Scientists are trying to better understand which people are more likely to get certain types of cancer. They also study the things we do and the things around us to see if they cause cancer. This information helps doctors recommend who should be screened for cancer, which screening tests should be used, and how often the tests should be done. It is important to remember that your doctor does not necessarily think you have cancer if he or she suggests a screening test. Screening tests are given when you have no cancer symptoms.

If a screening test result is abnormal, you may need to have more tests done to find out if you have cancer. These are called diagnostic tests.

PREPARE VEHICLES FOR HARSH WEATHER

Old batteries should be replaced before winter begins. Without a strong, properly functioning battery, engines cannot turn over. Most batteries last between three and five years. However, extreme cold can compromise batteries, especially those that have been around awhile. PROTECTIVE FACTORS FOR BREAST CANCER

Aromataseinhibitors(anastrozole,letrozole)andinactivators(exemestane) lower the risk of recurrence and of new breast cancers in women who have a history of breast cancer. Aromatase inhibitors also decrease the risk of breast cancer in women with the following conditions:

• Postmenopausal women with a personal history of breast cancer.

• Women with no personal history of breast cancer who are 60 years and older, have a history of ductal carcinoma in situ with mastectomy, or have a high risk of breast cancer based on the Gail model tool (a tool used to estimate the risk of breast cancer).

In women with an increased risk of breast cancer, taking aromatase inhibitors decreases the amount of estrogen made by the body. Before menopause, estrogen is made by the ovaries and other tissues in a woman’s body, including the brain, fat tissue, and skin. After menopause, the ovaries stop making estrogen, but the other tissues do not. Aromatase inhibitors block the action of an enzyme called aromatase, which is used to make all of the body’s estrogen. Aromatase inactivators stop the enzyme from working.

Possible harms from taking aromatase inhibitors include muscle and joint pain, osteoporosis, hot flashes, and feeling very tired.

ruway Failure

When are we going to admit the Thruway billings by mail is a complete failure! I have had it with billings that are months late, late fees automatically added because the state is late, and the $2 processing fees that are an added tax. It was better when we were paying cash at the booths. When the Thruway was built in the 1950s it was completed and “grandfathered” in time to have tolls before the moratorium, and the public was told there would be tolls only until it was paid for. The Thruway takes in hundreds of millions of dollars every year only to be mixed in with other wasteful spending the state does. We never needed the rest stops to be torn down and re-built; someone in Albany got really rich from doing that. This is NY. It’s all corrupt. It never had to be this way.

I am a cyclist myself, but I am constantly put off by the reckless behavior of some riders, including E-bikers. Bicycle riders must obey all NYS vehicle and traffic laws. History Is Alive in Medina’s Hart House Hotel

In its nearly 150-year history, the Hart House Hotel in Medina has seen all manner of people pass through its doors: high-end hotel guests and celebrity customers, hospitality staff and shirt factory workers. e building has changed hands in numerous business deals, recovered from a devastating fire, and has even been said to be haunted by a woman in white who appears on the stairs. Today, it’s owned by Andrew Meier, who purchased the building in 2005 and has spent years renovating it in a way that honors its long reputation in Medina.

e Hart House Hotel’s dramatic history begins even before it was constructed, with a local business deal that went a little wrong. “Hart House was built in retaliation,” says Renee Lama, author of Last Call: A History of the Service Industry in Western New York. According to Lama, in 1874, a group of businessmen devised a plan to reduce local competition between the existing hotels and liveries in Medina.

One hotel and livery, Medina House, would be split up and the hotel sold off as commercial property. Another hotel/livery business, Bancro House, would close its stables. “ is would give Bancro House a boost in sales, as it was one of only two hotels in Medina at the time,” says Lama. “In turn, closing his own livery would help Gorton and Deuel, who owned the former Medina House livery. But that did not go as planned.”

Bancro House kept its livery open, going back on its promise to reduce competition with the Medina House livery. So Gorton and Deuel built their own hotel on the livery property in 1876. e building, intended to be a “first-class” hotel, was originally called the Gorton House. A local paper called it “the handsomest and coziest hotel in Western New York,” boasting a brick building with white sandstone trim outside and beautiful woodwork inside, according to Last Call.

residents. Today, the first floor is commercial space, with the Shirt Factory Café, Newell Lounge, and Factory Espresso providing food and drink in spaces inspired by the decades the building spent as a shirt factory. e second floor became offices, and the third floor eventually became a boutique hotel, returning to the original use of the building.

Gorton and Deuel didn’t run their hotel for very long. In 1880, they sold the building to Elizur Kirke Hart of Albion, who renamed the building Hart House. Lama says, “Hart only owned the hotel for about five years, but the name stuck.” e Hart House operated as a hotel under different owners until around 1917, when it was purchased by Robert H. Newell, who used the building to run his shirt factory for over 80 years. Newell was known to have customers like Bob Hope, Dustin Hoffman, and even Winston Churchill. According to Last Call, each customer had a unique shirt pattern kept on file in the factory.

When Meier, a self-described “historic building nerd,” purchased the building 20 years ago, he was already familiar with some of its history as Hart House. Renovating the building in stages involved a mix of restoring what had previously been there and designing new elements that honored the building’s past while meeting the needs of current Medina

Meier led the design work for each room of the reimagined Hart House Hotel. For inspiration, he turned to the stories of Newell Shirt Factory’s famous customers. “Once we researched and decided who the customers were, everything fell into place,” Meier says. “We didn’t design in the abstract. Each room is thoughtfully appointed with the building’s history in mind as well as a particular customer of the shirt factory.”

For the Churchill-inspired room, everything came together around a wall-sized paneled wooden headboard, with an ornate armoire, golden chandeliers, and Oriental rugs finishing the look, whiletheHoperoomevokesmid-centuryHollywood. Meier says, “So much of the ethos of this rehab is about tapping into its historical identity. We want guests to feel as connected to this place as possible.”

According to the Haunted History Trail of New York State, e Hart House Hotel “has likely had more souls tread over its threshold than any other building in town,” so “with [its] long, storied past, original floors, and intact historic windows, it’s no surprise that this building [may be] haunted.” Shirt workers at Newell’s recounted seeing auras of a woman in white standing atop the first staircase, hearing names called aloud when no one’s around, or hearing inexplicable yet purposeful footsteps bounding down stairs.

As far as the tales of Hart House being haunted, Meier says they’re “probably” true, though he’s never experienced anything paranormal himself. “I have never seen anything, though I’m open to it. But a lot of people have seen something.” While ghosts may not be guaranteed at Hart House Hotel, one thing is for sure: the spirit of its history is alive and well.

Weight Loss Isn’t the Cause Behind Odd Globus Sensation

Q. A.

DEAR DR. ROACH: I was recently diagnosed with diabetes and immediately changed my diet and exercise regimen. I was able to lose 17 pounds in about two months but have started to experience a feeling of something being stuck in my throat. Could the sudden weight loss have brought this on? I was sent to get an ultrasound on my thyroid, and it came back negative. -- V.J.

ANSWER: A sensation of something stuck in the throat is common in clinical practice and can have many causes. It is referred to as a “globus” sensation. An ultrasound is a very reasonable test as a large thyroid mass (goiter) can cause a mechanical effect on the esophagus, which then leads to this sensation. Similarly, abnormalities in the esophagus, oropharynx and larynx may also lead to this sensation. Most experts recommend an evaluation with an endoscopy and a laryngoscopy for people with a persistent globus sensation.

Unintentional weight loss is a red flag in people with a globus sensation because if the cause behind the globus sensation is also leading people to lose weight, this means that it is more likely to be something serious. But this is not the case with you, so I can’t think of how losing weight could have been the cause behind your globus sensation.

If the sensation of something in your throat continues, I recommend further evaluation by an ENT physician.

A.DEAR DR. ROACH: My in-laws are being evaluated for infertility, and my brother-in-law has a varicocele. They say that he needs an operation. Is this necessary in order for them to have children? -- C.F.

ANSWER: A varicocele is a common enlargement of the veins in the male scrotum. There is some evidence that a large varicocele could reduce fertility in men. However, the repair of a varicocele is usually indicated for fertility only when both the varicocele is large and the man has an abnormal semen analysis.

The benefit in fertility is thought to be small; it is estimated that 17 men need to be operated on to produce one pregnancy, so there’s roughly a 6% chance that the operation will help the couple conceive.

DEAR DR. ROACH: I have high liver enzymes and bilirubin, most likely due to Gilbert syndrome. I’ve had elevated numbers since at least 2006 when I started to track them. I was recently prescribed a low-dose statin after receiving a slightly elevated coronary calcium score (49).

The statin did great at lowering my cholesterol but seriously elevated my AST and ALT numbers. (My AST and ALT levels were 12 and 20 U/L respectively before treatment, then 41 and 32 U/L while on the statin. Normal is less than 35 U/L.) So, I discontinued its use. Are there any options other than statins to help reduce cholesterol? -- S.M.

ANSWER: Gilbert syndrome is a relatively common liver condition caused by abnormal bilirubin metabolism. People with this condition can get recurrent jaundice and high bilirubin levels.

Statin drugs are generally considered to be safe in people with Gilbert syndrome. Many people (with and without Gilbert’s) will have elevations in their liver enzymes (ALT and AST levels) due to statin drugs. Most authorities recommend reducing the dose or stopping the statin treatment if the liver enzymes go over three times the upper limit of normal -- above about 100 U/L -- which yours didn’t get close to. Since the risk to your liver from a statin is small, and the benefit of a statin in a person with even modest coronary calcium is relatively large, there is more benefit than harm for most people in your situation.

People with disabilities may need assistive devices to reclaim some measure of their independence. For example, millions of people rely on wheelchairs or walking aids to get around. These are called assistive technology and rehabilitative devices, which include tools, equipment or products that can help people with disabilities get around more easily. These devices may be as small as magnifying glasses for reading to as large as wheelchairs.

The National Institutes of Health state that 2.2 million people in the U.S depend on wheelchairs for day-to-day tasks and mobility. More than six million use canes, walkers or crutches to assist with mobility. Assistive devices can be highly effective, but only when they are used properly and safely. Assistive devices should be measured for each user. Many walkers are height-adjustable. The width of a device also is important. A walker or another device that is too wide can affect mobility or require modifications to be made around the home. Weight is another consideration. The device should be lightweight so it can be maneuvered, but also weigh enough to provide enough stability to keep users upright. Users should proceed slowly and favor their stronger sides when relying on canes or walkers. Devices should not be used to climb atop other items like step stools. Children should not be allowed to play with or ride on assistive devices. If there are safety belts or locks on any assistive technology, they should be put in place before use to prevent further injury.

UNDERSTANDING CATARACTS

Cataract diagnosis and treatment: An eye doctor will examine dilated eyes to diagnose cataracts. A slit-lamp microscope can make it easier to spot abnormalities, states the AOA.

The eye professional will consider age and the severity of cataracts to determine a treatment strategy. Early loss of vision due to cataracts may be mitigated by stronger lighting or a change in eyeglasses or contact lens prescription.

Johns Hopkins says cataract surgery is one of the most common eye surgeries. It is safe and effective. An ophthalmologist will remove the cloudy lens and replace it with an artificial, clear lens. If cataracts are present in both eyes, each surgery will be performed separately. A possible complication of cataract surgery is when a part of the natural lens that is not taken out during surgery becomes cloudy and blurs eyesight. This can be remedied with a laser treatment.

BREAST CANCER TREATMENT

Most patients with breast cancer have surgery to remove the cancer. Sentinel lymph node biopsy is the removal of the sentinel lymph node during surgery. The sentinel lymph node is the first lymph node to receive lymphatic drainage from a tumor. It is the first lymph node where the cancer is likely to spread. A radioactive substance and/or blue dye is injected near the tumor. The substance or dye flows through the lymph ducts to the lymph nodes. The first lymph node to receive the substance or dye is removed. A pathologist views the tissue under a microscope to look for cancer cells. After the sentinel lymph node biopsy, the surgeon removes the tumor using breast-conserving surgery or mastectomy. If cancer cells were not found in the sentinel lymph node, it may not be necessary to remove more lymph nodes. If cancer cells were found, more lymph nodes will be removed through a separate incision. This is called a lymph node dissection.

Types of surgery include the following:

• Breast-conserving surgery is an operation to remove the cancer and some normal tissue around it, but not the breast itself. Part of the chest wall lining may also be removed if the cancer is near it. This type of surgery may also be called lumpectomy, partial mastectomy, segmental mastectomy, quadrantectomy, or breast-sparing surgery.

• Total mastectomy: Surgery to remove the whole breast that has cancer. This procedure is also called a simple mastectomy. Some of the lymph nodes under the arm may be removed and checked for cancer. This may be done at the same time as the breast surgery or after. This is done through a separate incision.

Breast cancer awareness campaigns have helped destigmatize the disease but have not had a significant impact on the incidence of Stage IV disease or on mortality. Stage IV breast cancer is disease that has, at initial diagnosis, spread to distant sites within the body. The incidence of this has not changed since 1975. In 2017, an estimated 40,610 women in our country will die as a result of breast cancer.

• Modified radical mastectomy: Surgery to remove the whole breast that has cancer, many of the lymph nodes under the arm, the lining over the chest muscles, and sometimes, part of the chest wall muscles. Chemotherapy may be given before surgery to remove the tumor. When given before surgery, chemotherapy will shrink the tumor and reduce the amount of tissue that needs to be removed during surgery. Treatment given before surgery is called preoperative therapy or neoadjuvant therapy.

Even if the doctor removes all the cancer that can be seen at the time of the surgery, some patients may be given radiation therapy, chemotherapy, or hormone therapy after surgery, to kill any cancer cells that are left. Treatment given after the surgery, to lower the risk that the cancer will come back, is called postoperative therapy or adjuvant therapy.

If a patient is going to have a mastectomy, breast reconstruction (surgery to rebuild a breast’s shape after a mastectomy) may be considered. Breast reconstruction may be done at the time of the mastectomy or at some time after. The reconstructed breast may be made with the patient’s own (nonbreast) tissue or by using implants filled with saline or silicone gel.

Arguably one of the most challenging - and often frustrating - components about puppyhood is training the dog to use the outdoors for bathroom needs. It’s easy to underestimate the time involved in housebreaking a puppy. The Humane Society of America says puppies typically can control their bladders for one hour for every month of age. That means if the dog is three months old, he can hold it for about three hours.

My Cat has become a picky eater

DEAR PAW’S CORNER: My cat “Jonesy” is 11 years old and has suddenly become a very picky eater. He is barely touching his dry kibble, and he will only eat canned pate or fresh chicken, shredded very fine. Is this something to be concerned about? -- Martha Y., Tallahassee, Florida

DEAR MARTHA: Sudden, unexplained changes in a cat’s behavior -- especially around food -- often signal a change in their health. Call the veterinarian and describe your cat’s symptoms; the vet will likely ask you to bring Jonesy in for a look.

One possible cause is a dental infection. After age 4, more than half of cats begin to experience some degree of dental problems, like gingivitis, tooth infections and tooth resorption. Symptoms of dental disease include bad breath, excessive drooling, blood-tinged saliva, pawing at the face, dropping food from the mouth, lack of appetite and several more -- all requiring a visit to the vet.

Your cat’s vet will give him a complete physical exam to determine if something else is causing his lack of appetite, and will take a close look at his teeth. X-rays may be ordered to determine the extent of the problem.

If Jonesy’s issue is an infection, antibiotics will be prescribed. If a tooth is involved, the vet will probably recommend pulling it; this is done under anesthesia, and will provide so much relief to your cat.

Once your cat has recovered,the vet will likely set up a toothbrushing routine -- a finger brush and flavored toothpaste made just for cats work best. They may recommend a dry cat food that helps fight plaque buildup. There are many more preventive treatments available for pets today than in the past, so ask your vet how you can keep Jonesy happy and healthy through his senior years.

Tipping men and women who work in the food service industry is commonplace in the United States and Canada, where citizens typically tip between 15 and 20 percent when dining out at restaurants. But American and Canadiantravelersabouttodepart for overseas destinations may want to familiarize themselves with the practices regarding gratuities before their planes touch down. Some countries automatically add gratuities onto customers’ bills. For example, the government in Dubai mandates that a 10 percent service charge be added to all hotel, restaurant and bar bills. This service charge is then divided by the staff at the establishment, making the charge the equivalent of a tip. Diners in Morocco, where the gratuity is sometimes included in the bill, might be surprised to learn that a 10 percent tip in that country is considered generous. Tips are included in the bill in Costa Rica, where travelers need not feel obligated to leave any additional gratuities. Service is often included in the bill within the United Kingdom as well, though travelers should inspect the bill before assuming gratuity was included. Atrial fibrillation, is the most common type of arrhythmia, which is a problem with the rate or rhythm of the heartbeat during which the heart can beat too fast, too slow or with an irregular rhythm. Atrial fibrillation, or AF, occurs when rapid, disorganized electrical signals cause the atria, which consists of the heart’s two upper chambers, to contract very fast and irregularly.

It’s impossible to say exactly what’s going on, but lately there are problems with our food supply, one after the other. Listeria and salmonella are at the top of the list.

Listeria shows up in water, waste and the soil and ends up in our meat, fresh produce and dairy products.

Some say the rising number of cases of listeria are because we’re eager to grab ready-to-eat foods like deli meats. Others point to better detection methods that find listeria during inspections. One big concern, however, is that cold doesn’t kill listeria. Even if it’s stored in the cold (or even frozen), it doesn’t die.

When it comes to salmonella, expert fingers want to point to climate change because warm temperatures help salmonella grow. But there is also the problem of large farming facilities where a single sick animal can infect the rest.

But no matter the reason for the increases and their sources, we seniors need to be especially careful. A bad case of listeria or salmonella can put us in the hospital.

What can we do to avoid problems with food contamination?

With salmonella, wash your fruit and vegetables. Cook foods to the correct temperature, especially eggs, meat and poultry. Invest in a thermometer to keep an eye on the temperature in the refrigerator.

Listeria is tougher. Unfortunately, hygiene is part of the problem. The contamination can start at the location where the food is produced -- and spread around the deli counter if slicers aren’t washed correctly. Once we have that lunch meat at home, we need to heat it to a high enough temperature. Which means it’s not safe to just grab a few slices of cold lunch meat to make a sandwich.

It is with great sadness we announce the passing of omas Ulatowski on Friday, October 17th, 2025 a er a brief illness.

Tom was born in Buffalo, New York to Henry and Regina (Zielinski) Ulatowski on January 17th, 1943. As a young man he was involved with car racing and customizing, and also ran many school dances at Bishop Ryan High School. He later graduated from Bryant & Stratton Business School as an accountant; a field he remained in his whole life. In 1964, he married his longtime sweetheart, Suzy. Within a few years, Tom was dra ed into the U.S. Army and was stationed on a firebase in Pleiku, South Vietnam. He was then transferred to Aberdeen Proving Grounds in Maryland to complete his service. A erwards, Tom returned to accounting and commuted daily from Buffalo to Avon, New York for Clark Forkli , until moving his family to Caledonia, New York. A er many years, Tom eventually went to work for the Genesee Valley Penny Saver as Controller. Ironically, his new office at the Penny Saver was the same exact building and office he worked in for Clark Forkli , many years earlier. Tom loved the Penny Saver, and all those who worked there with him. He worked with his GVPS family far past what he needed to – because he wanted to. Tom was treasurer of the American Legion Matthew Cleary Post, as well as financial secretary of the Knights of Columbus Fr. William Devereaux Council; both in Caledonia. Tom played on the Lake Keller bocce league for many years as well. He loved going to summer car shows, the Buffalo Bills, Knights and Legion meetings, country music (he even played the slide guitar) and spending time with his family.

Tom is survived by his loving wife of 61 years, Suzanne (Kennick) Ulatowski; his son, Brandon Ulatowski (Danielle); sister, Maryanne (Francis) Knighton; brother-in-law to Kathy (late Richard Fifer Sr.) and Karen (Tony) Wozniak; many nieces, nephews and friends. Tom touched many people’s hearts.

Friends called Tuesday, October 21, 2025 from 2-5pm at Alhart Funeral Home, 3068 W. Main St., Caledonia, NY. Mass was held Wednesday, October 22, 2025 at 11am at St. Martin de Porres Parish at St. Columba Roman Catholic Church, 198 North St., Caledonia, NY. Internment followed at St. Columba Cemetery. In Lieu of Flowers, donations may be made to the Big Springs Museum, 3095 Main St., Caledonia, NY 14423, or St. Jude’s Childrens Hospital, 501 St. Jude’s Place, Memphis, TN 38105.

FINDING WORK AFTER 50

Unemployment isn’t easy for anyone, regardless of their age. But unemployed men and women over 50 may find it especially difficult to find work.

Whether it’s a byproduct of age-related discrimination or any of a host of additional variables, jobless older workers often struggle to find work. In a 2016 analysis of government figures, the Schwartz Center for Economic Policy Analysis at the New School estimated that the jobless rate for workers 55 and older in August of 2016, six years after the Great Recession, was nearly 9 percent. At the time, the national jobless rate hovered around 5 percent.

Unemployed men and women over 50 who are struggling to find work can consider the following strategies as they look to rejoin the workforce. Revisit your résumé. Unemployed men and women over 50 have no doubt updated their résumés to reflect their most recent professional experience. But they may need to trim some of the fat in regard to their work life 10 or more years ago. Today’s hiring managers may only be concerned with recent experience that illustrates skillsets that are relevant to today’s jobs. Men and women over 50 may consider their experience from 20 years ago invaluable, but if that experience does not meet the specific needs of the jobs they’re now seeking, then they should remove it from their résumés so hiring managers can quickly access the more relevant information from their work histories.

Embrace 21st century job hunting. Finding a job in the second decade of the 21st century is unlike job hunting in decades prior, and wholly different from how men and women over 50 looked for jobs upon beginning their professional lives. Networking can mean the difference between unemployment and landing a job. Go to job fairs attended by hiring managers and join professional organizations that host events where professionals in your field can gather.

Turn your age into a positive. Men and women over 50 should accept the likelihood that their new managers and/or hiring managers will be younger than them. When interviewing for a job, men and women over 50 should make an effort to showcase their enthusiasm about working with and learning from younger colleagues, while also noting their desire to commit long-term to a company. Some hiring managers may surprise older applicants, viewing them as potentially more reliable than younger workers simply looking to gain some experience in a particular industry before moving on to the next opportunity.

Make use of your existing down time. Another strategy unemployed men and women over 50 can try as they look for work is to make better use of their existing downtime. Enrolling in online courses can give prospective employers the impression that applicants over 50 are both tech-savvy and willing to learn new things. Each of those things can help men and women over 50 overcome any unjustified, tech-related stigmas that hiring managers may attach to older job candidates.

Finding work after 50 is not always easy, and job seekers may need to adjust their approach before they can get back in the workforce.

BREAST CANCER TREATMENT

Radiation therapy is a cancer treatment that uses high-energy x-rays or other types of radiation to kill cancer cells or keep them from growing. There are two types of radiation therapy:

• External radiation therapy uses a machine outside the body to send radiation toward the cancer.

• Internal radiation therapy uses a radioactive substance sealed in needles, seeds, wires, or catheters that are placed directly into or near the cancer.

The way the radiation therapy is given depends on the type and stage of the cancer being treated. External radiation therapy is used to treat breast cancer. Internal radiation therapy with strontium-89 (a radionuclide) is used to relieve bone pain caused by breast cancer that has spread to the bones. Strontium-89 is injected into a vein and travels to the surface of the bones. Radiation is released and kills cancer cells in the bones.
