How to easily file a claim.

Follow these steps if you have a UnitedHealthcare Life Insurance, Short-term Disability, Long-term Disability, Accidental Protection, Critical Illness Protection or Hospital Indemnity Protection plan.

Steps to file a claim: 1 Notify your employer.

2 Access a claims packet on www.myuhcfp.com .

3 Complete, sign and date the forms.

How to access your claims packet.

1. Go to www.myuhcfp.com .

Note: You do not need to log in.

2. Select your claim forms.

4 Send completed forms to the contact listed.

After we’ve received all the completed paperwork, we will: Inform you by phone or letter that we are reviewing everything.

Ensure your claim receives a thorough, fair and objective evaluation.

Send benefit payments to you upon approval, if it applies.
