Useful information for playschemes and play activity weeks Only fill in the form on page 9 if you are booking a playscheme or play activity week. All the other summer activities can be booked through our leisure centres. Play leaders We employ and train our play leaders and play rangers. We also carry out criminal disclosure checks on all our employees who work with children. There will be someone trained in first aid at each site. What will your child need at the playscheme or activity week?

.. .. . .

Packed lunch and a plastic bottle of water T-shirt for messy activities Sun hat and a long-sleeved top for sun protection Sun cream labelled with child’s name Appropriate footwear and clothing - casual clothes as playing can get messy and active! Prescribed medication - you will need to complete a daily permission slip, available at the venue

Can my child take any personal items? We cannot take responsibility for any personal belongings, such as iPods, electronic games and mobile phones, and ask that children do not bring them. What happens if we arrive late? If your child is going to arrive at a playscheme session after 10.15am, please tell the play leader the day before, or contact the play development team before 10.15am on the day by calling 01543 308768.

Cancellation policy for playschemes and play activity weeks When booking a place on any of our playschemes or activity weeks, it is your responsibility to make sure that they are suitable for your child’s needs. If you cancel your child’s place on or before 9 July 2011, we will refund your money by cheque, less a £10 administration fee. You can transfer your child’s place up to 9 July, but only if the alternative place is available. We cannot offer any refunds if you cancel your child’s place after 10 July 2011. This is because Lichfield District Council subsidises each playscheme and play activity, and it is unlikely that the place can be re-sold after this date. How to book playschemes or activity weeks

.. . .

Booking starts on Monday 23 May. Complete a registration form for each child. You can either photocopy the form on page 9 or download one from www.lichfielddc.gov.uk/summerfun Include a stamped, self-addressed envelope with your booking form. If you are a member of Play on Prescription, please write your PoP card number on the bottom of the booking form.

Are playscheme places available on the day? You are welcome to come along on the day to see if there are any places available. If the playscheme is full, but a child has not arrived by 10.15am, you can take their place. Can my child book with a friend? If you put the application forms in the same envelope, we will do our best to place them on the same playscheme. How can I book at the concessionary rate? 10% of our playscheme places can be booked at a concessionary rate. However, places are limited so make sure you book early to avoid disappointment.
