LONG-DISTANCE CAREGIVING (NAPSI)—Chances are, at some point you will be a caregiver to a friend or family member. Providing care and support can be challenging in any circumstance, but caring for a loved one who lives far away presents a unique set of challenges.

When providing care for loved ones who do not live nearby, keep the following three tips in mind.

Plan ahead. Learn your loved one’s medical wishes, contact information for doctors as well as important financial and insurance information.

Research community options. Many older adults need a little help to stay healthy and independent. Area Agencies on Aging and other communitybased organizations can connect caregivers to programs that help with transportation, personal care services, nutrition, home modification and repairs, legal services, falls prevention programs and more.

Care for yourself. The Eldercare Locator can connect caregivers to local resources that can help, including caregiver support programs that can provide training, and support groups and other resources that help caregivers better support their loved ones—and themselves.

The Eldercare Locator can help both care recipients and their longdistance caregivers. Through its website, www.eldercare.acl.gov, and national Call Center at (800) 677-1116, the Eldercare Locator connects long-distance caregivers to resources that can provide assistance where it is needed most—in the care recipient’s own community. The Eldercare Locator is funded by the U.S. Administration for Community Living and is administered by the National Association of Area Agencies on Aging.

--DEAR DR. ROACH: My 18-year-old grandson has just been informed that he has left ventricular noncompaction cardiomyopathy.

He has been VERY active his whole life, playing football, baseball, basketball and soccer all through high school. He had no symptoms.

This was found when getting a physical for college, where he was supposed to play football, and he has been told that is now out of the question. The doctors are saying it’s a very rare condition, and they want to put in a defibrillator ASAP. -- Anon.

ANSWER: Left ventricular noncompaction is a cardiomyopathy (disease of the heart muscle) that is rarely diagnosed, but it probably has been under-recognized. It can run in families, so it’s recommended to examine relatives for the condition. It is diagnosed by echocardiogram, and some cases can be identified through genetic analysis.

LVNC can lead to heart failure, arrhythmias and stroke. There is no specific treatment for LVNC, so people with the condition are treated for the manifestations of their disease. For people with heart failure, this means they are treated with the appropriate medications, which hopefully will prevent or at least slow down any damage to the heart.

Management also can include an automated internal cardiac defibrillator (AICD) if the heart muscle is seriously weakened.

People with LVNC and a history of serious arrhythmias should get an AICD as well. Since arrhythmias are very common in LVNC, AICDs may be used more frequently than with other causes of heart failure.

Also, it is likely that your grandson’s doctor has more expertise than I in managing this condition, and certainly has more knowledge about his heart, so I would take the recommendation for AICD very seriously.

It is strongly recommended he avoid competitive endurance sports and weightlifting. *** DEAR DR. ROACH: I’m 40 and was diagnosed with sarcoidosis recently. It was found in my left eye when my eye doctor noticed inflammation. I also was having vertigo and face pain. It wasn’t until I had a PET scan and they biopsied a lymph node that I got the diagnosis.

I am on steroids and recently started taking methotrexate. I’m having horrible side effects (mostly mouth sores and losing my hair), and my original symptoms are coming back. I feel like my doctors don’t know how to treat this disease. I have been told that it is rare in my area.

Where can I learn more about this disease and see if there are better and more current treatments? -- J.O. ANSWER: Sarcoidosis is a multisystem disease with an unknown cause. It most often affects younger people and women of African descent. The most commonly affected organs are the eyes and lungs, and lymph nodes are commonly enlarged. The severity can range from quite mild to life-threatening. The diagnosis is confirmed by biopsy.

Initial treatment usually consists of steroids, and methotrexate is often used. However, mouth sores and hair loss are both common complications.

To find an expert with special expertise in treating sarcoidosis, I would start with your closest teaching hospital. Even though you haven’t told me about any lung disease, pulmonologists tend to have expertise in treatment of sarcoid, even when it affects organs other than the lungs.

Meatloaf is a dinnertime staple for many families. A favorite of adults and children alike, meatloaf can be prepared however cooks see fit, making it a versatile dish for family dinners. Parents who want to enjoy meatloaf with their families and do so while promoting healthy diets can try the following recipe for “Turkey, Vegetable & Oat MiniMeatloaves with Marinara Sauce” from Michelle Dudash’s “Clean Eating For Busy Families” (Fair Winds).

Turkey, Vegetable & Oat Mini-Meatloaves with Marinara Sauce

Serves 6 Expeller-pressed canola oil spray 1 8-ounce package crimini (baby bella) mushrooms (about 21/2 cups) 1 small yellow onion, cut into eighths 4 cloves garlic, minced 1 tablespoon extra-virgin oil 1 cup dry rolled oats 2 large eggs 1 pound lean ground turkey (or beef or bison) 13/4 cups pasta sauce, divided 3 tablespoons red wine vinegar 1 tablespoon Italian herb seasoning 1/2 teaspoon salt 1/2 teaspoon freshly ground black pepper Preheat oven to 375 F. Coat a 9 x 13-inch baking pan or 6 mini loaf pans with spray. Pulse mushrooms in a food processor until finely chopped and add them to a large bowl. Repeat with onion and garlic.

Place a large frying pan over medium heat and add oil. When oil is shimmering, add vegetables and sauté for 7 minutes or until water releases and evaporates completely, lowering the heat as necessary.

Set aside.

Process the oats until they are of a fine consistency. Whisk the eggs in the bowl used for the vegetables. Add processed oats, turkey, 3/4 cup of pasta sauce, cooked vegetables, vinegar, Italian seasoning, salt, and pepper and stir together with a fork until blended.

Scoop the mixture into 6 mounds on the pan or into each mini-loaf pan, about 3/4 cup each. Shape each mound into a 4 x 2-inch loaf. Spread remaining 1 cup of pasta sauce on top of the loaves, distributing evenly.

Bake on middle rack for 25 minutes, turning pan midway through cooking, until loaves are firm or a thermometer inserted in the middle reads at least 165 F. Allow loaves to rest for 5 minutes before serving.

Recipe Notes: You can finely chop vegetables with a knife rather than using a food processor if you prefer. Whole oats can be added to the meat mixture for a more rustic texture.

Help Reduce Animal Overpopulation One of the easiest ways to prevent pet overpopulation is to spay and neuter animals. Cats can reproduce at very fast rates. According to the Cat Rescue, Adoption & Foster Team of Central Oregon, two uncontrolled breeding cats can create the following situation if they have two litters a year at a survival rate of 2.8 kittens per litter: 12 cats in the first year, 66 cats in the second year, and 2,201 cats in the third year. Cats reach puberty between 4 and 12 months of age. Female cats reproduce between January and September, and might come back into “heat” every 14 to 21 days until they have bred or daylight decreases considerably. Cats can give birth 60 days after they have bred.

ELSIE’S STEWED APPLES Serves 6 to 8 2 to 3 tablespoons unsalted butter 2 pounds apples, peeled, cored and cut into thick slices 1/2 cup honey, preferably sage honey 1/2 cup white wine Zest of 1 lemon Juice of 1 lemon Fresh sage sprigs for garnish 1. Melt the butter in a large skillet over medium heat. Add the apples, turn the heat to high and sauté until they begin to brown on the edges, about 5 minutes. If some are getting too well done, remove them and place on a plate while the rest continue to cook, then return them to the skillet when all are done.

2. Reduce the heat to low and add the honey, wine, lemon zest, lemon juice, and 1/2 cup water.

Cover and allow to cook until the apples are tender but still firm; you don’t want them to turn into applesauce.

3. Serve this in a bowl, tuck in a couple sprigs of fresh sage from your garden, and tell your guests all about sage honey.

6 ways to make bathrooms a safer place Homes should be safe havens. But each year injuries in and around the home contribute to millions of medical visits and many fatalities each year.

Although anywhere in a home can be the scene of an accident, bathrooms tend to be the most dangerous room in the house.

Slippery tile, the presence of water, stockpiled medications, and many sharp and hard edges in a small space pose several different hazards in the bathroom, particularly for young children and people age 65 and up. The Centers for Disease Control and Prevention says falls, which can result in serious injuries like hip fractures and head trauma, are the cause of 80 percent of all bathroom accidents.

Many bathroom accidents are preventable with some easy modifications.

1. Reduce slippery surfaces. Wet tile is a recipe for slick conditions.

Bath rugs with rubber backing can provide traction in the bathroom, as can nonslip mats placed on the floor of the bathtub or shower enclosure. Water-resistant flooring made from recycled rubber is another option. It is softer, less slippery and more forgiving than traditional tile flooring.

2. Install lever-style fixtures. Round knobs in the bathroom can be difficult to grasp, especially for the elderly or those with arthritis.

Lever-style fixtures are easier to maneuver and can help alleviate scalding from not being able to adequately adjust the water temperature.

3. Utilize transfer benches and shower seats. A transfer bench can help reduce injuries that occur when trying to climb over a tub wall. Benches are placed outside of the tub and a person sits and then swings his legs over the ledge. Transfer benches also can be

4. Discard old medications.

Clean out the medicine cabinet of old or expired medications, including both prescription and over-the-counter drugs. This reduces the likelihood of medication confusion, and does not put potentially harmful pills and syrups into the hands of children.

5. Install grab bars. Properly installed grab bars around the shower and toilet area can provide leverage and stability. AARP says many injuries to seniors occur when they are attempting to sit or get up from the toilet. Grab bars or an elevated toilet seat can help.

6. Install motion-detecting lights. These lights turn on automatically upon detecting movement, making them beneficial for people who routinely visit the bathroom in the middle of the night.

Adequate illumination also can help reduce fall risk.

Bathroom safety should be made a priority. Various modifications can make bathrooms safer for people of all ages.

Sledding can be great fun and excellent winter exercise with a bit of preparation. Contributing factors such as the weather, sledding gear and winter wear should all be considered when preparing for your family’s winter outing. Even the most experienced sledding enthusiasts may learn something new from our sledding tips or discover a new favorite sledding hill from those below, recommended by our readers!

Check the weather forecast. Even though the temperature outside may not be especially low, other weather factors, such as wind chill, are important to take into consideration when you are planning to spend several minutes out of doors. Checking an hourly forecast, as well, can allow you to see potential storm fronts and temperature drops and prepare accordingly. Don’t forget to use SPF on any exposed skin!

Dress in warm, waterproof layers. When sledding, more complete coverage is essential, especially for children. Snow pants or a full-body snowsuit of waterproof/water-resistant material should be worn.

While warm gloves or mittens and coats come in absorbent materials, these are not ideal unless they are water-resistant. Boots must also be waterproof/waterresistant. Snow pant cuffs ideally secure around the outside of the boots, preventing snow from landing inside and soaking the socks and feet. Hats should completely cover ears. Underneath this winter gear, wear layers of natural, moisture-wicking fibers, such as cotton, to keep “pockets” of warm air next to the body.

Choose your sled. There is one question that stands out as important when choosing a sled, especially for a child. Does the sled have a steering mechanism? Some sleds have simple steering along the front. Many sleds do not. Younger riders and riders on busy hills may want to avoid sleds which they cannot quickly steer away from others or away from obstacles. Several types of sleds are directed by the way in which the rider leans; however, this may not be ideal for very young riders or for situations in which a rider may need to turn quickly.

Know your limits. Pay attention to your body’s well-being as well as to the well-being of those with you. Conditions such as frostbite and hypothermia can occur with exposure to cold weather. According to the American Red Cross, symptoms for frostbite include a lack of feeling in the affected area, skin that appears waxy, is cold to the touch, or is discolored (flushed, white or gray, yellow or blue). Hypothermia symptoms include such things as shivering which may stop as one’s body temperature continues to drop, numbness, a glassy stare, apathy, weakness, impaired judgment and loss of consciousness. Anyone experiencing any of these symptoms should be moved to a warm environment, and if frostbite and/or hypothermia is suspected, call 9-1-1 and seek medical attention immediately.

GET ON THE FAST TRACK TO BEING ORGANIZED • Keep a cleanup bin handy. Put an empty basket in a central location, using it to gather stray items scattered around main living areas. Once everything has been collected, items can be returned to their rightful place.

• Make the bed. One of the easiest ways to keep a room looking neat is to make the bed each day. Store clean linens in a pillow case so the entire set is together and ready to go.

• Purge closets and cabinets. There’s no point allowing old, stained or damaged items to take up space. Set aside days to clear expired medication from the bathroom cabinet, discard ripped or ill-fitting clothing from the closet, or to dispose of broken toys in a nursery.

• Write things down. Tasks seem more manageable when they are written down in black-and-white and can be crossed off as completed.

Writing things down also helps make the mind feel more organized.

• Exercise caution before buying. Don’t buy more than you need; otherwise, you’ll have to find space for extra items. Also, keep inventory of food and supplies in the house so you’re not unwittingly buying duplicates.

• Sort mail and paperwork. Establish different categories for papers so they can be sorted and discarded as needed. These may include bills, invitations, school papers, and junk mail. Reduce paper clutter by opting for digital correspondence when possible.

• Make one repair at a time. Tackle that source of stress by focusing on one repair and seeing it through to completion.

• Delegate the work. If kids want to help organize, let them. It can be a fun project for an adolescent to rearrange the pantry or offer their ideas for corralling messes.

I got in and drove about a mile and noticed an envelope stuck under my windshield wiper! I pulled over and got it off the windshield. I was thinking, "what the heck?"

On the envelope was written Season's Greetings. Inside was a $5 bill! At that very moment the song came on, "It's the Most Wonderful Time of the Year," then the sun came out, and then I cried tears of happiness. Little
