What is possible when a multinational engineering company partners with educators to encourage innovative thought in young learners?

Abdine Lewis is the Program Coordinator for the Next Engineers program in Cincinnati and is a staff member of the University of Cincinnati.

Abdine manages the grant through the Office of Inclusive Excellence and Community Engagement, part of the College of Engineering and Applied Sciences.

Joe Allen is the Chief Diversity Officer for GE Aviation, located in Cincinnati.

When the GE-funded independent charitable organization (GE Foundation) wanted to give back to the communities in which they have facilities, they thought long and hard about how best to leverage community connections and resources for a long-term impact, benefiting both the community, and GE. The solution: partner with local academic institutions like the University of Cincinnati to build a hands-on engineering program.

I interviewed Abdine and Joe, and we talked about the available programs, the benefit to local communities and the long-term vision for Next Engineers.

Innovation in healthcare draws from diverse aspects of engineering, including optics, computer science, electrical and of course biomedical engineering. Innovation Express is proud to feature this important and impactful program in this issue, and you can be sure we will be hearing more from Next Engineers and their students at AHIS2022 and Innovation Theater.

Currently a few months into the first iteration of the Engineering Academy program, it is a big hit with the students!

Called “Next Engineers,” the program is currently active in four communities: Johannesburg (South Africa), Staffordshire (UK), and Greenville, South Carolina and Cincinnati, Ohio (USA). In Cincinnati, GE is investing $5 million over five years with the goal of reaching more than 3,500 local middle and high school students.

The Next Engineers program aims to increase the number of diverse innovators and leaders who will make what’s next for the world even better (from their website).

There are three aspects to the program: Engineering Discovery (13-14 yo), Engineering Camp (14-15 yo) and Engineering Academy (15-18 yo).

I had selected Erica McAfee’s podcast, Sisters in Loss as my pick for this issue before her story was recently featured on the BBC. As an introduction to her work and mission, I think the BBC video does a better job that I can do with words.

Watch Here Based on her own experience in the US Healthcare systema s a black woman suffering miscarriage, Erica turned her hurt, sense of justice and pain into a healing place for others like her. Sisters in Loss provides a safe place for sharing stories, grieving, healing and support.

Erica’s podcast features women on their fertility journey, women who have lost infants or miscarried and experts in the field of maternal-fetal medicine.

Additionally, Sisters in Loss provides data, advice, and recommendations, all backed by reliable and trusted sources.

Dr. Cooper is an internationally recognized health services researcher and medical educator at the Johns Hopkins University School of Medicine. She has developed interventions that enhance patient-physician communication, improve health outcomes and reduce healthcare disparities. She is a 2007 recipient of the prestigious MacArthur Foundation "Genius" Award. Dr. Cooper was recently appointed to the President’s Council of Advisors on Science and Technology (PCAST) by President Joe Biden.

We first met Ashley Barrow and 12yr old Aaron Bess while ramping up for AHIS2021 when they submitted an abstract for our competition. For those of you unfamiliar with what an abstract is, “Abstracts are short summaries of your research and, although the format may vary slightly depending on where they are being submitted, there are standard guidelines about what should be in an abstract. The purpose of an abstract is to provide readers a brief summary of your study so that they may determine if they want to learn more about the research. An abstract should use proper terminology but should also be geared toward a reader who may have only a cursory familiarity with the research area.” (University At Buffalo Dept. Of Medicine) Ashley and Aaron’s abstract submission for their app blew us away.

What is Re-Assist and why is it so important?

Re-Assist is the ultimate referral assistance management app for patients in transitions of care, case management, and vendors to offer educated choices to improve quality of life. We need apps like this in healthcare to fill in the gaps. Did you know that nearly 1 in 5 Medicare patients are readmitted to the hospital within 30 days of discharge? Transitional care helps. Re-Assist helps patients in transitions of care to be educated on services and select service providers based on clinical recommendations, insurance, and locations.

From a dream to reality, the road from AHIS to now.

Ashley, born and raised in the Cincinnati area, has assisted people being discharged from acute care facilities to transition home, or to a rehabilitation facility for ongoing care for more than 15 years. Her professional experience, as well as personal experiences, allowed her to clearly identify a gap in the process that was allowing patients to fall through the cracks.

The concept for the application, which allows patients to be involved in their care transition and choose a suitable path for maximum quality of life, was Ashleys, but she had little knowledge of how to design such an app. Here, her son Aaron, 11 years old at the time, stepped in and did the research for the app and actually developed the prototype and the app itself. Aaron has always been interested in electronics and with his family background in engineering assisting his mom with the app was a no-brainer.

Ashley and Aaron were invited to attend AHIS2021 after we read their abstract submission. They were able to impress everyone in attendance with their innovative app concept and their dedication to patient care.

We were so impressed that we actually presented Aaron with The Young Innovator of the Year Award, which he said has motivated him to be even more involved with finalizing the app and finding ways to help more people. After AHIS Ashley says that they’ve been welcomed into the Cincinnati ecosystem and have been met with nothing but support for Re-Assist. Along with support from Main Street Ventures, and the SoCap Cohort they have made ReAssist a reality. Re-Assist is now in development with the first phase to reach completion in June 2022.

ReAssist is a fine example of why Cincinnati is THE place to start a small healthcare-oriented business. Local support and experience are unbounded and freely shared if you know where to look for it!

Ashley and Aaron’s participation in AHIS2021 allowed them to showcase their idea, and be seen by the industry disruptors and Social Capital investors. Join us in August for AHIS2022 to access these same opportunities.

GLOBAL MOBILIZATION TO ELIMINATE HEALTH DISPARITIES

The Center for Closing the Health Gap is a community-based grassroots non-profit health organization with a clear mission to eliminate racial and ethnic health disparities in minority populations within the Greater Cincinnati Area. Since 2004, we have reached over 420,000 people via our innovative, tailored evidenced-based programming. Our approach is to engage and empower African Americans, Latinos, and white Appalachians to live healthier lives by developing solutions through Community Based Participatory Research (CBPR). We also initiate solutions at the policy level, and collaborate with health care organizations to develop and implement evidence-based interventions and innovative programs to deliver our mission.

Our work is based on the principle that the people most affected by health disparities must lead the movement. Our programming enables people, families to formulate their own interventions and monitor their progress. The Health Gap’s Grassroots Mobilization Model engages, advocates, and empowers community movements. Data from our evidenced-based initiatives and examination of proposed solutions to alleviate racial/health inequities are reviewed for publication to encourage reapplication. innovationexpress.org

Health disparities that have persisted in the United States since it was first identified by the Health & Human Services Secretary in 1985. Black and Brown people in Hamilton and the nation have higher rates of hypertension, asthma, diabetes, and co-morbidities of these chronic diseases, (all of which increase the risk for serious complications from COVID-19). The higher levels of chronic disease are linked to our structural and institutional systems, including housing discrimination, neighborhood proximity to toxic areas, food deserts, higher uninsured rates. These Social Determinants of Health factors negatively impact the physical and mental health of Black Americans. and the psychological distress of being marginalized, dealing with racial microaggressions and instances of overt racism.

All of our 15 initiatives are directed toward to one goal: improving health outcomes for our Black, Latinx and Appalachian communities.

We meet people where they are. Engaging with people in the community across ages and socioeconomic status.

We support communities by equipping people on how to live healthier lives by activating their individual agency.

We promote organizational collaboration to meet the vast needs of the marginalized by galvanizing gifts, resources and funding to collectively develop effective, long-term strategies.

The We Must Save Us Campaign

Hamilton County partnered with the Center for Closing the Health Gap in Greater Cincinnati to develop and execute the Mitigating the Impact of Covid-19 on Marginalized Populations Initiative that began in November 2020. Over 60 community partners joined to increase the awareness, build knowledge, and expand capacity to mitigate the impact of COVID-19 on Black and Brown populations in Hamilton County. The objective of this initiative was to increase awareness, build knowledge, and expand capacity to mitigate the impact of COVID-19 on Black and Brown populations. This initiative included primary quantitative and qualitative research, development of the call to action We Must Save Us campaign, community support groups and outcome tracking. Our work continues with the commitment of getting more Greater Cincinnatians vaccinated.

ver the past two decades, Harvard professor Clayton M. Christensen’s concept of disruptive technology has become a staple of business innovation. A disruptive technology typically fundamentally alters the way a market segment operates. Some examples of disruptive technology are Moneyball; where math or data analytics fundamentally changed the way managers made decisions concerning baseball rosters and performance, or digital photography where digitization technology was made available to the masses on our phones.

The ability of companies to recognize when they have reached the peak of a market for a specific product and then find and incorporate newly developed technologies into their services has been at the core of many of the world’s largest corporation’s failure and success stories. Many are familiar with the Blockbuster and Netflix story, a prime example of one company’s failure to recognize and accept a disruptive technology entering their field. Netflix eventually passed Blockbuster with Blockbuster now being all but out of business, Netflix disrupted the home movie market to Blockbuster’s demise.

A common characteristic of a disruptive technology to a market is that the technology takes two disciplines, combines them in a new way and applies that new technology to a market to disrupt their operations.

Consider the example of Blockbuster who was the leader of video rentals for home entertainment. Netflix enters the home movie entertainment market but with a new technology; using the current home entertainment systems (TVs and screens) in combination with the internet and computer. Now you have home video rentals or purchases without the need for the VHS or DVD. Two technologies merged (home entertainment and the internet) to disrupt a market.

INNOVATION EXPRESS MAGAZINE In the world of sports medicine, traumatic brain injury (TBI) management has been an incredibly hot topic with millions of dollars poured into TBI prevention, management, and recovery research. As a result, the world of sports has witnessed TBI management change drastically over the past decade.

Specifically, Inneuractive has recently brought to the sports medicine market a form of training, developed and performed at the University of Cincinnati over the past 12 years known as NeuroVisual Training (NVT).

As a direct result of implementing this training, the University of Cincinnati has witnessed their TBI incidence rate decrease by 80% and the return to play time cut in half post-TBI. Inneuractive strives to change the way TBI’s are prevented, diagnosed and treated, especially for sports and athletes.

Gap in the market

Another market infamous for high TBI incidence is the Military. With over 1.3 million service members, the military has identified TBI as the signature injury in modern warfare and a primary health care mission. Most of their emphasis has been on treating the injured war fighter.

However, after identifying the University of Cincinnati’s concussion program as a flagship for concussion prevention, they have recognized the need for brain training and prevention of TBI to be added to their medical lexicon. There are programs underway to train US military service members in Inneuractive’s brand of NVT at multiple levels including service members at risk of TBI and the medical professionals taking care of them. What is NeuroVisual Training (NVT)?

NVT is somewhat analogous to the vision training eye doctors and vision training therapists might do for a young child with “weak eyes”. Also, NVT utilizes similar brain training that might be used by a therapist for a patient who has survived a stroke or severe brain injury. Now mix those two together and apply the new discipline of NVT to healthy people such as elite athletes or special forcestrained military service members. The same training that treats someone with a deficiency (weak eye or stroke deficit) can take a young healthy person and improve their already superior performance.

Prior to Inneractive’s philosophy of identifying room for improvement with our proprietary baseline, it would have seemed unnecessary to try to improve someone’s saccadic eye movement speed when they were at the 85th percentile without training. Noting that everyone has room for improvement, Inneuractive’s training could get that person to the 95th percentile. Small improvements in combat or sports skills for a high performing individual can be extremely important.

This changes the paradigm of how we currently approach preparedness and performance.

Claiming and proving that already high functioning individuals can still improve their craft with NVT is a concept that is disrupting sports performance and combat readiness disciplines.

While many aspects of NVT have been practiced for over 50 years, how we execute the training is novel. When military service members and their commanders came to the University of Cincinnati to observe our NVT program one of the members in their command structure had an important observation. Paraphrasing, he said that

This is a sage observation. The person making this observation was keenly interested in improving the pass rate of specialty-trained service members and immediately saw that the brain training basis of NVT would be able to aid him in improving the pass rate of his special forces trained recruits.

…when we first talked, I was thinking of a big machine with wires connected to a person’s head. I was surprised at the simplicity of many focused things coming together. There is a lot of elegance in the simplicity.

His interest quickly expanded to the combat readiness of his service members and their overall brain health. In today’s US military, there are specialized groups of service members who have a “heavy duty cycle” where they are exposed to intense and dangerous situations requiring very specialized training. These special operators are often very high functioning, physically and cognitively, and are ideally suited to engaging in Inneuractive’s NVT because there is still room for improvement for them despite their already high function.

The military is gravitating toward incorporating NVT into their training programs, especially with Special Forces Trained service members with a heavy duty cycle, because NVT has a large upside for little time investment. NVT can increase the pass-rate of some specialty trained military forces, it can increase retention of currently trained forces, aid in recovery post TBI and it can improve combat effectiveness at essentially all levels. All this can come from a structured training program using Inneuractive’s proprietary training modalities for 15 minutes at a time, three times per week.

Scanning like this can be very important for numerous sports and military service members. Using charts of alphanumerics called Hart Charts, Inneuarctive added a brain component to this task where the subject would spell out words and phrases with the alphanumeric and be tasked with recalling the words. Thus, adding a memory task to eye scanning training. Another proprietary modality that Inneuractive uses is decision making when looking or scanning far to near.

Picture a baseball batter who must track a pitch coming towards them, scan far to near, and make a decision to swing or not. So, decision making with far to near vision tasks is a tool that Inneuractive utilizes call Speed of Accommodation and Processing. This is a task and discipline important for many sports and many special forces trained military service members.

The distillation of the novelty and disruptive influence that Inneuractive’s NVT technology has is: 1. Standard techniques from eye doctors, 2. Standard techniques from neuro rehab specialists and 3, combine 1 and 2 into a new paradigm to improve performance of already high performing athletes and military service members, thus preventing head injuries, and aiding in recovery.

Who are Inneuractive?

Three people from Inneuractive (Inneuractive.com) have written this article concerning the innovative and disruptive technology of NeuroVisual Training.

Jon Vincent is the President of Inneuractive and former University of Cincinnati football player. Jon experienced Dr. Clark's NVT and founded Inneuractive because of its success and commercial potential. Currently Jon is a medical student in UK's MD/Ph.D. program and plans to continue working on disruptive technologies.

Dr. Joe Clark is the sports concussion specialist for the University of Cincinnati and currently the Chief Science Officer at Inneuractive. Dr. Clark's specialty is brain injury and stroke with an added emphasis on sports performance enhancement as he is also a Certified Athletic Trainer.

Blake Bacevich was a UC football player and has been accepted to Harvard Medical School. Blake has a strong interest in sport performance and joined Inneuractive as a vision trainer and outreach.

HOW DO WE DEFINE DISPARITIES? According to the agency for Healthcare Research and Quality, “Healthcare disparities are differences in access to or availability of medical facilities and services and variation in rates of disease occurrence and disabilities between population groups defined by socioeconomic characteristics such as age, ethnicity, economic resources, or gender and populations identified geographically.”

Why do we need a clear definition?

Health disparities, inequities, inequalities and. burden are slightly different, conceptually. Definitions are helpful in making sure resources are appropriately directed and to give structure to policies and guidance. A publicly available CDC article does a great job of explaining the differences and applications to research and policy.

Simply identifying that these disparities occur and the populations and diseases where the health gaps are most prevalent is merely the tip of a large and complex iceberg. Why are certain groups disproportionately burdened with healthcare issues but yet remain underrepresented in both pharmaceutical and device clinical trials?

There are recognized biological and anatomical differences among populations across the lifespan, people of diverse ethnic and racial backgrounds and genders.

While race is, in the US, not a reliable proxy for genetics, differences are evident. A study of FDA-approved medical devices showed that despite FDA recommendations there was no sufficient sub-group analysis to support approval in all adults(8). The FDA themselves reported that between 2008 and 2013, 20% of approved molecular therapies had differential efficacy or response across racial or ethnic groups, or both8. Bear in mind this does not include the hundreds of drugs and devices that were not approved!

The fact is, the population of patients that enroll in clinical trials almost never reflects the population that could most benefit the drug, process or device being trialed(6). Why not? At time of press, the US population is approximately 13% African-American and 18% Latinx, and by 2045, it is projected that non-whites will constitute 50% of the US population(9): this is clearly an issue that must be addressed.

What are the implications?

Testing a drug or device in a population that is relatively homogenous (all white, or all male for instance) can skew data and result in conclusions that are not, in fact, generalizable to the populations who will be actually using them. This is not only a scientific and medical issue, but also one of ethics and morals; excluding a population from a trial means that those people will benefit less if the drug/ device is approved, or possibly even be harmed.

What is being done?

I don’t think anything we have said here is news to you, but it may surprise you to learn that efforts by the major Federal Research Funding agency (National Institutes of Health) to ensure maximum equity and diversity, or at least to match the local demographic of the population being served, was not mandated as a review criterion until 2017(10), although guidelines suggesting appropriate inclusion of women and minorities were distributed in 1993(6).

This problem does not seem to be resolving as fast as policy-makers and patients would like, despite executive orders coming from the President himself(11). Issued on his first day in office, the executive order states

Because advancing equity requires a systematic approach to embedding fairness in decisionmaking processes, executive departments and agencies (agencies) must recognize and work to redress inequities in their policies and programs that serve as barriers to equal opportunity -President Joe Biden

It seems the best approach is to actually reach out to the communities where underserved populations are located and talk to them in their own cultural context to find out why patients who are asked to be a part of a study are refusing.

Solutions?

As we mentioned above, it is going to take more than increasing clinical trial enrollment to make all the changes that seem necessary (12,13). Several experts suggest a community-based approach. This has been tremendously successful in some specific cases, such as ischemic stroke(14). Engaging the community healthcare providers, religious and cultural leaders and trusted members in raising awareness and providing information in a form that can be easily understood is key to overcoming barriers of fear and stigma.

When I say the Cincinnati Tri-state area doesn’t get the recognition that it deserves in regards to innovators, particularly in healthcare, I am talking about innovators like Peeyush Shivastava.

Several years ago a Mason High School graduate, Peeyush sat at a bonfire with a bunch of friends and created a company based off of thoughts in his mind, with only a name and the drawings in his notepad.

This was the start of an innovation that will change how medical imaging is done. He knew that somehow he could measure the signal that comes from the chest naturally, break it back down to its individual cells that gave life to that signal and he would be able to use that information to predict heart attacks or strokes before it became systematic. Most people don’t question science but Peeyush, a self appointed nerd, always thought deeper than the average person. Peeyush knew the answer to what his future company would be able to do before he knew how he was going to do it or how successful it would be.
