When I initially started out on my career as an educator I realized that I was not only a trained Master Teacher, but also an advocate for families with challenged infants and toddlers. Who were experiencing crises in their lives? I promised my self that I would serve families in the urban communities as well as across the Diaspora. This was primarily due to the fact that I myself had been a victim, at the young age of (2) two years old my mother past away. My father soon made his transition. I became an orphan at the age of (10) ten years old. Theses were the promises that I made to myself. #! Promise: I will always pursue work that is in my passion, The Family! #2.Promise: I will apply my energies toward a good cause at all times. #3. Promise: I will create a pathway toward my goals each day. #4.Promise I will commit my work to excellent in education each day. #5. I Promise to do all things through Christ Jesus, my Elder Brother who strengthen me. #6. I Promise to use my tongue to speak life over my situation on every occasion. #7. I promise to collaborate with my higher Self and to cooperatively work with the God Head who lives inside of me.

Parents you can study this curriculum at your own pace, with the assistance of a certified Co-Parenting Resolution Assistant aiding and assisting you with your family Plan. 219-290-4408 Project 2019 Incorporated Co-Parenting Resolution Assistant Project INDEPENDENT PROVIDERS CONTRACTOR AGREEMENT This Independent Contractor Agreement (the” Agreement”) is made and entered between, Elaine A. Slawinski, an independent contractor hereafter referred to as “Contractor” And The Lake County Superior Courts, Domestic and Family Law hereafter referred to as “Organization”. In consideration of the covenants and conditions hereinafter set forth, Organization and Contractor agree as follows: 1. SERVICE Contractor shall perform the following services for the Organization (the “Work”). Conduct the Co-Parenting Screening, Needs Assessment, Parent, and Education Content. Grief Session Assessment Observation Instrument for a minimum of (10) ten Co-Parenting Students. That will enrolled in the Co-Parenting certificatecredentialing Program through Project 2019 Incorporated. Attendance at an orientation session, outlining the successful completion of the Co-Parenting training is required in addition to the purchasing of required books and materials... In the event that the Parent cannot afford to pay, that state will compensate Contractor for Co-Parenting training. Conduct a Pre- Observation Conference Call interview with all Trainees by, Judge Kerman. J.D. Ph.D. and Dr. Joy Jordan. Contact information shall be provided for each student enrolled in the Co-Parenting class such as; a completed application and emergency contact numbers.

Conduct a minimum of two on site classroom sessions per student enrolled in the Co-Parenting class, in order to complete the Family Plan Report, while guiding each candidate through the process of preparing a written report for courts. Also there shall be (1) one family visit in the child’s natural setting (In the home environment). Separate reports on the children are not compiled. However, there is only one flat fee for the contractor having a session with the children no matter how many children there are in the family. Complete an on site assessment of each child in the family for groupdynamics work and or individual sessions. Parent, Grand Parent, and Others and the Contractor shall make arrangements to establish all scheduled secessions appointments in advance prior to the next session For example; may be Teen share Activities, Peer Counselor Component, Session Timeline, Session on grief for the children. We also offer a six-week program on divorce education for the children; the Contractor is offering this program. The focus of the program is on the children, helping them with their individual needs as they pertain to the changes that their parents may be taking them through. Parents can expect to receive weekly handouts and both the video and the workbook will be used with the children in order to develop coping skills, while going through a divorce with their parents. The cost of the session for the children will be $70.00 per family session regardless of the number of sibling in the family. Sorry no reports will be prepared. Refer Parents to mediators and or arbitrators for issues that cannot be resolved in the Co- Parenting Report. A separate memorandum maybe drafted, if necessary and required by the courts. The types of services that the Contractor will offer are as follows; • Co-Parenting Three part, 6.5 hours “hands-on interactive, intensive training in Parent –to-Parent communication, if possible, if the couple can not work together, then separate classes will be established. I will encourage both parents to participate at the same time. However, if this is not possible then I will see them separately. • Mediation: is in process of being established with for services in the near future. 5


• Counseling & Consultation: Blended Families, Individual and Personal Crises Parent and Child reunification of infant and toddlers in addition to children and Youth. • Advocating from the Bench for Infant and Toddler, who have been Maltreated, assisting the courts with best practices for Early Intervention/ Prevention services under Section C of the Law for Birth to Three. • Therapy and &Divorce Education for infant& Toddlers and Children, This is a program to help the children get through the difficult times of separation and or divorce. I can perform Early Childhood Behavior • Therapy for infants and toddler who have delays in growth and development. However, upon request we can refer the clients to a Therapist who has a PHD in Psychology. 2. REPORTTNG Contractors shall report to Honorable, Elizabeth F. Tavitas as their immediate supervisor, while performing services for the courts of Lake County, Indiana, Family Courts, Co-Operative Parenting. Since, the approval rest with the three Judges of family courts in Lake County. I would like to petition each of the Judges to assist our contractors in carrying out the task of Co-Parenting Resolution Assistant. Location: The proposed location of the institute has been established in the city and state of: Merrillville, Indiana, physically located in a brick and mortar building. At Project 2019 incorporated, on the second floor of the conference room, where classes have been scheduled to be held three days a week on Wednesdays, Fridays and Saturdays. Project 2019 is a branch owner of Cornerstone Theological Seminary 6111 Harrison Street, Merrillville, Indiana 46410. The Co-Parenting Training of the Trainer facilitator’s classes has been held under this under Cornerstone Christian College ,under guidance of Judge Kerman and Juy Jordan.

Persons Sponsoring the Institute: The person that will be sponsoring the institute is Sherrill M. Garth President of the Northern, Indiana chapter of Project 2019. The institute will be residing at 6111 Harrison Street, Merrillville, Indiana 46410 Suite 275. The chapter’s institute will be name: Lilly Institute for Family Education under the guidance of Project 2019. Contractors: However, only those candidates who have completed the training through Project 2019, Co-Parenting Resolution Assistant Training with our institute may make a request to be placed on the registry with Lake County, Indiana Family Courts. Each Provider must contract as an individual, independent contractor, based on their credentials and experiences. Credentials and Certification of the Contractors: The candidates that have completed the 6.5 hour course come from a wide background such as; Within the group there are two Master Teachers, that are certified Teachers in the specialty areas of: Early Childhood Development with backgrounds in Curriculum and Instruction who have become master facilitators, state side as well as overseas in Europe. Some are from Purdue University, school of Consumer Science and specializing in Family Life. Also while, others have many years of experience in the areas of specialty of Education from Kindergarten through Twelfth. Grade. Also backgrounds in education will allow each of us to perform the dues of a Behavior Specialist, in the field of Child Development under section C. of Special Needs and challenged children. We are able to advocate for the infants and Toddlers of the community from the bench who have been Maltreated. We can offer specialized advice on the best practices for infants and toddlers who may be delayed in their growth stages, as an early intervention/prevention service. There are candidates with backgrounds in Family law, one of our students including myself are graduates of Roosevelt University’s Post Graduate Certificate program in the field of Paralegal/Legal Assistant work, in Chicago, Illinois. We have several candidates that are Independent Business owners in the non-for Profit area offering services in the area of social services for fathers struggling in the area of custody issues. There are some candidates with degrees in business and Administration. These businesses, owners are in the field of training and development for candidates re-entering the workforce, as well being a Certified Minority owned and Woman own 7


Business. Our program was founder on a faith base initiative, therefore our search initially begin with seeking out oft Men and Women of the Ministry and the clergy. We initially offered them a full scholarship in order to complete their Theology degrees. Some of Trainee actually has Ministerial licensing in theology. Judge Kerman is one of the co-authors of the curriculum. She has over thirty years in the specialty field of family law, Mediation, and Arbitration. She is also a Judge and a Psychologist. Her co-partner is Dr Juy. Jordan. Dr. Jordan feels that there would be a great advantage in employing the clergy in the field of family law, because of their previous training backgrounds in counseling. Dr. Guy Jordan is a trained and licensed Psychologist on our staff as an advisor. However, we are currently interviewing a local Psychologist whose husband has been trained as one of our Co-Parenting Resolution Assistant facilitators. Judge Kerman as well as Dr. Jordan both participated in the training of these facilitators. All Trainee have completed the Co-Parenting training in a conference call session with the facilitator., How These Classes would be consistent With the Goals of the Lake County Family Law Rules and Indiana statue. I sincerely believe that our classes would be more than consistent with the goals and regulations of the Lake County Family Law Rule and Indiana statue, due to the fact, that the statues of Indiana are in line with the curriculum, which has been prepared, by an Attorney, and a Judge who have practiced in the field of Family Law for over thirty years. Dr.Kerman is a practicing Georgia Divorce and Custody Attorney since 1976. She also an Educational Psychologist with a PhD in Psychology. In addition she completed four classes in Marriage and Family Therapy at the university of Georgia in order to practice in the area of Family Law, with a better understanding of family dynamics. She currently teaches courses in Psychology and family relation and has taught courses in law, communication, Psychology, sociology and related subjects. Dr. Kerman has tried domestic cases in Florida , Texas, Ohio , New York, New Jersey, Massachusetts, as well as Georgia, She is registered with the state bar of Georgia as a Domestic Mediator Case Evaluator Domestic Relation, Arbitrator, as well as being registered Guardian Ad Litem. She has also been trained as a juvenile court Mediator. Alone with her practice Dr. Kerman is the creator and host a live Internet Talk- Radio show. The Parents’ Hour, which focus solely on topic of importance to parents and their children and airs every Friday from 4:00.P.M. to 5:00 P.M. ET on 8

Dr. Kerman is member of the American Psychological Association, Georgia Psychological Association, and Georgia’s Association, Georgia’s bar foundation, Association for conflict resolution, various bar Associations, and the founder and past President of the Santiago Psych-Law Society. Dr. Kerman’s publication includes an article for feature entitled” Who Should Get The Kids After A Divorce?” Published the Atlanta Journal- Constitution, “Georgia Needs a Family Court Now,”Published in the Verdict, in book entitle” Step Parents? Should You Really Seek Custody Of Your Child”. “ New Georgia addition. How To Survive and Win As A Co- Parent, and How To Survive And Win as A Coparent (Second Edition) Also Dr. Guy Jordan is has had over a decade of experience working with families in the court system. Not to mention the other professional staff who will be advocating for not only families, but the. ”Child in the Middle” who requires that professional identify screen and refer for services. I feel that are goals are aligned with the mission statement of the statues. Guy W. Jordan, P.D. is a licensed Psychologist practicing in Gainesville, Georgia. He received a B.A. in Psychology from Valdosta State College in 1975. M.Ed in School Psychology from the University of Georgia in 1976, and a Ph.D. in Educational Psychology from the University of Georgia in 1981 Dr. Jordan became licensed for private practice in 1983. He is a member of the American Psychological Association, Georgia Psychological Association, and Georgia Psychological Association. Professional Academy of Custody Evaluators, American College of Forensic Examiner and American Board Disability Analysts. Dr, Jordan works a school Psychologist in Hall School System from 1977 to 1985. During time, he also served part time as an Adjunct Faculty from 1975 to 1985 with the department of Educational Psychology, University of Georgia and with Brenau College from 1979 to 1984. He worked the private practice 1983 entering full-time private practice in 1985. Dr. Jordan has 30 years experience as a Psychologist. He conducted over three thousand evaluations for court purposes. Evaluations have been for juvenile, superior, and federal courts. These evaluations has been for juvenile offenders, child abuse cases, custody cases, criminal cases, competency hearings, personal injury, and dispositions and or /sentencing hearings. Dr. Jordan has been qualified and testified in court as an expert witness over thousand times. Dr. Jordan has maintained a counseling practice that includes children adolescents, adults and family. He has present at state and national conferences for local agency. 9
