This past June, Connecticut became the first state in the country to require health

insurance coverage for fertility preservation based on medical necessity. Cancer

survivor, Melissa Thompson, shares her courageous journey of being diagnosed with breast cancer to becoming a patient advocate who worked tirelessly to have this new law passed.

Saturday, November 19 was an evening to remember as Greenwich Hospital held its annual gala, which raised significant funds for the hospital’s cardiovascular services.

More than 350 supporters, including special guests Jill and Harry Connick Jr., joined in the Parisian cabaret themed festivities at Greenwich Country Club. Gretchen Bylow, Kim Nichols, MD and Jennifer Seidel chaired the event. At the gala, Greenwich Hospital President Norman G. Roth presented James R. Sabetta, MD, director of Infectious Diseases, Internal Medicine and assistant chief of staff, with the President’s Award for his many years of dedicated service to the hospital and his outstanding achievement and commitment to his patients.

2017 Under the Stars

Guests were delighted by the soulful rock sound of Gavin DeGraw at this year’s Under the Stars event that raised over $700,000 for Women’s and Children’s Health at Greenwich Hospital. DeGraw is a GRAMMY® nominated, multi-platinum selling singer and songwriter, who performed many of his chart topping hits including “Not Over You,” “I Don’t Want To Be,” “She Sets The City On Fire” and his most recent hit song, “Making Love With The Radio On.” The June 24 benefit took place at Riverside Yacht Club and drew a crowd of music lovers and hospital supporters alike from Fairfield and Westchester counties.

Greenwich residents Karen Brown, Andrea Sinkin Jaffe, Carol Swift and Jennifer Turano served as this year’s co-chairs. During the evening’s program, Greenwich Hospital President Norman G. Roth noted the broad range of women’s and children’s health services that will benefit from the event. “Our medical experts care for the tiniest newborns up to very complex cancer patients, all right in our community,” he said. In thanking the audience and volunteers, Roth recognized that support from the community is what helps the hospital attract top clinicians, purchase the latest in technology and stay ahead of the latest developments.

Philip McWhorter, MD President’s Award Recipient

hen young Philip McWhorter set out for college at

St. Lawrence University in upstate New York, he was a physical education major on course to becoming a collegiatelevel coach. But in the fall of his junior year, he realized coaching wasn’t for him, often joking that he decided to pursue a career with a little less pressure. After taking extra prerequisite courses the following summer, he was ready to apply to medical school.

As chief of surgery, McWhorter leads with vast experience, humility and a great sense of humor. “His professionalism and dedication to medicine are unparalleled, and his leadership and surgical expertise continue to set examples for younger surgeons,” said Spike Lipschutz, MD, the hospital’s chief medical officer.

When asked how surgery has changed since the beginning of his career, Dr. McWhorter referenced two major factors – precision and technology. “At one time, most diseases were treated surgically with really big operations.

As we’ve come to understand the nature of the disease and the way things work scientifically, the surgeries have been able to become smaller and more precise. Radiation therapy, chemotherapy, improved gastroenterological techniques and medications allow for a team approach to issues that at one point all had to be done surgically,” he said.

Dr. McWhorter in the operating room When not in the operating room, Dr. McWhorter is likely to be found spending time with his family.

He just celebrated his 50-year wedding anniversary with his wife, Linda. They are active members of First Presbyterian Church and keep busy with their three children and six grandchildren. All are sports enthusiasts who enjoy being outdoors.

At Greenwich Hospital’s annual fundraising gala, Dr. McWhorter received the President’s Award for his many years of dedicated service to the hospital. Honoring Dr. McWhorter was an easy decision for Greenwich Hospital President Norman G. Roth. “Dr. McWhorter is a pillar of the Greenwich community,” said Roth. “Throughout his distinguished 40-year career, his leadership has had great impact on Greenwich Hospital.”

But Dr. McWhorter has never sought awards nor accolades.

“I’m just a workaday surgeon who keeps showing up,” he said. “I do this for the people I work with in the trenches and for the patients.” His good nature, love for his work and commitment to helping others are admired by all whose lives he touches. n

leads to success for oncology nursing development When Fred Flynn’s wife, Susan, died of ovarian cancer after a three-year battle, one thing was certain. He knew he wanted to do something special to honor her memory while giving back to an organization that helped his family through a most difficult time.

Flynn’s inspiration to develop the Susan D. Flynn Oncology Nursing Fellowship Program began with the work of Donna Coletti, MD, medical director of Palliative Care at Greenwich Hospital, and her team who managed Susan’s care during the last few weeks of her life in 2013. In September of 2013, he developed and successfully led a fundraising program involving a “matching gift” pledge that generated over $140,000 in support of palliative care. “I wanted to be sure that Dr. Coletti had the resources required to meet the growing patient services needs of the department and, in doing so, help raise awareness and broaden the donor base for her valuable function at the hospital,” he said.

But Flynn still wondered what other support he could provide. An idea came to him one evening as he was watching television and browsing through a brochure about the Carol Ghiloni Oncology Nursing Fellowship at Massachusetts General Hospital.

During the eight-week summer session, interns from top undergraduate nursing schools are immersed in applied oncology training. They work alongside veteran nurses in all areas of cancer care, including medical and surgical oncology, clinical research, palliative care, infusion services, radiation therapy, home hospice services and patient support programs.

This first-hand experience allows future oncology nurses to begin their careers with broad clinical exposure and expert training in compassionate care. Opportunities to attend educational programs within the hospital and meetings with key staff members provide added learning experiences. In 2016, the program’s scope was expanded to include a new pediatric oncology nursing track at three leading pediatric hospitals.

Former Flynn Fellow Begins her Career At Greenwich Hospital, Emily Kopas, RN completed her fellowship in 2015 and has been working in inpatient oncology services since August of 2016. Calling her internship an “eye-opening experience that gave me invaluable skills,” she praised the in-depth program for giving her “direct clinical experience in providing highquality, compassionate and palliative care to oncology patients.”

Kopas’s interest in oncology began when her grandmother was treated for cancer at Greenwich Hospital, but it was through her internship that she fell in love with her work and decided on oncology nursing as Kopas shares a tender moment with a career path. “I am a patient. grateful to this day for the mentorship and professional development opportunities Mr. Flynn provided me through this program,” she said.

Kopas also said she was inspired by the dedicated and passionate oncology team at Greenwich Hospital for motivating her to pursue her career as an oncology nurse. She has already started classes to become a nurse practitioner and also plans to earn certifications in palliative care and chemotherapy.

Elements for Success Flynn thanks the program’s preceptors and nursing staff for being key to its success. Two years ago, he earmarked significant funds from his annual golf fundraiser to further invest in professional development for oncology nursing staff at Greenwich Hospital. Planned program elements include financial sponsorship of advanced education certifications, memberships in key professional organizations, attendance at leading industry conferences, and various technical and professional skills training.

“It was a way to reinvest in the people who make my program so successful,” Flynn said. “Enthusiastic preceptors and highly skilled nursing staff serve as role models, mentors and coaches who foster extensive learning within the students.”

Flynn has attended every fellowship graduation since 2014. “It’s the best part,” he said. “At the ceremony, the students present an evidence-based research project on some aspect of oncology to a room full of many subject matter experts, which always amazes me.” He keeps in touch with all the program’s graduates, many of whom are working as oncology nurses at their host hospitals or at some of the country’s other leading cancer hospitals.

Flynn also credits the entire Greenwich Hospital organization for fully embracing the program, speaking highly of the strong commitment from top administrators to the entire nursing staff. “No one puts the kind of institutional commitment and ownership into the program that Greenwich Hospital does. Everyone is committed to the development of these aspiring oncology nurses.

It’s a wonderful learning environment and I’m very grateful for that.”

Kristina Capretti, RN, oncology program director, and Irene Piazza, RN, nurse manager of outpatient oncology services, oversee the fellowship program. “As mentors to the Flynn students, Irene and I are given the unique opportunity to invest in the education of future nurses and support the growth of oncology nursing care,” said Capretti.

“It’s energizing to the nursing staff because they’re happy to see the hospital focusing on attracting the best and brightest successors and staff support for them,” added Flynn. “It’s a symbiotic relationship where everyone wins.”

A concept that originally began to train one student intern at Greenwich Hospital continues growing to this day. The Susan D. Why nutritional support aids in the healing process

Most people think of integrative cancer care the same way they think about integrative medicine. Both incorporate nonconventional or complementary therapies into the care of the patient. Traditionally, these include acupuncture, Chinese medicine, herbal medicine, massage therapy, yoga and other supportive measures to help patients through their treatments for cancer and other diseases.

D. Barry Boyd, MD, MS, senior oncologist at Smilow Cancer Hospital’s Greenwich Hospital Campus, is a pioneer in the field of integrative cancer care, with a concentration on nutritional support for cancer patients. During his first several years in practice, the universal question patients had was, “What should I eat?” At that time, the majority of nutrition-focused healthcare was limited. Few people understood the role of nutrition as it related to diabetes and the obesity epidemic. This fueled Dr. Boyd’s interest in nutrition as a central concept to understanding the role of integrative medicine.

“Because there is so much less physician awareness and education regarding nutrition, it has become a wastebasket of theories filling the internet with ideas about treatments that are often alternative, nonconventional and dangerous,” he said. “The field of supplements and dietary approaches that are designed to avoid the need for conventional therapy is misleading.”

Too much of a good thing “We are learning more and more about the risk of taking too many micronutrients,” said Dr. Boyd. “For example,

very high levels of B complex have recently been shown to dysregulate metabolism and enhance cancer risk.”

He explains that people are taking supplements that are thousands of times more than what a regular, healthy diet would provide.

Many people are not aware of the dangers of taking high levels of nutrients because the supplement industry is protected by The Dietary Supplement Health and Education Act of 1994, a law passed by Congress to prevent the aggressive overview of dietary supplements. Supplements are often promoted to consumers without being carefully overseen or proven as safe and effective.

“People come to me expecting me to promote the use of supplements, but they end up getting an earful about the dangers and precautions,” said Dr. Boyd. “Patients in ill health may be nutrient- deficient, but basic supplementation, not excessive doses, is all that is needed to bring them up to normal levels.”

Increasing survival through a healthy diet and lifestyle Along with conventional medicine, Dr. Boyd provides a cancer recovery plan for patients, which includes education on the role nutrition and exercise play in their treatment and overall health. “To do integrative medicine well, you need to really understand cancer as a biologist, not just a physician,” said Dr. Boyd, who studied the origins of cancer and their relationship to healthy versus unhealthy lifestyles. “There’s a difference between thinking outside the box and recognizing that the box is bigger.”

Maintaining a healthy diet and lifestyle is increasingly recognized as an approach that aids cancer treatment and boosts survival. Studies have shown the supportive roles of vitamin D, exercise and energy balance are central to positive disease outcomes. “The key is to educate patients about this,” said Dr. Boyd. “This is no longer alternative medicine, but a part of mainstream care.”

A combination of a healthy, plant-based diet and avoiding excessive fat, sugar and highly processed foods is best, according to Dr. Boyd. Although there is no magic food, coffee has been universally shown to reduce the risk of many cancers as well as diabetes. A recent Harvard study showed that drinking more coffee can reduce the occurrence of colon cancers, as coffee is highly enriched in plant-based compounds that are very potent antioxidants. Through its effect on inflammation in the liver, coffee also reduces insulin levels.

People with obesity, diabetes, metabolic syndrome, high fat/high-sugar diets and inactive lifestyles have a greater cancer risk because they have too much insulin, which

encourages cancer cells to replicate faster. Even people who are at a normal weight can have high insulin levels. Growing evidence shows this type of metabolic obesity is associated with breast and colon cancer. Dr. Boyd helps patients focus on diets that influence insulin levels and metabolic status, which he explains is the heart of integrative medicine.

Care after cancer “When patients finish treatment for a curable cancer, they often feel incredibly vulnerable,” said Dr. Boyd. Appropriate diet and lifestyle approaches help them feel empowered, causing a positive psychological effect.

Enhanced physical activity after a cancer diagnosis has been shown to significantly reduce the recurrence of prostate, colon and breast cancers. Educating patients about this gives them something they can do on their own to reduce their risk, along with any feelings of helplessness. “By becoming more empowered, patients can then take a more active role in their future well-being,” said Dr. Boyd.

Controlling weight, becoming active, and reducing stress are not simply nuances of basic well-being; they are absolutely necessary for the successful treatment of cancer.

Even with the best medical care, including chemotherapy, a patient’s cancer treatment can be sabotaged if these areas are neglected. n

MELISSA’S LAW FOR FERTILITY PRESERVATION Connecticut’s landmark legislation opens access to fertility treatment On June 20, 2017, Connecticut Gov. Dannel Malloy signed and enacted Melissa’s Law for Fertility Preservation, ensuring that individuals diagnosed with cancer do not have to choose between their lives and their reproductive futures. The landmark legislation made Connecticut the first state in the country to require coverage for fertility preservation based on medical necessity.

Patient advocate Melissa Thompson was a catalyst to the bill’s passage. Her personal battle with Stage III breast cancer at the age of 32 fueled her passionate work to change the law for future cancer survivors.

“Many cancer patients don’t appeal insurance decisions because they are too sick to fight and end up going into medical debt. I appealed the reversal, but the insurance company kept denying it.”

A Devastating Diagnosis Just five weeks after giving birth to her daughter, Poppy, Thompson received the devastating news that she had cancer.

She described her delivery at Greenwich Hospital as normal, but when she began having persistent, high fevers, she knew something was amiss. Thompson was originally diagnosed with mastitis, a breast tissue infection frequently caused by clogged milk ducts in breastfeeding mothers.

Two courses of antibiotics did not alleviate her fevers. Sick and exhausted, she was sent to see a breast surgeon, who told her she likely had an abscess, but ordered a biopsy to be certain. Shortly thereafter, Thompson received the results. It was cancer.

“During the first few weeks of a cancer diagnosis, there is a lot of anxiety while waiting for test results,” said Thompson, who fell into a rare category of pregnancy-associated breast cancer. She needed a double mastectomy to remove seven centimeters of tumors.

During an initial appointment, her breast surgeon recommended that she see a fertility specialist. Thompson was grateful for the advice, as patients are often not informed that chemotherapy can cause infertility. According to the American Cancer Society, only 13 percent of cancer patients are educated about their fertility options.

“This was psychologically difficult as a brand new mom,” said Thompson. “I was learning to juggle being a mom with feeling sick and discovering I had cancer. The joy of new life and the fear of mortality were two concepts that did not mix.”

The timeframe is limited between diagnosis and starting treatment with surgery and chemotherapy. This meant Thompson needed to decide quickly if she wanted to freeze her eggs and preserve her chance to have more children. Thompson’s insurance company preauthorized the procedure. “The fertility treatment process took 16 days. I had egg retrieval on a Friday and a double mastectomy the following Monday,” she said.

Two days before her egg retrieval, Thompson’s insurance authorization was reversed on the grounds that she had cancer, not infertility. Having already gone through two weeks of giving herself hormone injections, and breast surgery set for the following week, she went through with

the egg retrieval, charging the $12,000 procedure on her credit card. “I went into this with the understanding the procedure was covered, but if I was presented with the choice of spending $12,000 on it, it would have been a difficult decision to make,” said Thompson.

Following surgery, an aggressive course of chemotherapy caused Thompson to spend several weeks in the hospital.

When she finally got home and started to feel better, she opened her mail to discover that her insurance appeal window had closed. “Many cancer patients don’t appeal insurance decisions because they are too sick to fight and end up going into medical debt. I appealed the reversal, but the insurance company kept denying it,” she said.

Thompson reached out to state Rep. Caroline Simmons of Stamford at the suggestion of the Oncofertility Consortium at Northwestern University, which she first called for help with her appeal. “Caroline was so wonderful,” she said. “She personally wrote a letter to my insurance company and put me in touch with the Connecticut Insurance Department and the Connecticut State Office of Healthcare Advocacy.”

“I was fortunate to get to know Melissa Thompson when she lived in my district in Stamford and to learn about her incredible story of bravery and perseverance,” said Simmons.

“Melissa has devoted herself to passionately advocating for insurance coverage for fertility preservation, so that other women do not have to experience the uncertainty and pain that she went through.”

Passing the Bill The Connecticut Insurance Department, which regulates the insurance industry in the state, told Thompson that her insurer was correct in that she was not covered for fertility preservation. Based on this decision, the Office of Healthcare Advocacy was unable to do anything further to help with her case.

“It felt really wrong,” she said. “They were effectively saying, ‘We know you’re going to have treatment that causes infertility, and you’ll be covered for in vitro fertilization once you’re infertile.’ Preserving fertility beforehand just seemed like a much more logical option.”

In the 1960’s, when he was 12 years old, Dr. Hollister became the first male candy striper at Greenwich Hospital. These wellknown volunteers, who were identified by their red and white striped pinafore dresses, called an “emergency meeting” about what he should wear.

Spotlight on Oncology Leaders at Greenwich Hospital

“Leading the team in the department of radiation oncology has been rewarding. I work with wonderful colleagues. We have built our volume and started new programs over the years, such as intraoperative radiation therapy, something many hospitals do not yet offer.”

Ashwatha Narayana, MD Director, Department of Radiation Oncology Education and Work Experience • Graduated from Bangalore Medical College, India

Dr. Narayana was instrumental in bringing brachytherapy, stereotactic radiosurgery, liquid radiation therapy and intraoperative radiation therapy to Greenwich Hospital.

• Attended Medical School at All India Institute of Medical Sciences in New Delhi, India • Completed his residency in radiation oncology at Loyola University Medical Center in Chicago, IL • Began his academic career at the University of Cincinnati and then served as head of the adult brain tumor section in Radiation Oncology at Memorial Sloan Kettering Cancer Center.

• Became the director of the neuro-oncology program and the Gamma Knife program at New York University with a joint appointment in both Radiation Oncology and Neurosurgery, before coming to Greenwich Hospital in 2013 as the director of Radiation Oncology.

Most Rewarding Experience Dr. Narayana has received the “teacher of the year” award from his residents five times in his career.

He keeps in touch with his past students, referring to them as his other “kids.” Although he loves teaching and research work, he most of all loves being a clinician and helping patients.“The beauty of my work here at Greenwich Hospital is that I have the ability to really get to know my patients in a community setting. I missed this sense of community when working at the university hospitals in New York.”

He has given over 100 lectures as an invited speaker around the world.

Having quality time to offer each of my patients is special. Our affiliation with Yale allows patients to have access to the very best protocols and clinical trials, but it is important to be available to patients and connect with them, looking beyond treatment and evaluating their health as a whole person.”

Dr. Narayana lives in Scarsdale, NY with his wife Shaila, a pediatrician.

They met as classmates in medical school in India. They have two sons, Aditya and Ashay.

“I knew I wanted to be a doctor since childhood. I always knew I wanted to help people. I also knew immediately upon entering medical school that I wanted to go into the field of Oncology.”

“Many times, we are treating a highly educated population who often seek second opinions at other hospitals, but they always come back to Greenwich Hospital for the best in treatment and compassionate care.”

Dr. Narayana loves the outdoors, especially rowing his scull along the Hudson River.

Linda LaTrenta, MD Director of Breast Imaging Education and Work Experience • Graduated from Harvard/Radcliffe University • Attended Medical School at Cornell University Medical College

Dr. LaTrenta grew up in Honolulu, Hawaii and was valedictorian of Punahou School in 1981.

• Completed her internship at Brigham and Women’s Hospital in Boston, MA and internal medicine residency at New York Presbyterian • Decided to specialize in radiology and completed her radiology residency at New York Presbyterian • After completing a breast-body fellowship at Memorial Sloan Kettering Cancer Center, she joined the staff there in the breast division • Began her career at Greenwich Hospital in 2003 as the director of Breast Imaging

Most Rewarding Experience “The most gratifying part of my job is finding small node negative breast cancers by imaging and not only theoretically saving someone’s life, but perhaps saving them from more extensive surgery or the need for chemotherapy. I also derive satisfaction from giving someone a negative result and relieving their anxiety. If the result is not negative and a biopsy is required, I try to make that experience less stressful by facilitating the process, addressing questions thoroughly and accurately, and providing compassionate care.”

Dr. LaTrenta knew she wanted to be a doctor when she was 12. Coming from a family of teachers, she knew she wanted to have a direct impact on people’s lives.

She became even more determined to pursue a career in medicine when her mother was diagnosed with breast cancer.

During medical training, the

evolution of the diagnosis and treatment of breast cancer shaped her interest in breast imaging.

“When not at work, I enjoy family life in Greenwich with my husband and two teenage children. My creative outlet is cooking, and eating! And I also love to travel!”

Dr. LaTrenta with her husband, Kurt Bloedel, on a trip to Provence.

“My kids joke that when I retire from being a doctor, maybe I could help the world by starting a food truck.”

“Breast cancer treatment has become a scientifically driven, multidisciplinary team effort that involves radiologists, surgeons, oncologists, radiation therapists, genetic counselors, nurse practitioners and social workers. That cooperation has not only enhanced the intellectual challenge for doctors, but improved the patients’ experience and increased survival.”

Susan Brown, RN, MSN, executive vice president of Operations and Anna Cerra, RN, DNP, vice president of Patient Care Services and chief nursing officer, discuss the critical role oncology nurses have in the healthcare setting.

What are some of the most important tasks that oncology nurses do?

Cerra: “Our nurses take care of oncology patients in both an inpatient and outpatient setting. Patients receive chemotherapy infusion at the Bendheim Cancer Center and Long Ridge Medical Center, as well as on our inpatient floor at the main hospital. These nurses are dedicated to providing care and comfort measures for patients and their families.”

Brown: “All of our oncology nurses are certified in giving chemotherapy, which requires extra schooling and a certification exam so that they are specialized in giving these very powerful medications.”

Cerra: “There is a lot of growth in this field and nurses receive continuing education to remain up to date with knowledge they need to care for this patient population.”

What kind of positive feedback do you receive from patients and their families about the care their nurses provided?

Brown: “We receive a lot of kudos regarding the patient experience. Countless stories are shared on a regular basis that bring to light the work nurses do when going above and beyond to care for their patients.”

Cerra: “I’ll share three stories that come to mind.

During the holidays last year, the nurses and staff got together to spread a little cheer to a family in need. A young woman battling lung cancer expressed concern that her younger brother might not get any presents for Christmas, since the family was busy caring for her and having some financial difficulties. Many of her caregivers got together and contributed to make a beautiful gift basket with a gift card for the family and purchase a red sled for the little boy.

“Last spring, a patient diagnosed with pancreatic cancer was coming towards the end of her journey. Her daughter was getting married and there was no way she would be able to leave the hospital to attend the wedding, so a whole team of nurses arranged for the daughter to come to the hospital in her wedding dress and they had another little ceremony in the lounge. The patient got to experience her daughter’s wedding in this special way, which made her very happy.

“I also heard a story about nurses embracing the culture of a Jewish patient by arranging a Shabbat meal in the lounge. The patient and her entire family appreciated how accommodating and respectful the staff were for their tradition.”

Brown: “These are three stories of many. This is why patients choose Greenwich Hospital. You’re not just a number here. For the most part, cancer treatment is very standardized around the country. But our specialty is that we care for the whole human being. That’s what we think attributes to their wellness. We take a holistic approach in caring for their physical, emotional and spiritual needs.”

Are there any special programs at Greenwich Hospital specifically for oncology nurses?

Cerra: “Earlier this year, several of our oncology nurses went on a fun retreat to paint at Pinot’s Palette in Stamford.

They were treated to this by a donor, Paula Kohn, who created an oncology nurses retreat fund in appreciation for their dedication to their profession. Paula’s only stipulation was that the nurses would have fun, and not be in a traditional classroom or workplace setting. It was a nice outing for them and a time to decompress.”

Do you have any advice for nurses looking to get into the field of oncology?

Cerra: “Being diagnosed with cancer is something that no patient chooses, and our nurses find it very rewarding to care for them. They go home and don’t have to question, “Did I do enough today?” Their days are very fulfilling because they are not only supporting the patient, but also taking care of the patient’s family. You need to have heart to take care of that kind of patient and be in this profession.

“There are many highs and lows. Nurses guide patients through their experiences. When they are cancer-free, they celebrate with them. There are also times when they hold the hand of a patient who is dying.”

Brown: “The diagnosis is never only for the one person.

Everyone is impacted by it. Our oncology nurses take care of the whole family. They’re extraordinary human beings.” n
