Ohana: An innovative model for youth mental health

n a world that is increasingly complex and stressful, our youth are shouldering the emotional toll. Pediatric behavioral health disorders have become more common during the past decade, now affecting as many as one in four children. The demand for pediatric mental health services has surged, overwhelming families and pediatric practices. This has resulted in long waitlists for dwindling resources, creating a crisis of access to quality support for the community at large.

At Ohana, we are building a program that expands resources and addresses this crisis in thoughtful and innovative ways. Over the past five years, we have grown from a small and intrepid team to a robust system of care with a campus that was thoughtfully designed to meet the needs of families in our community. We have met many families at a moment of great distress and have worked alongside each of them to identify the most effective ways to treat underlying illnesses while also expanding their strengths and well-being.

One of our goals over the past 18 months has been to reduce wait times for initial evaluation. It is not uncommon to hear about families waiting for months or even years for initial mental health appointments. Across the spectrum of pediatric mental health disorders, research has clearly demonstrated that early intervention reduces the severity and duration of symptoms and can lead to full remission faster. We are applying these findings to prioritize early diagnosis, enabling us to create a wellness plan, familybased psychoeducational services,

group treatment, and connection to appropriate community resources alongside recommended clinical treatments.

After evaluation, Ohana therapists offer empirically validated treatments that have been shown to address individual and family needs for the most common psychiatric problems of childhood, adolescence, and young adulthood. Our therapists are trained in cognitive behavioral therapy, dialectical behavioral therapy, dialogic therapy, and interpersonal

therapy, among others. We combine therapy with collaborative psychiatric care when deemed appropriate by our medical team. We also offer occupational therapy and psychological/ neuropsychological assessment. For youth and families with more acute needs, we offer partial hospitalization followed by intensive outpatient services which allow the child to receive full-day programming five days per week with a transition to after-school programming three days per week. This spring, we will open a residential treatment program that provides immersive treatment for patients showing dysregulation or distress that has made functioning in their current environment difficult.

We actively involve the family at all levels of care and coordinate with our community partners in caring for their patients. Hyperbaric oxygen therapy: An effective treatment for wound healing

The Wound Healing and Hyperbaric Medicine clinic at Community Hospital of the Monterey Peninsula is proud to offer hyperbaric oxygen (HBO) therapy treatments for a wide range of conditions identified in our community. HBO is a unique type of medical treatment where patients are placed into a clear pressurized chamber while breathing 100 percent oxygen which is delivered under increased atmospheric pressure. The treatments in general

are used for situations where there is compromised blood flow to an area of tissue. Common indications include diabetic foot wounds and radiation injuries as well as bone infections or problems involving acute loss of blood flow. There are more than 50 years of data on the efficacy and utility of these types of treatments for severe or nonhealing wounds. Medicare recognizes 15 different unique conditions that are reimbursed for HBO treatments and

other carriers have coverage for other more rare conditions, such as acute idiopathic blindness or hearing loss. In 2023 we performed 963 individual treatments (known as “dives”). 80 percent of the treatments were performed on a routine outpatient basis while the remaining 20 percent were hospitalized patients with acute conditions.

The treatments are performed in serial fashion. Treatments are generally 90 minutes and performed once daily during business hours. Patients are placed into a clear acrylic chamber on a soft bed. They may communicate via an intercom system where they can see and signal the technicians. The chambers are equipped with televisions and speakers for entertainment. Some indications such as acute healing problems after surgery are only treated for a few sessions, while other indications such as radiation injuries may be treated with as many as 60 treatments. The average patient receives four to eight weeks of treatment.

Insurance qualifying indications for treatment are highly specific and complex. The staff at the wound healing clinic are always available to discuss questions regarding this treatment modality with providers who may not be familiar with the regulations. We are happy to evaluate patients in consultation to help guide the process. The wound care clinic treats patients with wounds of all kinds and has a special focus on limb salvage. Our talented multidisciplinary team includes expert physicians from a wide variety of specialties, as well as RNs and wound technicians specifically certified in the treatment of wounds and hyperbaric medicine.

We are proud to announce a planned remodel this year and installation of two new larger state-of-the-art

hyperbaric chambers this summer. The new chambers provide greater flexibility in treating patients requiring acute inpatient care, enhanced safety profiles, and improved patient comfort during treatment.
