

The Ford Foundation provides the following benefits to New York-based full-time employees. You share the cost of medical coverage with the Foundation while other benefits are provided to you at no cost. Your contributions are deducted from your pay on a pre-tax basis and are based on your salary, the plan you elect and family composition (employee, employee + 1 dependent (spouse and/or child) or family coverage) – see monthly rate sheet attached. Proof of other health coverage is required if you elect to waive Foundation coverage. Eligibility for participation in the benefit program is on the first day of employment unless otherwise noted.
All employees working 30 hours or more on average per week are eligible to enroll in benefits. You may also enroll your legal dependents, including your legally married spouse and children up to age 26. Children may be natural, adopted, stepchildren, and children through courtappointed legal guardianship. Mentally or physically disabled children may continue coverage beyond age 26.
Unless you experience a life-changing qualifying event, you cannot make changes to your benefits until the next Open Enrollment period. Qualifying events include things like:
Marriage, divorce or legal separation
Birth or adoption of a child
Change in child’s dependent status
Death of a spouse/domestic partner, child or other qualified dependent
Change in residence
Change in employment status or a change in coverage under another employer-sponsored plan.
Telehealth virtual provider that provides $0 visits for cold, flu, pink eye, sinus infections and more.
An enhanced mental health benefit that will provide employees with oneon-one counseling services with licensed therapists at no cost to the employee.

Consolidated Administrative Services (CAS) will be your new Health Savings Account and Flexible Spending Account Provider.
Open Enrollment is the one time of year you can review your current elections and make changes for the upcoming plan year outside of a qualifying event. All benefit elections and changes will be effective January 1, 2024.
All employees can complete online enrollment on Paycom. The deadline to complete Open Enrollment is midnight Friday, November 17th.

You can contribute up to $4,150 on a pre-tax basis for individual, or $8,300 for family annually to help save and pay for current and future qualified healthcare expenses. Funds grow tax free.

Qualified dependents include: Your dependent children up to their 13th birthday or any dependents living with you for more than half the year and who is physically and mentally incapable of self-care.

Ford Foundation provides all benefit eligible employees with 3x your annual salary to $66,666 plus 1x salary over $66,666 to $400,000 maximum in group term life and accidental death and dismemberment (AD&D) insurance.
All eligible employees that are currently insured under this Voluntary Life Policy, may increase their Voluntary Life Insurance Benefit up to 1 units of 10,000 as long as the total benefit does not exceed the Guaranteed Issue Amount without completing Evidence of Insurability. You may also increase the Spouse Voluntary Life Benefit by 1 units of 5,000 if your spouse is currently insured, as long as the total benefit does not exceed the Guaranteed Issue Amount without completing Evidence of Insurability.

If, within 365 days of a covered accident, bodily injuries result in:
Loss of life, or total paralysis of upper and lower limbs, or loss of any combination of two: hands, feet or eyesight
Total paralysis of both lower or upper limbs
Total paralysis of upper and lower limbs on one side of the body, or loss of hand, foot or sight in one eye, or severance and reattachment of one hand or foot

You – You are eligible for coverage if you are an active full-time, regular employee of the sponsoring employer working 30 or more hours per week. You will be eligible to elect insurance for yourself and your dependents on the first of the month following receipt of your application by the foundation.
Your Family – You may elect to cover your lawful spouse under age 70 and your unmarried dependent children who are under age 19 (or under age 26 if they are full-time students). Children must be dependent upon you for support and maintenance. No one may be covered more than once under this plan. If covered as an employee, you cannot also be covered as a dependent.
You – You may select from $10,000 to $500,000 of coverage in units of $10,000 at an affordable price.
Your Family – Your Spouse’s benefit amount will be 50% of yours or 60% if you have no dependent children, subject to a maximum benefit of $250,000. Each of your covered children’s benefit amount will be 10% of yours or 15% if you have no eligible spouse, subject to a maximum benefit of $50,000.
Ford Foundation provides all benefit eligible employees with Long-term disability (LTD) insurance. The benefit includes:
• Monthly benefit equal to 60% of your monthly covered earnings to a maximum of $20,000 per month, due to an accident or illness.
• For employees who are continuously disabled for more than six months
Vacation: 3 weeks (15 business days) in first year of service; 4 weeks (20 business days) in subsequent years. Vacation accrues on a monthly basis and can be taken after the completion of four months of service.
Personal Days: 10 days per year – 3 days for any reason, plus 7 days for defined reasons.
Full-pay or half-pay for up to 26 weeks, depending on length of service and duration of illness.

• Eligibility: First of the month following 30 days of employment.
• Qualified tax deferred retirement savings plan. Maximum employee pre-tax contribution for 2024 is $23,000; and additional pre-tax catch-up contributions of $7,500 for participants age 50+.
• Employees are automatically enrolled with contributions of 2% of eligible salary on a pre-tax and/or post-tax basis and receive Foundation matching contributions of 3% of eligible salary.
• Employees have the option to elect a different contribution amount or to waive participation in the plan.
• Foundation contributes an additional 10% of employee’s eligible salary, even if employee does not contribute.
