Pulmonary Nodules Appear, Leading To Concern About CANCER DEAR DR. ROACH: I am a 76-year-old woman in what I thought was excellent health. On a recent coronary calcium CT scan that was ordered because of my age and slightly elevated cholesterol, an incidental finding was described in the report as “innumerable, tiny noncalcified pulmonary nodules bilaterally.” The largest of these is in the “left lower lobe with multiple additional 2- to 4-mm nodules,” and they recommended “clinical correlation and a consideration of a dedicated chest CT.” I found the report quite scary, especially considering my mother and grandfather died of lung cancer (although they were smokers and I never smoked). My primary physician has ordered the chest CT but wants to wait four weeks. She says that the nodules could be caused by inflammation or infection and might resolve on their own.

My fear is that they’re actually lung cancer and waiting four weeks might be problematic, not to mention extremely anxiety-producing. I would appreciate your input. -- M.S. ANSWER: I certainly understand why you are concerned about lung cancer given your family history, but primary lung cancer is very unlikely. Since there are so many of these tiny nodules, I agree with your doctor that these are highly unlikely to be lung cancer, which is generally only a single nodule.

Your doctor is correct that there are inflammatory and infectious conditions that can cause a similar appearance on a chest CT. Some of these infections are serious (including tuberculosis), but people generally have symptoms. So, I think this is unlikely. Exposure to lung toxins, especially silicosis and asbestosis, are generally occupational, but they are another potential cause.

Some cancers can spread to the lungs. While the appearance of nodules on a CT scan in a person with no known cancer would be unusual, if the CT scan in four weeks shows that the nodules are enlarging, then this would be concerning.

Getting another scan in less than four weeks might not be enough time to see a change.

I am sorry about the unavoidable anxiety in waiting for the scan, but it’s likely that the scan won’t show a change and you will be able to stop worrying.

DEAR DR. ROACH: I was experiencing hoarseness while taking ibuprofen. A friend got intestinal bleeding due to taking it. My doctor gave me 7.5 mg of meloxicam daily, but I’m not sure it’s OK for long periods of time. I’m 73 with osteoarthritis and osteopenia.

I exercise daily and eat a healthy diet, and I have high blood pressure that is controlled with losartan and amlodipine. -- T.M.H. ANSWER: Ibuprofen is generally a safe drug, but it does have the potential for side effects -- some of them being serious.

Gastrointestinal bleeding is a serious one, and it is most common in older women. I would not try a second nonsteroidal antiinflammatory drug (NSAID) like meloxicam in a person who has a history of serious bleeding with any other similar drug.

Hoarseness could be due to an allergy, due to direct irritation of ibuprofen on the throat, or from the worsening of acid reflux, which can cause hoarseness if acid gets all the way into the throat.

Meloxicam is also an NSAID but is chemically in a different class from ibuprofen (Motrin, Advil and others) or naproxen (Aleve and others), which are closely related. It’s reasonable to give it a try, but if it causes hoarseness again, then an NSAID probably isn’t a good choice for your arthritis. You could consider acetaminophen (Tylenol) or topical NSAIDs, which do not get absorbed enough to cause an increased bleeding risk.

Distracted Driver!

I am so very thankful to the kind man, lady & yellow lab in the red pickup truck towing a trailer in Hamlin! I was distracted—not from texting or talking on my cell, but mentally not focused on driving due to a recent death of a family member. I went through a stop sign & cut him off. He skillfully maneuvered his truck to avoid a collision. I apologized but want him to know how very thankful I was of his skill & kindness! I also want to make people aware of how we can become a “distracted” driver due to many circumstances. Please don’t drive if you physically or emotionally aren’t up to it. My cat, “Gerard,” is 6 years old and has become very overweight. I can barely pick him up! Servings: 24 1 sheet puff pastry, thawed 5 ounces Brie cheese 1/2 cup Musselman’s Apple Butter 1/4 cup pecans, chopped Heat oven to 400 F. Grease mini muffin pan.

Press seams of puff pastry sheet together. Cut sheet into quarters then each quarter into six pieces.

Use rolling pin to flatten each piece of pastry into square shape. Place one square into each muffin cup.

Put 1/2 teaspoon brie, 1 teaspoon apple butter and 1/2 teaspoon chopped pecans into each muffin cup.

Victor’s Blankets and Blessings Group CRAFTS FOR SLEEP IN H E AV E N LY P E A C E

In a secluded basement corner of First Presbyterian Church in Victor, a group of knitters and crocheters selflessly and determinedly makes an impact for Rochester’s most vulnerable children.

Blankets and Blessings, informally known as Stitch ‘n Chat, gathers monthly to produce colorful blankets.

These talented artisans, armed with knitting needles and crochet hooks, don’t shower their wares on grandchildren. Instead, they donate to the Rochester chapter of Sleep In Heavenly Peace, a volunteer organization that has assembled and delivered more than 1,400 beds in six years to children ages 3-17.

On a recent Tuesday, when morning temperatures hovered near single digits, five women gathered around a conference table, secure and snug in their commitment to make a difference for the children. Slip, purl, turn, and bind. They worked with 100 percent acrylic worsted weight yarn on individual, distinctive pieces. Nearby, six quilts were draped over chairs, awaiting delivery through SHP Rochester.

Their vivid work will land upon one of SHP’s 75-inch wooden bed frames that each come with a mattress, pillow, comforter, and sheet sets. Blankets and Blessings connected with SHP more than a year ago.

The knitted and crocheted blankets are heartwarming and treasured by children.

Blankets and Blessings organizer, Brenda Dean of Fairport, follows SHP’s Facebook postings about children receiving free beds, often spotting blankets her colleagues have made.

“I love seeing the pictures,’’ said the retired IT software consultant. “There is one child … He has this big smile around his face with his blanket wrapped around him.

I just want to hug him. We are putting our love around these children through our blankets.’’

SHP Rochester president Donna Ritchie said the blankets have become an essential part of the mission, saying, “When our volunteers tell the children that the cozy blanket we’ve given with their bed was made just for them, their eyes light up. It makes them feel special. The moms are so touched.

They are blown away that someone put that much time and work into making something so special for their child.’’

During the pandemic, Dean joined an online group through a North Georgia Episcopal church that made blankets for homeless individuals. Two years ago, she turned her attention to her own community and soon convinced her friend, Donna Schake, retired after 36 years as a lab technician at Strong Memorial Hospital, to launch the Victor ministry.

They avoid talking about national politics, but Victor politics are in play. Frayda said, “I haven’t seen any boxing matches yet!’’

When they counted grandchildren Schake decided she was getting off easier than the rest: “It means I have less Christmas shopping to do.’’

Roberts chuckled. “You don’t have to shop for this generation anymore. You just give them a gift card or cash.’’

On this day, the conversation was crisp and light-hearted as the starting lineup of knitters gathered: Eleanor Pagano, who taught preschool through Nazareth University teacher training classes; Barb Cromwell, a retired accounts receivable manager for the city of Canandaigua; Kathy Roberts, retired after 25 years as a library aide for Macedon Elementary School; Christine Frayda, a retired software and chemical engineer.

The group laughed. But they remain united and serious about their mission.

Schake was there on this day, but Dean had been benched by hip surgery. Dean said, “You never, ever know what you will talk about. They have become like sisters to me. They’ve been a tremendous help with my recovery.’’

“I believe we’re here to help others and give them a hand,’’ Dean said. “I hope our blankets will make a child warm, sleep well, and help them when they go to school. We want children to be what they are supposed to be. You can’t learn if you don’t sleep.’’

DID YOU KNOW? The American Heartworm Society advises dog owners to have their furry companions tested for heartworm annually. Such tests can typically be conducted during routine preventive care visits to the veterinarian. The AHS notes that such tests are necessary even for dogs that are taking year-round heartworm prevention medications. Tests can confirm that such programs are working or indicate if they are not effective. The American Veterinary Medical Association notes that antigen tests are the most commonly performed tests. These tests detect the presence of adult female heartworms and they are typically highly accurate. If a test is positive, veterinarians may recommend further testing, including radiographs, a blood profile and an echocardiogram. Such tests can determine a number of things about the infection, including its severity. The AVMA also notes that the additional testing can help determine the best course of treatment. Cat owners should know that it’s more difficult to detect heartworm in cats than in dogs. The AHS indicates that cats are much less likely than dogs to have adult heartworms, making the disease harder to detect in felines. Veterinarians may recommend cats receive both the antigen test and an antibody test, which detects exposure to heartworm larvae. The AVMA notes that more testing may increase the likelihood of detecting the disease in cats.
