Cook Children’s Center for Community Health
The Cook Children’s Center for Community Health (C4CH) prioritizes the findings from our Community Health Needs Assessment (CHNA) filled with information from leaders and parents to ensure that our work and support meets the needs of the community while aligning with system initiatives. Strategically, equitable access to care drives our six focus areas: asthma, healthy lifestyles, injury prevention, mental health, oral health, and parenting and family support.
