Editor’s Letter Hope and dreams have always been my companions. I believe in constant renewal as needed. What if? What if you changed your mind, your thoughts, your outlook, your focus, your self care, your daily practice of doing life all for the betterment of you – just you? Life is such an extraordinary gift and I am always amazed how the universe, God, your higher power steps in when you are doing you. You must believe in the greater good, the path of least resistance, that you have limitless possibilities and that it will all be okay. Over the past year, some extraordinary people, situations, and opportunities have been presented to me and to others that I know, love and respect. Oh, sure there have been challenges; life has kicked butt at times – it gave us a pause, made us weep, maybe made us take a turn. But because we believe in betterment--and in our wanderlust and in love and goodness--we trust that the universe has our back, and that there is good now and more good to be found. My motto, my mojo, my jam — To Move Forward with Purpose. In that I find my power, my thing, my self-love and my reason for being. What do you choose? Start with you!

Directors’ Letter Our bodies, minds, and lives perpetually move forward, leaving behind the memories, ravages, and burdens of the past. At the same time, they open up a new vista for renewal, and a fresh start. Like seasons, we go from hibernation to birth, from vitality to reflection. As long as we continually embrace forward movement, expanding to new horizons, our well-being will be intact. But if we’re wallowing and stuck, our spirit inevitably decays, rendering emotional discord and biological stagnation. Renewal is a vehicle that is loved by change and that your health gains from. Free yourself from spiraling into the abyss of negativity that is precipitated by inaction. Although our cultures elevate tradition to the highest level, it is new beginnings that fuel an inspired life. Every day we work with people who are releasing yesterday and welcoming tomorrow. In every case, when they become proficient in this glorious process, they achieve their desired goals. Early this morning, we sat with a woman who attended HHI’s transformation program eight years ago. She spoke about her struggle by remembering her childhood, her marriage, and her unhappy existence. Not long ago, I came to a realization that most of us, including me perpetually recycle thoughts that are not always helpful. Maybe it is our reptilian brains that imprint these minor and major traumas in our privy so that we attempt to prevent reliving the unpleasant experience. On the other hand, the magnitude of cluttered thoughts that most people harbor, stops us in our tracks from living a full, happy and productive life. First and foremost, renewal, is clearing the decks and allowing fresh and bright perspectives to become the guiding light.

Every cell in your body contains water and floats in water. Water transports nutrients to each cell and delivers these nutrients through cell membranes, while simultaneously removing the toxins that every cell creates. These toxins are then delivered to your kidneys and expelled from the body. Water delivers oxygen to each cell via red blood cells (which are mostly water) and removes carbon dioxide, which is then transported to the lungs to be expelled. Water is required for every bodily function: breathing, digestion, growth, movement, elimination of waste, heat dissipation, metabolism, secretion and all glandular functions. ey y’all! Yes.. you guessed it. I’m from Texas ha ha! My name is Kendall Dyal. I am a pain-free performance specialist, as well as the fitness academy manager here at Hippocrates Health Institute. I lead various group classes, including CampusFIT, Rebounding, and AquaFIT. I also train guests one-on-one with a more specialized approach to training to avoid injuries. As the fitness academy manager, I take part in helping guests lose their unwanted weight, as well as educating them about the plant-based lifestyle. I received my bachelor’s degree in exercise science from Schreiner University. Along with being a pain-free performance specialist, I am a former college basketball player, a certified personal trainer, a certified program design specialist, and I received my plant-based nutrition certification through Cornell University. I have a passion for fitness and living a healthy lifestyle. 1. MAKE SURE YOU DRINK ENOUGH WATER Dehydration can be a major factor in our performance & overall well-being. Staying hydrated can help with weight loss, brain function, flushing out waste, performance, regulating body temperature, etc. The body is made up of 70% water. We can live for weeks without food, but we can’t go more than 7 days without water. We need enough water on a daily basis to support our bodily functions. Water is just as important as the food we put into our body, as well as physical activity. Two ways you can assess your hydration include your level of thirst and checking your urine color. The darker your urine, the more dehydrated you are. The U.S. National Academies of Sciences, Engineering, and Medicine recommend 3.7 liters a day for men and 2.7 liters a day for women. I recommend investing in a water bottle or canteen to take with you wherever you go.

2. GET AT LEAST 8 HOURS OF SLEEP Sleep is an often neglected component in a person’s life, yet it is one of the most vital components in someone’s health. It’s just as important as healthy eating and exercise. Adequate sleep can help with appetite, weight management, productivity, performance, etc. Sleep allows the body to recover and repair before the next day. With individuals who don’t get enough sleep, studies have shown that 89% of children and 55% of adults are more likely to become obese. Neglected sleep has been shown to cause individuals to have a bigger appetite, have a decrease in performance levels, and individuals with very little sleep have an increased risk of developing heart disease. It’s important to get at least 7-8 hours of sleep daily.

3. FUEL YOUR BODY WITH THE RIGHT FOODS Think of your body like a car. If you don’t put the right fuel in the tank, it won’t run properly. Feeding your body with nutritious foods is important from not only a fitness standpoint, but for our overall health. It doesn’t matter what kind of shape you’re in; you cannot outwork a bad diet. It will catch up with you eventually. 4. ALWAYS WARM-UP AND COOL DOWN Most people hate the warm-up & cool down portions of a workout. People want to save time, get in, get out & go on about their day. Trust me... I get it. Yes, the actual workout is the main thing we’re focusing on; however, warming up and cooling down is just as important. Skipping these two portions can actually do more harm than good. The warm-up is essentially used to prepare our bodies for activity. We need to increase blood flow to the working muscles and allow our joints to move freely through a range of motion prior to the movement and strain you’re about to put on your body from the workout. We need our muscles to be as efficient as possible to take on the task. The cool down is needed to bring your heart rate and blood pressure back down to normal levels. Doing both portions can help with injury prevention and recovery, as well as reducing soreness. 5. LIFT WEIGHTS This tip is mainly for my ladies out there: toning is not a thing. The term “toning” is used to describe a type of look or physique individuals want to achieve. To get the “less flab, more definition” type look, you need to lift weights and lose body fat. If we don’t exercise and eat clean, nutritious food, body fat will most likely increase. The less we train our muscles, essentially, the smaller they will

get. Strength training and shedding the fat covering the muscles will help you achieve this look. I recommend a combination of resistance training to build muscle and cardio-based interval training to shed body fat.

6. HAVE A PLAN & STICK TO IT It can be intimidating walking into a gym if you’re new to fitness. Getting out of bed or getting to the gym after a long day at work is half the battle. Motivation comes and goes. Some days we don’t feel like working out, we’re too tired, etc. Staying disciplined and sticking to the plan is the key. There’s a difference between knowing when your body needs rest and lacking the motivation to get up off the couch. One thing to note: the time is going to pass anyway, so why not spend it trying to achieve your goals? Also, it does not matter where you are in your fitness journey; you just have to start. You can do anything you set your mind to. I think Nike said it best: “JUST DO IT.” 7. DON’T FOCUS TOO MUCH ON THE SCALE To piggyback off of bullet point #5, when training and going through an exercise program, don’t focus too much on the scale. The overall number may not change that much or may not look how you would like. Don’t worry. The body composition is the most important aspect to look at. The changes in your body fat percentage & muscle mass are far more important than your overall weight. We want to see a decrease in body fat and an increase in muscle mass. Remember, muscle weighs more than fat. I recommend using a scale that measures body composition as well, to see the underlying numbers that a normal scale doesn’t. If you tend to get discouraged or unmotivated because the number on the scale isn’t dropping, base it off of how you

By being more active we can sharpen our mental clarity, improve our physical health, and reduce the risk of developing chronic diseases. HAVE A BATTLE BUDDY Accountability is huge. Having a friend, family member, or even trainer being there by your side can help you stay motivated in your fitness journey. They keep you accountable during the hard times and celebrate with you when you reach a milestone. Research has shown

Be patient. Do workouts you will actually enjoy and stick with. Consistency is key. The best way to have consistency is to enjoy the journey. Set realistic short-term and long-term goals for yourself. Eventually, you’ll start to see changes. This will give you a sense of accomplishment and the motivation to continue to push harder.

Sleep Better Just about everyone wants to sleep better. If you have failed at resolving your sleep issue in the past, it is not your fault. There is a lot of information out there, and it can be confusing. Many times, that information overload keeps you from success. I understand how you feel. If you have been concerned in the past that you just cannot sleep well. I want to put these fears to rest. You can do this. You just need the right protocol. Big pharmaceutical companies and many medical professionals want you to think you need medications or other potions to fix your sleep. I am here to tell you they are wrong. They have their own reason for wanting you to think that. But that is not true. If you have ever thought that these powerful medical establishments want you to fail, you are right. They do not benefit from you succeeding. They want to keep you in need. The difference with Hippocrates Health Institute’s Sleep Program is that we care about you sleeping well and genuinely want to see you living a healthy and successful life. 10 STEPS TO BETTER SLEEP STARTING TONIGHT

STEP 1: FIX THE CIRCADIAN RHYTHM Circadian rhythm is a 24-hour cycle that is part of our body’s internal clock responsible for regulating our body’s system. If disrupted, it will be difficult to have good sleep. Fortunately, you can retrain your circadian rhythm. One of the easiest ways to do this is go to bed and get up at the same time daily, even on the weekends. And expose yourself to sunlight in the morning immediately after waking up to help keep your circadian rhythm healthy.

STEP 2: INCREASE SLEEP PRESSURE Sleep pressure is a biological process that makes our brains feel sleepy and more “pressured” to want to sleep the longer we are awake. Let us say, we are not tired enough yet to go to bed, then it is more likely we will lie in bed wide awake. The best way to increase sleep pressure is to avoid long naps during the daytime, even when you do not sleep well the previous night. STEP 3: MANAGE STRESS Undergoing stress activates the fight-orflight hormones that work against sleep. It is how our bodies respond to any important or dangerous situations. It can be from traumatic events, a toxic environment, or any disturbance to your daily routine and habits. Prolonged stress causes sleep deprivation and lack of sleep raises your stress until it becomes a vicious cycle. Some easy ways to counteract daily stress is through yoga, meditation, and physical activity.

STEP 4: DECREASE INFLAMMATION Inflammation which is the root cause of most health problems including Autoimmune disorders, Arthritis, Heart Disease, and Cancer can increase your body’s level of cortisol - a hormone that causes sleep disruption. And when your body lacks sleep, it can lead to more inflammation. The solution is to reduce inflammation through your diet. Stop eating gut irritants, such as gluten, sugar, dairy, and lectin. Consider eating a raw organic food diet. STEP 5: MENTAL HEALTH Sleep and mental health are intricately connected. Those with mental illness are more likely to have insomnia and other sleep disorders, and those sleep-deprived are prone to negative thinking and emotional vulnerability which lead to mental health problems like depression, anxiety, and even PTSD. For this, seek help from a professional. STEP 6: PROPER NUTRITION In today’s world, getting the proper nutrition from our diet is nearly impossible. Most people suffer from various nutritional deficiencies that can affect sleep quality, such as Vitamin D, B12, and Magnesium, just to name a few. Common blood tests can help determine what nutrients you are currently lacking. STEP 7: CHECK YOUR OXYGEN LEVELS WHEN YOU SLEEP Conditions like Obstructive Sleep Apnea (OSA) prevents good quality sleep. OSA is a condition in which breathing stops involuntarily for brief periods during your sleep. The flow of air stops because the airway space in the throat is too narrow. If you snore at night, consider using an overnight O2 monitor to determine if your oxygen levels fall when you sleep.

STEP 8: SLEEP TRACKING Tracking your sleep is a great way to analyzing how long you sleep, how much time you spend in each stage of sleep, and even making you aware of potential sleep issues like snoring and sleep apnea. STEP 9: COGNITIVE RECONDITIONING Cognitive behavioral therapy for insomnia (CBT-I) is a short, structured, and evidencebased approach to combating the symptoms of insomnia. CBT-I focuses on exploring the connection between the way we think, the things we do, and how we sleep. By identifying our thoughts, feelings, and behaviors that are contributing to the symptoms of insomnia, we can learn how to sleep better. STEP 10: SLEEP COACHING Most people simply do not make sleep a priority. Rarely does ignoring a problem help with its improvement. The fastest way to reach your goal of sleeping better is with someone supporting you. A good sleep coach can help monitor your sleep results, give you advice on the best things to improve your sleep, and make you accountable of doing what it takes to improve it. CONCLUSION: Sleep is essential to feel your best and the lack of it makes it impossible to achieve optimal health. It is crucial for everyone to master sleep and correct it. This is only a short list of ways to improve your sleep and you may need extra help. LEVY, MD “Since the mid-1970s, when I began my career, there has been a skyrocketing number of patients who are diagnosed as having some type of thyroid disorder,” says Dr. Elliot Levy, a thyroid specialist and Clinical Professor of Medicine at the University of Miami Miller School of Medicine.” On a sunny day at the Hippocrates Health Institute, Dr. Levy sat down with me to share some of his decades of experience and knowledge to help us understand common thyroid conditions. Hopefully, all of you will gain knowledge to understand those conditions and know which need treatment, which need further investigation, and which can just be followed by your doctor.

PLEASE TELL US ABOUT THE THYROID GLAND AND WHAT DOES IT DO? I would like to start with a little background about the thyroid gland. The thyroid gland is a butterfly-shaped organ located in the center of the neck, just below the “Adam’s Apple” or “cricoid cartilage.” Its sole purpose is to produce thyroid hormone. There are two thyroid hormones. Thyroxine (which is also called T4 because it has four iodine atoms in its structure) and Triiodothyronine (which is also called T3 because it contains three iodine atoms.) It is difficult to describe what the thyroid hormone does. The heart pumps blood around the body. The kidney excretes waste products into the urine. The lungs allow oxygen to enter the blood stream. But the thyroid does not have a specific function that is easy to understand. Thyroid hormone regulates body temperature in mice. It is responsible for the metamorphosis of a tadpole into a frog. But in humans, thyroid hormone is more of a permissive factor. It helps to regulate protein synthesis, and every cell in the body requires protein synthesis. We know what happens when there is too little thyroid hormone and when there is too much thyroid hormone, but when the amount of thyroid hormone is normal, things just run smoothly. The thyroid gland secretes predominately T4 which is converted in the liver and kidney into the active thyroid hormone, T3. Nearly all of the T3 comes from conversion of T4 into T3.

LET’S START WITH A VERY COMMON PROBLEM. WHAT EFFECT DOES ESTROGEN HAVE ON THE THYROID? The thyroid hormones, T4 and T3, do not travel in the blood stream alone. They are loosely bound to carrier proteins, the most common of which is Thyroxine Binding Globulin (TBG). So when a total T4 test is ordered by a doctor, the result measures all the T4 that is bound. Estrogen does not have any specific effect on the thyroid gland or its hormones but does effect those carrier proteins. Estrogen therapy, whether in the form of oral contraceptives or hormone replacement therapy for a post-menopausal woman, has a non-specific effect of increasing the levels of carrier proteins, including TBG. Since the amount of these carrier proteins rise when estrogen therapy is started, the total amount of T4 also rises. Since the total T4 test measures the total amount of thyroxine in the blood, including that which is bound as well as that which is free, estrogen therapy will cause an elevation in the levels of total T4. When your doctor needs to know the results of your thyroid hormone level to judge if your have enough or too much thyroid hormone in your blood stream, make sure that he or she orders a free T4 test, not a total T4 test. That test is very readily available. A more accurate way of diagnosing thyroid function levels (that is, too high or too low) is the TSH test which I will describe later. In summary, estrogen therapy has no effect on the thyroid gland, but only on the total T4 test.

WHEN DO THYROID PROBLEMS FIRST DEVELOP? Certainly, there are congenital thyroid conditions and the extremely rare thyroid cancers that can appear in childhood, but most common thyroid problems first appear during puberty. The only presentation might be a slightly enlarged and painless thyroid gland with completely normal levels of all thyroid hormones and TSH. Thyroid disease can be seen at all ages. There are no thyroid problems that only occur at a specific age, although it is felt that as people age, their levels of thyroid hormone tend to drift down, and mild hypothyroidism is very common in those over 65. Thyroid problems are also very common during pregnancy, and can be serious if not followed carefully. In some cases, there is a deficiency of iodine in a pregnant woman’s diet; other times auto-immune thyroid disease can occur (which will be discussed later) and are associated with either an underactive or even occasionally overactive production of thyroid hormone. Interestingly, many patients with auto-immune thyroid disease will have their thyroid condition go into remission during the last trimester of pregnancy. It is not known for sure why, but I think it is because of “immune tolerance” which occurs during pregnancy. Remember, a mother has a fetus inside her during her pregnancy which is composed of genetic material from her partner. In order to carry this fetus, the mother has to have her own immune system slow down. One specific thyroid problem called “hyperthyroidism” or “Graves’ disease”) is notorious for going into remission by the third trimester of a pregnancy, only to come back very strongly one to two months after delivery of the baby. The understanding of why this

happens and what to do about it should really be made by a thyroid specialist.

WHAT IS THE CAUSE OF MOST OF THE THYROID PROBLEMS? The cause of the most commonly seen thyroid conditions is an “auto-immune disorder” called “chronic lymphocytic thyroiditis,” also called “Hashimoto’s disease.” An auto-immune condition is caused by the body producing antibodies that are not normally there. These antibodies can cause painless inflammation in an organ which can result in destruction of the organ or inhibition of its producing the normal amount of hormone, in the case of the endocrine glands. Auto-immune problems are actually very common in our society and include such conditions as Type I Diabetes, Rheumatoid Arthritis, Psoriasis, Multiple Sclerosis, Adrenal Insufficiency (Addison’s disease), Systemic Lupus Erythematosus, Inflammatory Bowel Disease (Crohn’s disease, Ulcerative colitis), Sjoengren’s syndrome, Myasthenia gravis, and Pernicious anemia. However, auto-immune thyroid disease is, by far, the most common type of autoimmune disease that most physicians will see. In addition, doctors can order blood tests which measures thyroid antibodies (against Thyro-peroxidase (anti-TPO) and against Thyroglobulin (anti-Tg)) In the case of adolescent enlargement of the thyroid (any enlargement of the thyroid is called “goiter”), with normal hormone levels, no treatment needs to be offered and only reassurance given to the patient. An auto-immune thyroid disease can cause an enlargement of the thyroid, an infiltration of abnormal cells in the gland, and often, but not always, a reduction in the amount of thyroid hormone that can be produced. WHAT ABOUT MENOPAUSE? I don’t think that menopause in itself causes thyroid problems, but what we do see is an increased incidence of hypothyroidism as people get older. It seems like the thyroid gland just fails to produce enough thyroid hormone with time. Again, this is another presentation of the autoimmune process. Not everyone needs to be treated depending on levels of thyroid hormone as indicated by the results of specific blood tests. IN THE DECADES YOU HAVE PRACTICED, HAVE THYROID ISSUES GOTTEN WORSE? I think that thyroid problems have been diagnosed more commonly and earlier in life than when I started my career. I believe it’s a combination of more sensitive and more readily available blood tests and the ubiquitous use of neck ultrasound exams, often for conditions not related to the thyroid gland. When I first started my training in the late 1960’s, the only thyroid test available was Protein Binding Iodine, (PBI), an indirect measure of thyroid hormone. Then came measurement of total T4, then measurement of triiodothyronine (T3). For the past twenty years or so, free T4 levels are available through most laboratories. Free T4 tests are now often included in a biochemical screening that is done as part of a routine physical exam. Because it is ordered more often, more cases are diagnosed. The real breakthrough in thyroid testing was the development of the ability to directly

When the levels of thyroid hormone are too low, the pituitary produces more TSH. So the TSH level becomes a very sensitive way to detect mild or “subclinical” thyroid disease often before the levels of free T4 are out of the normal range. Since the testing of TSH has become so easy, many doctors also include TSH testing as part of the routine physical exam and blood work that patients receive as part of a new patient visit or yearly physical exam. If the doctor tests for common conditions (like hypothyroidism), these conditions can be diagnosed early. Another diagnostic test that has widespread use is ultrasound for making clinical diagnoses. Ultrasound devices have become more sensitive and have many more features than those early devices. They are truly “non-invasive” and are very easy to obtain, ultrasound exams are used more and more commonly. This use is especially for looking at structures in the neck, and other thyroid abnormalities that are often detected on examination for structural problems or lymph nodes in the neck or “hardening of the arteries” in the major blood vessels. The most commonly discovered abnormalities are nodules within the thyroid gland. Thyroid nodules are now thought to be very common in middle aged and older people. When some of these nodules are investigated by thyroidologists like myself, some of them actually contain thyroid cancer. Fortunately, most of these nodules with small cancers never progress

and never cause patients any problems if left alone. Because of all this use of ultrasound and the further investigation of the nodules, thyroid cancer is being diagnosed at a higher rate than any other endocrine cancer. Over the last 4-5 years, it has been discovered there is seldom a reason to surgically remove these tiny, incidentally discovered, thyroid cancers, unless it has spread to lymph nodes in the neck or other distant sites, or there is extreme anxiety on the part of the patient to have it removed. Nuclear disasters can be a cause of severe thyroid disease. In 1986, there was an explosion in the Chernobyl Nuclear Power Plant in the northern part of the Ukraine. Fires and steam from the reactor released a huge burst of radiation, including large amounts of radioactive iodine (131I). The winds blew the smoke around eastern Europe and further. Eventually the radioactivity settled to the ground. This “fallout” settled in the grass in farming areas, and cows ate the grass. This caused their milk to have high concentrations of radioactive iodine. Quickly, there was a high incidence of aggressive thyroid cancers in children, especially those under age 10, who drank the contaminated milk. These children developed very aggressive thyroid cancers at a young age. Fortunately, through a worldwide effort by doctors, all those children have been treated. Another environmental issue occurred In 1930s and 1940s, when x-ray was first used in clinical medicine. Back then, x-ray was used as a treatment for an enlarged thymus gland or enlarged tonsils, as doctors had no idea of the deleterious effects of radiation on the thyroid. Some of those individuals developed thyroid cancer 30 to 40 years later.

TELL US A LITTLE MORE ABOUT THYROID NODULES, HYPOTHYROIDISM, AND HYPERTHYROIDISM. Thyroid nodules are extremely common. Anywhere from 30% to 50% of patients will have nodules by the time they reach 65 years old. The vast majority of these nodules are small, seen only on ultrasound, and never cause any problems. They should be evaluated by a thyroid specialist if they are greater than 1 cm in size, but rarely turn into serious conditions. Multi-nodular goiter, thyroid cysts, and hyperplastic nodules present commonly as nodules, along with chronic lymphocytic thyroiditis. Although nodule should be evaluated, but only 10% of those will be cancerous, and only 10% of those cancers will be aggressive cancers. As discussed before, the most common type of thyroid disorder is called Chronic Lymphocytic Thyroiditis, also called, “Hashimoto’s disease or Hashimoto’s thyroiditis.” It is named after a Japanese physician and is a type of autoimmune disease as I discussed before. As in all auto-immune diseases the body produces antibodies that are directed against an organ. Often, the end-result is a loss of function or chronic inflammation. In the case of auto-immune thyroid disease, the thyroid gland is considered an innocent bystander. For most cases it leads to enlargement of the gland and to infiltration by certain types of white blood cells. Sometimes, there is blocking or inhibiting antibodies being produced which can cause scarring in the gland leading to loss of function. That is how hypothyroidism develops. Often the process occurs over decades, although sometimes it happens over months. Another type of auto-immune thyroid disease is hyperthyroidism, often called

HOW IMPORTANT IS LIFESTYLE TO REVERSING THIS MALADY? Honestly, I am not sure that lifestyle change can always reverse thyroid disease. Yet at the same time an interesting observation by most thyroidologists is that nearly everyone who develops Graves’ disease can trace it back to an emotional event that happened in their life. Often, a patient will know exactly when their disease started, and that event is always associated with an emotional episode of some type. It could be an accident, a fight, a divorce, a financial crisis, or a death in the family, and within a few days or weeks people can see their disease appear. We know there is a definite interaction between the emotional system and the endocrine system. Other auto-immune diseases have been related to emotional stress. It is not known why, but that definitely occurs. We also observe that many people who develop Graves’ disease tend to have a personality that is more the anxious, not the calm, cool, and collected type. Again, I am not sure why. WHAT’S IN THE FRONTIER OF MODERN MEDICINE THAT MAY HELP TO REDUCE THE FREQUENCY OF THYROID PROBLEMS? I already talked about early diagnosis with laboratory testing and with ultrasounds. Another area that has only recently been discussed is the observation of iodine deficiency in the United States something that we never realized until recently. Since the scientists tracked down the extremely high incidence of goiters, nodular thyroid disease and hypothyroidism in people living in the Great Lakes area of the U.S. in the 1940s, there was a big push to increase iodine consumption through the addition of iodine to salt. Iodine is tasteless so people would never know whether the salt they consumed contained iodine or not. Once that occurred, we always assumed that everyone

in the U.S. had sufficient iodine in their diets. Over the last 50 years there seems to be a reduction in iodine consumption as indicated by a reduced amount of iodine in the urine as looked into by research scientists. Long term this may be a problem that we should follow. Right now, we do have to watch our pregnant mothers carefully, because during pregnancy there is increased need for iodine. If a woman lives in an area with borderline iodine intake, then gets pregnant, her baby may be iodine deficient and might be born with intellectual difficulties. This problem is easily solved by making sure that all prenatal vitamins consumed contain at least 150 mcg, which most do nowadays. Another area that is rapidly changing is the diagnosis and treatment of certain aggressive thyroid cancers. Cancer is diagnosed by pathologists by its appearance under the microscope, something that is subjective in certain cases. Now, these cancers are being better characterized by biological markers both before surgery, as well as after the cancer is removed. Knowing those results may make it easier to prescribe certain treatments especially for those rare patients who have aggressive tumors. Hopefully, within the next 10 years, the cancer immunologists will have made tremendous inroads for treatment.

WHAT ABOUT TAKING THYROID HORMONE TO HELP YOU LOSE WEIGHT? It is very common for a patient to be given thyroid hormone for different reasons, which may not at all be related to thyroid hormone deficiency. One of those quasi-indications is to help people lose weight. Some people say “I can’t lose weight; it must be my thyroid.” Another such quasi-indication is “Doc, I feel cold, or I am tired all the time.” Doctors

As I said before, one of the complaints I used to get all the time is “Doc, I can’t lose weight, it must be my thyroid.” I’ve heard patients say so many times “nothing seems to help, I need medication.” My advice to those folks is that until they have tried a program like that of the Hippocrates Health Institute, in which they stay for at least 3 weeks, in a totally controlled environment, without any deviation by going off campus or eating foods off the program, and participating in the prescribed exercise program, they should not make such a statement. If those people can honestly say they had no weight loss at the end of the 3 weeks, then perhaps a metabolic workup is indicated. Honestly, that rarely happens. The vast majority of people need a combination of a very strict meal plan and regular exercise program, and getting it into their head that they are responsible for their lives and not a pill that the doctor may give.
