DEAR DR. ROACH: DEAR DR. ROACH: In a recent column, you wrote about reducing cholesterol levels with the help of statins along with a PCSK9i, such as Repatha or Praluent. My question is: Are these PCSK9is at all beneficial if one cannot take statins? -- D.C.S.
ANSWER: Yes, PCSK9 inhibitors are very effective at lowering cholesterol and reducing the risk of heart attacks in high-risk people. Because these drugs are given by injection and are quite expensive, they are not usually the firstline treatments. We have decades of experience using statins, so they remain the first choice for most.
However, if a person cannot take a statin, or one of the alternatives such as bempedoic acid, then PCSK9i drugs are a very reasonable choice. In people with familial high cholesterol and extraordinarily high levels of cholesterol, combination therapy with two or three drugs (a statin, a PCSK9i and sometimes ezetimibe) is frequently used.
DEAR DR. ROACH: My wife had COVID and has lost most of her smell and taste. This has lasted for two years. She is in her late 70s. Strong flavors like barbecue sauce and onions can be detected. Is there any hope that her condition will improve? -- P.M.
ANSWER: Most people recover their loss of smell after COVID. Having had this for two years is a bad sign; however, the fact that she does have some sense of smell is a good sign.
The most effective treatment for the loss of smell after a viral infection is olfactory training. The same scents that were used in clinical trials can be purchased online. Recent evidence has shown that a supplement combination, ultramicronized palmitoylethanolamide and luteolin, along with olfactory training, led to greater improvement. The dose used in studies was 770 mg.
DEAR DR. ROACH: My adult daughter (62) was diagnosed about three years ago with a skin disorder called prurigo nodularis. Her dermatologist has tried three different injections, but they’re not working. She still gets very itchy breakouts. Please give me your thoughts and more information about this. -- K.M.A.
ANSWER: Prurigo nodularis is an uncommon disease that affects about one person per 1,000. I have seen a handful of cases, and fortunately the cases I’ve seen have responded well to potent steroid creams.
Regular use of skin lotions, avoiding scratching especially at night (I recommend cotton gloves), and sometimes antihistamines like Benadryl are also the first-line therapies.
If a patient needs more effective treatment and only has a limited area of itching, then a dermatologist will often inject the area with a high-potency steroid. Since it sounds like this hasn’t been successful for your daughter, there are other options. One is a topical cream called ruxolitinib. About 45% of people responded to this treatment, compared to 20% who responded to a cream without an active drug. Ultraviolet light (narrowband ultraviolet B) showed about 80% effectiveness. For people in whom none of these therapies work, a very expensive injection treatment,dupilumab (Dupixent) or a similar drug called nemolizumab (Nemluvio), can be used, although the response rate is not as good as desired (roughly 40%).
Since many people with prurigo nodularis have other skin diseases, and some may have (or develop) depression or obsessive skin-picking, these conditions may also need to be treated to successfully improve symptoms

For a number of people, this may be the year they relocate to a new home or apartment. The moving rate in the United States has remained steady since 2008, with roughly 12 percent of the population relocating each year according to reports from the U.S. Census Bureau. Movement to the suburbs continues to be high, and renters tend to move more frequently than others. People who move cite a new job or a job transfer as one of the primary motivators to relocate. Acclimating to a new home and neighborhood after a move can be challenging. Even after boxes have been unpacked and items put away, some people still feel unsettled. Many people are familiar with the term “ozone layer.” But some may not know that the Earth’s atmosphere consists of several layers. The lowest layer of the Earth’s atmosphere is the troposphere, which the U.S Environmental Protection Agency notes extends from the Earth’s surface to roughly six miles in altitude.Thetroposphereiswhere nearly all human activities occur. Even climbers who make it to the peak of Mt. Everest are still in the troposphere, as Everest is less than six miles high. But anyone who has ever boarded a commercial flight has likely left the troposphere and entered the stratosphere, where the majority of commercial airplanes fly. The stratosphere begins at six miles above the Earth’s surface and extends to about 31 miles above the surface of the planet. The EPA notes that most atmospheric ozone is concentrated in a layer in the stratosphere that’s roughly nine to 18 miles above the Earth’s surface. The ozone layer in the Earth’sstratosphereabsorbssome of the sun’s radiation, including the UV and UVB radiation that has been linked to cancer. The third layer of the Earth’s atmosphere is the mesosphere, which extends from 31 miles above the Earth’s surface to roughly 53 miles above the surface of the planet. The height of the next highest layer, the thermosphere, varies due to changes in solar activity. The exosphere, which contains many of the satellites that orbit the Earth,istheoutermostlayerofthe Earth’s atmosphere.

Longer life is one of humanity’s greatest achievements, butitalsobringsnewchallengesforolderadultsandtheir caregivers. Few understand that landscape better than Ann Marie Cook, who has guided Lifespan for 29 years, transforming it into a trusted resource for thousands navigating aging with dignity and independence.
Cook’s interest in public service and the nonprofit sector began early. She attended St. Pius X Grammar School and Cardinal Mooney High School, then pursued higher education at SUNY Fredonia and SUNY Brockport, earning degrees in political science and public administration.

policy and funding mechanisms has been critical in guiding Lifespan over the years.”
Under Cook’s leadership, Lifespan provides more than 30 programs and services, offering guidance, education, and care management to older adults and their families across Monroe County and surrounding regions. e nonprofit hosts workshops ranging from Medicare essentials to fall prevention, runs support groups and respite centers, and connects caregivers to home care, transportation, and medical specialists.
Over the years, her passion for the work has only grown, as “People are living longer, healthier lives, and governmentfundinghaslargelynotkeptpacetosupport those changes.”
With the Rochester region’s shi ing demographics, skewing older than the state average, especially in rural communities, Cook says, “ at changes how we think about work, community design, and accessibility. Ageappropriate work strategies, safe crosswalks with longer light cycles, and inclusive public spaces all matter... to be an age-friendly community, older adults must have a voice.”
Healthspan, the ability to live healthier for longer, is central to Lifespan’s mission, and Cook says, “I’m not sure we can change longevity, but we can help people be healthier longer. We can reduce the likelihood of chronic illness. We promote physical and mental activity, strong social connections, and healthy eating. It’s not just about surviving; it’s about thriving.”
At the heart of Lifespan’s work are family caregivers, the unsung heroes who shoulder enormous mental and physical strain. Cook says education is the first line of support. Lifespan provides support groups so caregivers can connect with others going through the same experience, sharing advice and solidarity.

For Cook, a life well-lived is one marked by contribution and pride. “Everyone has a different definition. If you can be proud of your work, your children, or your community contributions, that’s a life well-lived. At Lifespan, our goal is to help others achieve that sense of purpose, connection, and dignity as they age.”

3 tablespoons melted butter
1 teaspoon vanilla extract
2 tablespoons brown sugar
1 teaspoon ground cinnamon
1/8 teaspoon ground allspice

8 cups popped popcorn
1 cup dried apple chips, broken into large pieces
1/4 cup toffee bits
Heat oven to 300 F. Whisk melted butter with vanilla. Toss brown sugar with cinnamon, allspice and nutmeg.
Toss popcorn with butter mixture. Sprinkle evenly with brown sugar mixture. Stir. Transfer to baking sheet lined with parchment paper. Sprinkle apple chips and toffee bits over top. Bake 15 minutes, or until toffee bits start to melt. Cool before serving.
Tip: Add chopped pecans for extra crunch.

I was wearing a ball cap which identified me as a veteran, and dining with my wife on 10/29 at Applebee’s in Geneseo. You can imagine my surprise when our waitress said, to thank you for your service, your dinner has been paid for by an anonymous person. This kind gesture meant so much to me. Wow, I hope that person knows how much I appreciated this thoughtfulness and generosity. It was a moment I will not soon forget. What is going on lately with so very many lost cats and dogs? It seems like every day there are notices of lost or missing cats and dogs. I’m sad for the animals and owners who are heartbroken. The other side of that is the sad amount of cats found wandering in back yards, mewing at people’s door steps, and lost dogs wandering on roads at all hours. There is info online to help pet owners understand about securing their homes, windows, and yards and providing an excellent safe pet home. Learning about a cat, dog, or any pet, is essential and understanding that by saying ‘Don’t go out’ or ‘No’ doesn’t do it. Sometimes loud noises, strangers, poorly socialized pets, getting ‘stuck’ someplace, or being just plain fearful can make an animal look for safety and escape, not knowing of course what the ‘big world’ out there is. While humane societies offer so many enticing adoptions, people should not get a pet ‘on a whim’ but rather study pet care. Let’s all keep our pets safe and happy at home. www.featuredmedia.com
Stop signs are not optional. The lines on the road are not there to just look pretty. After stopping at a stop sign when turning left, if you find yourself in the oncoming lane of traffic, you’re doing it wrong. Speed limits. If you can’t at least do the posted speed limit, please pull over and let the forever growing line of traffic go by you. We all make mistakes. If you pull out in front of someone, get going or pull over. Common courtesy needs to make a comeback!

DEAR PAW’S CORNER: My 12-year-old cat, “Gaston,” may be losing his vision. He used to jump on high objects easily; now he avoids jumping up or down. He loved chasing his laser light; now he just huddles down and seems to ignore it. At night, he meows loudly until I let him into my bedroom, where he insists on curling up with me. I have an appointment with the veterinarian for next week, but can his vision loss be reversed? -- Jeanne in Evanston, Illinois
DEAR JEANNE: I’m very glad that you’re taking Gaston to see the vet. With suspected vision loss, it’s very important to get your cat tested to determine how severe it is and the underlying cause.
Blindness in cats can be gradual or sudden, and there are many conditions that can cause it. Age, infection and common eye conditions like glaucoma, cataracts or retinal detachment are all in play here.
Can Gaston’s vision loss be reversed? That really depends on what the vet finds. I can say that the sooner a problem is diagnosed, the better the chances of treatment to reverse or at least halt vision loss.
Cats whose blindness is progressive or irreversible can adapt. Those who develop it gradually tend to adapt better, while those whose vision loss is sudden may experience more anxiety and be less prone to adapt.
You can support your vision-impaired cat by keeping them indoors (if they must go out, create a fenced-in area of your yard for them). Avoid moving furniture too often. And don’t coddle your cat by carrying them around. Let them explore and adapt -- their resilience will surprise you.

Use light wisely. The photography resource Expert Photography recommends taking photos primarily during the golden hours. These are right after sunrise and just before sunset. The light during these times of day has a softer, hazy quality that will illuminate subjects evenly and less harshly than midday, when shadows also may pose challenges. Taking photos during these times also means fewer people around and a better chance that wildlife will be out and about.

1 sheet puff pastry, thawed
5 ounces Brie cheese


Servings: 24
1/2 cup Musselman’s Apple Butter
1/4 cup pecans, chopped
Heat oven to 400 F. Grease mini muffin pan.
Press seams of puff pastry sheet together. Cut sheet into quarters then each quarter into six pieces.
Use rolling pin to flatten each piece of pastry into square shape. Place one square into each muffin cup.
Put1/2teaspoonbrie,1teaspoonapplebutterand1/2teaspoonchopped pecans into each muffin cup.
Bake 13-15 minutes until golden brown. Serve warm.

According to the American Osteopathic Association, one in five teenagers will now experience some form of hearing loss. That rate is roughly 30 percent higherthanitwas20yearsago.Though various factors have contributed to that increase, the AOA notes that many experts believe an increased use of headphones among teenagers is partly to blame. Listening to headphones at high volumes for extended periods of time can lead to lifelong hearing loss for children and teens. Parents can encourage children and teenagers to listen to music at reasonable levels when using their headphones,and one effective way to control volume without sacrificing sound is to use over-the-ear headphones instead of earbuds. The Cleveland Clinic notes that over-theear headphones are generally more effective than earbuds at cancelling outside noise. That allows over-the-ear headphone users to listen to music at lower volumes, which can significantly reduce their risk for noise-induced hearing loss.
