The Time Is Now The experience we went through this past year powerfully demonstrated the importance of Partners, and saw our agency grow and thrive due to a strong mission and solid pillars of SDOH innovation and outreach to the vulnerable, the at-risk, and the underserved. Where others struggled, Partners was able to nimbly pivot and address the crisis. Our work keeps people safe and at home – needs demanded by COVID. Our staff was able to maintain constant contact with clients– the housebound elderly, the technologically underserved, people of color, the disabled, and the poor. If they needed food, connection to a PCP, medications delivered, or just a friendly voice on the phone, our team members were there helping them get through each day with a bit more hope.

As other organizations saw our ability to connect and support people, requests for our care management, transitions, and engagement

services grew. Our services are evidence-based with outstanding outcomes. We are NCQA-accredited and nationally recognized for our innovations. With talented staff amongst the best you will find anywhere. We have a lot to be thankful for and are extremely proud of our response to the pandemic. We are equally proud to be honoring two amazing powerhouses within the medical world – Paul Viviano, the President and CEO of Children’s Hospital Los Angeles, and Paul Torrens, Professor Emeritus of Health Policy and Management and Founder of the Paul Torrens Health Forum at UCLA. There is no better time to be honoring these two healthcare

executives as they exemplify the characteristics that apply to every healthcare person we’ve come into contact with during the pandemic. Devotion to caring for individuals, attention to detail, and an incredible focus on their work, committed to service, knowing the critical nature and importance of what they are doing. Even though these two are at the top of their organizations, we feel they represent all our healthcare heroes – the institutions, the executives, the medical staff, the front-line workers, the entire spectrum of health care workers who showed up every day. And that is where we feel the paths of Partners and medical care intersect – in a shared devotion to providing the best services possible for those in our care. Our mission has emerged stronger than ever, and a brilliant indicator that The Time Is Now for transforming healthcare together. We have gained tremendous insights Marta Fernandez through this experience, as has healthcare. Let’s Board Chair put those together and to work for our community! 2021 Award for Vision & Excellence in Healthcare Leadership Paul S. Viviano

President & Chief Executive Officer of Children’s Hospital Los Angeles

Paul S. Viviano is a health care leader and prominent advocate of child health. For over three decades, he has directed academic, nonprofit, and for-profit health care organizations delivering excellence in clinical care. Mr. Viviano joined Children’s Hospital Los Angeles as President and CEO in 2015 and serves as a member of its Board of Directors and Board of Trustees. Before CHLA, Mr. Viviano served as CEO and Associate Vice Chancellor of the UC San Diego Health System, which is noted for medical research and patient care. He served as President and CEO of USC University Hospital and USC/Norris Cancer Hospital and held various CEO roles within the St. Joseph Health System, ultimately serving as the parent organization’s President and COO. He also served as Chairman of the Board and CEO of Alliance HealthCare Services. An advocate of child health, he serves as Board Chair for California Children’s Hospital Association and as Co-chair of the National Children’s Hospital Association CEO Advisory Council, organizations protecting acute and specialty health care for children. A member of the Public Policy Committee for the national Children’s Hospital Association, Mr. Viviano also serves as Board Chair of the Trustees of Loyola Marymount University and is on UCLA’s Health Policy Management Alumni Association Board. He is a trusted executive and mentor for legions of other health care leaders.

2021 Lifetime Achievement Award Paul Torrens, MD Professor Emeritus of Health Policy and Management and Founder of the Paul Torrens Health Forum at UCLA

Dr. Paul Torrens is Professor Emeritus of Health Policy and Management and founder of the Paul Torrens Health Forum at UCLA. In his more than 40 years at the Fielding School, Dr. Torrens has patiently guided, mentored and advised generations of students, helping many shape distinguished careers in public health. A physician by training, he had a long career in both healthcare management and policy. As a healthcare executive, Dr. Torrens held numerous executive positions in hospitals and health care organizations and served on the governing boards of Blue Shield of California and PacifiCare Behavioral Health of California. In the health policy arena, he served in a wide variety of advisory and consulting capacities to governmental and nongovernmental organizations in the United States and in 18 foreign countries. Thanks to his vision and leadership, the Paul Torrens Health Forum is a premier source of discovery and conversation about the prevailing public health issues of the day. It is a fitting legacy for a man who has done so much for so many.

2021 Tribute Dinner Co-Chairs John Baackes Chief Executive Officer L.A. Care Health Plan

John Baackes is Chief Executive Officer of L.A. Care Health Plan, the nation’s largest publicly operated health plan serving over two million members. L.A. Care is dedicated to providing access to quality and affordable health care for Los Angeles County residents through a variety of health coverage programs, including Medicaid, L.A. Care Covered, L.A. Care Cal MediConnect Plan, and PASC-SEIU Homecare Workers Health Care Plan. Mr. Baackes draws on more than 40 years of health care experience. Before joining L.A. Care, he served as president of Philadelphia-based AmeriHealth Caritas VIP Plans where he oversaw the Medicare Advantage business unit. Prior to that position, Mr. Baackes was CEO of Senior Whole Health in Cambridge, MA, a voluntary health care plan for more than 10,000 low-income seniors in Massachusetts and New York. Mr. Baackes holds a bachelor’s degree from Southern Illinois University, Carbondale, and is a native of Evanston, IL.

Cástulo de la Rocha President and CEO AltaMed Health Services Corporation As President and CEO of AltaMed Health Services Corporation, Cástulo de la Rocha is an enduring trailblazer in community health. Under his visionary leadership, AltaMed has transformed from a storefront barrio clinic in East Los Angeles into the largest Federally Qualified Health Center in the nation. He changed the face of one community,

then another, until AltaMed became one of the area’s largest health care systems, with approximately 50 medical, dental and PACE sites staffed by 3,000 employees, providing compassionate care to nearly 300,000 patients a year. Over the years, Mr. de la Rocha succeeded in building an essential resource for millions of underserved residents throughout Southern California, and in the process, created a model of excellence for health centers and communities everywhere. Mr. de la Rocha holds a bachelor’s degree from the University of California, Santa Barbara, and a JD from Boalt Hall School of Law at the University of California, Berkeley.

Thomas M. Priselac President and CEO Cedars-Sinai Health System Thomas M. Priselac has served as President and CEO of Cedars-Sinai since 1994 and was also named President and CEO of the CedarsSinai Health System when it was formed in 2017. An author and invited speaker on policy issues regarding the delivery and financing of healthcare, Mr. Priselac has served the health care field in various roles during his career at Cedars-Sinai. He is a past Chair of the American Hospital Association Board of Trustees and past Chair of the Association of American Medical Colleges. The holder of the Warschaw/Law Endowed Chair in Healthcare Leadership at CedarsSinai Medical Center, Mr. Priselac also serves as an adjunct professor at the UCLA School of Public Health. A native of Pennsylvania, Mr. Priselac obtained a bachelor’s degree in Biology from Washington and Jefferson College in Pennsylvania, and a master’s in Public Health, Health Services Administration and Planning, from the University of Pittsburgh.

Past Honorees 2019 Elaine Batchlor, MD, MPH Elaine Batchlor, MD, MPH, was honored as the CEO of Martin Luther King, Jr. Community Hospital, a state-of-the-art, community-oriented safety net hospital that provides compassionate, quality care to the South Los Angeles Community. Throughout her career, Dr. Batchlor’s number one priority has been to improve access and quality of care for underserved communities utilizing innovative and collaborative approaches. Her work to increase access for underserved populations has been recognized as an example of leading best practices and adopted throughout California.

2018 Diana S. Dooley Diana S. Dooley was honored as the Secretary of the California Health and Human Services Agency by Governor Jerry Brown at the beginning of his term in 2011. She has used the federal law, together with state legislative and administrative actions, to make improvements in the quality of healthcare delivery, increase transparency and consumer engagement, and promote the integration and coordination of medical, behavioral, and social services in the furtherance of whole-person care.

2017 Rod Hochman, MD Rod Hochman, MD, was honored as president and CEO of Providence St. Joseph Health. 2004 and 2018 Lifetime Achievement Award C. Duane Dauner In Memoriam C. Duane Dauner was honored as President and CEO of the California Hospital Association. A nationally known leader on health issues, he was a popular lecturer and authored numerous articles.

2003 Robert E. Tranquada, MD Dr. Tranquada’s leadership initiated and shaped many health care organizations. Although retired, Dr. Tranquada’s leadership continues with service on many local and national health care charitable, higher education, and civic boards, and as an author of numerous medical and policy articles.

2002 James E. Ludlam, JD In Memoriam Mr. Ludlam passed away in August of 2008. He was recognized as one of the founders of healthcare law who helped shape California’s healthcare environment.

2001 Yoshi Honkawa Mr. Honkawa served for many years as Vice President, Government and Industry Relations, Cedars-Sinai Medical Center, following a long and distinguished career in health care administration.

2000 Dr. Allen & Weta Mathies The Mathies Award was named in honor of this couple’s distinguished careers and Dr. Mathies’ remarkable leadership of the Visiting Nurse Association of Los Angeles (VNA-LA) and the founding Board of Partners in Care Foundation.

West was honored as the President and Chief Executive Officer of PIH Health, an integrated delivery system comprised of two hospitals, 22 medical offices, home health services, a medical foundation, and an independent physician association.

2011 Molina Family & Molina Family Healthcare, Inc. Molina Healthcare began in 1980 as an small medical clinic in Long Beach, California. As an emergency room physician, C. David Molina, MD saw the need for an alternative to going to the ER.

2010 Patrick Soon-Shiong, MD Dr. Patrick Soon-Shiong was honored as a physician, surgeon, and scientist. He pioneered treatments for both diabetes and cancer, publishing over 100 scientific papers, and holding 50 US patents on groundbreaking advancements across a myriad of fields.

Partners’ Innovations Partners has a history of bold, powerful approaches focused on mitigating the social determinants of health that we make available to others, including these innovations: • Evidence-based programs providing short-term bridges from hospital or nursing home discharge to home focused on patient safety and independence. • A statewide delivery system of high-impact health empowerment programs to help people master skills required to live better with chronic conditions such as diabetes, arthritis, or balance challenges. • HomeMeds℠ is our evidence-based medication home inventory software tool that catalogs and reports a person’s medications, identifying interactions that can lead to medical complications. • Development of an effective alternative workforce of Community Health Workers, working in partnership with physicians, health plans, and health systems addressing the social determinants of health. 28


• Partners at Home is our model statewide collaborative network of credentialed Community-Based Organizations allowing physicians and health plans to access locally-based services, senior centers, and Area Agencies on Aging through a single contract with Partners.

Partners’ Services Bridge the Gaps Between Social and Medical Services Community Wellness Programs Our evidence-based workshops serve a diverse group of clients with challenging chronic health conditions such as diabetes, heart disease, arthritis, chronic pain, fall risks, loneliness, and social isolation. These workshops are offered both in-person or virtually by telephone or using internet video applications. Participants learn to set realistic personal health goals and gain insights and skills to better manage their emotions and physical environment, adopt better health behaviors, and navigate their healthcare. Partners recognizes there are many ways to learn, which many ways feed ongoing innovation in both where and how Partners delivers this growing portfolio of powerful health self-management and wellness programs.

Safety Net Long-Term Services and Supports (LTSS) Partners operates programs for Medi-Cal beneficiaries serving over 3,700 very low-income community members with complex needs. These programs are unique in providing the ability to follow people over extended periods of time, assuring ongoing support for those facing complex challenges for independent living: Multipurpose Senior Services Program (MSSP) MSSP waiver clients are 65 years or older, low income, meet strict Medi-Cal eligibility requirements, and are functionally impaired. They need a level of care equal to that provided in a nursing home because their ability for selfcare is compromised. The MSSP team designs, implements, and monitors customized care plans to help avoid or end placement in a nursing home and ensure a dignified and safe aging experience in their own home. After the initial establishment of services, the team continues a supportive relationship with clients through monitoring calls and regular home visits. The service lasts as long as the client needs this support, often for years. Partners is now the largest MSSP provider in California, serving clients in two major sites within Los Angeles County, and smaller sites in Kern County and Santa Barbara County. Home and Community-Based Alternatives (HCBA) Partners provides care coordination and oversees complex in-home support services for highly disabled/medically complex children and adults. About 60% of Partners’ Los Angeles County caseload focuses on helping people move out of hospitals or nursing homes back to the community. The other 40% prevents placement in hospitals and nursing homes. Participants’ complex medical conditions include genetic disorders, immune deficiency disorders, and sports or motor vehicle accidents. Some are nonverbal, and all need considerable help with a variety of daily living activities, other key resources, and care coordination. Health Homes Program (HHP) Partners’ serves eligible Medi-Cal beneficiaries with chronic and complex medical and social needs who will benefit from enhanced care management and coordination. Our role is to identify participants, engage them, help them establish a medical home, and provide ongoing social care management to resolve critical social challenges. Many are experiencing 30


homelessness or on the cusp of experiencing homelessness. Participants have multiple chronic medical conditions and in the past year had a long hospital stay and/or have visited an emergency department three or more times. The program is open to anyone without age restrictions, but most program participants are between 40 and 65 years old. HHP is one of the few programs that truly meets people where they are in life and accepts people for services without a physical address and those who might be using illegal drugs – usually barriers to being served. Our Safety Net programs receive high ratings from funders, participants, and their families for the caring and quality of services and supports provided to these highly vulnerable recipients. As an accredited agency in complex case management by the National Committee on Quality Assessment, the highest standards of quality are built into each of our program services.

Short-Term Services and Supports (STSS) Under contract with health plans and hospital systems, Partners’ staff of social workers and health coaches work closely with individuals in need of coaching and/or care coordination to facilitate safe return home from the hospital or nursing home and avoid a decline in health or readmission. Services are usually provided for 30, 60, or 90-day periods of time – often following an individual’s discharge from the hospital or nursing home – a time of maximum risk for medication errors, falls or worsening health. Staff visit the home and use a specialized functional/psychosocial assessment to identify needs and risks and create a personalized plan to help the individual access benefits and services. Multiple studies have shown these interventions are effective at reducing readmission rates by 34% to 66%.

Partners’ Engagement Center Our state-of-the-art Engagement Center uses advanced dialing and result-tracking technologies to engage people in service programs, locally and throughout the United States. Staff are skilled at targeted outreach strategies, motivational interviewing techniques, as well as enrollment processing and scheduling. Clinics, offices, and major insurance companies frequently use the Center for outreach, due to a very high success rate in meeting patient activation goals. During the pandemic, the Center provided outreach and education to members of Blue Shield of California and Blue Shield of California Promise Health Plans. Of calls accepted by plan members, the Center had an outstanding 30% success rate in engaging the members in COVID-19 related services.

Partners at Home Network Partners also contracts with health plans and other providers statewide through its Partners at Home Network of agencies and community-based organizations. • A single point of access to a large spectrum of quality-accredited services, scalable for regional and state coverage • Coordination of resources (medication management, home services, meal delivery, etc.) provided at a competitive price • Continuity of services provided by culturally diverse providers with local expertise to engage patients to achieve their individualized outcomes
