A LETTER FROM THE BOARD PRESIDENT & EXECUTIVE DIRECTOR A Tough Year, A Remarkable Year 2009 was another challenging , rewarding and successful year in carrying out our Mission on behalf of people with intellectual/cognitive disabilities at Foothills Gateway, Inc. The programmatic and financial aspects of the organization were regularly surveyed by State of Colorado Quality Assurance personnel as well as State-selected Auditors Logan, Thomas, & Johnson, LLC. As in the past, we are pleased to be able to report that Foothills Gateway, Inc. is substantially in compliance with all rules, regulations, standards and guidelines established by the State of Colorado.

More importantly, however, is that high levels of client and family satisfaction were reported by these service constituents again through our annual satisfaction survey process.

This past year has been another year of significant change for services for people with intellectual disabilities.

Foothills Gateway, Inc., as the Community Centered Board serving Larimer County, finished the implementation of the Comprehensive Medicaid Waiver Changes in spring of 2009 and immediately began to inform clients, parents, guardians and other family members of impending changes in the Supported Living Services Waiver/Children’s Extensive Support program. Throughout this current fiscal year all clients in the Supported Living Services Waiver program will be transitioned into a new method of assessing client need and setting the annual service plan for each client.

As a back drop to all of these programmatic and resource changes, the State of Colorado has been wrestling with falling revenues and the related constitutional mandate to balance the budget. To date, the system for people with intellectual disabilities has taken some cuts to Medicaid provider rates. Beginning fiscal year 2010-2011, Foothills Gateway, Inc. and its related Program Approved Service Agencies will have taken a 4.5% provider rate cut in total. In spite of the difficult economic climate, Foothills Gateway, Inc. has managed to maintain high levels of quality services and has not reduced the number of clients served throughout this downturn. In fact, Foothills Gateway, Inc. is serving 1468 clients as compared to 1392 in fiscal year 2007-2008. This increase is due in part to Foothills Gateway, Inc.’s management efficiencies, enabled by local county Mill Levy funding.

The Foothills Gateway, Inc. Operating Board of Directors continues to proactively address two key issues: Public Information Accessibility (Transparency) and the analysis of the Conflict of Interest issue, as it pertains to Foothills Gateway and its service area of Larimer County. Specifically, in the past year the Board of Directors has been engaged with the community listening to and understanding how best to move forward. To that end, the Board assembled two groups: the Accessibility Committee and the Conflict of Interest Task Force.

The Accessibility Committee was charged with determining current levels of communication with service constituents. The committee was also charged with the task of developing new and innovative ways to make information more accessible to the public. Results of the committee have been presented to the Board and further discussion of the recommendations will be conducted through the Board of Directors meetings in the Spring of 2010.

The Board of Directors also created a Conflict of Interest Task Force to analyze the potential and perceived conflicts of interest in the Foothills Gateway, Inc. service area and to recommend changes to address the perception of conflict of interest. This task force, comprised of parents, advocates, service providers, staff and Board Members, was facilitated by professional staff from the Larimer County Administrative Offices. The task force met for a year to analyze the issue of conflict of interest, utilizing the State of Colorado’s commissioned “University of Southern Maine Report” as it’s “frame of reference” in order to be consistent with the efforts of the State of Colorado in mitigating the perceived conflict of interest.

The Board is currently working with these groups to prioritize recommendations and develop work plans to implement any further changes.

In 2009 members of Foothills Gateway, Inc.’s Operating Board also initiated meetings with other Community Centered Boards in Colorado to share information about how to better provide services. This communication is expected to strengthen in 2010.

Fundraising in all areas, and certainly with Foothills Gateway, Inc., has decreased dramatically. However, significant donations were made by the Fort Collins Service League and the Foothills Service League. The Foothills Gateway Foundation remained dedicated in their consistent work in raising significant funds through its “Annual Bonanza Auction” and two annual golf tournaments, “The Hogerty Open” and “Shanks for the Memories”.

The Operating Board of Directors and the Foundation Board of Directors have initiated a Joint Resource Development Committee, which is working to find additional sources of revenue, to make up for diminishing state and federal dollars.

While clients, families and those who support them ride out the rather protracted recession, the Foothills Gateway, Inc. Board and the Foothills Gateway Foundation Board remain steadfast in their efforts to provide the best services to the most people possible. New and innovative means of providing supports, informing the public and raising new funds are tasks that will be developed throughout the next year.

The ability to stay current with these tasks and other important systemic information will be available through internet capability. Our website (www.foothills gateway.org) will contain a plethora of new, informative and exciting details to keep you abreast of the dynamic times in which we find ourselves.

Foothills Gateway strives to make positive contributions to our community.

WE HAVE A COMMUNITY-ABILITY

Involve-Ability in Community Outreach and Fund Raising Throughout 2009, we continued working with our support groups to raise awareness and funds for our programs through annual fundraising events such as, the Fort Collins Service League’s Kitchen Kaper, the Foothills Gateway Foundation’s Bonanza Auction, the Shanks for the Memories & Hogerty Open golf tournaments, the Foothills Service League’s Taste of Loveland wine & food tasting, and their Comedy Night featuring Josh Blue. These groups also hosted several fun and social activities for our clients, including an Ice Cream Social, a Pizza Party, and the annual Christmas Gift Shop. A Songs of the Season concert was performed by the New Horizons Band, and the Family Support Council hosted several fundraising events throughout the year, including their calendar sales, Butter Braid sales, and their inaugural Flying Pigs Charity Run-Walk Race.

Other activities throughout the year included: being named O’Dell’s Brewing Company’s Charity of the Month in February, a dance performance by Dance Express, and the Round Up at the Fort parade. Whole Foods hosted a Chili Supper fundraiser and Grab & Give food drive for Foothills Gateway, and the Colorado Eagles hosted a Skate with the Eagles night for kids. We also have great help throughout the year from our various volunteers and volunteer groups, especially during the Make A Difference week.

Efforts are made to continue to inform and work with our local legislators on issues of importance to the developmental disabilities field through our annual co-sponsored Legislative Forum with the Larimer Center for Mental Health and by participating in several legislative functions sponsored

by Alliance and the Northern Colorado Social Legislative Network.

Intake and Waiting List Case Management Major Accomplishments In 2009, the Intake Team welcomed Justin Jones to our Resource Coordination Assistant position, as the Department of Resource Coordination and Development re-arranged staff and job responsibilities to better meet the requirements of the adult services waivers.

Intake Case Managers were busy as they handled over 570 intake calls, and processed 64 eligibility determinations throughout the year. The number of adult individuals admitted to services decreased this year due to a lack of new resources from the State and the State budget crisis. Admissions to the Early Intervention Services increased as well as the admissions into the Home & Community Based Services (HCBS) Children’s Waiver. The number of individuals on the Waiting Lists continued to grow.

The number of adults needing emergency services increased this year as well. Emergency situations included being homeless, planned release from jail, as well as financial and mental health issues.

Five persons from different parts of the state transferred their services to Foothills Gateway, Inc., an indication that Foothills Gateway, Inc. offers services that may not be available in other areas of the State. The increase of additional opportunities and choices available to clients and families is due to an increase of service provider options in Larimer County. In 2008, Foothills Gateway contracted with 16 Program Approved Service Agencies (PASAs). That has grown to 23 Program Approved Service Agencies in Foothills Gateway’s service area, giving our clients more choice than ever before.

Our outreach to the community continued with Intake Case Managers attending Individual Educational Plan (IEP) meetings at local school and foster care reviews. In addition, we continue to collaborate with local school districts in the annual Transition Fair.

Early Intervention supports children birth through 2 years of age who have developmental needs.

Foothills Gateway, Inc. served over 425 children through the EI program in 2009.

Grow-Ability & Sustain-Ability in Early Intervention Early Intervention is the Foothills Gateway program that coordinates services and supports for children in Larimer County ages birth through two years. Our Early Intervention (EI) program is proud of their successful grow-ability and sustain-ability.

We have sustained the program growth that we experienced last year, and have continued to grow, slowly but steadily this year. In 2008, our highest number of children enrolled at any one point in time was just over 240. In 2009, we had a month with just over 255 enrollees!

Our EI staff has grow-ability as well. Due to the increase in children and families served, another Service Coordinator was added to the team this year. Currently, the EI team has seven Service Coordinators that serve an average of 300 families per month. Service Coordinators in EI are unique when compared to other Case Managers in the agency, as they serve children who are enrolled in the program, as well as children and families who are in process of being determined to be eligible for services. Every child who is referred to the agency is assigned to a Service Coordinator who can assist them through the eligibility process.

The EI program has sustained its public awareness efforts, which includes outreach to family practice physicians, specialists and pediatricians. This outreach has resulted in a higher number of referrals for EI services coming from medical offices. This is essential as research and parents concur that the earlier infants and toddlers begin receiving intervention, the better the outcome! Speaking of outcomes, family feedback from our annual survey through the Division for Developmental Disabilities demonstrates that more than 90% of families served expressed satisfaction and felt that EI services helped their child develop and learn.

As the program grows, there are challenges in meeting demand and maintaining high quality in service delivery and family satisfaction. The EI program continues to work collaboratively in Larimer County to meet the changing and growing needs of infants and toddlers who are eligible for services. In EI, we have grow-ability and sustain-ability!

In addition to providing case management services, Foothills Gateway, Inc. also provides a variety of direct care services to clients.

WE HAVE A SERVICE-ABILITY

Foothills Gateway, Inc.’s Early Intervention Services Program provides direct care services to children in Larimer County. To help explain the difference such a program can make in just one family’s life, below is an article written by a speech therapist who is an independent contractor for Foothills Gateway and worked with this family.

A Family’s Story

By Julie Blair, M.N.S. CCC-SLP Logan Shaver and his family are recent graduates of the Early Intervention Program through Foothills Gateway. Logan’s mother, Jessica, would like to share their story in the hopes that it may help at least one other family navigate the difficult time that they may have in common. Logan began receiving therapy services through Foothills Gateway a couple of weeks before his first birthday. Logan’s family knew from the beginning that he wasn’t developing in typical ways and had initially pursued therapy services, paying out of pocket. Unfortunately, it wasn’t until Logan’s mother informed their Occupational Therapist and Pediatrician that they could no longer continue therapy due to the financial strain that it was putting on the family, that they were referred to Foothills Gateway. Logan and his family have been a great fit for the educational model of Early Intervention, as outlined in Part C of the Individuals with Disabilities Education Act (IDEA), that provides parent education and integration of therapeutic goals and strategies into the family’s daily routines within the home. Logan is a child that is very easily overstimulated and overwhelmed. When he received therapy at a center he would either cry or sleep through his sessions as a coping mechanism. Once he started to receive therapy in his home, he was able to participate and began to learn. Logan’s parents Jessica and Joe, and his big sister Destiny, have worked in a rewarding partnership with his therapy team to help Logan to play, to eat by himself, to move and walk, and to communicate. The nature of Logan’s disability has been a challenge to determine.

Figuring out what works for Logan has taken patience and persistence from his loving family and a lot of creative thinking by his therapy team. According to Jessica, “We have definitely struggled with finding support for Logan and our family. Without the help and support of our Foothills Gateway team, we wouldn’t be where we are today. Paige Sorensen, our Service Coordinator, was always there with an answer to my million questions about the different state programs available.

My therapists provided me with new activities and exercises to make Logan more independent and successful with daily activities. The entire team was always available when I needed an outlet or advice on the “next step”. Every person involved definitely went above and beyond their “job” and gave 110% to Logan and our family!”

Julie Blair CCC-SLP, Logan’s speech pathologist, says, “Every visit with Logan and his family has been a joy. To see him grin, to play with toys, to take his first bite of food by himself, to make sounds, and to practice his first steps is a major reason to celebrate. Logan is learning on his own schedule, but always learning, thanks to the great support that his family gives him.

And that is why I love being a therapist for Foothills Gateway.”

Logan has recently turned three and transitioned to an integrated pre-school class with therapy services through the Poudre School District. “I was reluctant about sending Logan to school, but he is doing great,” says mom. “We really only have one dream for Logan… that is, for him to be happy. No matter what happens from this day forward, we want him to be happy every day! Ideally, Logan will overcome any battles he’s had thus far and “catch up”, creating many more opportunities in life. We know in our hearts, no matter what, Logan will do great things. He already has! He is an inspiration to me everyday!!”

The Early Intervention Program at Foothills Gateway provides an array of services to qualifying children and their families.

Professionals from throughout Larimer County are contracted to provide speech therapy, occupational therapy, physical therapy, developmental intervention, nutrition, and social/emotional intervention. WE HAVE AN ANSWER-ABILITY

Adult Case Managers actively coordinate and monitor services and supports to participants and their families.

Answer-Ability in Adult Comprehensive Case Management Foothills Gateway, Inc.’s adult case management received a 4.5 out 5 in client satisfaction rating for FY 2008-2009. In reviewing comments and concerns from clients and families we recognized that there were some questions that needed to be answered.

Guardians, clients, families and sometimes even providers were confused about options available in each Home and Community Based Services (HCBS) waiver, processes to follow, and who could answer questions when they needed assistance. This feedback led to a massive restructuring in Adult Case Management. In July 2009, the Adult Case Management Team divided into two teams, one that supports clients in the HCBSSupported Living Services Waiver and one that supports clients in the Home & Community Based Services (HCBS)-Comprehensive/ Children’s Extensive Support (CES) Waiver. This change allowed Case Managers to become experts in waiver program rules, be aware of changes in policy and procedures and implement them in a much more timely fashion. Many questions have been asked about high turnover in Case Management the past few years. It is our hope that this change will also increase the knowledge, ability to meet job expectations and retention of the Case Management staff.

In October 2009 the Colorado Department of Health Care Policy and Financing (HCPF) set expectations that Case Management begin billing in 15 minute increments for Case Management services provided. There were many questions about how to accomplish this new billing process. With the support of Foothills Gateway’s Information and Technology (IT) team, new databases, and staff training we were able to answer all questions and accomplish this major task.

Case Management recognizes that services and resources can be limited. We are participating in committees, attending conferences, and working cooperatively with several local and state agencies to assure we are aware of any option that might be available to our clients.

The Comprehensive Adult Case Management team is committed to answering any question a client, family, guardian, or service provider may have. If you have a question about waiver services, please contact a Case Manager and we will assist you in getting answers.

Supported Living Services enables participants to live more independently at home and in our community.

Dur-Ability in SLS Case Management The past year posed many challenges for the staff, families and clients associated with the Medicaid Supported Living Services (SLS) and Children’s Extensive Support (CES) programs. The challenges were created by the July 1, 2009 implementation of the new SLS and CES Medicaid Waivers. Despite these challenges, everyone came together with optimism and professionalism to make the best of the situation proving that--even in the face of great and uncertain change-- the committed endure.

SLS and CES waivers are written for five year periods. They are the agreements that Colorado enters into with the Centers for Medicare and Medicaid Services (CMS), a federal agency. The July 2009 new waivers contained significant changes to the way SLS and CES must operate to be eligible for federal matching funds. These changes included caps on spending and individual services, uniform provider rates, changes in the types of services provided and changes to program oversight and management. These changes were not easy for many clients as they brought less flexibility with services, a more limited array of services, and potentially fewer services.

The first six months of the year were spent educating clients and families about the changes and strategizing with the teams to make necessary changes to the plans. Final information about the waiver changes was slow in coming, so at times the teams did their best to anticipate the changes and adapt as the information became final. At times this process could be frustrating, but Foothills Gateway staff would not allow the frustration to stand in the way of quality direct services, monitoring and goal setting.

As a result of these changes, Foothills Gateway determined that a higher degree of specialization was needed within Foothills Gateway Case Management. To meet this need, Foothills Gateway created specialized case management positions. Seven employees are now dedicated SLS and CES Case Managers. These case managers continue to face the uncertainties created by the changes while keeping positive attitudes as the Colorado Department of Division for Developmental Disabilities and Health Care Policy and Financing work the kinks out of the new waiver.

Housing-Ability in HUD-Section 8 Program Foothills Gateway, Inc. continues to assist clients with their independence-ability through the Section 8 Voucher program. In 2009, Foothills Gateway increased independence by serving the remaining 6 clients on the Section-8 waiting list with the issuance of State-funded housing vouchers. This program currently provides support for 129 clients/families through both rental and homeownership programs.

In addition, the State-wide waiting list opened again in 2009 and Foothills Gateway was able to submit an additional 25 applications and will serve additional clients/families in 2010.

Executive Summary-Information Management As is true with most corporations, Foothills Gateway, Inc. has always been and continues to be concerned with System performance management and outcomes. To evaluate the quality of service provided to our clients and their families, individualized surveys were sent to every client/family who receives any type of service from Foothills Gateway, whether it be a client who is on the waiting list and receives waiting list case management, or someone who receives direct services from any of our programs. We strive to attain a “360 degree� evaluation of ourselves to keep improving the quality of services every client receives at Foothills Gateway, Inc. Target Levels are set for each objective to be both attainable and appropriate. The overall objective is to maximize client and family satisfaction in all areas on an annual basis.

Clients and families receive separate surveys so that when information is received it can be used to determine where the organization is excelling and where additional work may be necessary. Clients and families are surveyed on multiple aspects of their services with Foothills Gateway, from satisfaction with their case manager to satisfaction with feeling safe and community inclusion. The predetermined target for client and family satisfaction with case manager is 95%. In 2008/2009, clients indicated a 98% satisfaction with their case manager (regrettably a 1% decrease from 2007/2008) and families indicated an overall 91% satisfaction rate with their case manager (no change from 2007/2008). Additional areas where FGI puts an emphasis on setting and achieving high standards are staff turnover and administrative expenses. The lower the turnover and administrative expenses, the more funds that can go directly to individuals with developmental disabilities in our service area.

Foothills Gateway, Inc. was able to decrease general and administrative expenses for the agency by 1.6% in the 2008-2009 Fiscal Year for a total of 8.5% of budget.

Foothills Gateway also experienced a decrease in staff turnover, reporting at 36% for the year, a 6% decrease from 2008. We plan to continue efforts to reduce turnover and maintain the target level of 35%.

Foothills Gateway strives to be both efficient and effective. To this point, Foothills Gateway, Inc. has formed many collaborations to make serving individuals with cognitive disabilities and receiving services from Foothills Gateway easier and more beneficial. We have accomplished such efficiencies with two organizations, Larimer Center for Mental Health (Larimer County) and Division of Vocational Rehabilititation (State of Colorado), by offering office space within the Foothills Gateway building to specialists within these organizations. This collaborative effort has proven beneficial to all three organizations as well as clients and families.

Foothills Gateway, Inc. will continue to look for simple and innovative ways to increase client and familiy satisfaction as well as increase collaboration with organizations serving the same population.

The number of individuals served by Foothills Gateway, Inc., continued to grow in FY2009.

The organization received an increase in funding from the State of Colorado. This funding increase was due to several factors.

Supports were provided to 5.5% more individuals than had been served in the prior year. Higher State support levels were received for children in the Early Intervention program.

Supports Intensity Scale (SIS) rates for Comprehensive programs were instituted statewide, which resulted in an increase in the level of reimbursement for individuals that had been being served in Larimer County. Despite these funding and service increases, the agency was able to decrease its expenditures for administrative supports while completing the building remodeling project which began the prior year.

The expenditures for Independent Contractors providing individualized Supported Living Services continued to show substantial growth of over 75% during the fiscal year. Likewise, the number of Program Approved Service Agencies providing contracted Comprehensive services grew from 12 to 14 provider agencies. This has again broadened the choices available to clients in selection of providers to meet their individual service needs.
