The Community Care Hub model is a means of engaging valuable social patient-care services with a demonstrated cost benefit that saves more than their cost.

Hub streamlines access across California to a network of community-based and healthcare organizations and fosters cross-sector collaboration in order to achieve better population health outcomes.

Integration Key to Successful Alignment

It isn’t enough for people to have a doctor, a medical plan, and access to social care. All those pieces need to work as an integrated clinical and social care collaboration. Partners is pioneering an approach that brings together medical practices, social care service providers, and health plans to assure that a person gets the precise and specific social care they need in a supportive and coordinated manner.

Partners’ Workforce Development Center is expanding the social care workforce in California and developing Community Health Worker training programs that bridge community-wide healthcare gaps.

For 25 years, Partners has been a forerunner in aligning social care and healthcare to address the social determinants of health.
