If your spouse is on your health plan, he or she must also complete the biometric screening, or your premium will increase.

How long will the biometric screening take? Please allow 15 to 20 minutes to complete the screening.

How do I prepare for the biometric screening? For the most accurate lab results, please do not eat anything for at least 12 hours before your screening appointment. Do not drink anything but water or black coffee 12 hours before your screening. You may take medications as prescribed.

Please note: If you completed or will complete a biometric screening through your physician between June 1, 2020, and Jan. 31, 2021, or if you are pregnant or one-year postpartum, you do not have to complete the biometric screening at the Health and Wellness Center. However, to avoid the premium increase, you will need to have your physician complete and fax the 2020 Physician Option Form, posted on the district website, to the Health and Wellness Center. The fax number is on the form.
