble standing for longer than five minutes. After a HomeMeds assessment, it was discovered that one of her medications was known to make symptoms of congested heart failure worse!

HomeMeds is Partner’s medication inventory and assessment tool that is invaluable in identifying medication problems that could lead to falls, drugs that affect mental state, harmful drug interactions, drug duplication (generic vs. brand name), and other issues that put older people at risk. When issues are identified, HomeMeds works with a pharmacist to suggest modifications to prevent medication related hazards and hospitalizations.

The Community Care Hub model is a means of engaging valuable social patient-care services with a demonstrated cost benefit that saves more than their cost.

Integration Key to Successful Alignment

It isn’t enough for people to have a doctor, a medical plan, and access to social care. All those pieces need to work as an integrated clinical and social care collaboration. Partners is pioneering an approach that brings together medical practices, social care service providers, and health plans to assure that a person gets the precise and specific social care they need in a supportive and coordinated manner.

Partners’ Workforce Development Center is expanding the social care workforce in California and developing Community Health Worker training programs that bridge community-wide healthcare gaps.

For 25 years, Partners has been a forerunner in aligning social care and healthcare to address the social determinants of health.

In all these ways and more, Partners continues to lead the way today for healthier outcomes tomorrow!

Charitable IRA Distributions can be set off against Required Minimum Distributions (RMDs) to reduce or eliminate income taxes on those RMDs

Charitable IRA Distributions are not included in the donor’s gross income and are not tax deductible

Consider a Gift to Partners’ Urgent Needs Fund and Change a Life

Partners maintains an Urgent Needs Fund to meet the critical and immediate needs of the very low-income people we serve. When funding is unavailable from any other public or private source, our Urgent Needs Fund contributes to the health and safety of our participants by making one-time purchases such as emergency housing support, adaptive equipment, home ramps, and other modifications essential to continued safe community living. These one-time investments change the course of lives by addressing basic human needs of safety and comfort and can be key to dramatic life changes.

One meaningful way to contribute to our Urgent Needs Fund is by donating “In Memory” or “In Honor” of someone special in your life.

Partners in Care Foundation Leading to Align Social Care and Health Care

Co-Designing a Social Care Delivery System

Currently in its second year, the Partnership to Align Social Care (“Partnership”) is a national learning and action network whose purpose is to advance the alignment of healthcare and social care service delivery. Consisting of leaders from across the social care and healthcare industries, this collaboration is leveraging the potential of cross-sector efforts to co-design scalable, sustainable, and equitable partnerships between healthcare entities and social care providers that are organized into community care networks led by community care hubs.

Partners in Care Foundation President & CEO, June Simmons, has served as Co-Chair of this innovative coalition since its inception in late 2021. In coordination with Co-Chair, Tim McNeill, COO of Freedmen’s Health, the Partnership to Align Social Care includes health plans, health systems, providers, community-based organizations, and government.

During its first year, this co-design initiative developed a suite of resources for healthcare and social care stakeholders. Informed by leading practices and focused on fostering infrastructure development, contracting, and payment standards, these tools aim to accelerate long-term system transformation and alignment between the social and health care ecosystems.

Federal agency leaders are also engaged as liaisons to the Partnership. According to Simmons, “This group offers a unique

collective commitment to ant the opportunity to effect meaningful, equitable, and system alignment. The synergy of this collaborative will ultimately impact positive health outcomes for the most underserved in communities across the country.”

Leading the Way on Aligning Health and Social Care

Throughout its second year, the Partnership to Align Social Care is focused on enabling and scaling efforts to ensure that community voices share in the design and governance of equitable health and social care ecosystems. Additionally, the Partnership will identify and elevate sustainable financing models, multi-stakeholder accountability efforts, and supportive policy changes at the federal, state, and local levels.

As McNeill observes, “The shift to value-based care will not be achieved unless sustainable models of SDOH delivery are implemented. The work of the Partnership to Align Social Care is an important initiative toward achieving Health Equity for all vulnerable populations.”

This work echoes the core of Partners’ mission for more than 25 years. Today, the importance of addressing the social determinants of health as a pathway to improve health outcomes is taking hold across California and nationally.

The work of the Partnership aims to support the growth of CBO networks and community care hubs that can act as equal partners to health care organizations and create a social care system that ensures the holistic needs of beneficiaries are met.
