LIFE MOVES FAST FOR ONE BUSY PARENT Improving Health PARENTS OFTEN MOVE FOCUS FROM THEMSELVES AND USE UP THEIR ENERGY TO TAKE CARE OF THEIR CHILDREN AND FAMILY INSTEAD. This fits the bill for Kayla Simpson, a mother with a busy family of three children.

Kayla is a working mom from Newton with a career at Grinnell Mutual in Grinnell. Over the years she has focused on her children and never really slowed down to focus on herself. Her family had always struggled with weight, and with each pregnancy she had gained more weight that never seemed to come off.

“I had tried every diet out there. I would have success at first, but then gain it all back plus more.

I had had enough,” Simpson says.

She knew it was time to look into a major lifestyle change. She turned to the support of her own mother, Paula Klosterboer, who had received weight-loss surgery (a Roux-en-Y Gastric Bypass) through the Grinnell Bariatric program in 2009 and has kept off over 160 pounds.

“My mom had major success with her weight-loss surgery. I knew I would have the support from her and the Grinnell Bariatrics team to accomplish the results I needed for my future,” Simpson says.

She adds, “I never really considered anywhere else because of the reputation that the Grinnell Bariatrics program has and because of the ease and convenience of working in Grinnell.”

Simpson underwent a gastric sleeve procedure with David Coster, MD, Surgical Associates, on January 30, 2018, and to date has lost over 130 pounds.

She notes the care she received even months after surgery was incredible. Even though she is not required, Kayla still meets with Stephanie Noun PA-C, Surgical Associates, program director for Grinnell Bariatric, monthly to have professional accountability to stay on track.

“I had an amazing experience with Grinnell Regional Medical Center and with Grinnell Bariatrics. The program was very informative and I felt very knowledgeable going into my surgery.”

Kayla wants others to know that they are not alone and to feel inspired to take the leap and look into this opportunity that awaits.

“I’ve gained so much more than the obvious benefits. Not only am I healthier and thinner, I have finally found myself. At 31, I am more comfortable in my own skin than I have ever been and that is attributed to the Grinnell Bariatrics team and this experience.

When Donald Crawford experienced a sharp pain in his leg during his walk in 2011, he didn’t think much about it.

“Out of nowhere a cramping, tight knot feeling happened in my right calf. It lasted about ten steps and then it was gone,” Crawford says.

“Today I realize that pain was the first signal to a serious problem.”

Crawford continued to experience pain in his legs until it became so intense, he was no longer able to work during the summer of 2013. Without medical insurance or the ability to cover the cost of a doctor’s visit, Crawford was reluctant to seek out medical care.

“I didn’t know what I was going to do,” Crawford says.

Crawford was new to the Grinnell area and didn’t have a primary care provider, so his first visit was to the UnityPoint Health® – Grinnell Regional Community Care Clinic.

The clinic provides care to people who have limited access to medical care, have a low income or are uninsured or underinsured with limited access to medical care.

Crawford’s appointment was with Kristin Phelps, PA-C. After discussing his symptoms, Phelps suggested Crawford have an ultrasound performed on both legs.

After being in pain for two and a half years, Crawford learned he had peripheral arterial disease

(PAD). PAD is commonly caused by atherosclerosis, a build-up of plaque deposited in the walls of blood vessels. This makes it harder for blood to flow to arms and legs.

The ultrasound found obstructed arteries in both of Crawford’s legs.

Phelps contacted the Volunteer Physicians Network (VPN) to assist Crawford. VPN provides free specialty health services and is the only program providing access to free specialty health care in the State of Iowa for residents who are at risk, have low income or are uninsured or underinsured.

The diagnosis caught Crawford’s condition early enough to prevent him from losing either limb.

The vascular surgeons at the Iowa Methodist Surgery Center performed a six-hour surgery to relieve the clot in each leg and to bypass a clot found in his abdominal aortic artery.

Crawford credits Phelps and the staff at the Community Care Clinic with discovering his illness and the work they did to have VPN perform surgery as saving not only his legs, but also his life.

After Crawford’s surgery, he was referred to Grinnell Regional Family Practice for further testing and to establish a primary care provider and medical home. A medical home is a coordinated effort by providers to replace episodic care based on symptoms and illness with directed, whole person care.

The goal is to build a long-term

healing relationship between patients and their primary care providers.

He has regular physical exams and health screenings with Phelps to make sure he is recovering well.

“The medical providers who helped me throughout this entire process treated me like I was a member of their own family,” Crawford says.

“I will be forever thankful.”

NOT A DIET, BUT A LIFESTYLE

Mediterranean Diet If you’re looking for a way to improve your health by making a few positive tweaks to your eating pattern, the Mediterranean Diet is a great option to consider.

While the word “diet” tends to lead people to think of restriction and unpleasantness, the Mediterranean Diet is really more of a lifestyle approach. It incorporates healthy eating and other components that characterize the culture and lifestyles of at least 16 countries bordering the Mediterranean Sea. Health Benefits Research has shown that following the Mediterranean Diet practices can lower a person’s risk of heart disease and can improve overall health. This lifestyle naturally limits the amount of highly processed foods you eat, which often have more sodium, saturated fat and added sugar. It also highlights enjoyment of your whole dining experience and flavorful foods.

What to Eat? Core foods encouraged every day are whole grains, fruits, vegetables, beans, herbs, spices, nuts and healthy fats such as olive oil. Fish and seafood are to be eaten about twice a week. Dairy foods, especially fermented dairy like yogurt and cheeses, are encouraged frequently in moderate portions. Eggs and occasional poultry are included in this eating pattern, but red meat and sweets are only had on occasion. Water and wine (for those who drink) are considered typical beverages.

If you are looking to try a new food, consider using farro in a side dish. Farro is an ancient grain that is a kind of wheat. It has a nutty flavor, a satisfyingly chewy texture and can be served warm or cold. Whole farro is a whole grain that requires soaking overnight, but semi-pearled farro has some of the bran removed and allows for quicker cooking time. If you cannot find farro in your grocery store, you could substitute brown rice, wheat berries or quinoa in the recipe on opposite page.

She also leads “Preventing T2” (a group organized to prevent type 2 diabetes) and provides counseling. In 2018, Lily was named the Young Dietitian of the Year for her leadership and expertise.

Winter Farro Salad Ingredients ½ cup chopped walnuts or another nut of your preference 1/ 3 cup plus 2 Tablespoons extra-virgin olive oil 1 small shallot, diced 1 cup farro, rinsed 2 sprigs fresh oregano ½ teaspoon salt Juice of 1 large lemon

Roasted Vegetables People often say they don’t like the bitter flavors some vegetables exhibit. Roasting vegetables in your oven is a great way to bring out a vegetable’s natural sweetness. It can also be a nice way to use up leftover vegetables. Root vegetables, like sweet potatoes, beets, parsnips and carrots, make good roasting vegetables, but you can also use vegetables such as broccoli, brussels sprouts, bell peppers, onions and zucchini.

Juice of 1 orange 1 small bunch Tuscan kale, stemmed and finely shredded ½ cup dried cherries or 1 cup fresh cherries, pitted and halved ½ cucumber, peeled and cut into ¼-inch pieces 4 ounces feta cheese

Directions

TIPS:

1. Put the walnuts in a small skillet over low heat. Cook, stirring frequently, until lightly toasted and fragrant, 8 to 10 minutes.

Cool completely on a small baking sheet.

1. Cut vegetables into uniform, bite-size pieces.

Remember, the softer the vegetable, the quicker it will roast.

2. Heat a medium saucepan over medium-high heat. Add 2 Tablespoons of olive oil and the diced shallot and cook, stirring often with a wooden spoon, until the shallots have softened and are fragrant, about 3 minutes.

2. Toss the vegetables in a bowl with about 1-2 tablespoons of oil (this is a good amount of oil for about 5 cups of cut up vegetables). They should be evenly coated.

3. Spread out vegetables on a pan in a single layer.

4. Sprinkle with salt, pepper, Italian seasoning and other spices/herbs you prefer.

5. Bake at 425°F for 30-45 minutes, turning the vegetables every 15 minutes.

3. Add the farro to the saucepan and toast it in the olive oil, stirring often, for about 4 minutes. Reduce the heat to medium and stir in 2 cups of water, the oregano and ½ teaspoon salt. Bring to a simmer and cook, stirring occasionally, until the farro is cooked through and tender, about 25 minutes. Remove the oregano if using fresh sprigs, drain the farro and set aside.

4. Whisk together the lemon juice, orange juice, remaining 1/3 cup olive oil and remaining ½ teaspoon salt in a large bowl.

Add the warm farro and toss to coat. Add the kale, cherries, cucumber and walnuts and toss to mix. Crumble in the feta cheese, toss gently to mix and then enjoy!

Makes 6 servings. Nutrition information: 400 calories, 25 g fat, 9 g protein, 35 g carbohydrate, 145 mg sodium, 7 g fiber.

Doyle is a certified registered nurse anesthetist in the Warren H. Bower Surgical Center. Doyle earned a master’s degree in nursing anesthesia from Rush University Medical Center and a master’s degree in nursing from Louisiana State University School of Nursing.

Dr. Hillebrand specializes in hepatology, the study and treatment of liver conditions. He earned his bachelor of science degree from Iowa State University and his medical degree from the University of Iowa College of Medicine.
