July 2022 Dear Garden Spot Community ~

Thank you for your willingness to partner with the Designation of Excellence (DoE) project team to field test some of the DoE manual contents and provide feedback so we can achieve the goal of offering a practical, accessible, and affordable process for achieving the Designation.

We understand that your participation at this stage of the project requires a commitment of time and resources, all of which are still in short supply for long-term care providers across the country. At our meeting scheduled for July 25, 2022, we will review this material, address your questions, and establish a meeting schedule for the months ahead.

The national project team is very busy analyzing data collected over the past 18 months and writing the person-centered care domain modules to field test with you and with other case study sites that will come on board over the course of the next few months.

Evidence Mapping The extensive scoping literature review with evidence mapping as conducted in Year One. Each PCC domain evidence was mapped according to whether the study was conceptual, operational, or had resident, family, or staff impact.

The next phase of mapping will include modifying the circles to represent studied practice standards and relative size based on importance to the Designation. Eventually graphic representations of each domain will be included in the Designation Manual that applicants will complete to achieve the Designation.

The Food and Dining Module You have agreed to review the Food and Dining module of the Designation guide, along with the introductory material and scoring process. We will provide the module content, the self-study questions, and the scoring sheet at our next meeting.

The Designation Process The project team has begun high-level thinking about the DoE process for Designation applicants. The graphic on the opposite page represents one possible trajectory that may take applicants about one year from start to finish.

These six steps represent the primary stages of Designation achievement. The process may take longer depending on an eligibility - or “readiness” - process before the start of the application process. The details of the Designation achievement standards, structure, and process will be developed throughout Year Two with input from expert panelists and from a select group of case study sites.

DoE Manual Concept The project team is developing Designation application “modules” that correspond to each person-centered care domain. You will review and provide feedback on one module. At a future time, we can share with you the remaining six modules.

We have included a few screenshots here but will provide an overview of all of the Food and Dining module at our next meeting.

Each PCC domain (e.g., Personhood) is represented in the DoE Application Manual as a module. The modules are further divided according to the components of the domain.

For instance, the scientific literature and our project data suggest that Personhood has several components, including Life Story and Narrative; Autonomy and SelfDetermination; Continuity of Self.

Each of these Personhood components have a section of the module with an explanation of the concept, based on the scientific literature to date; explanations of stakeholder views; self-study questions to help you consider how each component manifests in organizational culture, policies, practices, and training; and links to relevant resources and practices.

Each component has a self-scoring sheet and will eventually house a place to upload documents that demonstrate the score, and a verification plan for an on-site visit.
