Home Care
Our home care services bring high-level nursing, rehabilitation therapy, and aide care directly to patient homes and can be supported by telehealth as needed. We work closely with and are trusted by regional hospitals and physicians to manage goals of care.

Palliative Care
Palliative care is a person-centered approach intended for patients with serious, advanced illness to improve their quality of life and achieve goals of living. Patients who benefit from palliative care include those with cancer, COPD, ALS, Parkinson’s or dementia, kidney or liver disease, and heart failure, as well as those who experience uncontrolled symptoms due to other serious illness (pain, insomnia, shortness of breath, nausea, constipation, anxiety, etc.).
Our nurse practitioner-led palliative care program can offer:
• Relief from pain and other distressing physical, psychosocial, and spiritual symptoms
• Management of stress due to living with serious illness
• Support to help patients live as actively as possible
• Comprehensive integration of care to focus on what is most important to each patient—not just a diagnosis

Hospice Care
We provide comprehensive hospice care with an emphasis on quality of life, pain management, and symptom relief. Our team utilizes a holistic approach to offer physical, emotional, spiritual, and social support for patients and families facing life-limiting illness.
Our hospice program delivers:
• Patient-centered spiritual care by our chaplains as desired to assist people of all faiths
• Nurses, home health aides, and social workers who receive specialized, certified training in end-of-life, Alzheimer’s, and dementia
• Trained and certified volunteers for companionship, respite, and assistance
• Ongoing family support through education, bereavement programs, and groups

Pat Roche Hospice Home
Our 12-bed hospice home, located in Hingham, provides a dignified and peaceful environment for residents unable to remain in their homes but seeking hospice care in a homelike setting. The Pat Roche Hospice Home is the first non-profit, independent hospice home on the South Shore and is sustained due to the philanthropic generosity of our community.
The Pat Roche Hospice Home provides:
• Private rooms and bathrooms with 24-hour nursing and aide care
• Housekeeping, meal, and laundry services
• Interdisciplinary hospice team visits, including chaplains, volunteers, social workers, and counselors
• Common spaces for families and patients

Private Duty
NVNA WORKS offers a full range of private pay home care services to keep people healthy and safe in their homes. Care is customized to meet each client’s needs, including one or more of our services such as:
• Companion care
• Homemaking
• Personal care
• Private nursing and rehabilitation therapies
Navigator 27 is a concierge case management service that assists clients and their families as they traverse the challenges of aging and care transitions. We can:
• Advise on how to manage and coordinate short and long-term care plans for self or a loved one
• Accompany families to appointments to visit and evaluate facilities
• Offer specific guidance regarding memory impairment and services available
• Attend discharge planning meetings at acute care hospitals and skilled nursing facilities
• Help with interpretation and maximizing of insurance coverage options

Frequently Asked Questions
What services does NVNA and Hospice provide?
NVNA and Hospice has a variety of highly skilled clinical staff, including registered nurses, nurse practitioners, home health aides, certified diabetes educators, licensed social workers, and many other knowledgeable healthcare professionals who offer palliative care, hospice care, and bereavement services. NVNA and Hospice also offers programs and support groups through NVNA SUPPORTS.
When is home care necessary?
Home care may be necessary for a variety of reasons. We treat patients who are recovering from an illness, surgery, stroke, heart attack, or an accident. Our job is to support patients as well as their families, with an overall goal of maintaining dignity. We accomplish this goal by empowering individuals with the tools and information they need to stay healthy and happy within the comfort of their own homes.
How can I contact my caregiver?
If you need to transmit a message to your NVNA and Hospice caregiver regarding an appointment change or clinical need, please call our office at (781) 659-2342 and we will ensure your message is quickly relayed to them.
What if I have a question at night or on weekends?
If you need our help during non-office hours, NVNA and Hospice has a 24-hour answer service, 7 days a week. Please call us at (781) 659-2342.
If it is an emergency, please call 911.
For billing and general questions, please call during office hours, Monday–Friday 8:00am–5:00pm.
What if I live out-of-state and need more help for my aging parent?
We can help. In addition to offering a full continuum of home care, our private care division, NVNA WORKS, can help loved ones transition home from the hospital or skilled nursing facility, pick up medications or groceries, provide transport to doctor’s appointments, and perform light housekeeping. We can also help local adult caregivers who need intermittent assistance or plan to travel. See page 11 to learn more.
If a loved one is a hospice patient and needs help at night or on weekends, what should I do?
Health needs do not stop at 5:00pm, and neither do we. Once on hospice, the primary goal is to remain at home and not return to the hospital. Concerns that arise after normal business hours will be addressed by a hospice-trained caregiver collaborating with the patient’s physician to achieve the goal of remaining comfortably at home. We are on call 24/7 at (781) 659-2342.
