Message From the Chief Nurse Executive

Dear Colleagues:

It is with immense pride and gratitude that I present this year’s annual report, a testament to our shared commitment to propel Nursing Forward . As I reflect on our journey over the past year, I am humbled by all that our collective teams have accomplished.

We have celebrated numerous milestones in 2024 that elevate our standing as a world-leading health system and exhibited our contributions to the nursing profession. Our improved Press Ganey Your Voice Counts survey results reflect our dedication to staff well-being and quality care. The data clearly demonstrates substantial improvement across key areas of engagement, safety, resilience, and equity. We’ve seen a notable increase in sense of belonging, a stronger commitment to patient safety, and a more positive overall experience. This progress is a direct reflection of our nurses’ dedication, and the commitment of nursing leadership to act on their feedback.

Our leadership in nursing research and innovation through the Mount Sinai Center for Nursing Research and Innovation has been further reinforced by National Institutes of Health-awarded grants for pioneering nursing programs, as well as numerous publications and presentations at esteemed conferences. I am also proud to highlight the recognition of four Mount Sinai Emergency Departments with Emergency Nurses Association (ENA) Lantern Awards. Additionally, we have expanded our American Association of Critical Care Nurses (AACCN) Beacon Units, showcasing Mount Sinai’s commitment to nursing excellence, bringing our total to 12 units. The year 2024 also marked a successful Magnet re-designation for both The Mount Sinai Hospital and Mount Sinai Queens. Magnet Recognition is the gold standard for nursing excellence. The Mount Sinai Hospital has earned Magnet Recognition five times since 2004, a distinction that less than one percent of hospitals worldwide have attained.

These achievements are not just metrics; they reflect lives touched and communities transformed, and they would not be possible without a steadfast commitment to nursing excellence across all levels of nursing. Our patients, above all, are the primary benefactors of these accomplishments, and it is for them that we continue to strive for excellence. Edgar M. Cullman, Sr. Endowed Chair Department

of Nursing Installation

The Edgar M. Cullman, Sr. Chair of the Department of Nursing honors

Mr. Cullman’s admiration of the nursing profession , his dedication to improving the patient experience, and the Cullman family’s enduring connection to the Mount Sinai Health System.

The Edgar M. Cullman, Sr. Chair of the Department of Nursing was established in 2011 by the Cullman family in honor of the late Edgar M. Cullman, Sr. Mr. Cullman first joined the Board of Trustees of The Mount Sinai Hospital School of Nursing in 1950. He then became a member of the Mount Sinai Boards of Trustees in 1957 and served until his passing. In 2002, along with his brothers, Lewis Cullman and Joseph F. Cullman III, Edgar Cullman established the Joseph F. Cullman, Jr. Institute for Patient Care in honor of their late father.

The Edgar M. Cullman, Sr. Chair of the Department of Nursing honors Mr. Cullman’s admiration of the nursing profession, his dedication to improving the patient experience, and the Cullman family’s enduring connection to the Mount Sinai Health System. The installment of Beth Oliver, DNP, RN, FAAN into this esteemed Chair of the Nursing Department was celebrated on November 21, 2024.

Recognized as one of Modern Healthcare’s 50 Most Influential Clinical Executives in 2024, Dr. Oliver , has a proven track record as a transformational leader in health care for more than 30 years. As the Senior Vice President of Cardiac Services and the Chief Nurse Executive for the Mount Sinai Health System, one of the largest in New York City, including hospitals and extensive community-based services, she has been instrumental in developing inter-professional care, facilitating models that prioritize evidence-based, patient-centered care with seamless transitions between health care systems and their affiliated communities. Her contributions have not only enhanced the patient experience emphasizing quality and safety but also contributed to the professional development of nurses and the overall reputation of Mount Sinai Health System.

Nurse Leader Academy (NLA) Updates

In 2024, the Nurse Leader Academy (NLA) was held in person for the first time, following feedback from previous cohorts who felt that in-person learning would enhance the program. Graduates shared that face-to-face sessions would allow them to learn in a focused environment, free from the distractions of their daily responsibilities, establish relationships with peer nurse leaders, and engage directly with subject matter experts. The spring and fall cohorts welcomed 30 new graduates who completed 18 courses over four full days of learning. Since 2020, 120 nurse leaders have graduated from NLA, further strengthening leadership across the Mount Sinai Health System.

This award is presented by the American Organization for Nursing Leadership (AONL) and is an honor that recognizes a nurse who has made significant contributions to health care policy at the local, state, or national level.

The Nursing Pathway Program is a collaboration with the Department of Nursing, Office for Diversity and Inclusion (ODI), 1199 SEIU, and the New York City Department of Education. In 2024 85 high school students were paired with nurse managers to observe patient care units and took part in group events including an ODI Learning from Leaders Session and visiting the Mount Sinai Phillips School of Nursing Simulation Lab.

2024 System Initiatives and Accomplishments

At the onset of 2024, three key focus areas were identified by the council:

1 Implement sustainability initiatives focused on reducing supply waste while allocating appropriate supplies to key patient care areas

• A weekly forum was established which included Supply Chain and Nursing team members to execute initiatives

• Conversion of patient repositioning supplies from Sage to Medline, and change to individual items rather than kits (complete)

• Establish patient criteria and educate teams on appropriate utilization of turning and positioning supplies

• Changes to unit par levels (Only ICUs, Stepdown and ED, remainder will have small volume available)

• Explore options to provide easy access to key supplies in/near patient room (in progress)

2 Increase utilization of virtual patient observation (VPO) resources

• Patient observation needs have steadily increased and VPO provides additional resources to meet this demand

• Sites have increased additional cameras to better accommodate certain patient populations (ED)

• Cameras have been moved from underutilized sites to other sites that have advocated for more VPO resources

• Additional focus will be needed to ensure better utilization of resources for patients meeting criteria

3 Improve alignment of resources to manage staffing gaps

• Increase utilization of traveler resources to offset high reliance on overtime

• Establish “Float Pool” travelers to mitigate staffing gaps

• Increase 2025 RN budget in key areas to improve compliance with both CBA and New York State staffing requirements

• Establish systemwide data resources to better track and monitor deficits in staffing as union complaints are filed (“Patient Care Team Staffing–Complaint Tracker”)

American Heart Association Heart Walk

The Wall Street Run & Heart Walk took place on May 16, with Mount Sinai Health System earning distinction as the top hospital fundraiser. Mount Sinai teams successfully raised more than $47,000 for the American Heart Association, securing the honorary cup to commemorate these achievements for another year. The Wall Street Run & Heart Walk raises funds for cardiovascular research, lifesaving CPR training, and equitable health opportunities. More than 250 walkers participated in the 2024 walk from Mount Sinai Health System.

On June 4, the Department of Emergency Medicine and the Emergency Nursing Team hosted the 2024 Emergency Department New Graduate RN Fellowship Graduation . Mount Sinai Health System celebrated more than 75 RNs, making up cohorts 5, 6, 7, that have been a part of the Mount Sinai Emergency Departments over the last two years. Speakers from each cohort and preceptors shared learnings, motivations, and how to move into the future. The 24-week fellowship program is preparing new graduates and recruits to transition into lifelong careers as emergency nurses.

Current State: Mount Sinai Queens ICU nursing education materials are decentralized—some live in PEAK and others in binder on the unit and some on paper hung on the walls as flyers.

Goal: To transition from paper to electronic for all ICU-specific education materials and develop a platform where materials and modules can be accessed just-in-time.

Project relating to Social Determinants of Health (SDOH)

Goal: incorporate SDOH into standard clinical practice in order to address the population they serve

• Nurses completed a secure, anonymous webbased pre-survey to assess knowledge with applying SOH principles into their care delivery

• Following the survey, nurses were requested to view a 15 minute educational video about SDOH

Solution Option: Teams Channel: A Teams channel can store education materials in multiple formats (PPT, Word, video, etc.). The channel can be accessed on the WOW carts, local workstations, and on work phones. Both access and materials would be managed by the local Nursing Education group.

Develop Electronic Charge Nurse Rounding Tool

Current State: Paper-based rounding tool that charge nurses use to assist with shift hand-off.

Goal: To create an electronic charge nurse rounding tool that incorporates data from EPIC and some free text items (eg. lines, drips, fall risk etc.).

Solution Option: Charge Nurse Rounding Tool in EPIC: Similar to the OB Rounding Tool, this would be a popup smart form that lives in a column within Patient Lists activity. The documentation would not go into the EMR. The data can be pre-populated with answers as much as possible from existing documentation or columns can be added which already display data for overview such as “Isolation/Infection” or “Active Foley.” Data can be extracted into a report, if needed.

• Post-survey analysis showed an increase  in knowledge and confidence regarding incorporation of SDOH into practice

TOCC Readmission Impact 2024 Q1-Q3

• TOCC: successful calls continue to result in lower readmission rates across priority condition cohorts.

• The percentage of patients contacted by the TOCC that were billed with a priority condition as a primary diagnosis increased from 42.2 percent to 2023 to 47.8 percent in Q1-Q3 2024

• Patients that fall under the category of “Other” include patients with Sepsis, C-Section, Very high Risk of Unplanned Readmission (RoUR) scores and patients that were flagged for a priority condition but ended up not being billed for that condition.
