APDD – Adults with Physical & Developmental Disabilities: For individuals 21 and over who experience both physical & developmental disabilities.

CCMC - Children with Complex Medical Conditions: Serves individuals from birth to the last day of their 21st year.

IDD – Intellectual & Developmental Disabilities: Serves all ages; must have a formal IDD/DD diagnosis prior to age 21
