Nonalcoholic Fatty Liver Is Increasingly Common DEAR DR. ROACH: I am 59 years old and am having problems with my lower abdomen. After colonoscopy and ultrasound, my doctor called to say I have a fatty liver. When asked what that means, I was told to lose weight. I weigh 170 pounds, and am 5 feet, 6 inches tall. Research online suggests that alcohol is an issue, but I don’t drink. I may have a drink or a beer when I go out, but most times I am the designated driver, so I don’t drink. The only medication I take is estradiol-norethindrone 0.5-0.1; I take one daily. Diabetes does not run in my family, and I have not been tested for it. Can you give me information on “fatty liver” and treatment? -- J.A. ANSWER: Non-alcoholic fatty liver disease is an increasingly common problem; in fact, it is the most common liver disorder in industrialized countries. Recent studies have suggested that 30 percent to 46 percent of people in the U.S. have NAFLD. Most people are diagnosed in their 40s or 50s. Obesity, particularly abdominal obesity (as opposed to fat around the hips and thighs) is common in fatty liver disease. People with suspected NAFLD should have other possible causes evaluated, especially viral hepatitis (types B and C), hemochromatosis and autoimmune hepatitis. Simple fatty liver is a benign condition; however, some people will develop the more serious condition non-alcoholic steatohepatitis (NASH). Seventy percent of people with NASH are obese, and up to 75 percent have Type 2 diabetes. NASH without treatment can lead to fibrosis and even cirrhosis of the liver. Your body mass index is 27, so you are considered overweight, not obese. Still, I would recommend gradually losing 10 pounds or so. I also would recommend that you continue to drink very moderately or not at all. Exercise also can help. *** DEAR DR. ROACH: I am 77, female and in great health except for being diagnosed with achalasia. I know it is rare, and I was happy to find out why my digestive system had discomfort. Do yoga twists or shoulder stands worsen this? It seems worse when I do these. -- E.H. ANSWER: Achalasia is a disease of abnormal movement in the esophagus, the muscular tube that carries food from the back of your throat to the stomach. It is caused by damage to the nerves in the esophagus, possibly from destruction by the body’s immune system. Because of the nerve damage, the food gets stuck in the esophagus, leading to difficulty swallowing and vomiting of undigested food. Both solids and liquids are affected, as opposed to esophageal cancer, in which the problem is mostly solids. Gravity helps us swallow, so being upside down will make gravity work against you, and I would expect the condition to worsen with yoga positions that turn the esophagus upside down. I wouldn’t recommend those types of positions, especially since they may cause or worsen vomiting. Treatment of achalasia should be done by an expert in the condition, and may include dilation of the esophagus, injection with Botox into the muscle or surgery. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. Reflections from my little corner of the world I am mailing a submission for the “Holiday Coloring Contest” for a remarkable 8 year-old girl who lives across the street from my 87 year-old mother. Natalie is always helpful when she sees that my mother has groceries, or is carrying anything to the car for seniors’ Bingo. I’m 55 years-old and our house is the same age. I realized just recently that my mother is the last surviving original resident on the street. My father died earlier this year. We have been blessed by our surrounding neighbors. Nat’s father is a volunteer fireman in East Rochester and her younger brother Gavin is ill, and often at Golisano Children’s Hospital. That is what brought the family to our street – to be closer to Rochester. They miss the wide open spaces of their former home in the country but seem very happy here. Natalie’s older brother Evan (9) is active in football and very close friends with my mom’s next-door neighbor’s 10 year-old son. Chase, like most kids, is counting the minutes until he can go to the new Star Wars movie. This young boy has blessed our hearts. They have lived next door for 8-9 years. Chase loved my dad Ross, who was the size of a giant (6’5” and 300 lbs). He scared most kids, but he was Chase’s big best friend, this child bursting with joy who always needed to come over and tell my dad something. Chase’s parents Carvel and Katie are like brother and sister to me. I can count on them to watch over things next door and they are ready at a moment’s notice to pick something up at the store or change a light bulb. We are proud of our hero next door. Five years ago when a tree fell on a car on Main Street in East Rochester, Carvel was one of the first people at the scene. He picked up a branch, broke out a rear window and climbed through the massive fallen tree into the car. Carvel couldn’t tell if either of the two men had survived, so on instinct, he just stepped up. He found out later that the driver, like him, was an outdoor extremist; in fact, Carvel had biked with his finance the weekend before. They say there are no coincidences in life; things happen, and we might not know the reason, but there is one. I told Carvel that his awesome act of random kindness turned into so much more, because his friend’s finance didn’t die alone – someone connected was with him in that space within the forest on Main Street on that July day. For that moment something extraordinary happened. Carvel was a part of life that few experience. He was present in the moment soul-to-soul with another person, because he followed his heart. Carvel is also a devoted husband, married 16 years to Katie, the love of his life, who has been living with cancer since 2012. About four years ago, Katie mentioned a health concern which her doctor had dismissed to my mother. My mom, a breast cancer survivor diagnosed at age 76, insisted she get a second opinion. Katie was diagnosed with a rare type of breast cancer. Not much is known due to its rarity, but Katie is approaching 4 years. She’s had a reoccurrence, but, thankfully, it has not spread.

We hug a lot and pray a lot on our little corner of the street. We joke that my mother Carmella has a direct line to God – similar to the “Bat Phone.” Mom is a devout Roman Catholic and a parishioner at St. Jerome’s. She used to volunteer to clean the pews and she would embroider baptismal bibs with each baby’s name and date for close to ten years or more. So, when Katie is ill, we say, “Gotta get Mom on the God phone with some extra prayers.” Finally, when I think of Heaven, I think of Hannah, our next door neighbor on the other side, who is a nurse and an Earth Angel. Hannah is kind, caring and willing to be “on duty” when “off duty.” This amazing woman earned her BS in Nursing while working long hours in an ER, yet also managed to find time to tame the wild gardens at her new home. She’s currently enjoying her new work position. There are rental homes on our street that can be problematic, but there is no “Utopia’ on Earth,” I guess. I started talking about Natalie and the coloring contest and I have gone around the block. I was reflecting about the number of people my mother has watched come and go over the years. Get a head-start on tax season The dawn of a new calendar year often marks the end of the sometimes hectic holiday season. This time of year marks a return to normalcy for many families, as the kids go back to school and parents return to work. The beginning of January also serves as a great time to start preparing for tax season. While the deadline to file returns may be several months away, getting a head-start allows men and women the chance to organize their tax documents so they aren’t racing against a deadline come April. The following are a handful of ways to start preparing for your returns now. • Find last year’s return. You will need information from last year’s return in order to file this year, so find last year’s return and print it out if you plan to hire a professional to work on your return. • Gather dependents’ information. While you might know your own Social Security number by heart, if you have dependents, you’re going to need their information as well. New parents or adults who started serving as their elderly parents’ primary caretakers over the last year will need their kids’ and their folks’ social security numbers. If you do not have these numbers upon filing, your return will likely be delayed and you might even be denied potentially substantial tax credits. • Gather your year-end financial statements. If you spent the last year investing, then you will have to pay taxes on any interest earned. Interest earned on the majority of savings accounts is also taxable, so gather all of

Look for receipts of all contributions, contacting any charities you donated to if you misplaced any receipts. • Book an appointment with your tax preparation specialist now. As April 15 draws closer, tax preparers’ schedules get busier and busier. The earlier you book your appointment, the more likely you are to get a favorable time for that meeting. In addition, if you have gathered all of the information you need by early February, then booking your appointment early means you can file earlier and receive any return you might be eligible for that much quicker. Tax season might not be right around the corner, but it’s never too early to start preparing your return.

This is intended for the person who complained about the soccer game and being unfair. I honestly do not know who you’re talking to, but if you do not like the way the other team plays soccer then get your child off the field. It seems to me that you want to make sure your child does not get hurt. If that’s the case then make him a ballerina and dress him in a tutu. I am not trying to agree with the other team, but when you play sports, it’s for fun yes, but it’s competition too! I played soccer, ice hockey and football; when we got down and dirty no one complained. In closing, grow up, stop being a sissy and let your child play the game! To the person who shot our cat in November: You caused our family emotional pain and a very large pile of vet bills (which no one needs at the holidays). Also, much time was spent caring for our pet that may never fully recover. I want people to know pellet guns are not toys and injuring pets with them is animal cruelty and it is a crime. I live on a country road it’s very dark in early

morning, yet we have one or two people walk in dark clothes. You can’t see them! Why do they do this to drivers? Buy a yellow or orange vest and pants so you can be seen. Safety is good! Having the driveway sealed so I couldn’t pull in or out. I never asked for the deposit back but you said you’d mail it to me. You just wanted to know where I was moving to.
