Letter from the CEO “New,” “uncertain” and “challenging” were among the most commonly used words at Family Service & Guidance Center in 2014. That’s why I couldn’t be more proud of all 280 of our staff and the tremendous resilience, professionalism and positivity they brought through the doors every day to help our children and families. I was especially gratified by FSGC’s implementation of a number of innovative programs – none of them easy undertakings and all of them designed to meet important unmet community needs. • FSGC’s Wellness Connections Team became a “Health Home” for more than 1,000 Medicaid-eligible children in our community. Health Homes is a service that provides support for parents and helps coordinate their child’s health care among multiple providers. • FSGC staff members taught Mental Health First Aid in our community. MHFA is a free eight-hour training that teaches individuals how to recognize someone dealing with mental health issues and how to help them connect to resources. • FSGC and the University of Kansas are collaborating on “Families Supporting Recovery,” a pilot program in which youth ages 12-18 who are dealing with mental health issues and substance use disorders are treated for both at the same time. It is the only program of its kind in the nation. • FSGC’s Crisis Resolution Program moved into a larger facility that doubled its capacity. CRP is a short-term program designed to allow children and teens who are experiencing a mental health crisis to receive intensive care while remaining in their home community and avoiding a more restrictive, costly setting like inpatient hospitalization. It’s also noteworthy that 2014 was the 110th anniversary of FSGC’s founding. We are proud to have provided more than a century of service to our community, and we’re pleased to have established a reputation as a community leader, collaborator and a problem-solver. There’s no doubt that more “firsts” and “opportunities” are on tap for FSGC and the children we serve. Integrated healthcare, population health and outcomes-based reimbursement are on the radar screen for 2015. The philosopher Heraclitus said, “The only thing constant in life is change,” over 2,500 years ago. He would have made a great healthcare executive. I’m confident that the dedication, creativity and compassion of Family Service & Guidance Center’s staff are also constants, ones that more than 12,000 children, adolescents and families across northeast Kansas rely on to help them live happy, healthy lives. I’m eager to work to find solutions to the challenges that lie ahead, and I’m honored to work with so many people who are so deeply committed to the idea that every child deserves a happy childhood. Outreach takes services to those who need them most “A lot of people need our help but they can’t get to our offices, so we go to them.” That, according to Outreach Clinician Tara Wallace, is why FSGC has taken their services on the road with new Community Outreach services. FSGC Community Outreach isn’t exactly traditional, “come-into-my-office-and-have-a-seat” therapy, either. Wallace has conducted sessions in a gym, playing volleyball with a young client. She’s gotten her hands dirty in a community garden, helping young children plant and become more self-sufficient. She’s painted pictures and fingernails. Now, less than a year after Community Outreach’s launch, the program positive effects are already being felt. Wallace recalled a family that was receiving services at the Topeka Rescue Mission. The mother was dealing with multiple health issues. She didn’t have transportation so she had to walk everywhere she went. A staff member at the Mission thought FSGC’s Outreach Clinician might be able to help her and her family. “I had just started working with her,” Wallace said, “and she had gotten a letter from school about one of her children, and she had questions about it. We made an appointment with the school, and when we met with school staff, I pushed her a little by saying, ‘Mom, didn’t you have a question about that? Explain to them what you don’t understand.’ By having us there, she felt more empowered.” Over time, Wallace helped the mom build confidence in her own abilities to secure resources on her own, despite suffering from severe social anxiety disorder. It wasn’t long before she decided to go see a doctor about her health issues. Now, she’s working to address them. She secured a job on her own and can pay for a ride to work so she doesn’t have to walk. “After she got the job, she came to me and said, ‘One thing we haven’t talked about is budgeting. Can we talk about that?’” Wallace said, smiling. Once the mother got linked to people and organizations that could help her meet some of her needs, her life calmed enough to allow her to start working on meeting other needs and issues. That, Wallace said, is what the Community Outreach Program is designed to do. Anthony Bryan, LSCSW, Director of Integrated Care and Community Outreach, explained that the implementation of Community Outreach was a response to a broader need. “There are so many families that need help. We’re not just looking to help families dealing with mental illness,” he said. “We want to connect people to resources that can help meet their needs, whether they need mental health care or not.” FSGC selected high-risk, high-need neighborhoods for Community Outreach. Then, hubs within those neighborhoods were identified and partnerships were formed. 6


Community Outreach (con’t) Bryan said, “We spent a lot of time building relationships with organizations. It wasn’t always easy because they might see you as competition, even though that’s not the idea at all. It’s a tough balancing act.” Outreach clinicians, trained as both therapists and case managers, spend time at the Topeka Rescue Mission, HiCrest NET (Neighborhood Empowerment and Transformation), Pine Ridge Parents as Teachers, the Antioch Family Life Center and the Boys & Girls Club each week. People experiencing difficulties can come to those locations to speak to an Outreach Clinician who can help them determine what services would best meet their needs. Wallace said that being located in a community hub is a significant advantage when assessing needs and developing solutions. For example, if a family has a problem, an FSGC Outreach Clinician can go to the home and see if something in the environment is causing the problem. Sometimes, the solutions are relatively simple; other times, families need more help. Those relationships developed through Community Outreach’s up-close, hands-on approach gives her an advantage over other organizations that aren’t as proactive. “Some service providers don’t understand what resources people already have and what their home life is like because they’re not out in that environment,” she explained. The organizations at which Outreach Clinicians spend time are realizing benefits from having FSGC staff around, as well. Bryan said, “We’re a resource for those organizations, too. They may be serving individuals with needs that they just don’t know how to meet. The organization connects us to that individual. That’s where we get most of the people we work with now.” Those relationships with families and organizations are critical to the success of this exciting new program. Wallace said that without that trust, people won’t open themselves up to the help that is being offered. Once they do, she said, the changes can make a tremendous difference in their lives. “Our goal is to bring some stability into their chaotic environment. Once that happens, we can help them return healthy functioning to their family.”

FSGC and Health Homes improve children’s care FSGC’s Wellness Connections Team is making managing health care easier for hundreds of children and families in our community, thanks to Health Homes. Health Homes is an innovative set of services designed to reduce health care costs by helping Medicaid-eligible families learn to become more organized and proactive with their children’s health care. The program is paid for by KanCare, the state’s Medicaid program, so there is no cost for participants. “Families are notified by the State of Kansas that they can participate and that FSGC is their assigned Health Home. Once we know someone is assigned to us, our Wellness Connections staff contact them and try to get them engaged,” Anthony Bryan, LSCSW, Director of Integrated Care and Community Outreach explained. Wellness Connections staff members typically have to do a lot of education and reassuring about what Health Homes is and how it can benefit them and their child. Bryan said, “There are a lot of different reasons people are skeptical of a new service like this. Some are worried that we’re going to report them for something. Some people worry that they’ll go to a doctor and get bad news. Others worry that their health care is going to be expensive, even though they have insurance. But once a family does get engaged in Health Homes, they love it.” Families call FSGC and make an appointment with a Care Coordinator. At the appointment, the Care Coordinator works closely with them to assemble a Health Action Plan.

Health Homes (con’t) Rebekah Adams-Reyes, LMFT, Ph.D., Integrated Care Program Manager, said, “A Care Coordinator sits down with the child and the parent or caregiver. They assess the child’s health care needs, find out what services they’re already receiving and identify any health care gaps that need to be filled. Then they can help the family make appointments and keep track of what needs to happen and when.” Helping families locate resources is a big part of what the Care Coordinators do, she said. For example, not many dentists accept Medicaid. Dr. Adams-Reyes said Care Coordinators will help families find a dentist who does, especially if the child hasn’t been to the dentist for a while. Sometimes, more basic needs need to be met before a family can deal with health care issues. “A big part of what we do for families is basic stabilization,” Dr. Adams-Reyes said. “It’s hard for a parent to think about getting an annual checkup for their child when they don’t have a car or a way to get to the doctor’s office, or maybe they’re homeless. Once issues like those are settled, it’s easier for the parent to focus on their child’s health.” Bryan said, “One of the best things about Health Homes is that we help take some of the pressure off those families. They live hectic, uncertain lives. The more we help stabilize families, the healthier they become.” Health Homes staff members maintain regular contact with participating families, and the scope of the contact depends on the family’s needs. Some families only need a monthly “check-in” call. Others require more help in meeting basic needs and securing resources. There are also regularly scheduled Wellness Connections mailings that provide information on topics ranging from avoiding sunburn to the importance of annual physicals to getting good sleep. Wellness Connections staff also act as a central point of contact when it comes to a participating child’s health care. They can act as a liaison between families and providers, making appointments and ensuring that all of the child’s providers, including dentists, mental health clinicians and any specialists the child might be seeing, are on the same page. One local hospital has also been in contact with Wellness Connections when a Health Homes participating child has received care there. “That communication has been so helpful in coordinating services and making sure all of the child’s providers have complete information,” Dr. Reyes-Adams said. This kind of communication and cooperation will ensure the success of the Health Homes because there is so much flexibility in what the Wellness Connections team can do to help children and families. She said, “There aren’t many limitations on what we can do or what fits because, when it comes to a child’s health, it all fits. It’s nice to be able to talk to a family and find out what’s keeping them from getting health care for their kids because we’re able to say, ‘Yeah, we can figure that out.’” Bryan said this flexibility is key to Health Homes achieving its most important goal. “Our goal is to help families gain the confidence to do this on their own and be more self-sufficient. If parents can start to focus on maintaining their child’s health and wellness, they won’t need our help anymore,” Bryan said. 9
