First and foremost, we want to thank every one of you for your continued dedication to supporting the Mission of Ascension St. Vincent’s Foundation to serve all persons, with special attention to the poor and vulnerable.
In 2021, while the COVID-19 public health crisis continued to challenge our community, we witnessed our associates, physicians, donors and community partners step up in tremendous ways to support our patients. We cannot thank you enough for all you have done for our community.
Your generous gifts to Ascension St. Vincent’s Foundation support our outreach ministries that provide free healthcare, preventative healthcare education and hospital programs to those who would otherwise have no access to these services.
In 2021, we saw an increased need for access to primary healthcare. More people than ever turned to us for hope, health and healing. Whether it was testing emergency room patients for COVID-19 outside our Southside and Riverside hospitals during the most recent surge or devising a plan to deliver essential medications to our mobile health patients, our outreach ministries pivoted to serve where they were needed most. Thanks to your dedication and selflessness, the most vulnerable in our community had access to necessary medical equipment, life-sustaining medication and chronic disease management.

Ascension St. Vincent’s Foundation serves as the gateway to hope and health for Northeast Florida and Southeast Georgia’s most vulnerable residents. We raise funds and awareness to support the advanced medical treatment and quality, compassionate care offered in Ascension St. Vincent’s healthcare facilities, as well as to help care for individuals and families who are impoverished and underinsured through our community outreach ministries. Our Mission is based on the tradition of the Daughters of Charity of St. Vincent de Paul who founded us more than a century ago. Daughters of Charity founded St. Vincent’s in Florida with our Riverside campus. They named the hospital “St. Vincent’s” after Saint Vincent de Paul.
2008
St. Luke’s, founded in 1873 as the first private hospital in Jacksonville, joined the St. Vincent’s Healthcare family.
2013
St. Vincent’s Clay County hospital opened its doors thanks to the support of our community. In 2016, St. Vincent’s Clay County doubled in size through a $33.1 million expansion.
May 1, 2020, Delores Barr Weaver Heart and Vascular Pavilion opened at Ascension St. Vincent’s Riverside.
2021
Performance Orthopedics and Spine Specialty Hospital, (in partnership with HOPco) opened at Ascension St. Vincent’s Southside to provide personalized care for bone, muscle, joint and back pain.
Resurfacing of Ascension St. Vincent’s Southside completed. We installed more than 5,000 panels and 298 windows. The project took 162,000+ man-hours and more than 665 days to complete.
Daughters of Charity joined other like-minded sisters across the United States to create Ascension, a jointly sponsored Catholic health system that would provide maximized national advocacy, with each ministry continuing its important locally-driven work.
2019
St. Luke’s was renamed Ascension St. Vincent’s Southside. Since then, we have expanded to include the Performance Orthopedics and Spine Specialty Hospital, Heart and Vascular Health, Lung Institute, and Family Birth Place.

St. Vincent’s hospitals (Riverside, St. Luke’s, and Clay County) adopted the name Ascension St. Vincent’s.
Our past, present
Ascension St. Vincent’s St. Johns County opened in July 2022. Initial clinical specialties include cardiovascular care, neurology and neurosurgery, orthopedics, pulmonology, general surgery, ENT, gastroenterology, cancer care, family and internal medicine and pediatrics.
The hospital: 150,000 square feet, 56 beds (8 ICU), 6 operating rooms, 2 endoscopy suites, 1 catheter lab, 1 chapel, with room to grow
The need: 254,000 residents with only one hospital in St. Johns County. Ascension St. Vincent’s currently has a health center nearby. It opened in 2017 and since opening with our nine providers, we have served 115,000 patients.
The impact: Will bring 500 new jobs to St. Johns County with an average income of $81,000. Healthcare dollars will stay local, and there is room to grow as needed.
The access: Provides 24/7 emergency and surgical care close to home, combining compassionate, personalized care with the latest in advance surgical technology.
Since 2004, Ascension St. Vincent’s Mobile Health Outreach Ministry has served low-income patients in St. Johns County including migrants and homeless patients in the Hastings, Elkton and Armstrong communities (sites of care also included primary care clinics and labs).
Highlights of our outreach service to the St. Johns County, include:

Matthew
Matthew* came to us after he had undergone surgical repair of a hip fracture at a different healthcare facility**. Unfortunately, he was discharged to the street less than 24 hours after his surgery. Thankfully, Matthew found us a few days later. His health had rapidly declined and he was showing signs of a superficial surgical site infection. The Mobile Health team compassionately provided antibiotics, dressings and wound care for him during the next several months. Our continuous care of this homeless man prevented him from having to go to the Emergency Department.
Clarence
Clarence* had undergone surgery for squamous cell carcinoma (cancer) on his shoulder at a different healthcare facility**. This homeless patient was discharged to the street with nowhere else to go and no access to follow-up care. This put him at great risk for infection, as he did not have a clean, safe space to recover. Our Mobile Health team was his only hope. We cared for Clarence for almost one year. During each visit, his wound was cleaned and redressed. Finally, his wound healed and he has recovered completely thanks to the care we provided.
Many in our community face impossible choices each day. Some choose between paying their utility bills or buying insulin. Others elect to stay home — even with serious symptoms — in fear of paying for a trip to the emergency room. Our community outreach programs are designed to lessen these burdens and provide individuals and families with the opportunity to receive quality, compassionate healthcare in their communities.
• Mobile Health Outreach Ministry – Five doctor’s offices on wheels provide primary and acute care to patients in five Northeast Florida counties.
• Good Samaritan Fund – Patients receive necessary medical equipment, medications, life vests and transportation assistance upon discharge from the hospital.
• Dispensary of Hope – Patients receive free or discounted prescription medications.
• Medical Mission at Home – Provides a full day of free clinic services, including medical, dental, vision and pediatric care.
• Baby-Friendly Initiative – Safe sleep and breastfeeding education for mothers
• Seton Center – Health services for mothers and infants
• Reach out & Read – Promotes early literacy and provides books to children
• Emergency Pregnancy Services
• Momentum Program – Adolescent obesity prevention program
• Brighter Beginnings – Training for mothers to reduce rate of infant mortality
• Healthy Moms & Babies – Education and support for mothers
• Kids Together Against Cancer – Supports children of cancer patients
• Bayley School Nurse Program – Fully-funded school nurse position that provides care to five Jacksonville area schools
• Faith Community Nursing – Nursing associates provide educational programs for faith group partners, community events, monthly senior drive thru food distribution, spiritual care and emergency pregnancy services.

This year, Mobile Health saved $6,252,789 through preventative healthcare services and avoided emergency room visits. Ascension St. Vincent’s Mobile Health Outreach Ministry is the only mobile provider of free acute and preventive medical care for seniors, adults and children in Northeast Florida.
Our fleet of five fully-equipped doctor offices on wheels travels across five Northeast Florida counties provide award-winning services which improve the health of underserved residents in our community who would otherwise have no access to healthcare. This ministry provides thousands of our neighbors with quality, compassionate medical care, preventative health education and medication for chronic disease management.

The Arts in Medicine kits are made up of adult and children coloring kits; word search, crossword and sudoku kits and playing card kits. We utilized the nurses and patient care techs on the floors to distribute the materials to patients. The packets help patients reduce their anxiety, restlessness, isolation and depression while facilitating a positive experience with their caregivers -- especially the COVID positive patients.

Kathy is an 86-year-old grandmother who has been coming to our dental clinic for years. She is retired and lives alone on a fixed income of less than $1,000 a month. Kathy has thyroid problems, elevated cholesterol, glaucoma and sinus issues. With no dental insurance, dental hygiene appointments are not affordable and became just one more thing she needed to worry about. Fortunately, our Mobile Dental Clinic Outreach Ministry has been there to help Kathy, and many other seniors who struggle with the same concerns, to maintain their dental health.
Coleen is one of many patients suffering from Type 2 Diabetes and struggling to afford her insulin. Prior to enrolling with Dispensary of Hope, her uncontrolled blood sugar caused her to be in and out of the hospital. Over time, this took a toll on her body. She was ecstatic to hear that we could provide her with insulin at no charge. Since then, she has been doing well. She comes to our pharmacy regularly to get her insulin and reports to us that she has not been in the hospital at all since starting the program.
In April 2021, William was admitted to Ascension St. Vincent’s Clay County with severe heart disease. He was uninsured and needed a Life Vest (an external wearable defibrillator) to continuously monitor his heart rate and prevent sudden heart failure. However, he could not afford the equipment, being in between jobs and without insurance. Without insurance, his cost for a Life Vest would have been $3,400 each month. Thanks to the Good Samaritan Fund, we were able to bridge the gap and pay for the cost of the Life Vest until he started his new job. He was very grateful to Ascension St. Vincent’s and The Foundation. Without this program, we would not be able to provide patients with a feeling of safety and security.

Lung Institute
Pulmonary Ion Robot
Robotic tool to fight against lung cancer, this is a device used for robotic assisted, minimally invasive biopsy procedures in the lung, and allows for earlier diagnosis of the smaller nodules – that also allows the potential for earlier treatment.
Between March and October 2020, we’ve completed 251 ION cases and trained an additional four providers on the technology.

The Sisters of St. Francis Xavier are graciously supported by the Ascension St. Vincent’s Foundation.
The Congregation was founded on Aug. 26, 1897 in Pathein, Myanmar by French Missionary Bishop Alexander to serve those in need. The Sisters arrived in Jacksonville to support and volunteer in the Spiritual Care department.
The Sisters of St. Francis Xavier work closely with the Spiritual Care Team to assist with their ministry of love and compassion for holistic care to patients and families.
At our Ascension St. Vincent’s sites, the Sisters serve as sacristans coordinating the work of other volunteers in our chapels. Their duties include visiting and praying for patients, providing Communion to Catholic patients, and connecting them to a priest if needed. The Sisters
set up for the Mass, clean the Chapels, wash vestments and linens. They also assist with the Residents at St. Catherine Labouré Place.
The Sisters wear a religious habit that is seen by patients, staff, and families. This further enhances the Catholic identity of the hospital for those who experience them. They continue the experience of patient’s contact with religious sisters which has uninterruptedly been a part of Ascension St. Vincent’s from the roots of our existence. The Sisters provide a presence in nearby parishes as outreach to those whom they then often meet in the hospital.
