New Brain Health Initiative Could Unlock Mysteries Of Alzheimer’s, Dementia (NAPS)—Scientists are contemplating important questions about health, mind and age: Could your blood hold the molecular secrets to a fountain of youth, preventing agerelated brain disorders?

Are brain aging and Alzheimer’s disease caused by a failure of interconnected systems, triggering a dominolike cascade of disease? Can targeting the red blood cells and blood vessels jointly keep your brain healthy and prevent dementia?

The Problem As people live longer, Alzheimer’s and other age-related dementias are on the rise, projected to reach more than 75 million people worldwide by 2030. To date, no effective therapy has been developed for these disorders, which are not only deadly but exact a high financial and emotional toll on society.

The Research To find solutions, three large-scale research teams are exploring those questions as part of an initiative to merge research of the brain and the blood vessels to develop new understanding of—and, ultimately, better preventions and treatments for—age-related brain disorders such as Alzheimer’s disease.

The Scientists To help, American Heart Association, the world’s leading voluntary organization focused on heart and brain health, and The Paul G. Allen Frontiers Group, a division of the Allen Institute, launched the $43 million research project to bridge the science of vascular and brain health through revolutionary, out-of-the-box thinking. Additional supporters include the Oskar Fischer Project and the Henrietta B. and Frederick H. Bugher Foundation.

The three teams, headquartered at the Salk Institute for Biological Studies in La Jolla, Calif.; Stanford University School of Medicine in Stanford, Calif.; and University Hospitals Cleveland Medical Center in Cleveland, OH, respectively, work to develop new solutions to the urgent problem of age-related cognitive decline. The researchers are: • “Rusty” Gage, Ph.D., a neuroscience researcher and president of Salk, who will lead an eight-year project looking into new targets for therapeutic research and biomarkers of early-stage cognitive decline.

• Tony Wyss-Coray, Ph.D., a professor of neurology at Stanford, who will lead a four-year project studying how changes in the immune system affect cognitive health.

• Mukesh K. Jain, M.D., a cardiologist at University Hospitals Cleveland Medical Center and Professor of Medicine at Case Western Reserve University, who will lead a team of investigators on a four-year project investigating whether targeting red blood cells and blood vessels can prevent dementia.

The Hope The research could yield transformational discoveries to better detect, treat and prevent cognitive decline.

Scientists are seeking answers to important questions about how to preserve your mind over time.

PREP-YOUR-PLACE POINTERS (NAPSI)—Many people may be surprised to learn that fall is actually the most affordable season for home prep and repair projects. There’s less demand, so homeowners enjoy breaks on equipment and services.

An easy place to begin is at the window. Cracked and warped windows can often cause heat to escape. “Windows that produce a draft or feel cold on the inside should be replaced,” explains Richard Kasunic, a home repair expert with Window Nation, one of the country’s leading window replacement companies. Modern vinyl windows fit tightly to shield homes from wind, water infiltration and inclement weather overall. With that said, many homeowners have older windows and are often “heating the outside,” adds Kasunic.

“On average, homeowners spend 40 percent of their utility expense dollars on heating and cooling,” he says. “But if new windows aren’t in this year’s home budget, you might want to pick up insulated curtains, window and door gap seals, and add draft snakes on windowsills.”

He also suggests replacing the weather stripping around doors if you see any light around the edges. For less than $10, you can likely reduce electrical bills throughout the year.

Beyond windows and doors, exterior outlets, plumbing and utility access, chimneys and walkways should be sealed for unwanted holes, says Kasunic. And homeowners should have their furnaces and roofs professionally cleaned and inspected.

Finally, cover the water heater and change the furnace filter so your furnace doesn’t have to work as hard.

Why does the [local town] Police Dept. publish all the people arrested in their weekly “Police Report”? I am honestly wondering why this is done.

Not a “vent” - not a “brighter” - just some suggestions. For the New Year: Try some patience, friendly smiles, helpfulness toward others. Please, thank you, excuse me - these all go a long way in dealing with

others. With so many problems in the world, we can all use a little peace & joy!

I was recently sideswiped by a white box-type delivery truck. After the truck hit me, it continued down the road without stopping. There is over $5000 damage to my car. There were several witnesses to the accident, but no one stopped to ask if I needed help. I know there are several witnesses that could help to identify the truck. I guess this is where we are today. No one wants to get involved. It’s a sad day for this country.

I took the family to the Capital over the holidays.

At the Holocaust Museum

there are several exhibits of the concentration camps. While viewing a picture of children wearing prison uniforms, behind a wire fence, skinny and sickly looking, my 8-year-old turned to me and asked: “What did they do to be put in there?”

The children in this camp were about my child’s age. I didn’t want him to fear that this could ever happen to him. I told him they didn’t do anything wrong.

There were a lot of problems in the country and the leader didn’t want to be blamed, so he blamed outsiders: the Jews. He put them in the camps and let them die. Then my child asked: ”How could the good people let this happen?”

Brighter Side Congratulations to all the hardworking volunteers at the Henrietta Library Book Shop, the Shop is opening again and we can’t wait to see you all, we missed you.

DEAR DR. ROACH: I recently read an article on the pros and cons of a daily aspirin for protection against both heart disease and colon cancer. This is not a new topic, I realize, and I know that the benefits of daily aspirin apply only to certain segments of the population.

Here’s my question: If a person doesn’t experience any stomach or bleeding issues, does a much higher dose of aspirin daily also convey the same possibly protective effects?

I don’t believe that more is better, but I already take an average of nine 325 mg generic aspirin tablets daily for leg pain. I’m in that 50-59 age group, and there is heart disease on both sides of my family, so I’m hoping this dose confers some cardiac benefit.

I would love to find a cure for the deep ache I get in my legs -usually just the right calf. I’ve had these since I was a child, and I’ve reconciled myself to the fact that I will have them for the rest of my life, but the aspirin I take has never caused me stomach pain or excessive bleeding, and it works for the pain.

-- T.P. ANSWER: The studies showing that taking aspirin protects the heart mostly have been done using 81 mg daily or 325 mg every other day. However, the mechanism of the action, preventing platelets from forming a clot, is even stronger at higher doses, so it would make sense that higher doses would be at least as effective. However, they are much more likely to cause bleeding, even if most people can take higher doses without problems.

So it’s recommended you take the minimum effective dose to get the benefit with the least harm. In your case, however, I think you are likely getting all the benefit there may be.

I say “may be” because recent studies have failed to replicate the protection seen in earlier studies. I still believe that aspirin is useful in people at higher-than-average risk for heart disease.

More and more evidence is coming out that the same low dose of aspirin helps protect against several forms of cancer, especially of the colon. However, I don’t yet recommend it just to protect against cancer, but I will continue to pay careful attention to ongoing studies on aspirin and will revisit this topic when more research becomes available. *** DEAR DR. ROACH: In a recent test, my blood vitamin D level was good, at 43, but my calcitriol level was low at 16. I do have hyperglycemia, but how do I increase my calcitriol level?

I take no medications. -- J.I. ANSWER: If you don’t have kidney or parathyroid hormone problems, then calcitriol isn’t the right blood test to determine your bone health relative to vitamin D. Calcitriol is an activated form of vitamin D, and it best reflects calcium intake. I suspect low calcium intake when I see a high calcitriol. Since calcitriol removes calcium from the bones, it may worsen osteoporosis.

If you haven’t had your kidneys and parathyroid hormone level (which also affects calcium movement in the kidney and bones) checked recently, then do so. If those are OK, then be sure to get enough calcium, especially from dietary sources like leafy green vegetables. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible.

The GiGi’s Playhouse: A World of Possibilities

place to belong: it’s a human need we feel from birth, a need that doesn’t disappear if we happen to be born with an additional chromosome. Still, children with Down Syndrome often face an incredible lack of acceptance in their communities. GiGi’s Playhouse: Down Syndrome Achievement Centers is working hard to change all of that.

Gigi’s Playhouse has several locations across the country, and one is now serving the Greater Rochester area at Village Gate. “Gigi’s Playhouse is a national organization with a branch in Mexico,” said Amy Shulman, GiGi’s Playhouse Coordinator of Volunteers. “Our president and vice president, Jennifer Bustamante and Kim Guerrieri, both have children with Down Syndrome (Ds). They were on a local board for children with Ds when they heard about GiGi’s Playhouse. They thought it was something we could really benefit from here.” So, in April of 2017, GiGi’s Playhouse opened its doors at Village Gate.

“We are an extension of their education,” said Shulman, referring to the children with Ds who utilize GiGi’s. “We tutor children with D.S. in literacy and math. We have a science club, a GiGiFit program through which we help children living with Ds improve their muscle tone. We have cooking club once a month... anything we can do to be an extension of their learning, we do.” GiGi’s certainly is filling a need, exploding to about 1,000 participants in the Rochester area, and area residents are showing the love right back.

“I just can’t emphasize enough that we would be unable to do this without Rochester. It is an incredibly giving community,” said Shulman. “GiGi’s is 100% funded by the community, and what we offer is 100% free to the community. We are 99% run by volunteers who serve with this outpouring of love.” Shulman went on to describe the volunteers as the heart and soul of GiGi’s Playhouse, wonderful people who come in every day and give their all. “They do this out of the goodness of their heart and their love for the children,” she explained. Still, Shulman sees more opportunity to serve, stating that there are approximately 2,000 people living with Ds in the Rochester area and that GiGi’s is always reaching out to those whom they have not yet supported, reaching beyond the children to embrace the entire family.

“We also try to be a help to the parents and be a support system for them,” explains Shulman. “We do what we can to be a support system to both our participants and our families. Our center is a safe haven, and it is like home to most of them, a safe home environment. When participants come in, they feel welcome and are greeted with a hug.” The parents, says Shulman, have been so grateful to have this support system and a place of acceptance. “That is really what we want,” Shulman added. “To watch them grow and learn and succeed is so rewarding for us, and it is incredibly gratifying for the parents especially when children have these challenges.” The outstanding support GiGi’s provides starts young, even before birth.

“One of our goals is to reach couples prenatal if there is a diagnosis, which sometimes there isn’t,” said Shulman. “We are there all the way from prenatal to adulthood. We believe we are the first Down Syndrome advocacy group working to be right there offering a positive hand when a diagnosis of Down Syndrome is given.” Shulman then described an all-too-familiar scene. “Across the world, when a baby is born with Down Syndrome, doctors will all huddle around the baby in a corner, whispering. When a GiGi’s Ambassadors with the Brown Bag Lunches they doctor at last delivers made for the Open Door Mission. the news, it will be with expressions like, “I am so sorry.” This experience is something GiGi’s is working to change. “Why should any mother have to hear that?” Shulman asked.

“There is nothing to be sorry about! This is a wonderful, beautiful, healthy baby and a blessing.”

In fact, it is this current perception of Down Syndrome which leads to expressions of sympathy that GiGi’s is seeking to change, what Shulman refers to as “changing the face of Down Syndrome.” To this end, GiGi’s does its best to inform through social media and by getting its Ambassadors Club members out

GiGi’s Ambassadors Club is made up of some of its participants, teenagers through adults. “We get them out there to do community service, like the Brown Bag Lunches which they made and handed out at the Open Door Mission,” explained Shulman. “They have also participated in bell ringing for the Salvation Army, and we have them involved in public speaking as much as possible, especially at our events like our Gala. We give them a voice so that they can speak for themselves.”

Looking forward, Shulman sees an even bigger and brighter future for GiGi’s.

We have already outgrown our center,” she enthused. “We would like to add on.” Shulman explains that the first GiGi’s in Chicago, established by Nancy Giani, now includes several additional pieces. Hugs and Mugs, a coffee shop, is completely operated by the participants. GiGi University is basically a college for work skills where young people can learn real life skills to get out into the workforce and excel in real world jobs. “We see these things and more as a part of our potential future,” said Shulman. For now, GiGi’s is planning an extraordinary Gala on March 16th at the Riverside Convention Center near the Memorial Art Gallery in Rochester. Shulman hopes that this Gala will not only raise support but will increase awareness, as well. “We want people to know we are here for them,” she said. “We are a resource for all families facing that diagnosis.”
