Getting sober ‘gave me the resilience to come out of my own shell,’ says Moses Totalik

Like many people who struggle with addiction, there were many factors that went into Nunavummiuq Moses Totalik’s dark journey through substance abuse.

It evolved following a breakup from his first major relationship. Suddenly, Totalik’s dabbling with alcohol and marijuana took a serious turn for the worse. Occasionally partaking socially morphed into increasing secretive abuse, and the then-22-year-old was convicted of driving

under the influence. Although Totalik pulled over safely and avoided hitting anything after his vehicle ran out of gas, his licence was revoked, despite it being a first offence. He was charged with being involved in a “single vehicle collision.”

“I don’t remember all of it,” he admits. “I just remember putting my hand out… apparently I tried flagging people down. After that, I remember the solid bed in the jail cell and the breathalyzer.”

That incident became the catalyst that sent Totalik on a downward spiral of addiction.

“Honestly I’m not sure what I was thinking that day [in jail] because it was so many mixed feelings for me… heart-broken, guilt, shame.”

He then deliberately took the plunge into harder narcotics such as cocaine, Xanax, and fentanyl. Rather than a wake up call, the DUI pushed him into a darker place of desperation and hopelessness. He became further involved with the drug crowd, and used social media to make contacts in his search for relief from his mental anguish.

After moving to Ottawa, it became all too easy to find what he wanted on the street, particularly

Northmarket Street, an area he has to avoid now due to triggers. An average day consisted of starting by smoking marijuana, snorting coke “once every two hours” and inhaling micro-doses of fentanyl once every six hours or so.

This went on for about six months before Totalik, who hails from Gjoa Haven, reached out for help and moved into a group home and sobriety program. He now lives in a more relaxed facility, although he’s still monitored by camera 24 hours a day, has to turn in his electronic devices at night, and is under guardianship that oversees every aspect of his life.

Addiction and the system: “The new residential school” Now 27 years old, Totalik recounts his experiences “under the system” since age four, when he was removed from an allegedly abusive situation in a home where alcohol was abused. He considers himself to be a “stolen child” who went through 15 or more foster homes.

“I lost count,” recalls Totalik. “They were different homes all around Nunavut, so it was traumatizing. You don’t know who these [foster parents] are or what their background is. I call it the ‘new residential school.’”

He added that he considers his history and background to be factors leading to his addiction problems.

During his DUI court case, his court-appointed counsellor suggested guardianship. Deferring to the advice of his lawyer, and with no prior knowledge of what he was getting into, Totalik went along with the decision.

How much his guardianship has helped his recovery process is unclear.

“Being a guardianship client really limits my ability to live independently. They think I’m going to go back to my old life, and that’s not really my intention anymore,” he said. “It’s difficult to deal with because those are the reasons I became an addict. I had no control [over my circumstances].”

In addition, he mentioned that in a group home for recovery in Alberta, speaking Inuktitut was forbidden.

“I’ve lost my heritage. I don’t speak my mother-tongue. I don’t sound Inuk,” he said.

The same feelings of cultural isolation,

loneliness and lack of agency are all important factors when considering the complex puzzle that is addiction and its causes.

He feels that his support workers are just being paid, rather than genuinely invested in his well-being and recovery.

“I wish I could restart my life and say no to guardianship.”

However, this young man has made great personal strides to overcome the demons that have plagued him and the circumstances that hindered his success from an early age.

“Stability takes a lot of courage,” Totalik said as he reflected on his harrowing journey to sobriety.

“There’s a lot of times I wish I could go back, but I look at where I am right now. It really changed my perspective on life. I’m pretty quiet most of the time. But [getting sober] gave me the resilience to come out of my own shell.”

He also mentions a close friendship forged with an Inuk woman he met while in an Alberta treatment centre. She’s an Iqaluit resident who continues to aid him as a lifeline of support.

His advice to others who are struggling is to “reach out when you need help because there is someone who will actually listen.”

In five years time, Totalik can contest his guardianship. His ultimate goal is “freedom in the future for me, which I don’t have right now.”

He also contends that in his ideal world, “I’d definitely be living my own life, doing my own thing at this age” — free of the many constraints of guardianship that affect all aspects of his life.

While Totalik wishes to be a cautionary tale to others, he’s also an example of someone who can, despite adverse circumstances, overcome what life has thrown his way.

‘A ray of hope’ Nunavut’s Health minister expresses optimism over Aqqusariaq treatment centre and other initiatives

“Mental health and addictions pose huge challenges to many Nunavummiut throughout their lives,” Health Minister John Main said during the fall sitting of the legislative assembly.

A repeated thread of discussion for MLAs during the late October/early November legislative sessions revolved around strategies for combatting these stigmatized illnesses that continue to plague communities.

There is “a ray of hope” on the horizon, according to Main, as construction began in August on Aqqusariaq, or the $83.7 million Nunavut Recovery Centre.

“This facility will offer in-territory addictions and trauma treatment based on Inuit language and culture,” said the Health minister.

“The project is a trilateral partnership of Nunavut Tunngavik Incorporated, Indigenous Services Canada and the Government of Nunavut Department of Health.”

From Page A10 The name Aqqusariaq describes “a trail or pathway used to reach a destination and is highly symbolic of a recovery journey. The continuing tripartite investments and commitments being made to the development of Aqqusariaq represents the strengthening of in-territory mental wellness and substance abuse resources, part of an Inuit-designed, delivered and administrated addictions and trauma treatment system. Also, the need for in-territory addictions and trauma treatment infrastructure is being addressed in a meaningful, Inuit-focused and collaborative manner,” said Main.

“The establishment of Aqqusariaq will enable those seeking mental health and addictions support to access treatment within Nunavut, reducing the need to travel south for care,” he added. “It will serve as a hub for Inuit-led treatment opportunities that are trauma-informed, culturally sensitive, and closer to home. The facility will host a range of activities, including on-the-land healing camps, allowing Nunavummiut to reconnect with the land and traditional Inuit practices.

By combining evidence-based treatments with culturally grounded initiatives, we plan to support healing and empowerment.”

With construction currently underway at the Aqqusariaq location, “substantial completion” is planned for December 2025.

“As work at site progresses,” said Main, “the Aqqusariaq Development Team is building the programs… with a group of Nunavummiut subject matter experts. Health remains steadfast in our commitment to fostering a nurturing environment for mental health and addictions care… [the program] also includes on-the-land treatment in all three regions of Nunavut, and it also includes work to support developing a mental health workforce, particularly looking at getting more Inuit into the field.”

Housing for Aqqusariaq staff is being looked into by the Department of Health in consultation with the Nunavut Housing Corporation, and Main committed to provide updates on this issue so that “that the lack of housing is not a barrier to recruiting and hiring staff for the facility.”

Coming up on Addictions Awareness week, Nov. 19-25, Iqaluit-Manirajak MLA Adam Lightstone dove into discussion on the matter with Health Minister John Main during the Nov. 9 session of the legislative assembly.

three facilities to provide mental health and addictions services. My question is: what three facilities are being renovated and where?”

“Currently,” Main responded, “we have work on the books for a facility in Arviat that would be a surplussed building that’s being brought forward for consideration or moving towards converting it into programming space. There is some work in Iqaluit. I believe it’s a Greenstone Building… the Rankin Inlet Wellness Centre repairs, that’s the third one. For all those projects, they’re existing assets that are either being repurposed or upgraded.”

Iqaluit-Manirajak MLA Adam Lightstone shifted the discussion to developments in expanding education and training programs that provide Nunavummiut “with the necessary qualifications for professional and paraprofessional work in health care, mental health and addictions treatment.”

“There has been progress on a number of different fronts,” Main replied. “When it comes to our work with Nunavut Arctic College, they’re a valued partner. We’ve had the nursing program here in Iqaluit for a number of years. For the first time, Nunavut Arctic College is delivering a licensed practical nursing course in Rankin

Inlet. That’s exciting to see.

“We finished developing the personal support worker program with Nunavut Arctic College and that was piloted and delivered to students in four communities. Now we’re working with Nunavut Arctic College to see the first in-person delivery of that course and that’s more specifically looking to Rankin Inlet for that.

I’m happy to see that the college has secured funding for the next couple of years to deliver that program. It’s going to be important when we’re looking at needs around long-term care,” Main added, addressing another long-standing issue: the lack of Elder-care facilities in the territory.

“There are a number of different initiatives within the Department of Health that are also meant to strengthen our workforce; I’ll just mention two,” Main added. “The mental health paraprofessional positions that we have created where the positions are laddered, so there are four different positions and those are targeted specifically for Nunavut Inuit. I believe… 27 out of 31 paraprofessionals currently are Inuit and it’s really exciting because those paraprofessionals support the mental health professionals at the community level.”

“My questions today,” said Lightstone, “are regarding the complex treatment needs of those suffering with mental health and addictions. Mr. Speaker, as I mentioned on the first day of the fall sitting, there is a growing concern in Iqaluit about the state of public safety in the community. It’s apparent that we need a strategy to both improve public safety and address complex treatment of needs of those who are suffering from mental health and addictions.”

Lightstone then asked Main whether there have been any significant developments on this front.

The minister thanked Lightstone for raising “questions about mental health and the fact that the most vulnerable need help, on their behalf… in terms of the Iqaluit mental health and addictions services, there is a comprehensive suite of services available.

The mental health and addictions team provides assessment, crisis intervention, stabilization services, outreach support, and psychotherapeutic services.”

Main expounded on the networking that takes place.

“Some of them work just across the street at the Greenstone Building and they are spread in different facilities in Iqaluit. They do work on establishing connections with different organizations in the community and it’s those connections which help to build partnerships, which in the end serve residents of this fine city.”

Lightstone spoke about the link between addictions and mental health and crimes committed in the territory.

“Studies have shown that a majority of crime and violence is committed by a small number of persistent offenders,” he said.

“Many individuals contributing to the growing concerns of public safety in Iqaluit likely have regular interaction with the RCMP and may also be regular visitors of RCMP holding cells.”

He then asked Main to explain the direct interaction between mental health staff and the RCMP, and how this collaboration will result in better community outreach. Main responded that he was unable to provide detailed information on the relationship between the two groups due to a lack of data, but elaborated on the outreach work currently in place in the city.

“The mental health and addictions team works with the Department of Family Services, they work the men’s and women’s shelter, they work with the Elders’ home, the YMCA, the Umingmak Centre, and also with corrections facilities. Mr. Speaker, what does this look like? It means things like drop-in counselling sessions at the schools, and also an after-school program. It also means a weekly drop-in that’s offered at the Uquutaq Society, and there’s a weekly program at the Qimaavik Shelter in Apex. There is so much excellent work happening in terms of all of our partners. It’s really encouraging to see all the different organizations contributing to the needs and to serving people who need help.”

Lightstone responded that he was encouraged to hear about “a lot of proactive measures being incorporated there, but one area that the minister hasn’t mentioned was interaction between Health and the RCMP. If these discussions have not happened, I would like to ask the minister to commit to initiating these discussions between health officials and the RCMP to allow mental health staff to visit our RCMP holding cells to inform individuals there of what programs and services are available, should they make the choice to get there… and just be aware of the options and more importantly, that it is possible.”

Main reiterated that the Department of Health currently works “with the correctional staff, and we go over (to) their holding cells or the correctional facilities.”

He pledged to consult with Premier P.J. Akeeagok on Lightstone’s concerns and suggestion, and said he will return to the legislative assembly with more information and will “discuss the details” at a later time.

Health Canada supports communitybased organizations to address harms of substance abuse Additional $21M announced in October

The toxic drug and overdose crisis has touched every community nationwide and, as such, has been deemed a national public health crisis. Health Canada is pledging more than $21 million toward prevention, harm reduction, recovery, treatment, and enforcement.

Ya’ara Saks, minister of Mental Health and Addiction and associate minister of Health, in late October announced more than $21 million in federal funding for 52 projects through Health Canada’s Substance Use and Addiction Program. The budget is focused on improving health outcomes for people who are at risk of experiencing substance-related harm.

Since 2017, over $500 million has been committed through Health Canada’s Substance Use and Addictions Program and has benefitted more than 380 projects nationally, including Indigenous communities and low-income and low-education populations. Through new investments announced in the 2023 Federal Budget, the Government of Canada also proposes over $359 million over five years to support a renewed Canadian Drugs and Substances Strategy, which will continue to inform how care is provided to those at risk.

The Canadian Drugs and Substances Strategy launched in 2016 was also renewed and builds on the work done in previous iterations, including the Expert Task Force on Substance Use. The federal government is using an integrated approach to address the overdose crisis and other substance use-related harms by supporting community-based programs to help reduce stigma and meet the needs of those who are affected by getting the services and support when they need them.

“As a harm reduction nurse, I understand the incredible complexities of this devastating crisis. There are multifaceted ways to approach it. Compassion lifesaving interventions are not up for debate. I have seen firsthand the lifesaving impact of community-based initiatives, and we must listen to the perspective of those with lived and living experience because they know best what is needed,” chief nursing officer of Canada, Dr. Leigh Chapman, said in an emailed statement.

Addiction is a treatable medical condition, not a moral failing — yet many people affected by addiction continue to face stigma and shame. The language that is used has direct and profound impacts. Public education around addictions is a critical facet of overdose and substance harm prevention.

Following up on questions from Aivilik MLA Solomon Malliki, Baker Lake MLA Craig Simailak asked the minister of environment whether the government agrees with the designation of barren-ground caribou as threatened under the Species at Risk Act. “As the minister is aware, the Nunavut Wildlife Management Board exchanged correspondence with the federal minister of environment last year concerning the federal government’s proposal to list barren-ground caribou as ‘threatened’ under the federal Species at Risk Act,” said Simailak in the legislative assembly Nov. 6.

“In his letter dated July 20, 2022, the acting chair of the Nunavut Wildlife Management Board indicated that the board had, and I quote, ‘approved the listing of barren-ground caribou as threatened under the federal Species at Risk Act, with the condition that recovery planning will be undertaken on a herd-by-herd basis.’”

Daniel Qavvik, minister of environment, responded that the Government of Nunavut does not support the listing of barren-ground caribou as threatened.

“I want to take this opportunity to pay tribute to the Kivalliq members of the Beverly and Qamanirjuaq Caribou Management Board,” said Simailak. “Can the minister indicate what discussions he and his department are currently having with the board concerning the Species at Risk listing?”

Qavvik replied that the Committee on the Sta-

tus of Endangered Wildlife in Canada assessed barren-ground caribou as threatened in 2016.

Environment and Climate Change Canada was given between January and October 2029 to complete consultations on the potential listing of barren-ground caribou with affected communities and provincial and territorial governments.

“Relevant co-management partners were permitted an extension due to additional needs to identify the consultation,” said Qavvik. “Furthermore, Mr. Speaker, the community consultations by Environment and Climate Change Canada were considered inadequate by some communities and additional consultations for the listing of barren-ground caribou started in 2020 and (were) completed by Environment and Climate Change Canada. These additional consultations were completed, and the consultation record was provided by the Nunavut Wildlife Management Board.”

Simailak’s final question asked for clarification on how the environment department would be supporting hunter and trapper organizations in the Kivalliq to participate in herd management and recovery plans.

“The listing of barren-ground caribou under the Species at Risk Act would result in the federally required recovery strategy and the identification of critical habitat,” replied Qavvik.

“Once critical habitat is identified, federal staff would work with its partners to find the best methods to protect that habitat from activities that would negatively impact the consultation recovery strategy.”

He went on to say the department would continue to communicate its position based on the best available science and Indigenous knowledge

Referencing a community-wide tuberculosis screening clinic in Pangnirtung this September, Arviat South MLA Joe Savikataaq asked Minister of Health John Main whether one would be taking place in Arviat.

Main responded in the house Nov. 6 that when the chief public health officer identifies or declares an outbreak in a community, “it is a trigger for additional resources and additional supports in order to address that outbreak.”

Elaborating, Main said the determination for a community-wide screening in Arviat would be made by clinical experts and come from the chief public health officer’s office.

“It is sad that there still is tuberculosis in Nunavut,” replied Savikataaq. “I think, if we don’t act on it and eliminate it – and it was supposed to be eliminated by 2013, as the federal government had proposed that – it is sad that we still have it.”

He said Nunavut Tunngavik Inc. is “holding a large sum of money” that is supposed to go toward eliminating tuberculosis within the territory.

He asked what the total amount given to NTI is and how much of those funds the Department of Health has accessed to date.

“In terms of the accounting for the funds that flowed to Nunavut Tunngavik, I’m not in a place to speak to what the balance of the fund is within Nunavut Tunngavik’s coffers,” replied Main.

“I respect their end of the fact that they are an independent organization from the Nunavut government and that’s why I can’t answer that.”

For Pangnirtung’s community-wide clinic, NTI spent approximately $2.5 million, said Main.

“We really do appreciate the ongoing support and collaboration with Nunavut Tunngavik specific to Pangnirtung but also with regard to other work on tuberculosis,” he said.

If the chief public health officer needs to make a declaration to get a community-wide screening, said Savikataaq, “can I ask the minister why the chief public health officer has been vacant for so long if that office is needed to make recommendations to battle tuberculosis within the territory?”

Main said said CPHO Dr. Sean Wachtel has been on leave “for some time now” and acting chief public health officers have been filling that role.

“A familiar and trusted face, Dr. Michael Patterson, was acting in that role,” said Main. “We also had Dr. Jasmine Pawa acting in that role.

I would like to reassure the member that at no point has the role been vacant. There have been several individuals filling that role on an acting basis to ensure that the important work through that office continues.”

The screening clinic in Pangnirtung began mid-September and involves community members being contacted by household to attend screening sessions that include a questionnaire, skin test, chest X-ray, sputum collection and possible blood work. In 2021, the Government of Nunavut declared a tuberculosis outbreak in the community.

The GN also announced a tuberculosis outbreak in Naujaat this May.

A call for transparency, communication and accountability

From: Joe Savikataaq Jr. Mayor, Hamlet of Arviat I am writing today to the candidates of president and vice president in advance of the Kivalliq Inuit Association elections regarding the issue of transparency, communication and accountability.

I believe when the Inuit organizations were created, it was to help the Inuit. Surely over the years that has occasionally occurred. To the candidates of president and vice-president, will you be transparent to the Inuit of Kivalliq by releasing salaries of the Kivalliq Inuit Association staff including all their benefits the same way our regular MLAs, ministers and premier have to?

It seems like there is no accountability other than when the elections are held every four years.

Communication should be a twoway street, not one way. When you get a question, a request, or a complaint, it should be responded to even if you cannot do what is being asked or unable to provide some advice, at minimum acknowledgement is better than not responding.

Every aspect of communication should come to a full circle. How will you communicate to the Inuit of Kivalliq?

News Briefs Inuksuk Drum Dancers to attend Newfoundland festival thanks to $3,000 RCMP donation

Drum dancers from Inuksuk High School were presented with a $3,000 cheque from Nunavut’s RCMP on Nov. 10.

The group, created in 2008 by music teacher Mary Piercey-Lewis, will use the funds to travel to this year’s HarbourVOICES! International Festival of Collective Singing Traditions being held in St. John’s, Nfld.

The festival takes place the week of June 29July 4, when the group of 24 students and six Inuit tradition-bearers will share “traditional and contemporary Inuit songs in Inuktitut, throat songs, and choreographed drum dances.”

“We are so proud of the Inuksuk Drum Dancers and their talents!” reads the RCMP Facebook announcement.

Government, union sign healthcare memorandum

The Government of Nunavut and the Nunavut Employees Union signed a new memorandum for accredited healthcare professionals in the territory Nov. 10.

The memorandum will allow the GN’s Department of Health to enhance access to quality healthcare for Nunavummiut by providing incentives to attract and retain highly-skilled and dedicated healthcare professionals to Nunavut, says a press release from the government.

Incentives in the MOU include: -Hourly wage premiums that prioritize difficult-to-recruit positions and communities facing staff shortages.

-Incentives to enhance staffing during high-demand time periods.

-Recruitment and retention bonuses that recognize multiple years of service.

-Financial assistance for Nunavut students’

loan forgiveness of up to 20 per cent.

-A return-of-service pilot program that offers tuition reimbursement in exchange for fulltime service within the territory.

“The healthcare workforce shortage is reaching critical levels that extend well beyond Nunavut,” stated Human Resources Minister Margaret Nakashuk. “This is why the Departments of Human Resources and Health are rising to the challenge with innovative solutions and incentives to attract and retain skilled workers. Our commitment to investing in the healthcare workforce is reflected in this agreement.”

The agreement will remain in effect until Aug. 1, 2025.

Canada, Nunavut launch early learning and childcare action plan

The Government of Canada and Government

of Nunavut announced a three-year plan to improve and expand early learning and childcare services in the territory Nov. 10.

The agreement is part of the Canada-Nunavut early learning and child care agreement for 2021-2026, which aims to strengthen the territory’s childcare sector and improve access to care for Nunavut families.

Over the course of the three-year plan, more than $50 million is outlined to be spent in five areas: affordability of child care, access to child care, inclusion, quality and administration and reporting.

“I am pleased that through this action plan, we’re not just making child care more affordable, but also laying the groundwork for a system that is inclusive and high-quality,” stated Education Minister Pamela Gross in the news release. “With rates at $10-a-day in licensed centres, we’re eliminating financial barriers for families across Nunavut.”

Though Health Minister John Main has cited in the past that the number of people who would require a medical boarding home in Rankin Inlet is low for the threshold of the government making such an investment, he did float the possibility in the future in responses to questions from Baker Lake MLA Craig Simailak.

“We are looking at the numbers and we are looking at how we can improve the system of people getting stuck in between their departure and their destination,” said Main, interpreted from Inuktitut, in the legislative assembly Nov. 2.

“We see them sleeping in the terminal, which is not very good to see.”

He said the government is leasing rooms from the hotels, which has helped, but it adds to the expenses as well.

“In terms of the prefeasibility numbers, as has been discussed in this house, they are low in terms of looking at a boarding home in Rankin Inlet and whether we need to move to the next step of a feasibility study,” said Main.

But looking at developments in the community such as the new long-term care facility, dental services and a birthing centre, Main said, “there are things that are moving us towards a feasibility study for looking at a boarding home in Rankin Inlet.”

Simailak responded that he thought it was time to make that happen.

“Far too many times we all have heard stories of people getting stuck in Rankin Inlet either because of weather in their home community, or the airline went mechanical while in Rankin Inlet,” said Simailak, asking when a feasibility study would be conducted.
