Horizon Behavioral Health is the premier provider of mental health, substance use and intellectual disability services in Central Virginia. Horizon's mission is to support and promote the health, independence and self-worth of individuals and families in Central Virginia by providing a continuum of communitybased treatment, prevention, early intervention, and aftercare services for persons affected by mental health, intellectual disabilities, substance use and co-occurring disorders. As we reflect upon the past two years, Horizon has experienced many changes and challenges, not the least of which has been the Covid-19 Pandemic. Our Chief Executive Officer, Melissa Lucy, our Board of Directors, and our dedicated, professional staff, have met every challenge quickly and successfully, without interruption of services. One of the many ways Horizon continues to strengthen our service options has been to adjust to client preferences and needs by offering Telehealth as well as in-person service options.

The Multisystemic Therapy (MST) Program
serves youth and adolescents, ages 12-17, who have been introduced to the judicial system. The program empowers families and caregivers to assist and help deal with challenges their youth or adolescent may be experiencing by improving relationships they develop in schools, in their neighborhood, and at home.

The Too Good For Drugs™ Program
is an evidence based curriculum designed to promote life skills, character values, resistance skills to negative peer influence, and resistance to the use of illegal drugs, alcohol, and tobacco. This curriculum is taught in all 9th grade health classes in the City of Lynchburg, reaching approximately 582 students in the 2021-2022 school year.
Horizon was awarded the SAMHSA Grant to Benefit Homeless Individuals (GBHI) that provides outreach and engagement, screening and same-day assessment, treatment, case management, peer recovery support, and location of permanent housing, among other specialty services for homeless adolescents and young adults, ages 12 through 25.
The Assertive Community Treatment
(ACT) Program bridges the life-skills gap for adults that live with severe and persistent mental illness by providing longterm needed treatment, rehabilitation, and support services. This year, Horizon expanded this program with a community based team in Bedford County, providing nearly 100% of ACT services in the community or clients' home.
The Office Based Addiction Treatment (OBAT) offers medication assisted treatment for individuals diagnosed with a substance use disorder. This year, the OBAT program expanded it’s capacity by 50% by adding an additional provider, medical assistant, and care coordinator to the team, now serving up to 60 individuals at one time.

In May, Horizon opened a Project LINK site in the Lynchburg area. This program is now serving pregnant and parenting women, as well as their families, who are “at risk” or are diagnosed with a substance use disorder by providing intensive outreach case management, home visitation and support services with the goal of keeping the family healthy and together.

Horizon operates seven Intermediate Care Facilities (ICFs) located throughout Lynchburg and the surrounding areas. Horizon’s ICF programs offer adults with a primary diagnosis of intellectual disability an environment in which the individual can not only live safely but excel with support from a highly qualified, interdisciplinary team of professionals. Our team works together to care for individuals with individualized treatment plans, daily living support, and continuous, direct care twentyfour (24) hours per day, with 24/7 oversight and on-call availability.

This year was made very special with a ribbon cutting ceremony to honor the life and legacy of David Crickenberger. David was a beloved resident at Horizon's Bowyer
Home who called Horizon home for the last 6 years, before his recent passing. In loving memory, his brother donated a beautiful gazebo to Horizon so that clients, their families, and staff alike, could enjoy the outdoors as much as David did. David will be forever remembered by our clients and staff each time they enjoy it's shade.

When I create these models, it's a means of escape and it shows others how I spend my time when I am by myself. Also, it's a very visible way of receiving positive feedback that is gratifying and rewarding."
Not long ago, a client entered Horizon's Office Based Addiction Treatment (OBAT) Program after being released from jail. Her goals were to stop using opioids and live independently. With the assistance of her OBAT case manager, she was able to get a job, an apartment, buy her first car, and regain custody of her son. Last year, she decided it was time to wean off suboxone and did so successfully with her provider's help. She is currently a manager at her job and doing well in her recovery.

Therapeutic Outpatient Services are designed to address behavioral and emotional concerns that individuals experience throughout their lifetime which may include depression, anxiety, post-traumatic stress, substance use disorders, and attention deficit disorder among others. Horizon offers specialized program options for clients across the lifespan and their families. In addition to the more traditional outpatient therapies, we offer community based outpatient options to meet the individual needs of our clients.

Autumn was enrolled in Horizon's MST program when she was 16 years old. Still coping with the recent loss of her mother, she turned to substances, afraid that nobody would truly understand everything she was going through. With help from the MST program, Autumn has worked hard to strengthen her relationship with her father. She says "The more he saw me improve, the better our relationship got.” She is now working full-time, having graduated a year early from High School, with plans to further her education when she turns 18. Autumn shares that "Because I was so young when I first started using, I never got to experience what was out there for me outside of drugs. Now that I have the freedom to go places and spend my hard-earned money on things I like to do, I just have a lot more hope that I can stay clean and build a life for myself."

Horizon's Prevention and Wellness Services implements a comprehensive array of evidence-based programs and strategies to prevent substance misuse and suicide while promoting mental wellness in Central Virginia. The focus is to provide education and resources through evidence-based programs, trainings, presentations, and awareness campaigns. An emphasis is placed on community collaboration and mobilization, enabling groups to work together to address identified community needs. Horizon also focuses efforts on suicide prevention and the creation of trauma-informed, resilient communities.

Horizon’s team of School Based Clinicians are available to provide outpatient therapy and case management services in 22 contracted schools in central Virginia. Services provided are evidence-based and focus on assisting K-12 students in meeting their mental health needs while remaining in their academic setting. Therapists and case managers work closely with parents, teachers and other school personnel to meet the needs of the student and provide ongoing support during and after the school day.

INFORMATION TECHNOLOGY ADVANCEMENTS
Over the last year, Horizon's Information Technology (IT) Department has successfully undertaken a multitude of projects to advance Horizon's technology, equipment, and systems. Recently, Horizon has adopted the Microsoft 365 software, designed to help our teams achieve more with innovative Office apps, intelligent cloud services, and world-class security. Additionally, the new Avaya IP Office (VOIP) Telephone System was installed across all Horizon locations, as well as the successful completion of the Dell Server Farm Migration Project and Barracuda cloud email security gateway and archiving system.
The systems development team has also made significant contributions to bolster the efficiency of clinical reporting operations. Some of these contributions include the installment and user training development for Credible Orders 2.0 and eLabs 2.0, improved processes for appointment scheduling and provider coordination within Credible, complex Credible configurations to support the addition of Horizon's new Wellness Centers, as well as advancements in telehealth consent procedures to support client preferences and the overall client experience. STEP-VA PUTTING OUR BEST FOOT FORWARD
The Commonwealth's statewide initiative, STEP-VA (System Transformation Excellence and Performance), is designed to improve community behavioral health services for all Virginians. Two years into the four-year implementation timeline for STEP-VA, Horizon is proud to report 100% of outpatient clinical staff have received eight hours of trauma informed training, and 2,473 evidence-based assessments were conducted for new clients.

LYNCHBURG DRUG COURT GRADUATION
This year, five brave individuals were awarded their graduation certificate on May 26th, 2022, from the Lynchburg Adult Drug Treatment Court (LADC). This was a memorable milestone, indeed, celebrated by graduates, their families, and program partners including the court system, local law enforcement, Virginia Department of Corrections, and the Virginia Department of Health. Congressman Bob Good, Delegate Wendell Walker, and Anna Powers of the Supreme Court, joined Horizon in sharing this triumphant moment with the program’s 2022 graduates.

In February, Horizon was proud to receive a federal grant from the Bureau of Justice Administration (BJA) to expand the Lynchburg Adults Drug Treatment Court (LADTC) over the next four years.
With expanded services and supports, the LADTC will be able to enroll an additional 15 participants per year for the four-year duration of the project.

COMMUNITY HEALTH WORKER INITIATIVE
In response to significant rises in mental health and substance use challenges during the pandemic, Horizon launched a six week summer outreach community health worker initiative that reached residents in underserved communities’ right at their doorstep to provide home wellness checks and care packages at no cost. The community health workers were trained to identify signs of mental health and substance use conditions and provide information on how to access food, housing, employment, financial assistance, legal and medical services as well as additional resources to help prevent potential crisis'. Nearly 2,000 homes were visited and 1,000 care packages delivered from July to September, 2021.
MST CLINICIAN RECEIVES SUSTAINED EXCELLENCE AWARD
This year, Doug Poe, Multisystemic Therapy (MST) Clinician at Horizon Behavioral Health, was awarded an MST Sustained Excellence Award (an ESSY) in the category of Therapist Adherence from MST Services, the purveyor of the Multisystemic Therapy model. The ESSY is awarded on an annual basis to therapists who rank in the top one percent among their peers based on feedback from the families about their experience with MST offered by the provider organization. Within his role at Horizon, Doug has demonstrated outstanding and meritorious service to the youth and families he serves. His dedication to those he worked with and served is highly valued by Horizon, MST Services and his community.

BEDFORD FAMILY TREATMENT DRUG COURT
The Family Treatment Drug Court (FTDC) is an evidencebased model designed to restore, reunite, and strengthen families to ensure optimal health and safety of all members. After the first year of implementation, Horizon achieved 100% of expected program enrollment, having celebrated 11 graduates and 5 family re-unifications. Recently, the team has expanded to include a Program Ambassador and Peer Support Specialist. Families are linked to vaccines, infectious disease testing, and an array treatment options offered by Horizon.
EXPANDING TREATMENT IN RURAL VIRGINIA
A Health Resources and Services Administration (HRSA) Grant has afforded Horizon the opportunity to extend outreach and treatment in rural, Central Virginia. The desired outcome is to improve the quality of life of individuals, 18 or older, who meet criteria for severe mental illness and have barriers to accessing Mental Health and Substance Use Disorder treatment. The goal is to engage 150 clients in Amherst and Bedford Counties, over a period of 4 years, to address rural health disparities. So far, 630 hours of Illness Management & Recovery Model (IMR) training has been provided to 30 project partners and 100% of enrollment was achieved prior to the end of year one.
BEDFORD PHARMACY GRAND OPENING
On Thursday, September 29th, Horizon Behavioral Health and Genoa Healthcare held a Ribbon Cutting Ceremony to commemorate the grand opening of the new on-site pharmacy located at Horizon's Bedford Wellness Center. In the last fiscal year, Horizon served nearly 2,500 residents at the Bedford Wellness Center who will now have access to this full-service pharmacy for themselves, as well as their families.
Through this partnership with Genoa Healthcare, Horizon now offers two on-site pharmacies to serve clients in central Virginia which have been shown to improve care coordination and medication adherence rates. At both locations combined, Genoa has filled nearly 50,000 prescriptions in the last year, serving approximately 750 patients each month.

RIDE OF OUR LIVES MOTORCYCLE RIDE & RALLY
In September, the community came together in a powerful way to rally around the importance of mental health and suicide awareness. Horizon partnered with the American Foundation for Suicide Prevention, Combat Veterans Motorcycle Association, The Christian Motorcycle Association, Harley Davidson of Lynchburg, and the Suicide Prevention Awareness (SPA) Coalition of Central Virginia to host an awareness ride to promote open dialogue and early conversations around mental illness, addiction and suicide.
