Having a loved one who needs specialized care for Alzheimer’s disease or other forms of dementia presents many challenges. Our state-of-the-art memory care program provides the personalized support your loved one needs to live each day to the fullest.

Memory care at Fellowship Village follows an innovative household model of care that provides personalized support for your loved one. Residents benefit from having a secure and familiar environment, intentionally designed spaces, daily engagement activities and interaction with warm and caring team members. Our memory care team is specially trained in dementia care with certified dementia care practitioners on-hand so that every minute of every day, a nurturing team member is available to help your loved one.

Our sub-acute rehabilitation services are designed to restore optimal physical health following an injury or illness.

We have a reputation for excellence in providing rehabilitation services and post-hospitalization care for acute neurological, orthopedic, cardiopulmonary and other medical conditions. Our highly skilled therapy team works collaboratively with a physiatrist who specializes in physical medicine, pain management and rehabilitation.

We offer state-of-the-art equipment including RedCords® (an innovative neuro-activation system), LiteGait® (a body weight suspension system), NeuroGym® trainer (biofeedback device for motor control and coordination), and NuSTEP® cycles for general conditioning. Aquatic therapy is also available for a low-impact workout, and we provide community mobility training, including assessment and driver education for patients recovering from a stroke or complex surgery.

Residents can choose a private deluxe studio suite with a seating area and kitchenette. Our household model of care features an expansive common area with a gourmet kitchen for chef-prepared meals, as well as a dining room, living room, den and four-season porch. Also enjoy access to a spa with whirlpool tub and zero-entry steam shower.

Rehabilitation services are available for both residents of Fellowship Village and those who live in the greater tri-state community. We accept Medicare, Medicare Advantage and most private insurances.

Long-term care in a supportive setting

Our skilled nursing program offers exceptional healthcare services 24/7 in a welcoming residential setting that feels like home. A multidisciplinary care team works together to develop and implement a long-term care plan tailored to each individual. Our team members provide ongoing assessments, personalized long-term care and therapy services, helping residents maintain strength and mobility and maximize their independence.

Long-term care residents enjoy a private deluxe studio suite with a seating area and kitchenette. Our expansive common area features a gourmet kitchen for chef-prepared meals, a dining room, living room, den and four-season porch. Your loved one can choose from an impressive variety of delicious and nutritious meals as part of our 5-star dining program.

Direct admissions from outside our community are welcomed. We accept both Medicare and private-pay admissions.
