week and had pretty severe side effects, though in the range of what can be expected: high fever (101.5), intense shivering, a severe headache and 12 hours of extreme fatigue. The entire reaction began 11 hours after the shot and ended 18 hours after that. I am 61 and in excellent health, taking only 25 mcg of levothyroxine daily.

I have a few questions that I hope you can answer. I’ve tried looking at the clinical trial results for Shingrix, but I am not qualified to understand the information as presented.

1. Is it likely that I’ll have a similar (or worse?) reaction to the second shot? The package insert says one can react to the first, second, neither or both.

2. Are both injections identical?

3. When I had chickenpox as a child, I was way sicker than the average kid. Related?

4. If I get shingles, do I have a higher risk for a severe case? -- J.G. ANSWER: Compared with the previous one-time vaccine Zostavax, the new two-dose Shingrix vaccine is much more effective. However, it does have a higher risk of side effects. You have had the most common side effect, though only 10% of people will have symptoms as bad as yours. These symptoms are caused by your body mounting an inflammatory response to the glycoprotein in the vaccine (the new vaccine does not contain a live virus). You clearly have a robust system to fight off infection.

To answer your questions in order: You are at higher risk for a similar reaction for the second shot. I would plan your day accordingly, and premedicate with Tylenol (even though it may make the vaccine slightly less effective). The second vaccine is identical to the first. I have read reports that there have been shortages of the vaccine.

Chickenpox and shingles can affect you both by the virus attacking you and by an exuberant inflammatory and immune reaction. I would guess that you are at lower risk for viral complications (such as infection of the eye, brain or lungs) but at higher risk for symptoms due to your own system, such as high fever. In many cases, the body’s response to infection can be as damaging as the infection itself.

DEAR DR. ROACH: Last year I was treated for gout and was prescribed allopurinol (100 mg) once a day. Approximately five to six months after I started taking it, I became lethargic and was not feeling myself (I am a very young 77 years old) and suddenly lost my sense of taste. I was taken off the medicine and still have little taste sensation other than citrus fruits, apples and spices. I was told that this could last for months, years or forever. Do you have any suggestions for me? I am eating as before, hoping that I will recover my taste. -- N.K. ANSWER: I certainly found that allopurinol can cause loss of taste sensation, but the Food and Drug Administration case reports don’t say how long it can last. 5 Important Medicare Changes Coming For 2020 (NAPS)—If you’re enrolled in a Medicare Advantage plan, or are considering one, you will have more plan choices with a wider range of benefits to help you better manage your day-to-day health needs during this year’s open enrollment period.

Medicare’s Annual Enrollment Period, which runs from Oct. 15 through Dec. 7, gives Medicare enrollees a chance to revisit their Medicare insurance coverage, make sure it still fits their needs and make changes for the coming year.

Online health insurance broker eHealth outlines five of the most important Medicare changes to be aware of this year: 1. New benefits to help those with chronic illness. Starting in January, 2020, some Medicare Advantage Plans will include benefits for people with chronic illness that will help to support their care. A few examples include nutrition services, transportation to doctor’s appointments and home improvements like shower grips and wheelchair access. More traditional services like dental care, vision care and hearing aids also have been expanded.

* On Dec. 13, 1577, English seaman Francis Drake sets out from Plymouth, England, with five ships and 164 men on a mission to raid Spanish holdings on the Pacific coast of the New World. Three years later, Drake’s return to Plymouth marked the first circumnavigation of the earth by a British explorer. * On Dec. 10, 1901, the first Nobel Prizes are awarded in Stockholm, Sweden, on the fifth anniversary of the death of Alfred Nobel, the inventor of dynamite and other explosives. It is believed that Nobel created the prizes out of moral regret over the increasingly lethal uses of his inventions in war. * On Dec. 14, 1911, Norwegian Roald Amundsen becomes the first explorer to reach the South Pole, beating his British rival, Robert Falcon Scott, by more than a month. Amundsen used sleigh dogs, while Scott employed motor sledges, ponies and dogs.

2. Medicare Advantage premiums will be lower next year. The average monthly premium for Medicare Advantage plans is expected to be $23 in 2020, a decrease of 14% from an average of $26.87 in 2019, and down 28 percent since 2017. The average Medicare Advantage premium this year is the lowest it’s been since 2007.

3. Part D drug plan premiums will be lower, too. The average Medicare prescription drug plan premium for 2020 is $30, a decline of 13.5% as compared with 2019 costs. Savings on out-of-pocket costs may be available as well. A recent eHealth analysis of people using eHealthMedicare.com to compare Medicare plans found that fewer than one in ten were enrolled in the lowest cost plan for their personal prescription drug regimen. Those who switched to their optimal drug plan stood to save an average of $900 per year.

4. Seeing a doctor without leaving home will get easier. Many seniors will have access to Medicare Advantage telehealth benefits to connect with health care providers online or by phone. For 2020, more than half of all Medicare Advantage plans will offer added telehealth benefits, reaching up to about 1.7 million enrollees.

5. Medigap plan options are changing. Two of the most popular supplemental plans (so-called Medigap plans) used by those enrolled in original Medicare—Plan F and Plan C—will no longer be available to new Medicare members or those born after January 1, 1955. Fortunately, Plan G, another Medigap policy, offers the same level of coverage provided by Plan F—minus the Part B deductible—and will continue to be available after January 2020. The Part B deductible was $185 in 2019.

Even if you’re happy with your Medicare coverage, eHealth recommends that you review your current coverage during open enrollment because each year plans can and do make changes in costs, physician networks or drug benefits.

Now’s the time to decide which Medicare Advantage plan is best for you.

Mayer’s Cider Mill: Forming the Future; Preserving the Past

T

he farm that was destined to become Mayer’s Cider Mill, the oldest continuously operating cider mill in New York State, was built by Perry Langdon in 1835 and sold to Donat Mayer, who began pressing cider on the property, in 1876. Donat Mayer Jr. was the first to begin making donuts on the property, and donuts, made according to David Bower Sr.’s recipe, are still in high demand, especially apple cider donuts.

David Bower Sr. bought the cider mill from Harvey Mayer, Donat Mayer Sr.’s grandson, in 1962. David Bower Jr. continues historic and family traditions by successfully running the Webster cider mill. The Bower family has also expanded the family’s potent potables interests by opening a winery and offering craft beer and hard cider.

The Bower’s Lake Ontario Winery and Vineyards in Hilton, which will celebrate its 15th anniversary in 2020, sits on property which was originally purchased By Delos P. Tenney. He developed the land, establishing vast apple orchards and growing pears, grapes, plums, peaches and cherries. Now, the Bower family’s winery benefits from some of the richest wine country in New York.

Lake Ontario Winery and Vineyards is situated along what is known as the Niagara Escarpment which stretches from the Genesee River all the way to the Niagara River. It is also commonly called the Lake Ridge as it sits along the banks of Lake Ontario, or the Banana Belt for its shape as it begins north of Rochester with its tip resting at Niagara-on-the-Lake. This fertile region is filled with vineyards and wineries as it features a growing season of at least 220 days, second only to Long Island.

“We are protected from what is called a ‘microclimate,’” commented David Bower Jr. “It makes this a great region to grow native grapes, hybrid grapes, grapes used for European vinifera wines and all kinds of stone fruit.” Bower Jr. plans to bring vineyards back to Webster along Five Mile Line between 2020 and 2021.

The Bowers continue to expand their winery from what was purchased as a farm in 1983 to a destination and event venue comprised of the Lake Ontario Winery and Vineyards, a new Craft Haus and its nearly completed Estate Wedding Barn, featuring open, welcoming spaces, handcrafted Amish furniture, a roaring fireplace and beautifully high ceilings.

At the same time, the 184-year-old cider mill location in Webster is in no danger of being neglected. In fact, quite the opposite: David Bower Jr. has expanded the historic cider mill to include a satellite tasting area for wine, beer and hard cider as well as space to enjoy delicious edibles such as paninis, pizzas, cheese boards, fudge, cookies, and specialty ciders.

“Some things have changed,” explained Bower Jr. “There are more restrictions on products sold directly to customers. Cider

has to be pasteurized using either flash or UV pasteurization, unless the juice is specifically being sold for fermentation.” Even then, customers must sign a paper to that effect, and the jug itself is labeled as being only for the intent of fermentation.

Even so, many more things have stayed the same. “We still use the same press that the Mayer Family used in the 1930’s-40’s,” stated Bower Jr. “It was pressurized by water, and they put hydraulics into it. We also use a Pioneer Press which is a roll press.” The Bowers also continue to make their much-sought-after apple cider donuts, using an original recipe created by David Bower Sr. in 1962.

Bower Sr. also added a home beer and wine making retail store to the cider mill, as well, which sells home-brewing kits and malt extract to those interested in making their own product. “People used to make barrels and barrels of cider,” shared Bower Jr. “It was the poor man’s drink back then. They did a lot of their own fermentation just like they did their own canning.”

Growing grapes and producing wine have captured the interest of Bower Jr.’s son, David Bower III. Bower III, who studied viticulture and enology at Cornell as well as grape and wine production and science at Missouri State University, currently works in North Carolina as the Winemaker & Enology Instructor for Surry Community College’s Surry Cellars, a live winery project where students are taught Viticulture and Enology. He also teaches online classes for Sonoma State University Wine Business Institute in California, and he has won many awards for the schools at which he works and has worked.

Being a Caregiver is a very hard, stressful, demanding job whether it be for a family member or in a professional capacity. The physical, emotional and financial consequences for the family caregiver can be very overwhelming. You said that doing so is considered assault. Does this include someone touching my hearing aids? I have had someone reach out to touch them for one reason or another. Would I be right in calling the police next time someone did this?

As I was pulling into a parking space at [local store] in [local town], I saw a dog in a vehicle that clearly was in distress. I sat there for approximately 5 minutes and decided to call 911 as the car was probably extremely hot inside. After another 10 minutes and still no cop, the owner came out. Give them the opportunity to go out to lunch or have coffee with a friend, do some unhurried shopping, pamper themselves at the hairdresser or barbershop or take a relaxing bath. Just let them get away and take some time for themselves.

If this is not possible, offer to do the grocery shopping, take items to the dry cleaners, help with the house cleaning, shovel the driveway or whatever would be helpful. Caregivers often feel isolated and lonely.

Just being there for the caregiver, who is giving all of themselves to the one they are caring for, is so important.

I was having a bad day and went into the Mexican restaurant, Rancho Viejo on Ellicott St. in Batavia, and a young male waiter spontaneously paid for my meal. As I left the restaurant, we hugged.

Thank you to all the cheerful employees doing any kind of customer service during the holidays. I know from firsthand experience how difficult it can be to stay positive throughout this stressful retail season and service season. Thank you for making everyone’s day a little more jolly!

I have offered elderly persons help finding their car in a parking lot, assisted a person shorter than I in reaching a high shelf in a grocery store or even a child struggling to open a door. You see, there are people in this world who just want to be kind. Please don’t assume that, as you put it, you are “an object for pity” or that they are dwelling on the fact you are in a wheelchair.
